Extracellular flux analysis and proteomic profiling indicate ibogaine promotes synaptic plasticity and neural repair by a metabolic preconditioning mechanism.

We have examined how ibogaine may enhance synaptic plasticity and neural repair. Extracellular efflux analysis showed that ibogaine (3, 10 or 30 μM) produced a rapid (<5 min) and concentration-dependent suppression of mitochondrial function and glycolysis in mouse cortical neurons. Cell-based imaging demonstrated this metabolic inhibition was preceded by an elevation of the mitochondrial membrane potential and followed by increased mitochondrial reactive oxygen species levels. Like ibogaine, which blocks potassium channels, mitochondrial Kv1.3 channel (mitoKv1.3) inhibitors produce concordant

Risk Factors Associated with Spinal Cord Stimulation Failure in Patients with Chronic Pain: A Single-Center Retrospective Cohort Study.

Background/Objectives: Spinal cord stimulation (SCS) can provide long-term relief for selected patients with refractory chronic pain, but treatment failure may occur without device explantation. This study examined patient-, clinical-, and device-related factors associated with SCS failure after permanent implantation. Methods: This retrospective, single-center cohort study included 108 adults who underwent permanent SCS implantation at the Centre Hospitalier de l'Université de Montréal between 2007 and 2022. Failure was defined as completed or planned explantation, inadequate pain relief acco

355 nm UV nanosecond laser-induced spinal cord nerve ablation: Spatiotemporal optimization towards high-precision DREZotomy.

Dorsal Root Entry Zoneotomy (DREZotomy) requires extreme surgical precision to eliminate hyperactive nociceptive pathways without compromising adjacent functional spinal tracts. Traditional electrothermal tools often induce unpredictable collateral thermal damage, posing significant risks of postoperative neurological deficits. In this study, we investigated the efficacy and underlying mechanisms of a 355 nm ultraviolet (UV) nanosecond laser for the highly controlled ablation of lipid-rich spinal cord nerve tissue. By systematically evaluating spatiotemporal parameters, we characterized the cr

Manifold view of spinal motor control: A paradigm in motion.

The neural manifold framework has transformed our understanding of cortical motor control, focusing on population dynamics that is constrained to low-dimensional structures in state space, where behaviors correspond to structured trajectories with characteristic topology. Yet the spinal cord, which houses the core circuitry responsible for rhythmic motor output such as walking, has remained largely unexplored from a manifold perspective. For decades, spinal motor function has been interpreted either through the lens of alternating flexor/extensor modules or through genetic characterization of

Differential evolution of neuroinflammatory remodeling programs related to scar formation and regeneration in spinal cord injury: A cross-species multi-omics analysis.

Spinal cord injury in mammals often transitions from an acute inflammatory response to persistent neuroinflammatory and scar-associated remodeling, whereas zebrafish attenuate injury-induced inflammatory programs during regenerative repair. The molecular programs underlying these divergent outcomes remain incompletely understood. In this study, we integrated phase-aligned mouse and zebrafish spinal cord injury datasets using single-cell and single-nucleus transcriptomics, spatial transcriptomics, pathway and regulon analyses, expression-based ligand-receptor co-expression analysis, machine-lea

Medicare and Medicaid Professional Fee Reimbursement for Spinal Cord Stimulation: An Analysis of National and State-level Trends.

BACKGROUND: Spinal cord stimulation (SCS) is an important modality for treating chronic pain in patients who have failed conservative therapy and a substantial portion of the population rely on fee-for-service Medicare or Medicaid for coverage. We aimed to determine physician professional fee reimbursement rates and the ratio of Medicaid to Medicare professional fee reimbursement (MMI) for three common Current Procedural Terminology (CPT) codes related to SCS therapy (63650, 63685, and 63655) across states. Secondarily, we explored trends in utilization and reimbursement of these procedures by

Spinal subependymoma mimicking an intradural extramedullary tumor: illustrative case.

BACKGROUND: Spinal subependymomas are rare, benign tumors that typically arise within the spinal cord and usually appear as intramedullary lesions on MRI. The authors report a rare case of cervical spinal subependymoma with exophytic growth that mimicked an intradural extramedullary tumor on MRI. OBSERVATIONS: A 53-year-old man presented with right-sided paresthesia. MRI revealed a spinal tumor causing marked spinal cord compression. The lesion was isointense on T1-weighted images, slightly hyperintense on T2-weighted images, and showed no gadolinium enhancement. A focal CSF cleft between the

Loss of CRMP4 in astrocytes causes reduced scar formation and functional recovery of motor function after spinal cord injury.

Reactive astrocytes play a central role in shaping the post-injury microenvironment after spinal cord injury (SCI), yet the molecular mechanisms governing their proliferative expansion and contribution to scar architecture remain incompletely defined. Conditional deletion of CRMP4 in astrocytes significantly reduced astrocyte proliferation, altered scar organization, and attenuated the inflammatory milieu surrounding the lesion, including reduced microglial activation. These changes were accompanied by significantly improved locomotor recovery, as assessed by the Basso Mouse Scale (BMS) locomo

Cortex-wide imaging in naturally behaving mice with 1-gram wireless mesoscopes.

Revealing how distributed cortical circuits coordinate behavior in freely moving animals remains a central challenge in systems neuroscience, constrained by limited fields of view, low throughput, and the tethering required by current miniaturized microscopes1-4. Here, we present Airscope, a wireless mesoscope delivering 4 μm lateral resolution across a 6 mm field of view at 12.5 Hz, weighing 1.0 gram. Mechanical competence of proximal tibia in persons with spinal cord injury: development of a computational framework to quantify torsional and compressive stiffness and strength.

This study developed a framework to quantify the mechanical competence of the proximal tibia in spinal cord injury (SCI) and able-bodied (AB) participants. We created subject-specific finite element models of 53 proximal tibias (22 SCI, 5 AB) and quantified two torsional (stiffness, Kt and ultimate strength, Tult) and two compressive (stiffness, Kc and ultimate strength, Cult) metrics. We report the distributions of Kt, Tult, Kc, and Cult, and a minimum number of metrics required to quantify the mechanical competence of the proximal tibia. This work provides a foundation for confidence interva

Variable task difficulty with augmented sensory feedback for motor training in virtual reality.

INTRODUCTION: This study examined how motor task difficulty and augmented sensory feedback (ASF) modality influence task performance, physiological responses, and perceptual experience during virtual reality (VR)-based upper-limb motor training. Building on related brace- and VR-based ASF platforms previously examined in neurotypical and spinal cord injury cohorts, the long-term goal is to support neurorehabilitation-oriented adaptive VR training; however, the present study used neurotypical participants as an initial validation step. METHODS: Fourteen right-handed neurotypical participants u

Glis1 regulates metabolic programming by promoting H3K18 lactylation to alleviate neuronal senescence following spinal cord injury.

Neuronal senescence is a significant factor impeding spinal cord injury (SCI) repair. Research indicates that Glis1, a Kruppel-like zinc finger transcription factor, is pivotal in reprogramming senescent cells and enhancing genomic stability via lactylation. Histone lactylation, an epigenetic modification using lactate as a substrate, becomes prominent post-SCI due to increased neuronal lactate levels. However, the mechanisms linking neuronal senescence and histone lactylation following SCI remain poorly understood. This study elucidates the roles of Glis1, H3K18 lactylation, and neuronal sene
