Cellular protein profiles of the Hodgkin's disease cell lines L428, KM-H2 and HDLM-2: a comparative study.

The origin of the malignant mononuclear Hodgkin's cell and the classic Reed-Sternberg cell in Hodgkin's disease (HD) remains controversial despite extensive immunohistological and lymphoid gene analysis. Sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) in combination with protein staining and radioactive labelling has not been fully exploited in analysing the HD-derived cell lines available. The NP40 solubilised cellular proteins from the three HD cell lines, L428, KM-H2 and HDLM-2 were analysed using these techniques and compared with other haemopoietic cell lines and leu

International Database on Hodgkin's Disease: a cooperative effort to determine treatment outcome.

In an effort to better determine the outcome of patients treated for Hodgkin's disease, twenty institutions/cooperative groups in the western world agreed in 1988 to participate in the development of a large data base by sending data to be analyzed by a common statistical design. Data was obtained from both patients randomized on prospective trials and patients treated by standard protocol but not randomized. Overall statistics on 14,315 adult patients treated from the early 1960s to 1987 were collected. This data was first presented at the Paris International Workshop and Symposium held on Ju

Type II essential mixed cryoglobulinaemia: presentation, treatment and outcome in 13 patients.

The long-term clinical course of patients with primary Type II essential mixed cryoglobulinaemia is unclear as many reports fail to separate this group from patients with Type III disease. We have reviewed 13 patients with Type II essential mixed cryoglobulinaemia who presented to the Hammersmith Hospital between 1976 and 1990. All patients had a cryoglobulin level greater than 0.1 mg/ml (range 0.27-6.50 mg/ml), and characterization of the cryoglobulin in all cases revealed the presence of a monoclonal IgM kappa component with rheumatoid factor activity together with polyclonal IgG. All patien

Immunolocalization of the Bcl-2 protein within hematopoietic neoplasms.

The Bcl-2 proto-oncogene was discovered at the t(14;18) breakpoint found in most follicular B-cell lymphomas and some diffuse large-cell lymphomas. Bcl-2 is unique among proto-oncogenes, being localized to mitochondria and extending cell survival by blocking programmed cell death. We examined Bcl-2 protein expression in 82 hematologic malignancies and reactive lymphoid processes. All lymphomas with Bcl-2 rearrangement demonstrated high levels of Bcl-2 protein. However, most follicular and diffuse lymphomas without Bcl-2 rearrangement also displayed intense Bcl-2 staining. In these cases, mecha

High-dose chemotherapy containing busulfan followed by bone marrow transplantation in 24 children with refractory or relapsed non-Hodgkin's lymphoma.

Twenty-four children with relapsed or refractory non-Hodgkin's lymphoma underwent high-dose chemotherapy (HDC) with bone marrow transplantation (BMT). HDC comprised in all cases busulfan (16 mg/kg or 600 mg/m2), with either cyclophosphamide (200 mg/kg or 4.4 g/m2) and/or melphalan (140 mg/m2). Twenty-three of these children had received second-line therapy before receiving HDC. There were 16 B cell and eight T cell lymphomas. Twenty-three patients were evaluable at day 30 post-BMT; 19 were in complete remission, four did not respond. Eight patients are long-term survivors between 62 and 296 we

[Angioedema due to acquired complement-C1-inhibitor deficiency in a female patient with non-Hodgkin lymphoma and autoimmune hemolytic anemia].

A case of angioedema due to acquired deficiency of the regulatory protein C1-esterase-inhibitor (C1-INH) is reported. The edematous attack occurred 3 1/2 weeks after initiation of successful therapy for autoimmune-hemolytic anemia in the course of long-standing non-Hodgkin's lymphoma. At the time of acute edema the complement profile was typical: virtual absence of C1-INH function was associated with diminished concentrations of the components of the classical pathway of complement (C1q, C1r, C1s, C2, C4) and reduced complement hemolytic activity (CH50). Anti-C1-INH-autoantibodies were not det

Aggressive non Hodgkin lymphoma in the elderly. A retrospective study of 72 patients with clinical features and treatment.

