Neoadjuvant immune checkpoint inhibitors for patients with resectable stage III/IV melanoma: a nationwide real-life study in France (NEOMEL).

BACKGROUND: Despite the clear therapeutic benefits of neoadjuvant treatment (NT) with immune checkpoint inhibitors (ICIs) in clinical trials, the efficacy of NT with ICIs (NT-ICI) and the optimal regimen for patients with resectable metastatic melanoma (RMM) remain to be confirmed in real life. OBJECTIVES: To assess the efficacy and safety of NT-ICI among patients with RMM in real life. METHODS: NEOMEL is a French retrospective multicentric cohort study. Dermato-oncologists from the French Groupe de Cancérologie Cutanée (Cutaneous Cancer Group) included patients treated with NT-ICI for RMM (

Loss of ten-eleven translocation 2 (TET2) facilitates aggressive behaviour in cutaneous melanoma by inducing peroxisome proliferator-activated receptor-γ coactivator 1α expression and oxidative phosphorylation.

BACKGROUND: The induction of peroxisome proliferator-activated receptor-γ coactivator 1α (PGC-1α; encoded by PPARGC1A) expression and activation of oxidative phosphorylation (OXPHOS) is associated with disease progression and treatment resistance in patients with cutaneous melanoma. Loss of the TET2 (ten-eleven translocation 2)/5-hydroxymethylcytosine (5-hmC) epigenetic pathway is linked to melanoma aggressiveness, although the underlying mechanisms remain unclear. OBJECTIVES: To explore a relationship between TET2-mediated DNA hydroxymethylation and the induction of PPARGC1A/PGC-1α expressio

Incidence and tumour location of iris melanoma in Denmark: a nationwide cohort study (1943-2021).

BACKGROUND/AIMS: Iris melanoma differs from posterior uveal melanoma in clinical behaviour and location. Situated anteriorly, it is directly exposed to ultraviolet radiation (UVR), unlike posterior melanomas shielded by the lens. However, epidemiological patterns and UVR's role in iris melanoma development remain poorly understood. Therefore, we investigated long-term incidence trends of iris melanoma in Denmark, focusing particularly on tumour location in relation to sun exposure. METHODS: We conducted a nationwide retrospective cohort study of patients diagnosed with iris melanoma in Denmar

Sentinel Lymph Node Biopsy for Cutaneous Squamous Cell Carcinoma.

BACKGROUND: Cutaneous squamous cell carcinoma (cSCC) often presents localized in the skin, but when it metastasizes to regional lymph nodes prognosis declines. Despite this, the role of sentinel lymph node biopsy (SLNB) in detecting metastasis is not well defined. This study uses the National Cancer Database (NCDB) to evaluate the role of SLNB in cSCC. STUDY DESIGN: Patients with cSCC who underwent complete surgical excision (CSE) alone or CSE + SLNB were selected from the NCDB from 2012 to 2020. Overall survival (OS) in these cohorts was compared. Subgroup analyses of SLNB status and additio

Management of In-Transit Metastatic Melanoma.

Approximately 10% of patients with melanoma develop in-transit disease (ITM). Although resection may be performed in patients with limited, fully resectable disease, many patients with ITM present with bulky, unresectable disease, which can pose a significant therapeutic challenge. Regional perfusion chemotherapy procedures and intralesional therapy have high response rates and can provide durable locoregional control while limiting systemic toxicity. Systemic therapies, including immune checkpoint inhibitors and targeted therapies, may also be used especially in patients with concomitant dist

Characterisation and impact of intratumoural stroma in melanoma and carcinoma brain metastases.

Research on the tumour microenvironment in brain metastases (BM) has predominantly focused on the immune response, while the presence, morphological patterns, and potential clinical relevance of intratumoural stroma remain less intensively investigated. We retrospectively analysed 604 BM (529 carcinomas, 75 melanomas) from 556 patients. Intratumoural stroma was histomorphologically classified into absent/unclear (Group 0), present without desmoplasia (Group 1) or with desmoplasia (Group 2). Associations with histological features, clinical parameters, and survival were evaluated. Intratumoural

Circulating Tumor DNA Is Prognostic of Patient Outcome and Enables Therapy Monitoring in Metastatic Uveal Melanoma.

PURPOSE: Circulating tumor DNA (ctDNA) refers to small DNA fragments, shed from tumor cells into the bloodstream. Measuring ctDNA provides a noninvasive tool for real-time disease monitoring. Although ctDNA predicted overall survival (OS) in metastatic uveal melanoma (mUM) treated with tebentafusp, its broader prognostic value across treatment modalities remains unclear. In this study, we assess the prognostic relevance of longitudinal ctDNA detection and mutant allele fraction (MAF) in patients with mUM treated with different modalities. EXPERIMENTAL DESIGN: To assess ctDNA monitoring as a t

Cortactin Suppresses mTOR-Dependent Senescence in Circulating Tumor Cells.

