To report a case of IgG4-related disease (IgG4-RD) involving the eyes, pituitary gland, dura mater and temporal region simultaneously. The patient was a 55-year-old middle-aged male with a protracted course of more than 13 years. The main clinical manifestations included swelling of the left eye accompanied by exophthalmos, which gradually worsened, and progressive visual deterioration. Imaging examinations revealed multiple pseudotumorous masses in the bilateral extraconal spaces (superior to the bilateral superior rectus muscles and inferior to the bilateral inferior rectus muscles) and the

Disruption of the natural light regimen is considered a key pathogenetic factor in the development of desynchronosis and associated diseases. The pineal gland, as a neuroendocrine organ, is a central component of the circadian system and the primary target of the pathogenic effects of disrupted lighting. However, it remains unclear whether the mechanisms of pineal damage differ between different types of light stress. In this study, 120 male Wistar rats underwent a comprehensive analysis of the morphofunctional state of the pineal gland under two lighting regimens: constant lighting and interm

Symmetrical, bilateral intracranial calcifications, commonly involving the basal ganglia, constitute the characteristic features of Fahr's syndrome, which is an uncommon neurometabolic condition. Its incidence in conjunction with congenital parathyroid agenesis, however, has rarely been reported. The present case emphasises how crucial it is to identify the metabolic origins of intracranial calcification and differentiate Fahr's syndrome from primary familial brain calcification to prevent repeated neurological consequences through adequate therapy. A 26-year-old man with congenital absence of

Moyamoya disease (MMD) is a progressive cerebrovascular condition marked by carotid narrowing and collateral vessel formation. Patients are often diagnosed after a stroke, but here we describe a case of galactorrhea as the presenting symptom of MMD. A 30-year-old gravida 1, para 0 woman noted milky, nonbloody discharge leaking from both breasts after manual compression during sexual intercourse. Prolactin was mildly elevated at 29.4 ng/mL (SI [International System of Units]: 29.4 µg/L) (reference range, 3-23.1 ng/mL [SI: 3-23.1 µg/L]). Magnetic resonance imaging of the pituitary gland showed n

Prolactinomas account for approximately half of the pituitary adenomas and predominantly affect women of reproductive age. Hyperprolactinemia commonly causes infertility by suppressing hypothalamic gonadotropin-releasing hormone (GnRH) secretion and impairing gonadal function. Because dopamine agonist therapy can rapidly restore fertility, clinicians caring for women with prolactinomas should be qualified to manage problems that may arise during pregnancy. Pregnancy is characterized by physiological enlargement of the anterior pituitary gland and progressive hyperprolactinemia, primarily drive

INTRODUCTION: Multiple endocrine neoplasia type 2A (MEN2A) is a rare autosomal dominant hereditary cancer syndrome, predominantly caused by mutations of the RET proto-oncogene on chromosome 10q11.2. It typically presents with medullary thyroid carcinoma (MTC), pheochromocytoma, and primary hyperparathyroidism. Cases with a MEN2A phenotype but negative RET testing are uncommon, posing challenges for diagnosis and patient management.
CASE PRESENTATION: We report the case of a 63-year-old female who was diagnosed with MTC and pheochromocytoma. and cystic parathyroid gland. Genetic workup was neg

INTRODUCTION AND IMPORTANCE: The incidence rate of paraganglioma stands at approximately 1 case per 300 000 person-years. Symptoms resembling anxiety and panic attacks are prevalent within the general populace, rendering the identification of individuals afflicted with pheochromocytoma or paraganglioma a formidable task for healthcare practitioners.
CASE PRESENTATION: A 43-year-old presented with ischemic signs that turned into necrosis at his left foot due to diffused thrombosis in his main arteries, furthermore Echodiagram indicated the presence of left ventricular thrombosis, through which

BACKGROUND: Primary hyperparathyroidism (PHPT) is caused by overproduction of parathyroid hormone (PTH) by a hyperfunctioning parathyroid gland (most commonly a parathyroid adenoma, PTA) or glands. Historically, the most commonly described surgical approach has been bilateral neck exploration (BNE). This study focuses on the outcomes of targeted minimally invasive parathyroidectomy [unilateral neck exploration (UNE), focused exploration (FE)-including endoscope and/or exoscope-assisted parathyroidectomy]. Intraoperative measurement of parathyroid hormone levels (ioPTH) is generally considered

BACKGROUND: Benign thyroid nodules frequently require surgical intervention. Balancing thyroid‑function preservation and re‑operation‑related neuro‑parathyroidal risk represents an unmet clinical challenge. This study aimed to investigate the feasibility of translocating residual normal thyroid tissue into the subcutaneous space after resection of benign thyroid tumors. Conventional thyroid resection commonly compromises native thyroid function and leads to postoperative hypothyroidism. Residual thyroid tissue retained in the neck complicates surveillance and increases risks to the recurrent l

Plasma adrenocorticotropin (ACTH) concentration is measured to support a diagnosis of pituitary pars intermedia dysfunction (PPID). However, baseline ACTH and ACTH concentrations after IV administration of thyrotropin releasing hormone (TRH) vary with season and assay used. To compare two ACTH assays (Scantibodies immunoradiometric assay [IRMA] and Immulite 2000Xpi chemiluminescent immunometric assay [CIA]) for diagnosis of PPID, ACTH was measured before and 10 minutes after administration of TRH in three groups of horses: 1) four young horses (6 ± 3 years, no hypertrichosis and a pituitary gl

