Interlesional genomic heterogeneity in small bowel adenocarcinoma: evidence from matched primary and peritoneal metastatic lesions.

This study analyzed the spatiotemporal heterogeneity of metastatic small bowel adenocarcinoma (SBA) with peritoneal involvement by examining genomic profiles of six SBAs and their corresponding peritoneal metastases using next-generation sequencing. Four cases had synchronous peritoneal metastases, showing complete genomic concordance with the primary tumors. In contrast, one of the two cases with metachronous peritoneal metastases developed an additional KRAS mutation compared to the paired primary, shifted from moderately to poorly differentiated morphology, and exhibited mismatch-repair def

Long-Term Prognosis Analysis of Gastrointestinal Stromal Tumors in the Jejunum and Ileum: A SEER-Based Study.

IntroductionThe prognosis differences of gastrointestinal stromal tumors (GISTs) between the jejunum and ileum have rarely been studied. This study aimed to evaluate the long-term survival outcomes in patients with jejunal and ileal GISTs.MethodsThis population-based retrospective cohort study utilized data from the Surveillance, Epidemiology, and End Results (SEER) database from 2000 to 2019. Kaplan-Meier analyses and Cox proportional hazards models were employed to assess overall survival (OS) and cancer-specific survival (CSS). A 1:1 propensity score matching (PSM) approach was implemented

Biological and comorbidity factors outweigh lymph node metrics in predicting outcomes after small-bowel neuroendocrine tumour resection.

BACKGROUND: Small-bowel neuroendocrine tumours (SBNETs) frequently involve mesenteric lymph nodes. While guidelines suggest minimum lymph node yields (LNY), the independent prognostic relevance of LNY and lymph node ratio (LNR) regarding recurrence remains uncertain in contemporary cohorts. METHODS: A retrospective analysis of 102 patients undergoing R0/R1 resection for SBNETs (2005-2023) at a single tertiary institution was conducted. The primary outcome was overall survival (OS), and time to recurrence (TTR) was analyzed as a secondary endpoint, with the caveat that recurrence detection is

Efficacy of targeted therapies for metastatic small bowel adenocarcinoma treatment: A retrospective study by AGEO.

BACKGROUND: Small bowel adenocarcinoma (SBA) is a rare malignancy with poor prognosis. In metastatic disease, evidence regarding the efficacy of chemotherapy (CT) combined with bevacizumab or anti-EGFR agents is limited to small studies. This study aimed to assess, in real-world practice, the effectiveness of first-line CT combined with targeted therapies (TT)-either antiangiogenic (AA) or anti-EGFR-compared with CT alone in patients with metastatic SBA (mSBA) and proficient mismatch repair/microsatellite stable (pMMR/MSS) status. PATIENTS AND METHODS: This retrospective multicentre study inc

The ILEOSTIM trial: A multicentre randomised controlled trial evaluating the impact of efferent loop stimulation prior to ileostomy reversal on postoperative ileus.

BACKGROUND: Strategies to optimise bowel function before stoma reversal surgery (SRS) are needed to improve postoperative outcomes. Efferent bowel stimulation (EBS) prior to stoma closure has been proposed as a potential strategy to reduce postoperative ileus (POI) incidence and improve the patient's postoperative course. METHODS: A multicentre randomised controlled trial was designed to evaluate the effectiveness of pre-SRS EBS in reducing POI incidence. Patients scheduled for elective SRS were randomised 1:1 to either an intervention group, receiving daily EBS with saline and a nutritional

Lymph node ratio, not nodal stage, predicts overall and disease-free survival in non-ampullary primary small intestine adenocarcinoma.

Lymph node ratio (LNR) has emerged as a strong prognostic indicator in colorectal cancer (CRC), but its significance in non-ampullary primary small intestine adenocarcinoma (SIA) has been only sparsely reported in the literature. Moreover, LNR has not been examined alongside key CRC histologic prognostic variables, including tumor budding (TB), desmoplastic reaction (DR), and tumor-infiltrating lymphocytes (TILs). Eighty-nine cases (mean age: 63 years, range: 20-88) of SIA were included from 2008 to 2023; most tumors arose in the duodenum (40%). Mean follow-up was 40.9 months (range: 0-189). T

Gut-reproduction axis: genome-wide pleiotropic and prospective evidence linking inflammatory bowel disease with gynecological disorders.

OBJECTIVE: We aimed to explore shared genetic architectures and potential causal associations between inflammatory bowel disease (IBD) and gynecological diseases, including ovarian cysts (OC), pelvic inflammatory disease (PID), and endometriosis (EMs), and to evaluate whether the findings are consistent with a gut-reproductive connection. METHODS: We used genome-wide association study (GWAS) data to assess genetic correlation between IBD and gynecological diseases. Cross-trait pleiotropic loci and genes were identified using PLACO, colocalization, and functional annotation analyses. Stratifie

Primary tumor resection in the setting of unresectable metastatic disease in small bowel NETs: consensus and controversy.