UNLABELLED: Tumor senescence is a critical mechanism underlying tumor progression and recurrence. A better understanding of how premetastatic circulating tumor cells (CTC) exploit senescence to survive in the bloodstream could help reveal vulnerabilities for therapeutic intervention. Using patient-derived melanoma CTC lines and xenograft models, we identified a role for the cytoskeletal regulator cortactin in mTOR/p53-dependent senescence. Cortactin localized to Rab7-positive endosomes and maintained late-endosomal homeostasis. Depletion of cortactin induced aberrant endosomal aggregates with

Unusually aggressive malignant glomus tumour of cutaneous origin.

Glomus tumours are uncommon, typically benign neoplasms of specialised dermal thermoregulatory structures known as glomus bodies. Wide local excision is the gold standard management for such tumours with good recurrence-free outcomes. Rarely, they may exhibit malignant and metastatic behaviour. The efficacy of systemic treatments to manage widespread or metastatic disease has been trialled with varying degrees of success but remains poorly understood. This case report describes a cutaneous glomus tumour in a female patient. Despite prompt wide local excision of the primary lesion and subsequent

Phase III Study Investigating the Safety and Efficacy of TM5614 in Combination With Nivolumab for the Treatment of Unresectable Malignant Melanoma: Protocol.

TM5614, a plasminogen activator inhibitor-1 (PAI-1) inhibitor, has demonstrated potential in overcoming nivolumab resistance in patients with unresectable, anti-PD-1 antibody-refractory malignant melanoma. Previous Phase II trials have shown promising response rates, warranting further investigation through a randomised, placebo-controlled Phase III trial. This Phase III, randomised, double-blind, placebo-controlled, investigator-initiated clinical trial aims to evaluate the efficacy and safety of TM5614 in combination with nivolumab for treating patients with unresectable, anti-PD-1 antibody-

Phase II Trial of Interleukin-12 Followed by Interferon Alfa-2b in Patients with Metastatic Malignant Melanoma: Results from CALGB 500001 (Alliance).

The ability of IL12 to stimulate production of IFNγ suggested it might improve the efficacy of low-dose IFNα. In this phase II trial, patients with metastatic malignant melanoma were administered recombinant human (rh) IL12 followed by IFNα2b. Primary endpoints were clinical response and progression-free survival. Secondary objectives were to evaluate the effect of endogenous IFNγ on JAK-STAT signaling and IFN-regulated genes in peripheral blood mononuclear cells (PBMC). Patients with advanced melanoma received rhIL12 on day 1 and IFNα2b on days 2 to 6 of a 14-day cycle. rhIL12 was given intra

Retrospective Analysis of the Recurrence of Atypical Fibroxanthoma When Treated With Curettage and Cautery at a Single Centre.

Atypical fibroxanthoma (AFX) is a rare spindle cell neoplasm that arises within the dermis of chronically sun-exposed skin. Although wide local excision is standard practice, patients are regularly elderly and highly comorbid and therefore, curettage may offer a suitable alternative as a same day treatment with minimal aftercare. A retrospective case series was conducted analysing the files of all patients diagnosed with histopathological AFX at a single dermatology practice in Fremantle, Western Australia between the years 2016 and 2025. Kaplan-Meier analyses estimated recurrence rate with su

The quadruple immunotherapy for colorectal cancer (QuICC) trial for mismatch repair-proficient metastatic colorectal cancer.

BACKGROUND: Despite successes in other malignancies, immune checkpoint blockade (ICB) has not demonstrated reproducible efficacy in mismatch repair-proficient (pMMR) colorectal cancer (CRC). Preclinical studies indicate that multitargeted combination immunotherapy may sensitize resistant tumors to immune-mediated killing. This trial was designed to test this hypothesis by combining agents to induce and augment an immune response in pMMR CRC. METHODS: In this single-center, phase 1/2 trial, eligible patients had pMMR, RECIST v1.1-measurable metastatic CRC, ≥2 prior lines of therapy, and no pri

The prospective phase I "Flash-Skin I" trial: ultra-high dose rate radiotherapy implementation and quality assurance at a clinical linear accelerator.

BACKGROUND AND PURPOSE: The combination of reduced normal tissue damage and unaffected tumor control is referred to as Flash-effect. This phenomenon has been observed with ultra-high dose rate (UHDR) radiotherapy in preclinical models. The Flash-Skin I (NCT06549439) treated seven patients with melanoma skin metastasis. The treatment protocol included 2 × 9 Gy UHDR followed by 1 × 9 Gy conventional radiotherapy. Here we report on the implementation of the study at a converted C-arm clinical linear accelerator. MATERIALS AND METHODS: A dedicated preclinical radiation therapy quality assurance (

An extension of the validation cohort of the Dutch Early-Stage Melanoma (D-ESMEL) study for stage-specific analyses.

There is a high need for accurate prognostic models among stage II melanoma to determine who may benefit from (neo)adjuvant systemic therapy. The Dutch Early- Stage Melanoma (D-ESMEL) study was designed to identify new prognostic features in a population-based sample of stage I/II melanoma patients in addition to American Joint Committee of Cancer (AJCC) staging. The validation cohort of the D-ESMEL study employs a nested case-control design. Initially, controls were randomly sampled to develop prognostic that included both known and new prognostic factors to assess the additive value of new p
