Biomimetic PD-1-nanovesicles inducing tri-modal sonodynamic-chemo-immunotherapy for breast cancer therapy and lung metastasis inhibition.

BACKGROUND: Current cancer therapeutic approaches demonstrate constrained efficacy and fail to effectively address tumor metastasis, underscoring the critical need for innovative treatment modalities. RESULTS: In this study, we developed an innovative PD-1+ cell membrane-coated nanoplatform (PD-1-Au@DOX) that integrates ultrasound (US)-responsive gold nanoparticles (AuNPs), doxorubicin (DOX) delivery, and PD-L1 blockade into a synergistic therapeutic system. In vitro studies demonstrate that PD-1-Au@DOX enables targeted delivery of DOX and AuNPs to PD-L1-expressing breast cancer cells (4T1 an

HDAC6 orchestrates metastatic and immunosuppressive programs in small cell lung cancer through S100A2-TGF-β/SMAD and CSF1R signaling.

BACKGROUND: Small cell lung cancer (SCLC) remains a highly lethal malignancy with limited therapeutic options. The purpose of this study was to investigate the central role of histone deacetylase 6 (HDAC6) in SCLC progression and its regulatory mechanisms to identify novel therapeutic strategies. METHODS: Preclinical SCLC models were utilized alongside molecular, cellular, and immunological techniques to elucidate HDAC6's mechanistic functions. The deacetylation of S100A2 and its impact on downstream signaling were analyzed, compensatory responses to HDAC6 inhibition were assessed, and the eff

Utilization of Indocyanine Green for Augmentation of Pulmonary Metastases Resection in Pediatric, Adolescent, and Young Adult Sarcoma Patients.

BACKGROUND: Complete pulmonary metastasectomy is central to curative-intent therapy for sarcoma, but lesion localization can be challenging. Indocyanine green (ICG) near-infrared fluorescence offers real-time intraoperative guidance, though data in pediatric and adolescent/young adult sarcoma patients are limited. METHODS: A retrospective review of patients with metastatic sarcoma who underwent pulmonary metastasectomy was performed. Patients were dosed preoperative ICG (dose: 4 mg/kg, 24 h before surgery) between April 2019 and November 2022. Demographics, tumor histology, operative details,

Development of Antibody-Drug Conjugates Targeting L1CAM to Treat Metastatic Cancer.

Effective treatment for metastatic cancer has remained elusive because of the persistence of drug-resistant metastasis stem cells (MetSC) that drive relapse. MetSCs are tumor cell subpopulations enriched for their ability to reinitiate and sustain metastatic growth, displaying phenotypic plasticity and resistance to chemotherapy. These cells express the L1 cell adhesion molecule (L1CAM), a transmembrane protein detected in numerous human solid tumor types and at multiple disseminated organ sites. As a selective surface marker of MetSCs, L1CAM is a promising candidate for molecularly targeted d

Superiority of Lateral Decubitus Strategy in Preventing Atelectasis From Obscuring Targets During Robotic Bronchoscopy: Lateral Decubitus Strategy vs Ventilatory Strategy to Prevent Atelectasis Trial.

BACKGROUND: Atelectasis during peripheral bronchoscopy can cause CT-to-body divergence, can cause false-positive radial-probe endobronchial ultrasound images, and can obscure a target. As shown in prior studies, ventilatory strategies to prevent atelectasis, although useful, cannot completely avoid this phenomenon. RESEARCH QUESTION: Is a lateral decubitus superior to a ventilatory strategy in preventing atelectasis from obscuring targets in patients with lung nodules in dependent zones? STUDY DESIGN AND METHODS: This was a randomized controlled study (1:1) in which patients undergoing robot

Abscopal effect following combined radiotherapy and immunotherapy in pulmonary metastases.

Abscopal effect refers to the shrinkage or regression of metastatic tumours that are distant from the irradiated field. Radiotherapy combined with immunotherapy can increase the occurrence of the abscopal effect. This case report describes a female patient with lung metastases from hepatocellular carcinoma who exhibited shrinkage and regression of non-irradiated lung lesions following combined radiotherapy and immunotherapy. The patient is currently stable and remains on treatment and follow-up. We discuss the potential mechanisms of the abscopal effect to provide clinical evidence supporting

Matched MRI, Segmentations, and Histopathologic Images of Brain Metastases from Primary Lung Cancer.

