Clinical Flow Cytometric Testing in Chronic Lymphocytic Leukemia.

Flow cytometry is the cornerstone for establishing the diagnosis of chronic lymphocytic leukemia (CLL), owing to its characteristic and well-defined immunophenotype that enables accurate distinction from other leukemias and lymphomas. Beyond diagnosis, flow cytometry provides essential prognostic information and allows sensitive detection of minimal residual disease (MRD), a strong predictor of clinical outcome. CLL MRD assessment is increasingly used to guide risk stratification, therapeutic decision-making, and treatment duration in the era of targeted therapies and immunotherapies. This cha

Adverse Family Outcomes in Children With B-Cell Acute Lymphoblastic Leukemia From the Multicenter Trial Children's Oncology Group AALL0932.

BACKGROUND: Treatment for B-cell acute lymphoblastic leukemia (B-ALL) lasts >2 years and can impact families. The Children's Oncology Group multicenter therapeutic trial AALL0932 randomized patients with NCI standard-risk, average-risk B-ALL (NCI-SR AR B-ALL) to receive maintenance vincristine/dexamethasone every 4 or 12 weeks (VCR/DEX4 or VCR/DEX12, respectively). Embedded in AALL0932, this study aimed to (1) longitudinally measure adverse family outcomes overall, and (2) determine whether patients on the VCR/DEX4 regimen (vs VCR/DEX12) had fewer adverse family outcomes. METHODS: Patients wi

Inhibition and Knockdown of Monocarboxylate Transporter 1 Induce L-Lactate Accumulation in T-Cell Acute Lymphoblastic Leukemia Cells.

Tumors exhibit the Warburg effect, characterized by elevated glucose metabolism through both oxidative phosphorylation and glycolysis to produce energy. To maintain intracellular pH and metabolic homeostasis, cancer cells depend on monocarboxylate transporters (MCTs) to export the glycolytic end product, l-lactate. Similar to other cancers, glucose metabolism is enhanced in T-cell acute lymphoblastic leukemia (T-ALL), supporting rapid cell proliferation. Although MCT1 expression and its sensitivity to inhibition have been reported in certain T-ALL models, the relative contributions of other co

Emerging drug profile: bexobrutideg.

Bruton's tyrosine kinase (BTK) plays an important role in B-cell development and has become a well-established target in B-cell malignancies. Over the past two decades, BTK inhibitors (BTKis) have transformed the treatment landscape for chronic lymphocytic leukemia (CLL) and other B-cell lymphoid malignancies. Despite their efficacy, acquired resistance to BTKis remains a significant challenge, highlighting the need for alternative therapeutic strategies. Targeted protein degradation has emerged as a promising approach to eliminate BTK and thereby inhibit both its kinase and non-kinase (i.e. s

Mimicking Polycystic Liver Disease: A Case Report of Innumerable Cystic Hepatic Metastases From a Pancreatic Neuroendocrine Tumour and Diagnostic Role of Endoscopic Ultrasound.

Pancreatic neuroendocrine tumours (PNETs) are malignant processes and are identified as being the rarest of all pancreatic cancers. Here we present the case of a 62-year-old male with a background of chronic lymphocytic leukaemia (CLL) who was investigated over 18 months for progressive hepatic cystic lesions with no known aetiology. Imaging revealed main pancreatic duct dilatation without an identifiable intrinsic or extrinsic cause, coupled with enlarged mesenteric lymph nodes. Endoscopic ultrasound identified multiple cystic liver lesions of different sizes, some reflecting the presence of

Prefibrotic bone marrow microenvironment is a hallmark of clonal hematopoiesis.

Clonal hematopoiesis of indeterminate potential is associated with systemic inflammation and increased malignant risk in humans, yet understanding of how it reshapes the bone marrow (BM) microenvironment has been limited. Using single-cell transcriptomic profiling of freshly isolated BM samples, combined with histological and spatial transcriptomic analyses, we identified fibro-inflammatory remodeling as a hallmark of clonal hematopoiesis of indeterminate potential. Fibroblasts were markedly expanded and acquired cancer-associated fibroblast-like transcriptional states, while mesenchymal and v

FAT1-associated membranous nephropathy after hematopoietic stem cell transplantation: a case report and review of the literature.