OBJECTIVE: The risk-benefit balance of pituitary stalk (PS) preservation during craniopharyngioma (CP) surgery remains insufficiently investigated. Although preservation requires meticulous microsurgical dissection, frequent invasion of the PS by the CP also raises the question of the feasibility of preservation. Herein, the authors assessed the impact of preserving the PS in a large consecutive series of adult patients who underwent endoscopic endonasal surgery for CP.
METHODS: The authors performed a retrospective single-center study of adult, surgery-naive patients with supradiaphragmatic

BACKGROUND: We evaluated experience with probe-based near-infrared autofluorescence (NIRAF) system (PTeye) in parathyroid gland (PTG) identification during total thyroidectomy.
METHODS: In this retrospective study, patients undergoing total thyroidectomy using PTeye (2020-2022; n = 84) were compared with a historical visual-identification cohort (2018-2019; n = 103).
RESULTS: PTG identification rates were comparable, although 42% of PTGs detected with PTeye were initially visualized with low or moderate confidence. Significantly fewer patients in the PTeye cohort had PTGs incidentally identi

Deficiencies in oxytocin are associated with neurodevelopmental and behavioral disorders, and the oxytocin gene (Oxt) can be regulated by thyroid hormone. We have previously shown that a systemic excess of thyroid hormone during development causes deficits in oxytocin that are associated with major abnormalities in mood-related and social behaviors. To determine whether the disruption in oxytocin physiology is responsible for these aberrant behaviors we targeted thyroid hormone excess to oxytocin neurons using a mouse model of conditional deficiency of the type 3 deiodinase (DIO3), the neurona

BACKGROUND: The objective of this study was to verify that a strategy involving prophylactic exposure of the inferior parathyroid glands and preservation of their venous blood flow-achieved through early identification and protection of their venous drainage-can enhance the success rate of maintaining glandular perfusion and effectively reduce the incidence of postoperative hypoparathyroidism.
METHODS: This study included 600 patients who underwent total thyroidectomy with concomitant bilateral central neck dissection between November 2019 and December 2025. Patients were divided into two gro

A 4-month-old female-intact German Shepherd mixed breed dog was evaluated for polyuria, polydipsia, oral discomfort, tachypnea, and pyrexia. Blood testing documented total and ionized hypercalcemia with an increased parathyroid hormone concentration. Computed tomography identified bone abnormalities of the mandibule and mild, diffuse pulmonary changes. Ultrasonography of the neck documented one mildly enlarged parathyroid gland that was surgically removed. Histopathology indicated diffuse chief cell hyperplasia. The ionized calcium concentration normalized within 7 days postoperatively.

BACKGROUND: Plasma adrenocorticotropic hormone (ACTH) concentrations are used to diagnose equine pituitary pars intermedia dysfunction (PPID). The reference chemiluminescent immunoassay for immunoreactive ACTH (iACTH) cross-reacts with corticotropin-like intermediate peptide (CLIP), which is also abnormally high in PPID.
HYPOTHESIS/OBJECTIVES: To compare the performance of a novel point-of-care (POC) immunoassay that concurrently measures ACTH and CLIP against the reference iACTH assay.
ANIMALS: Twenty-eight university-owned horses (13 PPID, 15 non-PPID).
METHODS: Prospective observational

Chronic Kidney Disease (CKD) is a condition of abnormal kidney structure or function, pathologically characterized by chronic inflammation, tubular atrophy, vascular rarefaction, glomerulosclerosis, and interstitial fibrosis. CKD potentially represents a risk factor for several disease conditions, such as cardiovascular disease, mineral bone disorder, and electrolyte imbalance. Hormone dysregulation within the pituitary axis is yet another condition that CKD influences. Notably, CKD dysregulates several anterior pituitary hormones, either by directly influencing hypothalamic-pituitary secretio

We report a 45-year-old woman with a papillary tumor of the pineal region (PTPR), a neuroepithelial neoplasm of the pineal gland, who presented with a one-year history of headaches, visual disturbance, and gait impairment and was found to have a pineal mass with obstructive hydrocephalus. She underwent endoscopic third ventriculostomy (ETV) with biopsy; the initial radiologic impression favored pineocytoma. Immunohistochemistry was nondiagnostic on the limited sample, and the diagnosis of PTPR was established only after review at two academic neuropathology centers. DNA methylation profiling w

Pituitary apoplexy (PA) is an acute, potentially life-threatening neuroendocrine emergency caused by hemorrhage or infarction within the pituitary gland, most often arising in a previously unrecognized pituitary neuroendocrine tumor (PitNET). Reported precipitants include surgery, anticoagulation, dynamic pituitary testing, hypertension, and head trauma; alcohol is rarely implicated. We report a 61-year-old man with long-standing hypertension, type 2 diabetes mellitus and coronary artery disease (on aspirin after coronary stenting) who developed sudden thunderclap headache and complete right o