Brain metastases occur in approximately 30-40% of lung cancer patients, and outcomes are heterogeneous, dependent on a combination of clinical, radiologic, pathologic, and molecular characteristics. Though artificial intelligence-based prognostic models show promise, their use is limited by a lack of high-quality training data. Thus, there is a critical need for a large, heterogeneous, annotated, open-access brain metastasis dataset with matched radiologic and histopathologic imaging. We present such a dataset composed of 111 cases of magnetic resonance (MR) and histopathologic imaging from pa

Antioxidant N-Acetylcysteine Facilitates Breast Cancer Metas-Tasis via Immunosuppressive Reprogramming of Neutrophils.

N-acetylcysteine (NAC) is a widely used antioxidant. It has also attracted significant research interest with regard to its role in cancer progression, although the mechanisms involved remain controversial and poorly understood. Here, using murine models of breast cancer metastasis, we found that systemic NAC administration significantly enhanced pulmonary metastasis without altering primary tumor growth in immunocompetent mice, whereas this metastasis-promoting property of NAC was abrogated in T cell-deficient mice. This phenomenon was not due to the direct effects of NAC on T cells or tumor

Practical Aspects of Delivering Radiation for Polymetastatic Disease.

Patients with polymetastatic cancer have traditionally been treated with systemic therapies, whereas radiation therapy has played a mainly supportive role, typically using simple, nonconformal techniques for palliation of the most symptomatic sites. More recently, however, stereotactic ablative radiotherapy (SABR) therapy has emerged as a feasible and safe option for select patients with widespread polymetastatic disease, building on its established efficacy in the oligometastatic setting and supported by encouraging data from recent clinical trials. In parallel, low-dose radiation therapy (LD

The Role of Hedgehog Signaling in Non-small Cell Lung Cancer: Targeting Tumor Invasion, Therapy Resistance and Novel Therapeutic Strategies.

Although essential for normal development and tissue homeostasis, aberrant activation of the Hedgehog (Hh) signaling pathway is implicated in non-small cell lung cancer (NSCLC) progression and treatment resistance. This review details the contribution of Hh signaling to NSCLC, focusing on its promotion of tumor invasion and therapeutic resistance, and establishes a rationale for disrupting this pathway to improve treatment efficacy. Malignant phenotypes in NSCLC are driven by dysregulated Hh pathway activity, often via autocrine or paracrine loops. We specifically assess how Hh pathway activat

What we can learn from the lepidic growth pattern of pulmonary metastases.

In lung adenocarcinoma, cancer cells can grow in a peculiar modality, replacing normal pneumocytes and creating glandular-like structures based on the alveolar architecture. This lepidic growth pattern is characterized by well differentiated epithelial cancer cells, that preserve their monolayer disposition, avoiding invasion of the interstitial space and dispersion into the airways. The same growth pattern can be assumed also by cancer cells originating from distant organs and a new interest has arisen around this modality of growth related to pulmonary metastases. Here we systematically revi

Telehealth-delivered exercise and nutrition intervention to improve outcomes in patients with early stage non-small cell lung cancer: protocol for the multicentre STARLighT phase II (neoadjuvant) and phase III (adjuvant) trial.

INTRODUCTION: In early stage non-small cell lung cancer (NSCLC), recurrence is frequent despite surgery and systemic treatments. Observational studies suggest that physical exercise and nutrition could improve outcomes, such as survival and treatment tolerance; however, solid evidence is lacking. The STARLighT trial aims to assess the effects of a telehealth-delivered combined exercise and nutrition intervention on clinical, biological and patient-reported outcomes in early stage NSCLC. METHODS AND ANALYSIS: STARLighT is a multicentre master protocol study conducted in Italy, comprising two c

Is pneumonectomy still relevant for non-small cell lung cancer? Long-term overall survival from a 15-year experience.

BACKGROUND: Pneumonectomy has long been shown to be curative for lung cancer but may lead to several acute and chronic adverse physiologic changes. Limited data are available on long-term follow-up of patients who have undergone this procedure. We sought to evaluate whether pneumonectomy is still a valuable therapeutic procedure when it is the only curative approach available and to report on long-term survival after the procedure as well as operative mortality and morbidity. METHODS: The primary outcome of our study was long-term overall survival after pneumonectomy. Secondary outcomes were
