Bilateral cranial polyneuropathy associated with an intracranial mass in a cat with concurrent tonsillar lymphoma.

A 14-year-old spayed female Siamese cat was presented with signs consistent with a rapidly progressive cranial polyneuropathy. Magnetic resonance imaging of the brain revealed an extra-axial mass at the calvarial floor, extending caudally to involve bilateral trigeminal nerves and rostrally to involve the pituitary gland and optic chiasm. Additional findings included contrast-enhancing content within the bilateral tympanic bullae, mild mandibular and medial retropharyngeal lymphadenopathy, and a left tonsillar mass. Cytologic and histopathologic findings confirmed tonsillar lymphoma. The concu

Fulminant Synchronous Epstein-Barr Virus-Positive MALT Lymphoma and Diffuse Large B-Cell Lymphoma Monomorphic Post-Transplant Lymphoproliferative Disorder in a Child Following Heart Transplant: A Case Report.

BACKGROUND: Post-transplant lymphoproliferative disorder (PTLD) is a devastating complication following solid-organ and hematopoietic stem cell transplantation. While PTLD is often effectively managed with immunotherapy or immuno-chemotherapy, rare cases present in a fulminant manner and are rapidly life-threatening. METHODS: We describe a case of an Epstein-Barr virus (EBV)-negative 16-year-old who developed EBV DNAemia and rapidly fulminant monomorphic PTLD within five months of receiving a heart transplant from an EBV-positive donor. RESULTS/IMPORTANT CLINICAL FINDINGS: Three months after

Immunohistochemistry for c-MET, HLA class II, and CD68 does not show prognostic value in early-stage classic Hodgkin lymphoma patients in the EORTC-GELA H9 intergroup randomised trial.

BACKGROUND: In classic Hodgkin lymphoma (cHL), identifying high-risk patients is crucial for tailored treatment. Clinical risk factors are useful, but there is room for improvement. Although many diagnostic tissue-based biomarkers have been described to correlate with prognosis, none have achieved the accuracy to be included into daily clinical practice. METHODS: The most promising tissue biomarkers, c-MET, HLA class II, and CD68, were tested in early-stage cHL for prognostic validation, individually and in combination with each other and established clinical risk factors. All biomarkers were

Translocation-induced enhancer rewiring reveals cryptic oncogenic circuits in multiple myeloma.

Immunoglobulin heavy chain (IGH) enhancer translocations drive multiple myeloma (MM), but routine diagnostics resolve only 32 to 38% of cases. Here, we introduce TransFinder, a multiomic pipeline integrating SMRT long-read sequencing (LRS), Hi-C, H3K27ac ChIP-seq/CUT&Tag, and RNA-seq to systematically resolve cryptic translocations and their hijacked oncogenes by deconvoluting enhancer-promoter neo-loops at rearrangement breakpoints. Applied to MM, TransFinder identified that t(16;22)(q23;q11), a canonical translocation known to activate MAF, also hijacks enhancers to activate RAB36. It furthe

HIV and Antiretroviral Therapy Influence Tumor Microenvironment Architecture, Subclonal Evolution, and Immune Evasion Strategies in Diffuse Large B-Cell Lymphoma.

The tumor immune microenvironment (TIME) and tumor heterogeneity shape prognosis and therapeutic response in Diffuse Large B-cell Lymphoma (DLBCL), but HIV-associated (HIV+) DLBCL remains poorly understood. To characterize the TIME and tumor-intrinsic features of DLBCL across a spectrum of HIV-associated immune dysregulation, we applied imaging mass cytometry (IMC), spatial transcriptomics (ST), and whole exome sequencing to 61 pre-treatment DLBCL biopsies, stratified by HIV status and antiretroviral therapy (ART) exposure. By IMC, a tumor-immune hot neighborhood associated with inferior survi

Emergency diagnosis of multiple myeloma: a SEER-Medicare study.