Aggressive non Hodgkin lymphoma (NHL) occurs frequently in the elderly and because of drug related toxicity treatment remains controversial. Seventy-two patients over 65 years (68-83 y, median 73 y) with intermediate and high grade NHL were retrospectively studied. We found 22 cases of high grade NHL and diffuse large cell the most frequent type encountered (54%). Seven patients had stage I, 8 stage II, 14 stage III, and 43 stage IV disease. Treatment for localized disease was radiotherapy or chlorambucil, and disseminated stage patients were treated by three different chemotherapy regimens, g

Cerebral lymphomas: review of 70 cases.

Seventy cerebral lymphomas from 37 men and 33 women were reviewed and reclassified using the modified Kiel classification. Fifty two of the patients were over the age of 50 years (age range 23-85 years). Only two patients were known to be immune deficient--one case of AIDS and one renal transplant recipient. Necropsies only were performed on 31 cases, biopsies only in 23 cases, and both biopsies and necropsies in 16 cases. Systemic tumour was found in 16 of 47 necropsy cases. All the tumours available for review were classic diffuse non-Hodgkin's lymphomas. Where immunocytological studies were

[Non-Hodgkin's lymphomas. Results of alternating chemotherapy versus sequential chemotherapy].

An analysis is presented of the results attained in 115 cases of advanced non-Hodgkin's lymphoma (stages III and IV) with high or intermediate grade histology (with exception of lymphoblastic lymphoma) which had received no previous treatment. The patients were distributed at random into two groups, A and B. Patients in group A received the classical CHOP-Bleo regimen, and they were given six courses at three-week intervals. The patients of group B received alternate chemotherapy consisting of two CHOP-Bleo courses, two CMED (cyclophosphamide, methotrexate, etoposide, dexamethasone) courses, a

Ki-M6 immunostaining in routinely processed sections of reactive and neoplastic human lymphoid tissue.

A monoclonal antibody, termed Ki-M6 (CD68), which shows a restricted reactivity to cells of the monocyte/macrophage system, has been evaluated primarily with the use of cryostat sections. In this study the authors could assess that the Ki-M6 antibody recognizes a fixation-resistant epitope in most human macrophages. The Ki-M6 immunoreactivity with monocyte/macrophage-related cells was established by testing on routinely processed samples of reactive and neoplastic lymphoid tissues; it was compared with the staining for vimentin (V9) and S-100 protein antibodies, with visualization of the stati

Association of bcl-1 rearrangements with lymphocytic lymphoma of intermediate differentiation.

Previous studies using classical cytogenetics have demonstrated the presence of the t(11;14) (q13;q32) chromosomal translocation in some cases of lymphocytic lymphoma of intermediate differentiation (IDL), a distinct type of low grade B-cell lymphoma. This finding suggested that the bcl-1 region (located at band q13 of chromosome 11) might be involved in this neoplasm. Using a genomic probe from the major breakpoint area of the bcl-1 locus, we identified rearrangements of the bcl-1 region in 10 of 19 cases, 2 of which comigrated with a rearranged allele of the immunoglobulin heavy chain gene j

Induction and progression of human lymphoproliferative lesions by Epstein-Barr virus.

Epstein-Barr virus (EBV) is involved in numerous lymphoproliferative diseases. In addition to classical lesions such as endemic Burkitt's lymphoma and infectious mononucleosis, there are other disorders of the lymphoid system that are discussed in relation to EBV: B-cell lymphomas in immunosuppressed individuals. Hodgkin's disease and, to some extent, primary extranodal lymphomas. Studies of the EBV expression in classical and nonclassical lesions could lead to the better understanding of different EBV mechanisms in lymphomagenesis.

Pathology of Hodgkin's disease: anything new?

This presentation deals with the gross and microscopic pathology of HD. Recent advances in immunohistochemistry, gene rearrangement studies and cell culture are not discussed, except where they shed light on the pathology. Clinical and pathological experience, over the past 2 decades, suggests that HD should be divided into six subtypes, as originally proposed by Lukes et al. (1966b), rather than the four subtypes included in the Rye classification. Nodular lymphocyte/histiocyte predominant HD forms a clinicopathological entity separate from the other subtypes. It most frequently presents at a

Pseudo-Gaucher cells preceding the appearance of immunoblastic lymphoma.

