Association of PCSK9 inhibitors versus statins with cancer incidence: a target trial emulation.

BACKGROUND: The relationship between PCSK9 inhibitors (PCSK9i) and cancer incidence remains poorly understood. This study is to investigate the association between PCSK9i use and cancer risk compared to statin therapy. METHODS: We conducted a retrospective cohort study using target trial emulation principles on de-identified electronic health records from the TriNetX network (July 2015 – June 2023). Adults with indications for lipid-lowering therapy who initiated PCSK9i (alirocumab, evolocumab, or inclisiran) or statins (no prior statin use) were included. New PCSK9i users were propensity-scor

Pastoral Care Provision for Medical Students: What Is the Role of the Clinical Teaching Fellow?

BACKGROUND: The clinical teaching fellow (CTF) role is often taken by resident doctors between training programmes to develop skills within medical education. This project explored CTF experiences of pastoral care provision at one medical school in the United Kingdom (UK), specifically focusing on their own role and responsibilities to inform practical support strategies for doctors as future medical educators. METHODS: This study adopted a qualitative, hermeneutic phenomenological approach. Five focus groups were conducted at different hospital sites where medical students undertake clinical

Maxillary chondrosarcoma: a case report.

INTRODUCTION: Chondrosarcoma is a malignant tumor of cartilaginous origin, which is rarely seen in the maxillofacial region, especially in young individuals. The most common clinical manifestation is painless swelling of the bone. The anterior maxilla and the posterior mandible are considered the most common sites of occurrence, with a slightly higher incidence reported in female compared with male individuals. Chondrosarcomas typically present in older individuals, most commonly during the fourth to sixth decades of life. The most widely accepted treatment approach for this tumor is radical s

Clinical and 18F-FDG PET/CT Imaging Characteristics of Post-radiotherapy Sacral Insufficiency Fractures in Cervical Cancer Patients.

INTRODUCTION: Sacral Insufficiency Fractures (SIFs) are a common yet frequently misdiagnosed late complication following pelvic radiotherapy for cervical cancer. Accurate differentiation from bone metastases is crucial to avoid unnecessary interventions. While MRI is sensitive for early marrow edema, integrated 18F-FDG PET/CT offers a unique simultaneous assessment of bone metabolism and systemic tumor status. However, comprehensive studies detailing the qualitative and quantitative PET/CT characteristics of post-radiotherapy SIFs are lacking. This study aims to systematically define these fea

Real-time PCR-based intraoperative molecular boundary diagnosis of corticotroph pituitary neuroendocrine tumors.

BACKGROUND: Corticotroph pituitary neuroendocrine tumors (PitNETs) causing Cushing's disease require complete surgical resection to achieve endocrinological remission. Although endoscopic transsphenoidal surgery is the standard first-line treatment, accurate intraoperative delineation of tumor boundaries remains challenging because the pseudocapsule is often thin or ill-defined and tumor cells may infiltrate adjacent tissues. Given that intraoperative histopathology and conventional molecular methods are limited by time constraints, rapid molecular detection of tumor-specific mutations may ser

The stress-sensing MARCH5 axis governs blood cancer apoptotic resilience via tractable protein-protein interactions.

The mitochondrial E3 ligase MARCH5 has consistently emerged as a dependency in unbiased screens in acute myeloid leukemia and myeloma, yet the underpinning mechanism remains ill-defined. Here, we show that MARCH5 cooperates with UBE2J2 and MFN2, forming a stress-sensing complex at mitochondria-ER contact sites (MERCS) that restrains apoptosis in response to diverse organellar damage signals. Loss of MARCH5 potently sensitizes diverse blood cancer cell lines to BCL-2 and BCL-XL inhibition and compromises stress tolerance. By contrast, non-hematopoietic cell lines exhibit a phenotype largely res

Masked divergence in cancer mortality following COVID-19 disruption.

BACKGROUND: While overall cancer mortality in the United States has declined steadily for decades, systemic healthcare disruptions during the COVID-19 pandemic-including delays in screening, diagnosis, and treatment-may have created heterogeneous impacts across different malignancy types that are not visible in aggregate data. OBJECTIVE: To determine whether site-specific cancer mortality during the COVID-19 pandemic (2020-2023) deviated from expected trajectories based on established pre-pandemic trends. METHODS: This population-based analysis utilized U.S. National Vital Statistics System

Necrotizing Soft Tissue Infection at a Self-Administered Subcutaneous Etanercept Injection Site in an Immunosuppressed Patient With Rheumatoid Arthritis: A Case Report.

Necrotizing soft tissue infection (NSTI) is rapidly progressive and can be fatal; outcomes depend on early recognition and prompt surgical debridement. In immunocompromised patients, local findings and inflammatory responses may be atypical, increasing the risk of delayed diagnosis. Although injections breach the skin barrier, NSTI originating at injection sites can be overlooked as a benign injection-site reaction; procedure-associated NSTI has been reported to have poor outcomes. A woman in her 60s with rheumatoid arthritis was receiving methotrexate, tacrolimus, and prednisolone and had sta

Estrogen influences class-switched memory B cell frequency only in humans with two X chromosomes.

Sex differences in immunity are well-documented, though mechanisms underpinning these differences remain ill-defined. Here, in a human-only ex vivo study, we demonstrate that postpubertal cisgender females have higher levels of CD19+CD27+IgD- class-switched memory B cells compared with age-matched cisgender males. This increase is only observed after puberty and before menopause, suggesting a strong influence for sex hormones. Accordingly, B cells express high levels of estrogen receptor 2 (ESR2), and class-switch-regulating genes are enriched for ESR2-binding sites. In a gender-diverse cohort

Risk factors for mortality and implications on therapy for Stenotrophomonas maltophilia bacteraemia.

