CAR T-cell therapy for relapsed and refractory B-cell non-Hodgkin lymphoma: a single-centre 5-year real-world experience study at Ghent University Hospital.

BACKGROUND: Relapsed or refractory (R/R) B-cell non-Hodgkin lymphoma (B-NHL) remains challenging to treat, particularly in patients ineligible for high-dose chemotherapy and autologous stem cell transplantation. Chimeric antigen receptor (CAR) T-cell therapy has emerged as a promising treatment. AIMS: We describe the epidemiology, demographics and outcomes of patients receiving CAR T-cell therapy for R/R DLBCL (including transformed follicular lymphoma [TFL]), high-grade B-cell lymphoma (HGBCL) and mantle cell lymphoma (MCL) at Ghent University Hospital between 2019 and 2024. We assessed asso

Retained epcoritamab activity despite routine CD20 assay negativity approximately one terminal half-life after mosunetuzumab in B-cell lymphoma.

CD20 testing shortly after CD20-directed therapy can be difficult to interpret. We report an 89-year-old woman with multiply relapsed B-cell lymphoma who underwent excisional biopsy of an enlarging right axillary lymph node 17 days after the last dose of mosunetuzumab. A prior lymph node biopsy showed follicular lymphoma grade 3A with preserved CD20 expression. The relapse specimen showed diffuse large B-cell lymphoma of the germinal center B-cell subtype. Immunohistochemistry using the CD20 clone L26 failed to detect CD20 expression, whereas PAX5 and CD79a remained positive. Flow cytometry sh

Pathological Diagnosis of Nodal T Follicular Helper Cell Lymphoma, Angioimmunoblastic Type.

We herein review the pathological diagnosis of nodal T follicular helper (TFH) cell lymphoma, angioimmunoblastic type (nTFHL-AI), a mature T-cell neoplasm with a TFH phenotype that was previously designated as angioimmunoblastic T-cell lymphoma (AITL). The history of AITL from its discovery to the WHO 5th classification, which includes Japanese pathologists' and hematologists' contributions, is discussed. We then focus on what true neoplastic T cells of nTFHL-Al are in histopathology. Since the number of neoplastic cells is small in some cases of nTFHL-AI, the findings of recent single-cell an

Diffuse Large B-Cell Lymphoma Presenting as an Infiltrative Masticator Space Lesion Mimicking Inflammatory Disease: A Case Report.

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma in adults. Although extranodal involvement is frequent, presentation as an infiltrative lesion of the masticator space is rare and may mimic inflammatory, odontogenic, salivary gland, or soft tissue disorders, leading to diagnostic delay. We report the case of a 34-year-old woman who presented with progressive right mandibular swelling, facial paresthesia, trismus, weight loss, and night sweats. Initial computed tomography suggested an inflammatory process, whereas magnetic resonance imaging revealed an in

How brain pulsations drive solute transport in the cranial subarachnoid space: insights from a toy model.

Hepatitis B virus (HBV) infection has been associated with an increased risk for B-cell lymphomas. We previously showed that 20% of diffuse large B-cell lymphoma (DLBCL) patients from China, an endemic area of HBV infection, have chronic HBV infection (surface antigen-positive, HBsAg+) and are characterized by distinct clinical and genetic features. Here, we showed that 24% of follicular lymphoma (FL) Chinese patients are HBsAg+. Compared with the HBsAg- FL patients, HBsAg+ patients are younger, have a higher histological grade at diagnosis, and have a higher incidence of disease progression w

Population-wide introduction of dose-adjusted EPOCH-R in high-grade B-cell lymphoma with MYC/BCL2 rearrangements, DLBCL morphology.

High-grade B-cell lymphoma with "double-hit" MYC and BCL2 rearrangements (HGBCL-DH-BCL2) is associated with poor outcomes following standard chemoimmunotherapy, prompting dose-intensive regimen use. However, the benefit of intensification is unclear due to rarity precluding randomized trials, and selection bias in retrospective comparisons. In 2015, BC Cancer introduced a provincial guideline recommending dose-adjusted etoposide, prednisone, vincristine, cyclophosphamide, doxorubicin, and rituximab (DA-EPOCH-R) for fit patients aged ≤75 years with HGBCL-DH-BCL2 identified by routine cytogeneti

Investigating rituximab-induced infusion-related reactions in patients with non-Hodgkin lymphoma, with a focus on follicular lymphoma: a retrospective observational study.

INTRODUCTION: The incidence of infusion-related reactions (IRRs) caused by rituximab is very high, and many studies have explored factors related to the onset of IRR; however, most analyses have been conducted on a mixed range of disease indications. This study aimed to investigate rituximab-induced IRR in patients with non-Hodgkin lymphoma (NHL), particularly follicular lymphoma (FL), and examine the factors related to its occurrence. METHODS: Data on sociodemographic and blood test findings were collected on the closest possible date prior to rituximab administration. Cytokine/chemokine leve

Assessing Trends in Conditional Survival for Non-Hodgkin Lymphoma and Chronic Myeloid Leukemia via Novel Extensions to the Joinpoint Survival Model.

BACKGROUND: Five-year relative survival for non-Hodgkin lymphoma (NHL) and chronic myeloid leukemia (CML) has improved, reflecting therapeutic advances, but prognosis beyond 5 years after diagnosis is less understood. We introduced a method to estimate trends in conditional survival, the probability of further survival given survival to a specific time. METHODS: We developed a landmarked joinpoint survival (JPSurv) model that uses data beginning at a prespecified follow-up interval. We apply this approach to NHL (overall and by major subtype) and CML survival data from the Surveillance, Epide

Biological heterogeneity in adult IRF4-rearranged large B-cell lymphoma: Stratification by age and anatomical site.

