Isolated Maxillary Extracranial Metastasis of a Suprasellar Germinoma.

Intracranial germinomas are exquisitely radiosensitive, and a tissue diagnosis obtained by minimal biopsy followed by radiotherapy (with or without chemotherapy) provides excellent long-term control; aggressive resection adds morbidity without oncological benefit. We report a 20-year-old man whose sellar and suprasellar mass was preoperatively interpreted as a pituitary adenoma and was treated by aggressive endoscopic endonasal transsphenoidal resection after an intraoperative frozen section suggested adenoma; final histology demonstrated germinoma. The postoperative course was complicated by

Outcomes of endoscopic and microscopic transsphenoidal pituitary surgery: evidence from a systematic review, meta-analysis, and institutional experience.

To compare surgical outcomes between endoscopic and microscopic transsphenoidal pituitary surgery using international comparative evidence and an institutional cohort. A PRISMA-based systematic review and meta-analysis of comparative transsphenoidal pituitary surgery studies was performed. Gross total resection was the primary endpoint. Secondary outcomes included operative time, intraoperative blood loss, cerebrospinal fluid leak, epistaxis, meningitis, diabetes insipidus, SIADH, and visual outcomes. Fixed- and random-effects models were applied. Fifty-six comparative studies including 26,295

Successful pre-operative endovascular intraventricular meningioma embolization: a systematic review and pediatric case analysis.

BACKGROUND: Intraventricular meningiomas are rare tumors accounting for a small fraction of intracranial meningiomas and are often associated with significant surgical challenges due to their deep location and rich vascular supply. Pre-operative endovascular embolization has been proposed as an adjunct to reduce intraoperative bleeding and facilitate safer resection; however, its role in intraventricular lesions remains poorly defined. METHOD: We present a case of a 9.5-year-old boy with a highly vascular intraventricular meningioma who underwent pre-operative endovascular embolization follow

Multidisciplinary management for watertight surgical closure of pneumatized mastoid breach using fascia lata graft, following neurosurgical retrosigmoid approach: a technical note.

The retrosigmoid approach is a standard surgical corridor to the posterior fossa but may lead to mastoid air cell opening and subsequent cerebrospinal fluid (CSF) leakage or infectious complications. Achieving durable closure can be challenging, particularly in revision surgery due to inflammatory changes and altered anatomical landmarks. We report the case of a 76-year-old patient who developed recurrent infectious complications following a right retrosigmoid approach for posterior fossa meningioma. Imaging revealed a subcutaneous abscess associated with persistent mastoid air cell opening. A

Beyond the blood-brain barrier: humanised mice, the missing link in glioblastoma research.

Glioblastoma (GBM) remains a major challenge in neuro-oncology, associated with a high rate of mortality despite decades of intensive research and therapeutic advancements, underscoring the urgent need for innovative preclinical platforms that can more accurately recapitulate the biological and pathological features of human disease. While conventional animal models have contributed to our understanding of GBM biology and the evaluation of treatment efficacy, they fail to capture the full complexity and heterogeneity of the tumour microenvironment (TME). Ex vivo models are associated with cert

Cognitive impairments associated with meningiomas and gliomas in adults: A systematic review.

OBJECTIVE: Meningiomas and gliomas are common adult brain tumors often linked to cognitive impairments. Meningiomas are frequently overlooked as a potential contributor to such impairments, due to their benign, extra-axial, and slow-growing nature. This systematic review compares the type and frequency of cognitive impairments associated with these brain tumors, prior to any therapeutic intervention. METHOD: Systematic search conducted in ProQuest, PubMed, Scopus, and Cochrane Library identified records on cognitive status of adults with untreated, nonrecurrent gliomas or meningiomas, publish

Residual Tree Damage During Forest Operations: a Comprehensive Review of Operational, Silvicultural, and Human-related Factors.

PURPOSE OF REVIEW: Minimizing residual tree damage during mechanized forest operations is vital for sustainable forest management since this damage can affect stand structure, timber quality, and long-term forest health. Although various studies have examined tree damage factors, evidence remains fragmented across regions and systems. The review synthesizes the state of knowledge of residual tree damage during forest operations, identifying how operational, silvicultural, and human factors have been reported to influence damage characteristics and synthesize mitigation strategies. RECENT FIND

Neuroepithelial Tumor with AAV Integration after Intracisternal Magna Vector Delivery.

