Clear Positive MOG Antibodies in Adults With Multiple Sclerosis Features: Diagnostic Challenges.

BACKGROUND AND OBJECTIVES: Distinguishing multiple sclerosis (MS) from myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) can be challenging because of some shared clinical and radiologic features. We aimed to describe adults with clear positive serum MOG antibodies (MOG-Abs) presenting with MS features and rethink their nosological framework. METHODS: We conducted a retrospective longitudinal study within the French NOMADMUS cohort and included adults from 2015 to 2025 tested clear positive for MOG-Ab in the serum with at least 1 MS-suggestive feature: (1) biological (CS

Insulin resistance and brain sulcal morphology in non-diabetic adults.

Insulin resistance exerts direct effects on brain structure, yet most neuroimaging evidence comes from populations with established diabetes, and conventional gray matter volumetry may lack sensitivity to detect early structural changes. Here, we examined associations between insulin resistance and sulcal morphology, a measure that integrates cortical thinning, subcortical atrophy, and white matter degeneration into a single outcome, in 728 non-diabetic adults (518 men, 210 women; mean age 61.4 ± 11.6 years). Brain volumes were quantified using OpenMAP-T1, an automated deep learning-based parc

Memory-Based Learning Disabilities: A Syndrome without a Pigeonhole-Towards a Mechanism-First Assessment Framework.

Memory-based learning disabilities represent a distinct and underrecognized clinical entity that current diagnostic classification systems fail to capture. This theoretical synthesis argues that these difficulties constitute an identifiable syndrome defined by neuropsychological vulnerabilities in declarative memory systems, particularly those dependent on hippocampal function. Evidence from cognitive neuroscience, developmental neuropsychology, and educational psychology converges to identify a distinctive phenotypic profile that differentiates memory-based learning disabilities from other de

Regional brain iron deposition and functional alterations in thalassemia: a multimodal human study with murine validation.

BACKGROUND AND OBJECTIVES: Chronic anemia and systemic iron overload in thalassemia major (TM) patients may result in neurological deficits, although researchers have not completely elucidated their specific impacts on the brain. This study employed a multimodal research methodology to investigate the characteristics and intrinsic connections of cerebral iron deposition, cerebral dysfunction, and cognitive impairment in TM patients, providing theoretical support for brain monitoring and neuroprotective interventions in this condition. METHODS: We prospectively enrolled 88 patients with TM, in

Conflict-Dependent Gain Control in Area VIP during Visual-Vestibular Self-Motion Processing.

Visual and vestibular signals are continuously integrated to estimate self-motion, yet these sensory cues are often inconsistent under natural conditions. How cortical circuits regulate multisensory interactions under cue conflict while preserving stable heading representations remains poorly understood. Here, we recorded neurons in male macaque ventral intraparietal area (VIP) during passive movement stimulation while systematically varying visual-vestibular heading offsets. VIP neurons exhibited strong conflict-sensitive gain modulation: congruent cues enhanced neuronal responses, whereas in

Interpretable Multiscale Directed Temporal Graph Learning for MEG-Based Identification and Lateralization of Temporal Lobe Epilepsy.

Objective: To address the limitations of existing deep learning methods for brain networks in jointly modeling directional interregional connectivity, multiband information, and short-term dynamic features, this study proposes a multiscale directed temporal graph convolutional network (MSD-STGNN) for the three-class classification of healthy controls (HCs), patients with left temporal lobe epilepsy (lTLE), and patients with right temporal lobe epilepsy (rTLE). Methods: Resting-state magnetoencephalography (MEG) data were obtained from 43 subjects, including 14 HCs, 13 patients with lTLE, and 1

Preoperative neuropsychological profiles in adult parietal glial and glioneuronal tumors: A retrospective descriptive study.

The parietal lobe is a highly interconnected cortical hub involved in language, sensory integration, visuospatial processing, and cognition. Although neurological and cognitive consequences of parietal tumors have previously been described, detailed preoperative neuropsychological profiles obtained using comprehensive assessment batteries remain incompletely documented. This study aimed to describe preoperative neuropsychological impairments in adults with parietal glial and glioneuronal tumors and to explore descriptive patterns according to hemispheric laterality and parietal subregion. The cerebellum in migraine: a narrative review of anatomical, imaging, and clinical evidence.