Small bowel neuroendocrine tumors represent a rare malignancy, and most patients present with metastatic disease at diagnosis. While current guidelines from the North American Neuroendocrine Tumor Society and the European Neuroendocrine Tumor Society agree that resection is indicated for symptomatic tumors causing obstruction, ischemia, or bleeding, the role of primary tumor resection in otherwise asymptomatic patients with unresectable metastatic disease remains controversial. This article reviews the existing evidence and highlights the role of primary tumor resection in the setting of metas

Primary multifocal gastrointestinal epithelioid angiosarcoma with MYC amplification: a diagnostic pitfall mimicking poorly differentiated carcinoma: a case report.

BACKGROUND: Primary gastrointestinal epithelioid angiosarcoma (EAS) is an extremely rare and aggressive malignancy. Due to its epithelioid morphology and aberrant cytokeratin expression, it frequently mimics poorly differentiated carcinoma, creating significant diagnostic challenges. Furthermore, the molecular landscape of primary EAS, particularly regarding MYC status, remains underrecognized compared to radiation-associated angiosarcomas. CASE PRESENTATION: An 82-year-old man presented with refractory melena and severe anemia. Imaging revealed multifocal lesions involving the stomach and sm

A phase II study of cabozantinib in metastatic or unresectable refractory gastrointestinal stromal tumour(GIST): a single-centre study from India.

BACKGROUND: Beyond third-line therapy, options for metastatic/advanced GIST are limited, especially in low- and middle-income countries (LMICs) where access to fourth-line ripretinib is often restricted. Cabozantinib, a multi-kinase inhibitor targeting KIT and related resistance pathways, could serve as a pragmatic alternative. This Phase II study evaluated the efficacy and safety of cabozantinib in patients with metastatic/advanced GIST who had progressed after ≥ 3 prior Tyrosine Kinase Inhibitors (TKIs). METHODS: This is a single-arm phase II trial that enrolled patients with advanced GIST

Survival trends in the management of carcinoid heart disease among patients with neuroendocrine neoplasms.

BACKGROUND: Nearly 20% of patients with neuroendocrine tumors (NET) develop carcinoid syndrome (CS), and 20-25% of those with CS develop carcinoid heart disease (CHD). Contemporary risk factors for CHD and prognostic variables in NET management remain limited. This study aimed to characterize the clinical and imaging factors associated with the development, and prognostic impact of carcinoid heart disease in patients with neuroendocrine neoplasms. METHODS: A single-institution, retrospective analysis of well-differentiated NET patients from January 2010 to December 2024 was conducted. Cox pro

[A Case of Small Intestinal GIST with Small Intestinal Torsion during Follow-Up as an Ovarian Cyst].

An 87-year-old woman with no prior history of laparotomy visited her local doctor complaining of anorexia. She was diagnosed with intestinal obstruction and initially managed conservatively with fasting; however, her condition did not improve, and she was referred to our hospital on the seventh day after symptom onset. Abdominal CT revealed a 2-cm cystic lesion on the anterior surface of the sacral pelvic cavity and a whirlpool sign near the end of the ileum. Based on these findings, a diagnosis of small intestinal volvulus and an ovarian cyst-previously noted-was made, and emergency surgery w

Chronic intussusception revealing primary diffuse large B-cell lymphoma of the small intestine: A case report.

Adult intussusception is an uncommon clinical entity, representing approximately 5% of all intussusception cases and 1%-5% of intestinal obstructions in adults. (1) In contrast to the pediatric population, adult intussusception is most often secondary to an underlying organic lesion, frequently a neoplasm. Malignant lesions account for nearly 30%-50% of small bowel cases. (2) Among these, primary small intestinal lymphoma is a rare cause, with diffuse large B-cell lymphoma (DLBCL) being particularly uncommon. We herein report a case of chronic intermittent intussusception revealing primary DLB

Histotripsy for neuroendocrine liver metastases: Early single-institution outcomes and safety.

BACKGROUND: Histotripsy is a novel, noninvasive, nonionizing focused ultrasound technology that mechanically destroys tissue via cavitation. Neuroendocrine tumors, which frequently metastasize to the liver and follow an indolent course, may be particularly well-suited for histotripsy, and we report an early experience. METHODS: This retrospective analysis summarizes the first 32 patients consecutively treated with histotripsy in a neuroendocrine practice. Intraoperative targeting and cavitation were performed with the EDISON system. Clinical outcomes, postprocedural imaging, and adverse event