Recombinant adeno-associated virus (AAV) vectors are predominantly nonintegrating, but rare genomic integration events have been associated with oncogenesis in neonatal murine models. Here we report a case of a neuroepithelial tumor that developed in a 5-year-old boy with severe mucopolysaccharidosis type I (MPSI, Hurler subtype) 4 years after intracisternal magna administration of AAV serotype 9 gene therapy. The patient underwent successful resection of the primary tumor. Postoperatively, he has continued to have advanced cognitive function for his age, a finding that indicates mitigation of

When sulcal enhancement is not leptomeningeal metastasis: diagnostic value of T1-T2FLAIR matching for differentiating benign contrast leakage.

The incidence and imaging characteristics of non-neoplastic sulcal enhancement on contrast-enhanced T2-weighted fluid-attenuated inversion recovery (CE-T2FLAIR) imaging in routine neuro-oncologic magnetic resonance imaging (MRI) were investigated. Contrast-enhanced T1-T2FLAIR sequence matching was used to differentiate benign contrast leakage from leptomeningeal metastasis (LMS). This retrospective, single-center study included 337 patients who underwent contrast-enhanced brain MRI for evaluation of suspected intracranial metastasis. Sulcal enhancement on CE-T2FLAIR was classified as absent, c

Proteomic Analysis Identifies ATE1-Dependent Arginylation Dysregulation across Meningioma Grades.

Meningiomas are the most common primary brain tumors, yet the molecular pathways that distinguish grade 1 from grade 2 lesions remain insufficiently understood. Among post-translational modifications, N-terminal arginylation─catalyzed by ATE1─regulates protein stability and cellular stress responses, but its role in meningioma biology has not been explored. Here, we integrated mass-spectrometry-based proteomics, immunoblotting, and transcriptomic reanalysis to investigate pathway regulation across tumor grades. Grade 1 meningiomas displayed higher ATE1 expression and increased arginylation of

"Chipped but not broken" - patients' symptoms and challenges beyond five years post radiotherapy for meningioma: a qualitative interview study.

BACKGROUND: Meningioma, the most prevalent brain tumor in adults, is predominantly benign and often treated with radiotherapy and/or surgery, generally with positive treatment outcomes. Radiotherapy can be administered using either conventional radiotherapy (CRT) or proton beam therapy (PBT). The treatment can cause chronic symptoms such as headache, fatigue, and neurocognitive impairments that can significantly affect daily life. However, there is a lack of research on the long-term consequences of treatment, particularly concerning patients' own experiences of related symptoms. Therefore, th

Optic neuritis and meningoencephalitis associated with an ovarian teratoma in a dog.

A 3-year-old intact female Pit Bull Terrier mix-breed dog was examined because of acute vision loss. Ophthalmologic examination was consistent with bilateral optic neuritis. Abnormalities were not detected on the remainder of the neurological examination. Brain MRI identified multifocal lesions involving the optic nerves and cortical gray matter, while cerebrospinal fluid analysis demonstrated mixed pleocytosis. Abdominal ultrasonography revealed a cystic ovarian mass. Ovariohysterectomy was performed and histopathologic examination was consistent with an ovarian teratoma. Neural tissue within

Upregulation of TNFR2 Precedes TOX Expression by Exhausted T cells and Restricts Antitumor and Antiviral Immunity.

PURPOSE: Exhaustion represents a collection of programmed T-cell differentiation states and an important mode of T-cell dysfunction. T-cell progression from progenitor to terminal exhaustion is associated with upregulation of the transcription factor thymocyte selection-associated HMG box (TOX). Our understanding of factors regulating TOX expression and the transition from progenitor to terminal exhaustion, however, remains incomplete. EXPERIMENTAL DESIGN: Single-cell RNA sequencing was used to evaluate the expression of TNF receptors on human and murine tumor-infiltrating CD8+ T cells. Flow