Migraine is usually framed through trigeminovascular pathways and interactions among the brainstem, hypothalamus, and cortex. The cerebellum has received less attention, although human studies place it within circuits that process trigeminal nociception and vestibular signals. This narrative review examines anatomical and experimental work together with imaging, genetic, and clinical studies, giving greatest weight to direct human migraine evidence. Repeated-measures imaging suggests that cerebellar connectivity changes across the migraine cycle, but the cohorts are small. Structural findings

Putative Transcriptomic Endotypes Reveal Neuronal, Immune, and Metabolic Heterogeneity in Temporal Lobe Epilepsy with Hippocampal Sclerosis.

Temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) is a biologically heterogeneous disorder in which neuronal loss, gliosis, neuroinflammation, and metabolic stress may coexist but are unlikely to be uniformly represented across all patients. To investigate this heterogeneity at the molecular level, we analysed publicly available hippocampal transcriptomic data from individuals with TLE-HS. Our analyses identified three transcriptomically defined putative endotypes within this shared neuropathological diagnosis. These states were associated with distinct molecular programs, including a

Reassessment of the DTI-ALPS index in focal epilepsy.

OBJECTIVE: Recent evidence highlights the limited specificity of diffusion tensor imaging (DTI) along perivascular spaces (ALPS) as a noninvasive glymphatic biomarker because microstructural tract degeneration and crossing-fiber geometry confound it. We aimed to reassess the interpretation of the DTI-ALPS index in people with focal epilepsy. MATERIALS AND METHODS: Seventy-nine people with drug-resistant focal epilepsy (PWFE) underwent comprehensive presurgical work-up and 3-T magnetic resonance imaging (MRI). Etiologies included MRI-negative epilepsy, malformations of cortical development, te

Very Low Frequency of SCA27B Among Undiagnosed Cerebellar Ataxia Patients in Peru.

Spinocerebellar ataxia type 27B (SCA27B) is a recently described autosomal dominant ataxia caused by a pathogenic GAA repeat expansion in the FGF14 gene. The frequency and the distribution of FGF14 repeat alleles remain largely unexplored in underrepresented populations, particularly in Latin America. To investigate the frequency of SCA27B and characterize the allelic distribution of the GAA-FGF14 microsatellite locus in a Peruvian cohort of individuals with ataxia of unknown molecular etiology. We analysed a cohort of affected individuals evaluated at a referral center for neurogenetic disord

Hydrocephalus and hearing loss associated with chronic Brucella ceti infection in stranded bottlenose dolphins Tursiops truncatus.

Brucella ceti has a global distribution and is associated with neurologic disease, systemic infection, and reproductive losses in cetaceans. B. ceti has been associated with perinate mortality and sporadic systemic infections in bottlenose dolphins (BNDs) Tursiops truncatus from strandings both during and outside of unusual mortality events on the southern and eastern coasts of the USA. In 2014-2015, 2 BND from the waters of Texas and Florida stranded on separate occasions, were admitted to rehabilitation facilities, and were later placed under permanent professional care. Both dolphins had su

Naturalistic spatial education engages distinct neural processes for mental rotation, cognitive load, and linguistic demands.

Neuroscience studies typically isolate spatial cognition using simplified tasks targeting a single cognitive process. However, during formal spatial education, the brain rarely performs one cognitive operation at a time. Here, we used functional magnetic resonance imaging (fMRI) to assess neural systems involved in dynamically processing an educational spatial visualization lesson. We quantified lesson content using an instructional-design framework capturing variation in spatial complexity, cognitive load, and verbal density. We then characterized how instructional demands modulated brain fun

Whole-Brain 4D MRA With Improved Vessel Delineation Using Undersampled Multiple Overlapping Thin-Slab Radial Acquisition.

Time-resolved arterial spin labeling (ASL)-based noncontrast 4D MRA enables visualization of cerebral arterial flow dynamic patterns and holds great potential for evaluating cerebrovascular disease. However, conventional ASL 4D MRA implementations using look-locker acquisitions are susceptible to progressive RF saturation during extended readouts, leading to substantial ASL signal attenuation and loss of vessel delineation in distal arteries. These limitations potentially hinder a wider clinical translation of 4D MRA, especially when assessment of whole-brain vascular anatomy and flow dynamics

The Mask of Delirium: Uncovering a Presumed Cardioembolic Stroke Through Comprehensive Clinical Evaluation.

Delirium and aphasia exhibit overlapping clinical features, complicating the diagnosis of acute neurological disturbances in older adults. Both conditions impair communication and orientation, leading to incoherence; however, they arise from distinct pathophysiological mechanisms and require different management strategies. This case concerns an 83-year-old woman with a 48-hour history of acute-onset confusion and abnormal speech. Magnetic resonance imaging (MRI) revealed an acute infarct in the inferior division of the left middle cerebral artery (MCA) territory, with cardioembolism considere