PURPOSE: Non-specific presenting symptoms make multiple myeloma (MM) one of the most difficult hematologic cancers to diagnose, increasing the risk of late diagnosis and emergency presentation (EP). We examined the frequency of EP in MM, estimated associations between EP and patient-level factors, and MM symptom-related healthcare utilization in the year before diagnosis. METHODS: We studied Medicare beneficiaries age ≥ 66 years diagnosed with MM between 2008 and 2017. Emergency presentation (EP) was defined as diagnosis preceded by a recent ED visit, sub-classified as: inpatients (EP-IP) for

Gustatory dysfunction under talquetamab therapy in multiple myeloma: a cross-sectional study of patient-reported course, nutritional changes, and quality of life.

The purpose of this study is to characterize patient-reported taste disturbances, recalled symptom course, nutritional changes, and quality of life in patients with relapsed/refractory multiple myeloma treated with talquetamab. This single-center cross-sectional study combined taste-strip testing with a study-specific questionnaire, EORTC QLQ-C30, and PHQ-4 in 32 patients. Analyses used recorded aggregate correct-identification counts without a normative hypogeusia classification. Symptom timing and changes since treatment initiation were recalled at one assessment. All patients reported taste

Primary pulmonary mucosa-associated lymphoid tissue lymphoma presenting as a right hilar mass: A case report.

RATIONALE: Primary pulmonary lymphoma (PPL) is an extremely rare malignant lung tumor that is frequently misdiagnosed as pneumonia or lung cancer due to its atypical radiological and clinical presentation. The rationale of this report is to elucidate the diagnostic value of combining computed tomography (CT) and 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) in evaluating a rare presentation of PPL. PATIENT CONCERNS: A 44-year-old male was admitted after an incidental lung lesion was discovered during routine screening. He was completely asymptomatic,

Clinical Pharmacology of Lisocabtagene Maraleucel in B-cell Malignancies.

Chimeric antigen receptor (CAR) T-cell therapies represent a distinct therapeutic modality whose clinical activity is governed by in vivo cellular kinetics. Lisocabtagene maraleucel (liso-cel) is an autologous CD19-directed, 4-1BB CAR T-cell product in which in vivo expansion plays an important role in clinical pharmacology assessments. Across B-cell malignancies, characterization of liso-cel cellular kinetics demonstrates substantial interindividual variability in expansion, with no single evaluated clinical covariate exerting a clinically meaningful effect. Higher in vivo expansion has been

Production of anti-human cannabinoid receptor 2 (CB2) monoclonal antibody using non-viral vector induced human CB2 expressing myeloma as an immunogen.

BACKGROUND: Cannabinoid receptor 2 (CB2) of the cannabinoid system is predominantly expressed on immune cells and involved in a diverse range of immune functions. However, the role of CB2 in immunoregulation is still controversial. One of the challenges in the detailed characterization and functional study of CB2 is the lack of CB2-specific monoclonal antibodies (mAbs). OBJECTIVE: We aimed to produce mAbs against a native form of human CB2 using human CB2 expressing mouse myeloma cells as immunogens. METHODS: Non-viral vector expression system was used to generate stable human CB2-expressing

Patient preferences for treatment‑free intervals in multiple myeloma: mixed‑methods insights on monitoring trade‑offs and supportive care improvements.

PURPOSE: Continuous and maintenance therapy remains central to multiple myeloma (MM) care, but repeated treatment-related healthcare encounters may impose physical, psychosocial, and logistical burden. We explored patient preferences regarding intervals between active treatment contacts, monitoring trade-offs, and opportunities to improve inpatient and outpatient care. METHODS: A 12-month, single-center mixed-methods survey was conducted among 100 patients with MM at a German tertiary center. The survey assessed the perceived importance and preferred length of treatment-free intervals (TFIs),

Assessing clinical relevance of off-target effects observed at suprapharmacological concentrations of SGLT2 inhibitors. Reply.