A 49-year-old white woman who presented with multiple lytic bone lesions was found to have Gaucher-like storage cells in multiple bone marrow aspirates and a percutaneous bone marrow biopsy several months before the development of overt disseminated lymphoma. Open bone biopsies of three different sites at initial presentation revealed only necrotic debris and a nondiagnostic polymorphic infiltrate. Elevated leukocyte beta-glucocerebrosidase levels ruled out the diagnosis of classic Gaucher's disease. The ultrastructural characteristics of these storage cells were found to be identical to those

Response of rheumatoid arthritis to chemotherapy for Hodgkin's disease in a patient with IgA deficiency and overlap connective tissue disease.

A patient with IgA deficiency presented with classical rheumatoid arthritis (RA) and subsequently, features of multiple other connective tissue diseases (CTD). When Hodgkin's disease later developed the patient received combination chemotherapy, which induced remission of both neoplastic and connective tissue diseases. As the RA remained in remission for three years this case may offer insight into a mode of use of cytotoxic agents in CTD.

Lectin activity as a marker for Hodgkin disease cells.

Treatment of cultured Hodgkin disease (HD) cells with neuraminidase results in decreased reactivity of monoclonal antibody VIM-D5 with its antigen, the X hapten, a fucosyl-N-acetyllactosamine. The other feature characteristic of HD cells is the expression of high levels of ectosialyltransferase activity. We present evidence for a cause-effect relationship between these two findings in that VIM-D5 antigenicity can be restored on neuraminidase-treated HD cells by modulating transferase activity. This can be interpreted in terms of a lectin activity of the ectosialyltransferase that binds the X h

[Immune thrombocytopenia in Hodgkin's disease].

Thrombocytopenic purpura of the idiopathic thrombocytopenic purpura (ITP) type is an unusual and poorly documented complication of Hodgkin's disease. In an eight year period, three patients out of 140 with Hodgkin's disease developed thrombocytopenia; two of them presented a clinical picture undistinguishable from classic ITP. In both platelet antibody could be demonstrated. The third patient developed thrombocytopenia during an acute exacerbation of the underlying disease. Successful therapy of thrombocytopenia was achieved by methylprednisolone alone in one case and by cyclophosphamide in co

[Coronary stenosis after radiotherapy. Clinical study of 5 cases and review of the literature].

Coronary artery stenosis is one of the possible complications of radiotherapy to the mediastinum. Although less frequent than pericardial disease, anatomopathological studies have shown it not to be uncommon. Five cases with different clinical presentations are reported and the 30 previously described cases are reviewed. Radiotherapy was performed for Hodgkin's disease in 70% of cases and for carcinoma of the breast in 10% of cases. The average delay before onset of the symptoms was 4 years but in some cases delays of up to 10 years were observed. The most common presentation was an inaugural

Lymphoproliferative malignancy in rheumatoid arthritis: a study of 20 cases.

A series of 20 patients with definite or classical rheumatoid arthritis who subsequently developed a lymphoproliferative malignancy are described. The mean time between the onset of the 2 diseases was 13.2 years. A wide range of types of non-Hodgkin's lymphoma and Hodgkin's disease were found; there were no unusual histological features in the lymphomas. Although many of the patients had had gold, penicillamine, and other second-line drugs, none of them had received cytotoxic drugs, and there was no evidence that therapy was a cause of their malignancies. The likely cause of the association is

A case of large "thymic granuloma". Neoplasm of T-zone histiocyte.

An unusual case of classic thymic granuloma is reported. A 16-year-old boy presented with a huge mediastinal mass and the superior vena caval syndrome. Irradiation and combination chemotherapy induced complete remission, despite several episodes of lymph node recurrence. Examination of the first biopsy specimen was considered to indicate a histiocytic tumor or malignant form of eosinophilic granuloma, because of the diffuse proliferation of histiocytes and marked infiltration of eosinophils. Late involvement in the lymph node appeared to have the same histologic characteristics as the mediasti