We report a 55-year-old man who developed nephrotic syndrome 52 months after allogeneic hematopoietic stem cell transplantation for acute lymphoblastic leukemia. He had chronic graft-versus-host disease controlled with low-dose prednisolone and tacrolimus. Asymptomatic proteinuria was detected five months before referral, followed by progressive edema, massive pleural effusion, and ascites. At presentation, laboratory testing showed profound hypoalbuminemia, mildly reduced kidney function, and nephrotic-range proteinuria. Kidney biopsy demonstrated membranous nephropathy, with periodic acid-me

Enhancing medication adherence with blister-packed oral oncolytics: A case example in chronic lymphocytic leukemia.

IntroductionMedication nonadherence is a prevalent issue in the oncology population and can be costly on the healthcare system while leading to worsened outcomes, including increased mortality. One intervention that has been shown to help promote medication adherence to oral therapies for chronic conditions is blister-packaging. However, no literature is available assessing the potential impact of blister-packaging in oncology. Therefore, we aimed to quantify the impact of blister-packaging oral oncolytics on medication adherence and healthcare costs.MethodsAn economic model was designed to as

Navigating Asparaginase Treatment for Patients With Acute Lymphoblastic Leukemia: US Consensus Panel Recommendations on the Role of Multidisciplinary Care.

Asparaginase is a critical treatment component for patients with acute lymphoblastic leukemia/lymphoblastic lymphoma (ALL/LBL). However, the successful delivery of asparaginase-based therapy remains challenging across care settings due to its complex administration, distinct toxicity profile, prolonged treatment duration, and the need for coordinated multidisciplinary care. These challenges are further compounded by variability in institutional infrastructure, provider experience, and access to specialized supportive services. In this article, we describe key operational and clinical barriers

SLC28A3 expression and fludarabine-emergent autoimmune hemolytic anemia in chronic lymphocytic leukemia: A new player in an old equation.

Fludarabine treatment in patients with chronic lymphocytic leukemia (CLL) has been associated with immune dysregulation and the development of autoimmune hemolytic anemia (AIHA). Prior research indicates a potential correlation between the expression of the SLC28A3 gene, which encodes human concentrative nucleoside transporter 3 (responsible for fludarabine uptake), and treatment response. This retrospective study investigated the relationship between baseline SLC28A3 expression levels, quantified via reverse-transcriptase polymerase chain reaction (qRT-PCR), and the incidence of AIHA in a coh

Off-the-shelf dual CAR-iNKT cell immunotherapy eradicates medullary and leptomeningeal high-risk KMT2A-rearranged leukemia.

Current therapies, including autologous chimeric antigen receptor (CAR) T-cell immunotherapy, fail to cure half of infants with KMT2A-rearranged acute lymphoblastic leukemia (KMT2Ar-ALL), a disease characterized by frequent central nervous system involvement, poor treatment response, early relapse, and lineage switching. More effective treatment strategies, including the availability of off-the-shelf immunotherapies, is particularly relevant in infants. PROM1/CD133 is a direct target of KMT2A-fusion oncoproteins and is expressed on leukemic cells. Allogeneic invariant natural killer T (iNKT) c

Prognostic impact of KMT2A-PTD in acute myeloid leukemia in the NGS era: a multicenter retrospective study.

The KMT2A gene is frequently altered in acute myeloid leukemia (AML). KMT2A abnormalities include partial tandem duplication (PTD), single-nucleotide variants (SNVs), and chromosomal rearrangements such as t(9;11)(p21.3;q23.3) and t(v;11q23.3). However, with the widespread adoption of next-generation sequencing (NGS), PTD has been assessed less frequently. We evaluated the clinical features and prognostic impact of PTD and other KMT2A abnormalities. We analyzed the KMT2A abnormalities and coexisting genetic alterations in 585 patients with de novo AML diagnosed across 25 institutions (1991-202

CAR HEMATOTOX independently predicts outcomes after CD19 CAR-T therapy for acute lymphoblastic leukemia.

Chimeric antigen receptor (CAR) T-cell (CAR-T) treatment for B-cell acute lymphoblastic leukemia (ALL) induces high initial response rates, but most patients relapse. Low disease burden (often defined as <5% blasts in the bone marrow) is associated with better outcomes. CAR HEMATOTOX (HT) is a score using prelymphodepletion hematologic and inflammatory parameters to predict outcomes in lymphoma. Here, we assess its prognostic utility in a large multicenter adult B-cell ALL cohort. Patients who received brexucabtagene autoleucel across 33 centers in North America were included as part of the Re

Genomic determinants of response and resistance to pirtobrutinib in relapsed/refractory chronic lymphocytic leukemia.