Large B-cell lymphoma (LBCL) with IRF4 rearrangement (LBCL-IRF4-R) is a rare subtype predominantly diagnosed in children and young adults. Whether adult LBCL cases with IRF4 rearrangement (IRF4-R) should be classified as LBCL-IRF4-R remains unclear. Clinicopathological and molecular features of 61 adult LBCL cases with IRF4-R were analyzed and compared to diffuse large B-cell lymphoma, not otherwise specified (DLBCL, NOS), to assess biological heterogeneity. The 61 cases grouped by patient age and site were classified as follows: Group 1 included 13 patients aged ≤40 years, whose features supp

Recurrent Urinary Tract Infections in a Patient with Diffuse Large B-Cell Lymphoma and Severe COVID-19: A Single Case of Suspected Immunosuppression Where Antibacterial Therapy Was Not Enough.

Background: Non-Hodgkin lymphoma (NHL) is a malignancy of the immune system that includes several subtypes, most commonly diffuse large B-cell lymphoma and follicular lymphoma. Its etiology is multifactorial, with risk factors such as immunosuppressive therapy, infections, chemical exposure, and advanced age. A key aspect is the bidirectional relationship between lymphoma and immunodeficiency, which increases susceptibility to recurrent infections and complicates disease management. Case presentation: One particularly challenging case during the COVID-19 pandemic involved a patient with a pers

Clinicopathological Spectrum of EBV-Related Primary Splenic Tumors Identified by Splenectomy: A Case Series.

Background: Epstein-Barr virus (EBV)-related primary splenic tumors are exceptionally rare and encompass a heterogeneous group of entities, including inflammatory pseudotumor (IPT), IPT-like follicular dendritic cell (FDC) tumors or sarcomas, and EBV-positive diffuse large B-cell lymphoma (DLBCL). Because clinical presentation and imaging findings are often nonspecific, establishing a definitive diagnosis remains challenging and frequently necessitates splenectomy for histopathologic confirmation. Methods: We retrospectively reviewed patients who underwent laparoscopic splenectomy for suspecte

Robust Multimodal Deep Learning for Lymphoma Subtype Classification Using 18F-FDG PET Maximum Intensity Projection Images and Clinical Data: A Multi-Center Study.

Background: Previous attempts to classify lymphoma subtypes based on metabolic features extracted from 18F-FDG PET imaging have been hindered by inconsistencies in imaging protocols, scanner types, and inter-institutional variability. To overcome these limitations, we propose a multimodal deep learning framework that integrates harmonized PET imaging features with structured clinical information. The proposed framework is designed to perform hierarchical classification of clinically meaningful lymphoma subtypes through two sequential binary classification tasks. Methods: We collected multi-cen

Prognostic significance of PD-L1/PD-1 co-expression and CXCR3-driven inflammatory signatures in Egyptian patients with lymphoproliferative neoplasms.

BACKGROUND: Lymphoproliferative neoplasms (LPNs) such as chronic lymphocytic leukemia and non-Hodgkin lymphomas are clinically heterogeneous and frequently associated with recurrence and poor outcomes. Immune checkpoint markers, including programmed death-ligand 1 (PD-L1) and programmed cell death protein 1 (PD-1), as well as C-X-C motif chemokine receptor 3 (CXCR3) and inflammation-based indices [systemic immune-inflammation index (SII) and systemic inflammation response index (SIRI)], have shown promise in risk stratification but remain underexplored in Egyptian populations. AIM: To assess

Gemcitabine, dexamethasone and cisplatin with or without rituximab is highly effective as a mobilization regimen in relapsed or refractory lymphoma.

INTRODUCTION: A combination of Gemcitabine, Dexamethasone and Cisplatin (GDP) with or without rituximab (±R) is an outpatient treatment widely used for relapsed or refractory (R/R) lymphoma. Limited data are available on stem cell mobilization following GDP treatment. METHOD: A retrospective multicenter study was conducted from January 1st, 2014, to December 31st, 2020. Patients received GDP±R before autologous stem cell transplantation (ASCT) and were mobilized with either GDP±R or intermediate-dose cyclophosphamide (ID-CY). The primary objective was to determine the percentage of patients a

From Widespread Cutaneous to Systemic Nodal Involvement: Serial FDG PET/CT Findings in Follicular Lymphoma.

A 57-year-old woman with follicular lymphoma (FL) presented with diffuse cutaneous and subcutaneous involvement on baseline 18 F-FDG PET/CT. Following therapy, cutaneous uptake largely resolved; however, follow-up PET/CT demonstrated new, widespread confluent hypermetabolic lymphadenopathy, indicating systemic progression. This striking post-therapy redistribution highlights the importance of serial whole-body PET/CT in atypical FL.

Differential role of CREBBP missense and truncating mutations in germinal center development and lymphomagenesis.

Truncating and missense mutations of the CREBBP gene are highly prevalent in follicular lymphoma (FL) and diffuse large B-cell lymphoma (DLBCL), the most common lymphoid malignancies. These mutations are acquired early during tumor evolution by a common precursor cell (CPC) and lead to either complete protein loss or single amino acid substitutions in the lysine acetyltransferase (KAT) domain. As a result, CREBBP is impaired in its ability to acetylate enhancer histones and nonhistone proteins implicated in the germinal center (GC) reaction, the structure from which these tumors originate.
