Occult breast carcinoma presenting with axillary lymph node metastases: report of two cases.

We report two unusual cases of occult breast carcinoma in which extensive axillary node metastasis was the first manifestation. A microscopic lesion of intraductal carcinoma was found in the tissue of one resected breast and a small infiltrative ductal carcinoma, 0.8 cm in size, in the greatest dimension in the other. Extensive metastasis was found in 26 out of 51 and 10 out of 15 axillary nodes, respectively. Results of immunohistochemical stains for estrogen and progesterone receptors on fresh frozen sections were negative for metastatic nodes in one case and both were positive for primary a

Combined use of 123I-labelled BCD-F9 and 4C4 monoclonal antibody with dissimilar specificity for breast cancer: implication for the detection limit of immunolymphoscintigraphy in the assessment of axillary lymph node metastases.

To determine whether the detection limit of immunolymphoscintigraphy (ILS), reported to be > or = 1 cm, can be improved by comparing imaging after administration of breast cancer-specific monoclonal antibody (MAb) BCD-F9 and breast cancer-nonspecific 4C4, 25 patients with suspected breast cancer were given injections of both 123I-labelled MAbs. The ILS was performed independently for both MAbs, the 4C4 scans serving as an ipsilateral negative control, and was used preoperatively to detect lymph node metastases. Twenty-one patients had breast cancer of whom 11 patients suffered from axillary in

Effect of menstrual phase on surgical treatment of breast cancer.

1175 premenopausal women whose date of last menstrual period was known were followed up for up to 20 years (average 8 years) after surgery for breast cancer. 525 patients were in the follicular phase and 650 in the luteal phase. We observed 192 unfavourable events among patients operated on during the follicular phase (36.6%) and 192 among patients operated on during the luteal phase (29.6%). The effect of phase was restricted to patients with positive axillary nodes. The 5-year relapse-free survival was 75.5% in 246 node-positive patients operated on during the luteal phase and 63.3% in 190 n

Hormonal receptors, cell proliferation fraction (Ki-67) and c-erbB-2 amplification in breast cancer. Relationship between differentiation degree and axillary lymph node metastases.

In view of the limitations of conventional prognostic factors such as differentiation degree, metastatic lymph nodes, hormonal receptors and others, especially when early lesions are found, additional new markers have been studied, such as gene amplification and cell proliferation index, in order to choose the appropriate treatment. Primary breast carcinoma tumors from 97 patients were examined for differentiation degree, metastatic lymph nodes, hormonal receptors, c-erbB-2 amplification and cell proliferation index (Ki-67). A negative relationship with hormonal receptors and c-erbB-2 amplific

Benign melanocytic nevus cells in axillary lymph nodes. A prospective incidence and immunohistochemical study with literature review.

Benign nevus cell aggregates (NCAs) in lymph nodes usually present as intracapsular or trabecular collections of small, uniform melanocytes that resemble those seen in intradermal melanocytic nevi. The surgical pathologist must be aware of nodal NCAs because they can mimic micrometastasis of carcinoma. Although not uncommon, the frequency with which NCAs occur is controversial. Two previous studies attempted to determine the case incidence of NCAs in axillary lymphadenectomies; widely different results were reported, ranging from .33% to 6.2%. In this study, the authors examined prospectively

Breast conservation in elderly women for clinically negative axillary lymph nodes without axillary dissection.

BACKGROUND: A prospective study was initiated to explore an approach of limited therapy in elderly patients with early clinical stage breast cancer. METHODS: Between 1982 and 1989, 73 women with American Joint Committee on Cancer Stage I/II, clinically negative axillary lymph nodes aged 65 years or older (median age, 74 years) were enrolled in a treatment program consisting of tumor excision, breast and regional lymph node irradiation, and, in 66 patients, tamoxifen. Patients were assessed for disease outcome and complications. RESULTS: At a median follow-up of 54 months, 8-year rates of loc

Radiotherapy and the management of the axilla in early breast cancer.

The role of radiotherapy in the management of the axilla in early breast cancer is examined. A few, carefully selected, clinically node-negative postmenopausal women may require no intervention to the axilla. Otherwise, surgical clearance is the preferred sole management of the axilla, resulting in an excellent level of local control and providing optimal information for the use of systemic adjuvant therapy. Axillary radiotherapy can also provide equivalent levels of long-term control in the clinically node-negative axilla, but the chronic disabling syndrome of brachial plexopathy is documente

An evaluation of the prognostic significance of HLA-DR expression in axillary-node-negative breast cancer.

BACKGROUND: Adjuvant treatment for node-negative breast cancer remains controversial. It is important to pick out the high-risk groups who may benefit from adjuvant systemic therapy and avoid the unnecessary additional therapy for the favorable prognostic groups. METHODS AND MATERIAL: Retrospective study of immunohistochemical staining for HLA-DR on tumor cells from paraffin-embedded tissue specimens of 32 patients with node-negative invasive breast cancer at this hospital from 1986 to 1991 was performed with the aim to investigate its prognostic significance. RESULTS: HLA-DR staining was po

Lymphatic mapping and sentinel lymphadenectomy for breast cancer.