Pirtobrutinib, a noncovalent, reversible Bruton tyrosine kinase inhibitor (BTKi), demonstrated efficacy in patients with chronic lymphocytic leukemia (CLL), resistant to covalent BTKi (cBTKi). We analyzed genomic correlations with response and resistance to pirtobrutinib in relapsed/refractory (R/R) patients with CLL pretreated with cBTKi enrolled in the phase 1/2 BRUIN trial. DNA sequencing was performed on peripheral blood mononuclear cells at baseline, on treatment, and at progressive disease (PD). Common alterations at baseline included mutations in BTK (43%), TP53 (38%), SF3B1 (25%), NOTC

The clinical relevance of RAS pathway gene mutations in childhood B-cell acute lymphoblastic leukemia.

OBJECTIVE: Notably, FLT3 and RAS pathway gene mutations represent a significant proportion of childhood B-cell acute lymphoblastic leukemia (B-ALL) cases. Our study explores whether these patients have distinct clinical features and prognostic outcomes. METHODS: We retrospectively analyzed 488 pediatric B-ALL cases from the CCCG-ALL-2015 cohort using whole transcriptome sequencing to assess the clinical features of FLT3 mutations, RAS pathway alterations, and their cooperative effects. RESULTS: 192 patients (39.3%) carried at least one RAS pathway alterations, while 76 patients (15.6%) had F

Central nervous system involvement in CLL: an international retrospective study by ERIC, the European Research Initiative on CLL.

Central nervous system involvement (CNSi) of chronic lymphocytic leukemia (CLL) is a rare condition with no consensus on diagnostic criteria and limited evidence for management and outcome. Here, we report an international, multicenter, retrospective study conducted by the European Research Initiative on CLL. The study defined CNSi of CLL by the following: (1) detection of CLL cells in the cerebrospinal fluid or confirmation of CLL infiltration of the CNS based on a tissue biopsy, (2) clinical or radiographic evidence of neurologic disease, and (3) the absence of other explanations for the neu

Multiomics Profiling of T-cell Leukemia and Lymphoma Enables Targeted Therapeutic Discovery.

UNLABELLED: T-cell leukemias and lymphomas (TCL) form a heterogeneous group of rare and often aggressive malignancies. Because of the rarity and heterogeneity of TCL subtypes, clinical trials are challenging to conduct, making pharmacogenomic studies in cell line panels critical for the discovery of targeted therapeutics. The scarcity of data repositories with integrated multiomics and drug screening data hinders the preclinical evaluation of drug vulnerabilities and the identification of molecular markers predictive of responses to monotherapies and combinations. To address this gap, we condu

DNA methyltransferase inhibitors in hematological malignancies and solid tumors.

Epigenetic modifications such as DNA methylation play a fundamental role in oncogenesis and the progression of neoplasms neoplasias. DNA methyltransferase inhibitors (DNMTi) constitute a family of therapeutic agents that impede the methylation at the 5-position on cytosine nucleotides, thereby modulating the epigenetic regulation of tumor suppressor genes, oncogenes, and other key regulatory genes. The first-generation DNMTi azacitidine and decitabine have demonstrated substantial efficacy in the treatment of medically non-fit, older patients with acute myeloid leukemia (AML) or myelodysplasti

Cell State Transitions Drive the Evolution of Disease Progression in B-Lymphoblastic Leukemia.

UNLABELLED: Cancer stem cells (CSC) are hypothesized to promote tumor progression through innate chemoresistance and self-renewal. CSCs reside in the CD34+/CD38- immunophenotypic subpopulation of acute myeloid leukemia (AML). Isolation of CSCs from B-lymphoblastic leukemia (B-ALL) has proven difficult, and the cells of interest apparently are not isolated to the CD34+/CD38- compartment. This may be explained, in part, by temporal variations of CD34 and CD38 expression which result in stochastic cell state transitions (e.g., from CD34+/CD38+ to CD34+/CD38-). We present a mathematical model of t
