Diagnostic and Prognostic Value of Sonographic Optic Nerve Sheath Diameter in Patients with Ischemic Stroke or Hypertensive Intracerebral Hemorrhage.

BACKGROUND: Elevated intracranial pressure (ICP) is a critical complication in nontraumatic brain injury, yet invasive monitoring carries significant risks, and noninvasive alternatives remain undervalidated. Optic nerve sheath diameter (ONSD) ultrasonography represents a promising bedside technique, but data in nontraumatic populations are limited. This study evaluated the diagnostic and prognostic value of ONSD in neurocritical patients with ischemic stroke or hypertensive intracerebral hemorrhage. METHODS: This prospective observational cohort study enrolled 100 neurocritical intensive car

Mapping of pikachurin heparan sulfate modification to a region dispensable for synaptic localization in photoreceptors.

Rod and cone photoreceptors (PRs) are specialized neurons that detect light, then transmit information to bipolar cells at synapses in the outer plexiform layer of the retina. Pikachurin (PIKA) is a multi-domain heparan sulfate (HS) proteoglycan expressed in PRs. It is secreted into the synaptic cleft, where it binds to presynaptic dystroglycan, postsynaptic orphan receptor GPR179, and postsynaptic adhesion molecule LRRTM4. Knockout of PIKA results in ultrastructural and functional defects at rod synapses. However, the role of HS in PIKA function at synapses is unknown. Using a panel of deleti

Optic nerve sheath decompression-related perioptic pseudocysts: histo-biochemical features.

Two patients (Female; 9 and 32 years in age) with papilledema secondary to venous sinus thrombosis underwent bilateral medial transconjunctival optic nerve sheath decompression for visual loss. Postoperative imaging revealed perioptic cystic lesions in 3 eyes associated with persistent papilledema and increased retinal nerve fiber layer thickness. For treatment, the cystic lesions were excised through a transconjunctival approach after aspiration of their fluid content. The cystic fluid, analyzed by gel electrophoresis, tested positive for beta-2 transferrin, confirming its cerebrospinal fluid

Prenatal tobacco exposure and visual disorders in children.

Maternal smoking during pregnancy is associated with adverse fetal and perinatal outcomes and may also affect visual system development in offspring. Increasing evidence suggests that prenatal tobacco exposure may also be associated with abnormalities of visual system development in children. However, the extent to which these associations reflect direct effects of tobacco exposure versus indirect pathways mediated by adverse perinatal outcomes remains uncertain. This narrative review summarizes the available evidence regarding the association between prenatal tobacco exposure and visual abnor

Therapeutic extracellular vesicles isolated from mesenchymal stem cells: properties, isolation, characterization, and applications in optic neuropathies.

Extracellular vesicles (EVs) are promising cell-free therapeutic agents because of their low immunogenicity, excellent biocompatibility, and intrinsic capacity for intercellular communication. Of these, mesenchymal stem cell-derived EVs (MSC-EVs) have attracted considerable interest for their potential in treating optic neuropathies; however, their clinical translation remains limited by standardized manufacturing, quality evaluation, and mechanistic understanding. This review summarizes key advances in therapeutic EV development, including MSC culture, EV production, isolation, and characteri

Caveolin-1 defines astrocyte mechanobiology in the glial lamina of the optic nerve head.

The optic nerve head is the principal site of retinal ganglion cell axonal injury in primary open angle glaucoma, where astrocyte mechanobiology and extracellular matrix remodeling are thought to play critical roles in disease progression. Caveolae, which are mechanosensitive plasma membrane invaginations formed by Caveolin and Cavin proteins, are key regulators of cellular responses to mechanical stress, yet their involvement in optic nerve head pathophysiology has not been defined. Here, we characterize the presence, regulation, and mechanobiological relevance of Caveolin and Cavin proteins

Trefoil Family Factor 1 Expression in the Indian Cohort of Treatment-Naïve Retinoblastoma.

INTRODUCTION: Retinoblastoma (RB) is the most common intraocular malignancy of childhood. Advanced RB has a poor prognosis and is associated with high-risk histopathological factors (HRPFs), metastasis, and relapse. Trefoil family factor 1 (TFF1), a secreted peptide previously associated with adverse clinicopathological features in RB, has emerged as a potential biomarker of aggressive disease; however, its clinical significance in Indian patients remains poorly understood. This study evaluated TFF1 expression in correlation with HRPFs, metastasis, and survival outcomes. METHODS: This study i

Systemic mitochondrial injection protects against methanol-induced optic neuropathy through restoration of mitochondrial function and attenuation of oxidative stress.

Methanol poisoning can cause severe and irreversible optic neuropathy, for which effective neuroprotective strategies remain limited. Mitochondrial dysfunction is important contributors to methanol-induced neuronal injury. Therefore, this study evaluated whether systemic mitochondrial injection could attenuate methanol-induced optic nerve injury. Adult rats were randomly assigned to control, methanol, methanol + single mitochondrial injection, and methanol + double mitochondrial injection groups. Following methanol intoxication, freshly isolated functional mitochondria (250 µg/kg) were adminis

Research Progress on Intraocular Pressure-lowering Drugs, Commonly Used Clinical Adjuvants, and Emerging Neuroprotective Agents for Glaucomatous Optic Neuropathy.

Glaucomatous optic neuropathy (GON) is the leading cause of irreversible blindness worldwide, characterized by progressive retinal ganglion cell (RGC) loss. Current clinical treatment relies primarily on IOP-lowering drugs, but a subset of patients continues to experience progressive optic nerve damage. Adjuvant therapies such as antioxidants and calcium channel blockers have limited efficacy. Emerging neuroprotective strategies including NMDA receptor antagonists, neurotrophic factors, and gene therapy remain in preclinical or early-stage trials. This review systematically summarizes the mech

MOG Antibody Status Is Associated With Distinct Clinical Profiles and Treatment Responses in Chronic Relapsing Inflammatory Optic Neuropathy.

BACKGROUND AND OBJECTIVES: Chronic relapsing inflammatory optic neuropathy (CRION) is a steroid-dependent form of optic neuritis with incompletely understood pathophysiology. The identification of myelin oligodendrocyte glycoprotein antibodies (MOG-IgG) in a substantial patient subset has challenged the diagnostic and therapeutic management. The aim of this study was to investigate clinical profiles and treatment outcomes of patients with CRION, comparing MOG-IgG-positive (MOG+) and seronegative (MOG-) subgroups. METHODS: Patients from 6 European tertiary centers fulfilling diagnostic criteri

Comparison of Ultrasound and Computed Tomography Optic Nerve Sheath Diameter for Intracranial Pressure Assessment in Pediatric Neurocritical Care.

BACKGROUND: Noninvasive intracranial pressure (ICP) assessment remains challenging in pediatric neurocritical care. Optic nerve sheath diameter (ONSD) measured by ultrasound (ONSD-US) and computed tomography (ONSD-CT) has been proposed as a surrogate marker of ICP, although comparative pediatric data remain limited. METHODS: Prospective observational study conducted in a tertiary pediatric intensive care unit. Patients requiring invasive ICP monitoring were consecutively included. ONSD-US measurements were obtained simultaneously with invasive ICP measurements and, for patients undergoing CT,