In this study, using single-cell RNA-seq, cell mass spectrometry, flow cytometry, and functional analysis, we characterized the heterogeneity of polymorphonuclear neutrophils (PMNs) in cancer. We describe three populations of PMNs in tumor-bearing mice: classical PMNs, polymorphonuclear myeloid-derived suppressor cells (PMN-MDSCs), and activated PMN-MDSCs with potent immune suppressive activity. In spleens of mice, PMN-MDSCs gradually replaced PMNs during tumor progression. Activated PMN-MDSCs were found only in tumors, where they were present at the very early stages of the disease. These pop

Linvoseltamab outcomes by frailty status in patients with relapsed/refractory multiple myeloma: a subgroup analysis of the LINKER-MM1 study.

There is a paucity of data in frail patients with multiple myeloma (MM) receiving bispecific antibodies (bsAbs). In LINKER-MM1 (NCT03761108), linvoseltamab (human BCMA×CD3 bsAb) demonstrated high response rates and a generally manageable safety profile in relapsed/refractory (RR)MM. This LINKER-MM1 post hoc analysis evaluated outcomes of linvoseltamab 200 mg in patients categorized for frailty using the International Myeloma Working Group Frailty Proxy (IMWG-FP), Simplified Frailty Score (SFS), and patient-reported frailty phenotype (PRFP). As of July 23, 2024, 117 patients received linvoselta

Standardized FDG-PET interpretation during CAR T-cell therapy in R/R DLBCL: a DESCAR-T study.

PURPOSE: CAR T-cell therapy has changed the management of relapsed/refractory (R/R) Diffuse large B-cell lymphoma (DLBCL). Fluorodeoxyglucose Positron Emission Tomography Computed Tomography (PET/CT) plays a central role in lymphoma response assessment, but its prognostic use remains insufficiently standardized in this setting. METHODS: Patients from the French DESCAR-T registry treated in third line or beyond with commercial anti-CD19 CAR T-cells in real-life, and having centrally reviewed PET/CT before infusion, and at one month (M1) or three months (M3) post-infusion were included. The association between inborn errors of immunity and Hodgkin lymphoma-A register-based case-control study in Sweden.

Immune dysfunction is implicated in the aetiology of Hodgkin lymphoma (HL), but the association between inborn errors of immunity (IEI) and HL remains poorly understood. We conducted a retrospective, register-based case-control study in Sweden over five decades to investigate the relationship between IEI and HL. Individuals diagnosed with HL in the Swedish Cancer Register between 1969 and 2019 were matched with population controls. Data were linked to the National Patient Register to identify recorded the International Classification of Diseases (ICD)-coded IEI diagnoses. Odds ratios (ORs) for

Concurrent presentation of distinct extranodal and splenic marginal zone lymphomas.

Marginal zone lymphomas (MZLs) encompass extranodal, splenic, and nodal subtypes that are thought to clonally arise from antigen-exposed B-cells. Here, we present the case of a patient with a concurrent diagnosis of extranodal marginal zone lymphoma (EMZL) of the ocular adnexa and splenic marginal zone lymphoma (SMZL). These were identified on a core biopsy of the ocular adnexa and a bone marrow biopsy, respectively. While sequential occurrences of MZLs have been previously documented, this represents a concurrent diagnosis of two distinct MZLs.

Diagnostic challenges posed by the wide morphological and immunophenotypic spectrum of plasma cell neoplasms.

Plasma cell neoplasms (PCNs) are a group of malignant neoplasms originating from the monoclonal expansion of immunoglobulin-secreting, terminally differentiated B cells, i.e., plasma cells (PCs). PCNs include monoclonal gammopathy of undetermined significance (MGUS), plasmacytoma (solitary plasmacytoma of the bone and extramedullary plasmacytoma), multiple myeloma (MM)/plasma cell myeloma, and rare PCNs associated with paraneoplastic syndromes. MM is a multifocal, bone marrow-based clonal PCN associated with end-organ damage and a monoclonal paraprotein (M protein) in the serum and/or urine. M
