Approach to the pregnant patient with prolactinoma.

Prolactinomas account for approximately half of the pituitary adenomas and predominantly affect women of reproductive age. Hyperprolactinemia commonly causes infertility by suppressing hypothalamic gonadotropin-releasing hormone (GnRH) secretion and impairing gonadal function. Because dopamine agonist therapy can rapidly restore fertility, clinicians caring for women with prolactinomas should be qualified to manage problems that may arise during pregnancy. Pregnancy is characterized by physiological enlargement of the anterior pituitary gland and progressive hyperprolactinemia, primarily drive

Medullary thyroid carcinoma with pheochromocytoma and cystic parathyroid degeneration in a ret-negative patient: a case report.

INTRODUCTION: Multiple endocrine neoplasia type 2A (MEN2A) is a rare autosomal dominant hereditary cancer syndrome, predominantly caused by mutations of the RET proto-oncogene on chromosome 10q11.2. It typically presents with medullary thyroid carcinoma (MTC), pheochromocytoma, and primary hyperparathyroidism. Cases with a MEN2A phenotype but negative RET testing are uncommon, posing challenges for diagnosis and patient management. CASE PRESENTATION: We report the case of a 63-year-old female who was diagnosed with MTC and pheochromocytoma. and cystic parathyroid gland. Genetic workup was neg

The use of intraoperative parathyroid hormone assay in predicting the success and reduce morbidity of targeted minimal invasive parathyroidectomy for primary hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism (PHPT) is caused by overproduction of parathyroid hormone (PTH) by a hyperfunctioning parathyroid gland (most commonly a parathyroid adenoma, PTA) or glands. Historically, the most commonly described surgical approach has been bilateral neck exploration (BNE). This study focuses on the outcomes of targeted minimally invasive parathyroidectomy [unilateral neck exploration (UNE), focused exploration (FE)-including endoscope and/or exoscope-assisted parathyroidectomy]. Intraoperative measurement of parathyroid hormone levels (ioPTH) is generally considered

Subcutaneous translocation of residual thyroid tissue: a novel functional-preserving technique for partial thyroidectomy with 1-year follow-up retrospective case series.

BACKGROUND: Benign thyroid nodules frequently require surgical intervention. Balancing thyroid‑function preservation and re‑operation‑related neuro‑parathyroidal risk represents an unmet clinical challenge. This study aimed to investigate the feasibility of translocating residual normal thyroid tissue into the subcutaneous space after resection of benign thyroid tumors. Conventional thyroid resection commonly compromises native thyroid function and leads to postoperative hypothyroidism. Residual thyroid tissue retained in the neck complicates surveillance and increases risks to the recurrent l

Plasma adrenocorticotropin concentrations vary with assay used in horses with and without pituitary pars intermedia dysfunction.

Plasma adrenocorticotropin (ACTH) concentration is measured to support a diagnosis of pituitary pars intermedia dysfunction (PPID). However, baseline ACTH and ACTH concentrations after IV administration of thyrotropin releasing hormone (TRH) vary with season and assay used. To compare two ACTH assays (Scantibodies immunoradiometric assay [IRMA] and Immulite 2000Xpi chemiluminescent immunometric assay [CIA]) for diagnosis of PPID, ACTH was measured before and 10 minutes after administration of TRH in three groups of horses: 1) four young horses (6 ± 3 years, no hypertrichosis and a pituitary gl

Partial or full preservation of the pituitary stalk in craniopharyngioma surgery: improving endocrinological outcomes without an increased risk of recurrence.

OBJECTIVE: The risk-benefit balance of pituitary stalk (PS) preservation during craniopharyngioma (CP) surgery remains insufficiently investigated. Although preservation requires meticulous microsurgical dissection, frequent invasion of the PS by the CP also raises the question of the feasibility of preservation. Herein, the authors assessed the impact of preserving the PS in a large consecutive series of adult patients who underwent endoscopic endonasal surgery for CP. METHODS: The authors performed a retrospective single-center study of adult, surgery-naive patients with supradiaphragmatic

Probe-Based Near-Infrared Autofluorescence for Parathyroid Identification During Thyroid Surgery: Clinical Outcomes, Surgeon Confidence, and Autofluorescence Thresholds.

BACKGROUND: We evaluated experience with probe-based near-infrared autofluorescence (NIRAF) system (PTeye) in parathyroid gland (PTG) identification during total thyroidectomy. METHODS: In this retrospective study, patients undergoing total thyroidectomy using PTeye (2020-2022; n = 84) were compared with a historical visual-identification cohort (2018-2019; n = 103). RESULTS: PTG identification rates were comparable, although 42% of PTGs detected with PTeye were initially visualized with low or moderate confidence. Significantly fewer patients in the PTeye cohort had PTGs incidentally identi

The Effect of Prophylactic Protection of Inferior Parathyroid Gland Vein Blood Supply on Reducing Postoperative Hypoparathyroidism.

BACKGROUND: The objective of this study was to verify that a strategy involving prophylactic exposure of the inferior parathyroid glands and preservation of their venous blood flow-achieved through early identification and protection of their venous drainage-can enhance the success rate of maintaining glandular perfusion and effectively reduce the incidence of postoperative hypoparathyroidism. METHODS: This study included 600 patients who underwent total thyroidectomy with concomitant bilateral central neck dissection between November 2019 and December 2025. Patients were divided into two gro

Primary hyperparathyroidism in a 4-month-old German Shepherd mixed breed dog.

