Transdural Cisternotomy in the Extended Middle Fossa Approach.

Dolenc's pretemporal transcavernous approach and Kawase's s anterior transpetrosal approach are the two primary extradural middle cranial fossa approaches (EMFAs) to the central skullbase. The extended extradural phase of the surgery may potentially result in temporal lobe edema, compromising the surgical exposure and sometimes a postoperative temporal lobe venous infarction. The authors describe an alternative technique of cerebrospinal fluid (CSF) drainage called "transdural cisternotomy" in EMFA. In this, CSF is released from the chiasmatic cistern during the extradural stage of surgery. Tw

Tumor immunogenicity shapes CNS immune niches and CD8⁺ T cell fate in glioblastoma.

Immunotherapeutic strategies have largely failed in glioblastoma (GBM), in part because the spatial and temporal organization of anti-tumor immunity across central nervous system (CNS) compartments remains poorly defined. Here, using longitudinal, compartment-resolved profiling of the deep cervical lymph nodes (dcLNs), cranial meninges, and tumor tissue in two preclinical GBM models of differing immunogenicity, we delineate how tumor immunogenicity shapes CNS immune engagement. Although early immune priming occurred in both models, as evidenced by similar temporal patterns of dcLNs activation,

Profiling of rare immune cell populations and integrative analysis identify immune ecotypes in newly diagnosed meningiomas.

Meningiomas (MGMs) are the most common primary intracranial tumors in adults with a substantial subset exhibiting aggressive clinical behavior. Immunotherapy represents a potential alternative treatment option, even though MGMs have traditionally been considered “immunologically cold” tumors. This study explored less characterized immune cell subsets —B cells, natural killer (NK) cells, and granulocytes— and their associations with major immune cell populations as well as their prognostic implications. For this purpose, we performed tissue cytometry analysis in a clinically well-annotated mult

Dexamethasone Suppresses Already Low Estrogen Receptor Levels in Meningiomas.

Intracranial meningiomas (ICMs) are the most common primary adult brain tumor. They are more frequent in women, respond to female hormones, are associated with breast cancer and are often progesterone receptor-positive (PR+), consistent with hormonal sensitivity. Yet <20% are weakly estrogen receptor-positive (ER+). This work reviews the literature to investigate this paucity of ER by first testing if Dexamethasone (Dex), which has been used since 1984 to reduce peritumoral brain edema, is suppressing ER. Ligand-binding assays after 1984 have shown a significant decrease in any and supra-thres

Meningeal-tumor interactions define distinct modes of leptomeningeal colonization in Group 3 medulloblastoma.

Medulloblastoma, the most common malignant brain tumor in children, shows a pronounced tendency to spread to the leptomeninges. Leptomeningeal metastasis accounts for nearly all medulloblastoma-related deaths, yet the cellular and molecular mechanisms driving this process remain poorly understood. Progress has been hindered by limited access to patient samples, the fragile anatomy of the leptomeninges, and the lack of robust preclinical models. Here, we developed an in vitro model that captures key features of the early leptomeningeal niche. We demonstrate that human meningeal cells promote me

Survival of lung cancer patients with central nervous system metastases: from clinical trials to mouse models.

BACKGROUND: Central nervous system (CNS) metastasis is common in patients with non-small cell lung cancer (NSCLC) and is associated with poor prognosis. Evidence guiding optimal clinical management remains limited, and comprehensive studies on animal models that recapitulate different CNS metastatic patterns are lacking. METHODS: A total of 316 NSCLC patients with CNS metastases were retrospectively enrolled between May 2019 and February 2024. Patients were categorized into four groups: brain metastases only (BM, n = 145), leptomeningeal metastases only (LM, n = 43), concurrent brain and lept

Local metastatic expansion versus secondary intra-organ dissemination: two causes of neurological death explained by fundamentally different metastatic colonization patterns.

BACKGROUND: Neurological failure contributes to 15-50% of deaths in patients with brain metastases, yet the underlying mechanisms remain poorly understood. Clinical causes range from local compression to meningeal metastasis. In this context, a link between infiltrative histopathological growth patterns (HGPs) and meningeal metastasis was recently described and prompted this reverse translation study. METHODS: We conducted a retrospective postmortem histological assessment and a prospective MRI-based proof-of-concept study to explore neurological decline mechanisms in two experimental brain m

Preliminary assessment of an MRI-based grading system for leptomeningeal disease: an exploratory prognostic framework.

BACKGROUND: Current diagnostic approaches of leptomeningeal disease (LMD) rely heavily on cerebrospinal fluid (CSF) cytology, which shows significant limitations and the requirement for invasive procedures. We aim to develop an MRI-based grading scores for LMD diagnosis and prognosis that address current diagnostic limitations and provide standardized, reproducible assessment criteria. METHODS: We conducted a retrospective analysis of 32 adult cancer patients evaluated for suspected LMD. Two experienced neuroradiologists independently assessed MRI studies using our novel grading system, which

Malignant Progression of Subependymal Giant Cell Astrocytoma-Imitating Fibrous Meningioma in a Child Carrying a Germline CHEK2 Mutation.

INTRODUCTION: The subependymal giant cell astrocytoma (SEGA) predominantly occurs in patients with tuberous sclerosis. Here, we present an unusual aggressive transformation of SEGA-imitating fibrous meningioma in a child carrying a germline CHEK2 mutation. METHODS: This case study was conducted at a tertiary pediatric oncology center in accordance with current diagnostic and therapeutic standards. Tumor classification followed WHO CNS5 criteria and was complemented by genome-wide DNA methylation profiling. Comprehensive molecular workup included germline and somatic whole exome sequencing, co

Continuous Intrathecal Infusion of Nivolumab in Advanced Melanoma With Concomitant Leptomeningeal Disease: Efficacy, Safety, and Pharmacokinetics.