OBJECTIVE: The authors report the feasibility and accuracy of intraoperative lymphatic mapping with sentinel lymphadenectomy in patients with breast cancer. SUMMARY BACKGROUND DATA: Axillary lymph node dissection (ALND) for breast cancer generally is accepted for its staging and prognostic value, but the extent of dissection remains controversial. Blind lymph node sampling or level I dissection may miss some nodal metastases, but ALND may result in lymphedema. In melanoma, intraoperative lymph node mapping with sentinel lymphadenectomy is an effective and minimally invasive alternative to ALN

Tc-99m MIBI uptake in breast carcinoma and axillary lymph node metastases.

Two female patients with breast cancer and axillary lymph node metastases had Tc-99m MIBI chest scintigraphy. The Tc-99m MIBI uptake in the primary breast cancer and metastatic lymph nodes were noted and discussed. The findings suggest that Tc-99m MIBI chest scintigraphy for palpable breast masses and axillary lymph nodes is a helpful test in detecting malignant breast masses and metastatic lymph nodes.

The importance of locoregional control in the treatment of breast cancer and its impact on survival.

BACKGROUND: Different schools of thought regarding the natural history of breast cancer and how it relates to the importance of local control in affecting survival have generated controversy over the optimal treatment of early breast cancer. Parting with traditional thinking that some disease is localized, in which cases local control is important for survival, another view of the natural history of the disease argues that local control is not of value to survival. This commentary will review the evidence and attempt to evaluate the effect of modern adequate local treatment on survival. METHO

Correlation of DNA flow cytometry and hormone receptors with axillary lymph node status in patients with carcinoma of the breast.

Axillary lymph node dissection (ALND) remains a standard staging procedure for patients with primary carcinoma of the breast. In a retrospective review of 63 patients with primary breast cancer who underwent ALND, statistical analysis showed that only tumor size was correlated with axillary lymph node involvement. Neither hormone receptor status, ploidy, nor percent S-phase contributed more to prediction of nodal size. Statistical analysis also indicated a significant correlation between receptor-negative status and the presence of positive axillary lymph nodes.

Intraoperative assessment of nodal status in the selection of patients with breast cancer for axillary clearance.

Axillary node status was assessed by intraoperative contact cytology of four sampled nodes in each of 114 consecutive patients with operable breast cancer treated by wide local excision or mastectomy. At a mean follow-up of 18.7 months, 43 node-positive patients who underwent immediate axillary clearance had two locoregional and six distant recurrences with two deaths; four had arm oedema and one shoulder stiffness. There were no recurrences or arm and shoulder morbidity in 71 node-negative women who had no clearance or radiotherapy. The mean hospital stay was 4.3 days for patients having wide

A case report of infiltrating ductal carcinoma originating from aberrant breast tissue.

We report a case of carcinoma originating from aberrant breast tissue in a 32-year-old female who had a soft subcutaneous mass on the anterior axillary fold. Histopathologic examination showed an infiltrating ductal carcinoma of aberrant breast tissue without metastasis to lymph nodes. We believe that subcutaneous nodules of uncertain origin around the periphery of the breast and especially in the axilla should always be viewed with suspicion and treated by early excision.

Management of the axilla in patients with breast cancers one centimeter or smaller.

Very small breast cancers are being diagnosed with increased frequency, and, until recently, little information regarding the incidence of axillary lymph node metastases in these most favorable tumors was available. Moreover, scarce data exist regarding axillary failure in this cohort as a function of initial treatment, be it surgery, radiation, or simply observation. In the present study, limited to women with invasive cancers measuring no more than 10 mm, the incidence of pathologically positive axillary nodes was 12.3%. The incidence of nodal metastases was influenced by tumor size (albeit

Prediction of axillary lymph node status in breast cancer patients by use of prognostic indicators.

BACKGROUND: If axillary lymph node status of breast cancer patients could be accurately predicted from basic clinical information and from characteristics of their primary tumors, then many patients could be spared axillary lymph node dissection. Tumor size alone does not allow the identification of groups with very low or high risk of being axillary node positive. PURPOSE: Our goal was to investigate the possibility of using prognostic indicators to predict axillary node status of patients with primary breast cancer. METHODS: Data from 26,683 patients from the National Breast Cancer Tissue

Epstein-Barr virus in Hodgkin's disease and site of origin of tumour.

Epstein-Barr virus (EBV) may be involved in the pathogenesis of Hodgkin's disease. We investigated whether EBV in Hodgkin's disease is related to the site of origin of the tumour. In 40 patients with stage I disease, there was a significant association between EBV latent membrane protein (LMP-1) expression and presentation in neck lymph nodes. There was no association in stage II-IV disease (57 cases). Nodular sclerosing subtype was rarely associated with LMP-1 expression. In some cases of Hodgkin's disease of mixed cellularity or lymphocyte predominant subtype originating in the neck, EBV may

[A case of lung cancer with axillary nodal involvement].

A 57-year-old man with lung cancer was reported. Primary tumor was located at left S1+2, and directly invaded to chest wall (from 1st. rib to 4th rib). Hypercalcemia and delirium were observed. Whole body examination showed that no distant metastasis except for nodal swelling of left axillary region. Left upper lobectomy combined with chest wall resection was performed. Hilar, mediastinal and axillary nodes were also dissected. Histological examination revealed that nodal involvement was not present at neither hilar or mediastinal region, but was present in axillary node. It was thought that l