BACKGROUND: Stenotrophomonas maltophilia is an important nosocomial pathogen. Bacteraemia is associated with significant morbidity, despite antibiotic therapy. Optimal treatment strategies for Stenotrophomonas maltophilia bacteraemia remain ill-defined. Thus, we retrospectively examined the clinical presentation, microbiological characteristics, treatment options to identify risk factors for mortality. METHODS: We performed a retrospective single-centre analysis of Stenotrophomonas maltophilia bacteraemia from 1 Jan 2012-30 Jun 2024. Data on the clinical presentation, source of infection, mic

Retroperitoneal hibernoma mimicking a metastatic germ cell tumor: a case report and review of the literature.

BACKGROUND: Hibernomas are very rare fat-derived benign tumors most commonly developed from residual brown fat sited in the interscapular region, the neck, the axillary, and the mediastinal areas. Retroperitoneal location is extremely rare and is usually confused with other malignancies such as liposarcoma and teratoma. We herein described an additional case of retroperitoneal hibernoma and discussed the challenging diagnosis. CASE PRESENTATION: A 32-year-old White North African man presented with a left-sided testicular tumor mass. The testicular ultrasonography showed an ill-defined hypoech

Mediastinal Teratoma With Somatic-Type Adenocarcinoma: An Enigmatic Discovery by Fine Needle Aspiration Cytology.

Mediastinal masses often present acutely as medical emergencies, necessitating prompt and accurate diagnosis. Imaging-guided fine needle aspiration cytology (FNAC) plays a pivotal role in rapidly identifying rare mediastinal tumours and differentiating them from other potential aetiologies, enabling timely intervention. Primary mediastinal germ cell tumours (PMGCTs) constitute approximately 15% of adult mediastinal neoplasms. The development of somatic-type malignancies, particularly colonic-type adenocarcinoma, in PMGCTs is rare and typically associated with a poor prognosis. Combining cytolo

Paediatric Langerhans Cell Histiocytosis: A 20-Year Single-Centre Retrospective Study of 35 Cases.

Background Langerhans cell histiocytosis (LCH) is a rare proliferative disorder of bone marrow-derived histiocytes that most often affects children. Skeletal involvement is the most frequent manifestation of paediatric LCH, but the lesions can mimic malignancy or infection, posing a diagnostic challenge. We present a retrospective case series of 35 paediatric patients with LCH, highlighting skeletal lesion patterns, multisystem involvement, treatment approaches, and clinical outcomes. Methodology Medical records and imaging of 35 paediatric patients diagnosed with LCH between 2005 and 2025 in

The neuropeptide pigment-dispersing factor signals independently of Bruchpilot-labelled active zones in daily remodelled terminals of Drosophila clock neurons.

The small ventrolateral neurons (sLNvs) are key components of the central clock in the Drosophila brain. They signal via the neuropeptide pigment-dispersing factor (PDF) to align the molecular clockwork of different central clock neurons and to modulate downstream circuits. The dorsal terminals of the sLNvs undergo daily morphological changes that affect presynaptic sites organised by the active zone protein Bruchpilot (BRP), a homolog of mammalian ELKS proteins. However, the role of these presynaptic sites for PDF release is ill-defined. Here, we combined expansion microscopy with labelling o

Mass spectrometry identifies tau C-terminal phosphorylation cluster during neuronal hyperexcitation.

Tau is a microtubule-associated protein implicated in Alzheimer's disease (AD) and other neurodegenerative disorders termed tauopathies. Pathological, aggregated forms of tau form neurofibrillary tangles (NFTs), impairing its ability to stabilize microtubules and promoting neurotoxicity. Indeed, NFTs correlate with neuronal loss and cognitive impairment. Hyperphosphorylation of tau is seen in all tauopathies and mirrors disease progression, suggesting an essential role in pathogenesis. However, hyperphosphorylation remains a generic and ill-defined term, obscuring the functional importance of

Molecular Retention Limitations for Prevascularized Subcutaneous Sites for Islet Transplantation.

Beta cell replacement therapies utilizing the subcutaneous space have inherent advantages to other sites: the potential for increased accessibility, noninvasive monitoring, and graft extraction. Site prevascularization has been developed to enhance islet survivability in the subcutaneous zone while minimizing potential foreign body immune responses. Molecular communication between the host and prevascularized implant site remains ill-defined. Poly(ethylene oxide)s (PEOs) of various hydrated radii (i.e., ∼11-62 Å) were injected into prevascularized subcutaneous sites in C57BL/6 mice, and the cl

PERK recruits E-Syt1 at ER-mitochondria contacts for mitochondrial lipid transport and respiration.

The integrity of ER-mitochondria appositions ensures transfer of ions and phospholipids (PLs) between these organelles and exerts crucial effects on mitochondrial bioenergetics. Malfunctions within the ER-mitochondria contacts altering lipid trafficking homeostasis manifest in diverse pathologies, but the molecular effectors governing this process remain ill-defined. Here, we report that PERK promotes lipid trafficking at the ER-mitochondria contact sites (EMCS) through a non-conventional, unfolded protein response-independent, mechanism. PERK operates as an adaptor for the recruitment of the
