Management of Anterior Third Ventricular Tumors: A Comprehensive Review and Clinical Experience.

Anterior third ventricular tumors, encompassing colloid cysts, gliomas, craniopharyngiomas, and neurocytomas, are rare intracranial neoplasms that present significant diagnostic and therapeutic challenges due to their deep-seated location and proximity to critical neurovascular structures. This chapter synthesizes current literature on their natural history, diagnostic approaches, and management strategies, integrating our clinical experience with 193 cases treated between 1995 and 2023. We explore observation, microsurgical resection, endoscopic surgery, and stereotactic radiosurgery (SRS), p

Wide opening of the uvulo-tonsillar space to facilitate early intraoperative control of feeding arteries in fourth ventricular tumor surgery: How I do it.

BACKGROUND: The feeding arteries of fourth ventricular tumors often arise from the posterior inferior cerebellar artery (PICA), particularly the tonsillomedullary and telovelotonsillar segments. Achieving early control of these feeders is critical to minimize bleeding. METHOD: We present a surgical technique that facilitates early control of feeders from the telovelotonsillar segment, which courses dorsally to the inferior medullary velum and tela choroidea, by wide and deep opening of the uvulo-tonsillar space before tumor manipulation. CONCLUSION: This technique enables early interruption

Metastatic rosette-forming glioneuronal tumour presenting as isolated primary amenorrhoea.

Primary amenorrhoea is defined as the absence of menarche by 15 years of age in individuals with normal secondary sexual characteristics and is most commonly due to constitutional delay, gonadal dysgenesis or outflow tract abnormalities. We describe an adolescent female who presented with isolated primary amenorrhoea despite normal pubertal onset and progression and no neurological symptoms. Biochemical evaluation demonstrated a central hypothalamic-pituitary pattern of hypogonadism, prompting neuroimaging. MRI revealed a large intraventricular mass causing obstructive hydrocephalus with compr

Third ventricle cavernous malformations: Diagnostic pitfalls and optimal surgical strategies - Scoping review with institutional series.

OBJECTIVE: Third ventricle cerebral cavernous malformations (TV-CCMs) are rare lesions. They are often misdiagnosed due to imaging overlap with other TV pathologies. Moreover, they appear to exhibit more aggressive behaviour than parenchymal CCMs. The goal of this study is to evaluate diagnostic errors, the therapeutic options (conservative management vs surgery) chosen and their related outcomes. METHODS: The PRISMA-ScR (1980-2025), combined with an institutional case series, analyzes diagnostic accuracy, bleeding risk, and surgical outcomes. Literature scoping review (42 studies) and 3 inst

Astroblastoma-like tumor with an EWSR1::BEND2 fusion in the fourth ventricle: a case report and narrative review.

PURPOSE: To report an EWSR1::BEND2 fusion astroblastoma-like tumor arising in the fourth ventricle of a 7-year-old girl and to highlight the diagnostic value of molecular pathology in astroblastoma-like tumors with an atypical immunophenotype. METHODS: Clinical, histopathological, immunohistochemical, and molecular findings of the present case are presented. RESULTS: Despite gross total resection followed by radiotherapy, the patient died 7 months after surgery. Histologically, the tumor showed typical astroblastoma-like morphology but an unusual immunophenotype characterized by complete los

Papillary Tumour of the Pineal Region, in a Child With a Germline PTEN Pathogenic Variant, Arisen in the Fourth Ventricle.

We describe a unique case of papillary tumour of the pineal region (PTPR) arising in the fourth ventricle without any demonstrable anatomical continuity with the pineal region, in a 2-year-old male patient harbouring a germline PTEN pathogenic variant. This case arisen in the fourth ventricle shows epigenetic characteristics consistent with PTPR, suggesting the possibility that PTPR develops outside the pineal region. The tumour was composed of epithelioid-looking papillary structures (A, Haematoxylin and eosin [H&E]), intermingled with more densely cellular areas displaying perivascular pseud

Broadband electronic resonance coherent anti-Stokes/Stokes Raman scattering microscopy.

Coherent Raman imaging (CRI) enables label-free chemical imaging based on intrinsic molecular vibrations, but its applicability is often limited by low sensitivity, hindering the detection of low-abundance biomolecules. Although electronic resonance enhances sensitivity, most resonance CRI implementations rely on narrowband excitation and/or detection, which limits spectral coverage and complicates the differentiation of target molecules from complex backgrounds. Here, we address these challenges by developing broadband electronic resonance coherent anti-Stokes/Stokes Raman scattering (BER-CAR

Intraoperative diagnosis of myxoid glioneuronal tumor guiding safe limited resection: a case report.