BACKGROUND AND OBJECTIVES: Prognosis of melanoma with leptomeningeal disease (LMD) is poor (median overall survival of 5.1 months). Intrathecal (IT) nivolumab appears safe, although its efficacy remains uncertain. We investigated the feasibility, safety, and efficacy of continuous IT nivolumab. METHODS: This was a retrospective analysis of 11 melanoma patients with progressive LMD (MelBase; NCT02828202) treated as part of the patients' care with continuous IT nivolumab administered through spinal (n = 10) or ventricular catheter (n = 1). RESULTS: Patients (5 women, median age 52) with Easter

KMT2C Loss Promotes NF2-Wildtype Meningioma Progression and Ferroptosis Sensitivity via Epigenetic Repression of Hippo Signaling.

High-grade meningiomas remain clinically challenging due to their aggressive behavior and limited therapeutic options. Although mutations and dysregulation of KMT2 family members have been implicated in various cancers, their functional significance in meningioma remains unclear. While NF2 alterations are the most common drivers of meningioma pathogenesis, the mechanisms regulating NF2 transcription in NF2-intact tumors are poorly understood. Here, we demonstrate that KMT2C expression is markedly reduced in high-grade meningiomas and that loss of KMT2C promotes proliferation and invasion in NF

Meningeal blood vessel blockage enhances anti-glioblastoma immunity.

The dura mater, the outermost meningeal layer that samples and presents central nervous system (CNS)-derived antigens, is a pivotal interface for CNS immunosurveillance. Here, we show that meningeal blood vessel blockage effectively suppresses glioblastoma (GBM) progression in murine models. Single-cell profiling of dura reveals a resident border-associated macrophage (rBAM) subset characterized by high neonatal Fc receptor expression, which endows rBAMs with superior capacity for presenting tumor antigens and activating CNS-patrolling T cells. Meningeal blood vessel blockage preserves dural c

Causal Pathways Linking Gut Microbiota, Serum Metabolites, and Meningioma Risk: A Mendelian Randomization Analysis.

OBJECTIVES: Meningioma is a common tumor of the adult central nervous system (CNS), but its origin remains unclear. Increasing evidence suggests that the gut microbiota affects CNS disorders through the "microbiota-gut-brain axis," yet its link to meningioma is still uncertain. This study used Mendelian randomization (MR) to explore causal relationships between gut microbiota, serum metabolites, and meningioma, and to investigate potential mediation by serum metabolites. METHODS: A two-sample MR framework was applied using publicly available genome-wide association study data on 473 gut micro

High frequency and unique subtypes of meningioma in patients with BAP1 tumor predisposition syndrome.

PURPOSE: BAP1-tumor predisposition syndrome (BAP1-TPDS) is associated with four main cancers: uveal melanoma, cutaneous melanoma, malignant mesothelioma, and renal cell carcinoma. However, additional cancers are found more rarely in BAP1-TPDS patients. The aim of this study was to investigate the association, clinical, and pathologic characteristics of meningioma in BAP1-TPDS. METHODS: We conducted a retrospective chart review of meningiomas in two independent cohorts of patients with germline BAP1 pathogenic or likely pathogenic (P/LP) variants at The Ohio State University Wexner Medical Cent

Integrative CSF profiling identifies disease-specific immune responses in leptomeningeal disease.

Leptomeningeal disease (LMD) is a devastating manifestation of advanced cancer, marked by rapid neurological decline and limited treatment options. Immune profiling in central nervous system (CNS) neoplasms, including LMD, is critical for understanding disease biology and guiding therapy. Here, we use single-cell RNA and T cell receptor (TCR) sequencing of cerebrospinal fluid (CSF) from patients with CNS lymphoma (CNSL), brain metastases (BrMs), and glioblastoma (GB), alongside deep TCR sequencing of blood and spatial transcriptomics of brain lesions. We uncover distinct, disease-specific CSF

Laser interstitial thermal therapy for intracranial dural-based lesions: A single center case series of 20 patients.

PURPOSE: Laser interstitial thermal therapy (LITT) is a minimally invasive alternative to craniotomy for intracranial lesions in patients who are poor surgical candidates, but its role in treating dural-based lesions remains underexplored. This study evaluates the safety and feasibility of LITT for dural-based lesions. METHODS: We retrospectively reviewed 431 LITT cases at our institution and identified 20 patients who underwent LITT for 22 dural-based lesions (two patients received double LITT for two separate lesions). Primary outcomes included postoperative complications, neurologic defici

Pediatric Fourth Ventricular Atypical Meningioma With a Whorling-Sclerosing Pattern: A Report of a Rare Tumor.

Meningiomas are rare in the pediatric age group and also very rare in the fourth ventricle. We describe a report of an 11-year-old boy who presented with frequent involuntary movements and an imbalance while walking. His MRI scan showed a well-defined T2 and FLAIR hyperintense lesion in the fourth ventricle, suspected to be a medulloblastoma. Post-excision of the tumor, histology showed a brain invasive atypical meningioma with a peculiar whorling-sclerosing architecture. The tumor cells were positive for EMA, PR, SSTR2A and showed retained MTAP and H3k27me3 expression. The MKI67 index was 8%