A 4-month-old female-intact German Shepherd mixed breed dog was evaluated for polyuria, polydipsia, oral discomfort, tachypnea, and pyrexia. Blood testing documented total and ionized hypercalcemia with an increased parathyroid hormone concentration. Computed tomography identified bone abnormalities of the mandibule and mild, diffuse pulmonary changes. Ultrasonography of the neck documented one mildly enlarged parathyroid gland that was surgically removed. Histopathology indicated diffuse chief cell hyperplasia. The ionized calcium concentration normalized within 7 days postoperatively.

Validation of a point-of-care assay for simultaneous quantification of equine adrenocorticotropic hormone and corticotropin-like intermediate peptide.

BACKGROUND: Plasma adrenocorticotropic hormone (ACTH) concentrations are used to diagnose equine pituitary pars intermedia dysfunction (PPID). The reference chemiluminescent immunoassay for immunoreactive ACTH (iACTH) cross-reacts with corticotropin-like intermediate peptide (CLIP), which is also abnormally high in PPID. HYPOTHESIS/OBJECTIVES: To compare the performance of a novel point-of-care (POC) immunoassay that concurrently measures ACTH and CLIP against the reference iACTH assay. ANIMALS: Twenty-eight university-owned horses (13 PPID, 15 non-PPID). METHODS: Prospective observational

Chronic kidney disease as a potential risk factor for pituitary hormone dysregulation: a review of the underlying mechanisms.

Chronic Kidney Disease (CKD) is a condition of abnormal kidney structure or function, pathologically characterized by chronic inflammation, tubular atrophy, vascular rarefaction, glomerulosclerosis, and interstitial fibrosis. CKD potentially represents a risk factor for several disease conditions, such as cardiovascular disease, mineral bone disorder, and electrolyte imbalance. Hormone dysregulation within the pituitary axis is yet another condition that CKD influences. Notably, CKD dysregulates several anterior pituitary hormones, either by directly influencing hypothalamic-pituitary secretio

Papillary Tumor of the Pineal Region Presenting With Obstructive Hydrocephalus: A Diagnostic and Management Challenge.

We report a 45-year-old woman with a papillary tumor of the pineal region (PTPR), a neuroepithelial neoplasm of the pineal gland, who presented with a one-year history of headaches, visual disturbance, and gait impairment and was found to have a pineal mass with obstructive hydrocephalus. She underwent endoscopic third ventriculostomy (ETV) with biopsy; the initial radiologic impression favored pineocytoma. Immunohistochemistry was nondiagnostic on the limited sample, and the diagnosis of PTPR was established only after review at two academic neuropathology centers. DNA methylation profiling w

Case Report: Pituitary apoplexy temporally associated with acute alcohol intoxication, presenting as complete oculomotor nerve palsy in an SF-1 lineage gonadotroph PitNET.

Pituitary apoplexy (PA) is an acute, potentially life-threatening neuroendocrine emergency caused by hemorrhage or infarction within the pituitary gland, most often arising in a previously unrecognized pituitary neuroendocrine tumor (PitNET). Reported precipitants include surgery, anticoagulation, dynamic pituitary testing, hypertension, and head trauma; alcohol is rarely implicated. We report a 61-year-old man with long-standing hypertension, type 2 diabetes mellitus and coronary artery disease (on aspirin after coronary stenting) who developed sudden thunderclap headache and complete right o

Bilateral cranial polyneuropathy associated with an intracranial mass in a cat with concurrent tonsillar lymphoma.

A 14-year-old spayed female Siamese cat was presented with signs consistent with a rapidly progressive cranial polyneuropathy. Magnetic resonance imaging of the brain revealed an extra-axial mass at the calvarial floor, extending caudally to involve bilateral trigeminal nerves and rostrally to involve the pituitary gland and optic chiasm. Additional findings included contrast-enhancing content within the bilateral tympanic bullae, mild mandibular and medial retropharyngeal lymphadenopathy, and a left tonsillar mass. Cytologic and histopathologic findings confirmed tonsillar lymphoma. The concu

Step-by-Step Cadaveric Dissection of the Carotid Region: Anatomical Landmarks, Surgical Exposure, and Clinical Applications.

The carotid region represents critical neurovascular spaces of the anterolateral neck, where a precise three-dimensional understanding of vascular, neural, and fascial relationships is essential for safe cervical surgery. This article describes a standardized, step-by-step cadaveric dissection protocol linking consistent anatomical landmarks to their surgical relevance. Unilateral dissections were performed on a formalin-preserved adult human cadaver using standard gross anatomical techniques. Specimens were dissected from superficial to deep layers according to a six-step sequence based on cl

Pineal melatonin modulates responsiveness to exogenous melatonin via the IFN-γ-CXCL9/10-CXCR3 axis in gastric cancer.

BACKGROUND: Melatonin is a pineal-derived neuroendocrine hormone with pleiotropic actions on tumor biology and immune regulation. Although exogenous melatonin has been reported to exert antitumor and immunomodulatory effects, whether these actions depend on an intact endogenous pineal melatonin background remains unclear. In particular, the role of endogenous melatonin in conditioning chemokine-mediated antitumor effects has not been defined. METHODS: A male mouse gastric cancer model was used to investigate the interplay between endogenous and exogenous melatonin in tumor growth, immune infi

Improving Nursing Knowledge in the Care of Patients Undergoing Pituitary Surgery: A Quality Improvement Project.

Background: Surgical treatment of pituitary adenomas places patients at substantial risk for life-threatening postoperative complications due to the pituitary gland's unique anatomical position and endocrine function. When preoperative patient education is insufficient, early signs of complications such as excessive bleeding, cerebrospinal fluid leaks, and diabetes insipidus may go unrecognized, resulting in delayed intervention. Objective: To support consistent and high-quality preoperative education across the continuum of care, this quality improvement project aimed to increase perioperativ