Myxoid glioneuronal tumor (MGT) is a newly delineated CNS WHO grade 1 neoplasm genetically characterized by PDGFRA p.K385L/I mutation. We describe a 14-year-old boy presenting with gelastic seizures caused by an intraventricular MGT. Intraoperative frozen section allowed minimal resection, preserving surrounding eloquent structures. Histology showed oligodendrocyte-like cells within myxoid stroma and scattered floating neurons, while molecular analysis revealed the canonical PDGFRA p.K385L mutation. At 13-month follow-up, the residual tumor remained stable without any further neurological dysf

Successful pre-operative endovascular intraventricular meningioma embolization: a systematic review and pediatric case analysis.

BACKGROUND: Intraventricular meningiomas are rare tumors accounting for a small fraction of intracranial meningiomas and are often associated with significant surgical challenges due to their deep location and rich vascular supply. Pre-operative endovascular embolization has been proposed as an adjunct to reduce intraoperative bleeding and facilitate safer resection; however, its role in intraventricular lesions remains poorly defined. METHOD: We present a case of a 9.5-year-old boy with a highly vascular intraventricular meningioma who underwent pre-operative endovascular embolization follow

Fully endoscopic transvallecular transforaminal approach to the fourth ventricle epidermoids - cadaveric anatomy, surgical technique & case series.

Epidermoid cysts in the fourth ventricle are usually treated with either a microscopic or endoscope-assisted microscopic technique. The authors share their experience with four cases of fully endoscopic removal of fourth ventricular epidermoids using the transvallecular transforaminal approach, discussing the technical details and benefits. From June 2021 to July 2022, four patients with fourth ventricle epidermoid cysts underwent surgery at our center using a fully endoscopic transvallecular transforaminal approach (FETTA). Clinical details and surgical outcomes were analyzed retrospectively.

Neuroepithelial Tumor with AAV Integration after Intracisternal Magna Vector Delivery.

Recombinant adeno-associated virus (AAV) vectors are predominantly nonintegrating, but rare genomic integration events have been associated with oncogenesis in neonatal murine models. Here we report a case of a neuroepithelial tumor that developed in a 5-year-old boy with severe mucopolysaccharidosis type I (MPSI, Hurler subtype) 4 years after intracisternal magna administration of AAV serotype 9 gene therapy. The patient underwent successful resection of the primary tumor. Postoperatively, he has continued to have advanced cognitive function for his age, a finding that indicates mitigation of

In silico insights into shared molecular mechanisms and drug repurposing for non‑small cell lung cancer and tuberculosis.

UNLABELLED: Universal trauma-informed screening is increasingly recommended across pediatric settings, yet it remains unclear which forms of adversity most accurately and equitably identify youth at risk for psychopathology. In response, the current study tested whether defining maltreatment by cumulative quality (i.e., number of subtypes) versus cumulative frequency (i.e., number of experiences regardless of subtypes), as well as potential differences within each subtype, yielded more accurate and fair predictions of trauma-related psychological distress. Participants included 839 adolescents

Sporadic Creutzfeldt-Jakob disease presenting with cerebral atrophy following traumatic brain injury mimicking hydrocephalus: A case report.

Sporadic Creutzfeldt-Jakob disease (sCJD) is a rapidly progressive neurodegenerative disorder for which no effective treatment exists, typically resulting in death within 1 year. Nowadays, medical examinations have enhanced the accuracy and sensitivity of disease diagnosis. This study reports a patient who exhibited cognitive deficits, gait instability, and urinary incontinence 8 months after a traumatic brain injury. Admission computed tomography images revealed cerebral ventricle dilation. Further diagnostic evaluations, including magnetic resonance imaging, cerebrospinal fluid analysis for

Electroacupuncture modulates cortical functional connectivity and network topology in healthy adults: an fNIRS study.

Acupuncture has shown clinical benefits in post-stroke rehabilitation, yet its central neural mechanisms remain incompletely understood. In particular, how cortical activation, inter-regional functional connectivity, and brain network topology are modulated during electroacupuncture stimulation has not been systematically characterised. This study aimed to investigate the central effects of verum electroacupuncture (VE) applied to the non-dominant hand in healthy adults. In a randomized, crossover design, functional near-infrared spectroscopy (fNIRS) data were collected from 24 participants du

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or
