BACKGROUND: Melioidosis is a serious infection caused by the bacterium Burkholderia pseudomallei with a case fatality rate of up to 40% in Northeast Thailand. Diabetes mellitus (DM) increases the risk of developing melioidosis by 12-fold. A similar, but less marked relationship with DM is seen in patients with tuberculosis, with a 3-fold increased risk of developing tuberculosis in people with DM. However, the mechanisms underlying the impact of DM on infection are not fully understood.
METHODS: Eighty-one patients with acute melioidosis from Northeast Thailand and 151 patients with tuberculo

Melioidosis is an infection caused by the environmental saprophyte Burkholderia pseudomallei and is endemic to tropical regions. It has diverse clinical manifestations; however, vascular involvement in the form of a mycotic aneurysm is rare and associated with significant morbidity and mortality. We describe a 50-year-old male farmer from Kerala, India, with a nine-month history of recurrent fever, weight loss, and fatigue who underwent multiple admissions and exhaustive evaluation for pyrexia of unknown origin. Sequential blood cultures over this period identified a Pseudomonas species and a

BACKGROUND: RAS-associated Autoimmune Lymphoproliferative Disease (RALD) is a rare, non-malignant lymphoproliferative disorder caused by somatic mutations in RAS genes that impair lymphocyte apoptosis. Patients often present with features of lymphoproliferation, autoimmune manifestations, and an increased risk of malignant transformation.
CASE PRESENTATION: We report the case of a 3-year-old boy with splenic microabscesses and Burkholderia pseudomallei IgM serology positivity, who was treated as melioidosis. He had recurrent respiratory infections and chronic rhinorrhoea since the age of one

BACKGROUND: Leptospirosis is a zoonotic disease prevalent in low and middle-income countries and tropical regions. It can be mimicked by endemic infectious diseases that present simultaneously, like dengue, malaria, melioidosis, or scrub typhus. However, there is a dearth of documented evidence about the frequency of coinfection, as well as related symptoms and mortality. The study aims to determine the frequency and geographical extent of coinfections in leptospirosis patients in India.
METHODOLOGY: We systematically searched databases, including Scopus, PubMed, Embase, and Web of Science, f

Melioidosis, caused by Burkholderia pseudomallei, is an endemic infectious disease with high mortality in tropical and subtropical regions. Large-scale epidemiological data remain insufficient in China, while comprehensive data integrating genomic epidemiology are rare worldwide. Herein, performed a retrospective analysis of 554 culture-confirmed melioidosis cases in southern China from 2003 to 2022. Genomic characteristics and their relationship with antimicrobial susceptibility and clinical characteristics were analyzed via whole genome sequencing. Core-genome SNP phylogenies were constructe

Neuromelioidosis, a rare but severe complication of melioidosis caused by B. pseudomallei, is associated with a high mortality rate. These cases exhibited diverse neurological manifestations, including quadriparesis, hemiparesis, encephalopathy, and abscess formation. All cases were confirmed through culture and received appropriate antibiotic therapy. India is considered to have significant underreporting of melioidosis, leading to delayed diagnosis and treatment challenges. Early identification and initiation of antimicrobial therapy are crucial to improve patient outcomes. Clinicians should

Burkholderia pseudomallei causes melioidosis, a potentially life-threatening infection that can rapidly progress to septic shock and multi-organ failure if not promptly recognized and treated. Current international guidelines recommend meropenem as first-line therapy for severe melioidosis, whereas prolonged oral eradication therapy is essential to prevent relapse. We report a 39-year-old man with diabetes mellitus and chronic hepatitis B who initially improved on intravenous ceftazidime for disseminated melioidosis, but relapsed after an inadequate eradication phase consisting of only one wee

BACKGROUND: Research on melioidosis has expanded steadily over recent decades, but its global research landscape, knowledge structure, collaboration patterns, and thematic evolution remain insufficiently characterized.
METHODS: We searched the Web of Science Core Collection for melioidosis-related Article and Review records published from 1990 to 2025. Bibliometrix-biblioshiny, VOSviewer, CiteSpace, Microsoft Excel, and R were used to analyze publication output, collaboration networks, institutional and author contributions, reference co-citation patterns, keyword co-occurrence, and trend top

Melioidosis is a life-threatening infectious disease caused by Burkholderia pseudomallei. Early and accurate diagnosis is critical for timely treatment and improved outcomes. Although highly endemic in tropical regions, it remains underdiagnosed due to limitations of current diagnostic methods. Bacterial culture, the diagnostic gold standard, is time-consuming, prone to misidentified species, requires specialized laboratory facilities, and has low sensitivity. The indirect hemagglutination assay is also unreliable due to poor sensitivity and specificity. We developed a second-generation immuno

Melioidosis, caused by Burkholderia pseudomallei, is an emerging disease in the United States and was declared endemic to the Gulf Coast region in 2022. Melioidosis has been sporadically reported in the American continents, the Caribbean, and more recently in Africa. We conducted lethal dose analyses in BALB/c and C57BL/6 mice exposed to small particle aerosols of B. pseudomallei strains from the Western Hemisphere and Africa. Those isolates exhibit a variety of virulence patterns, including rapidly-lethal disease and delayed onset of fatal disease. We found that the isolates we tested grew si

This article is about how melioidosis, a historically little-known infectious disease, came to be constructed as an object of evidence and rendered visible within global health. I consider the production of knowledge on melioidosis in Thailand, focusing particularly on spaces of scientific research and policy, and examining the social relations required to make the disease visible at both a national and international scale. My research demonstrates how barami, a social capital distinct to Thai culture, is cultivated alongside scientific knowledge in ways that give prominence to the researchers

Melioidosis, caused by Burkholderia pseudomallei, carries a high mortality rate, particularly in acute severe cases. This study aimed to develop a practical prediction model for in-hospital mortality using routinely available clinical data. We retrospectively analyzed 283 melioidosis patients from Hainan General Hospital (2010-2024). Compared to survivors, non-survivors had significantly higher neutrophil percentage, C-reactive protein (CRP), and urea nitrogen levels, and lower platelet counts (PLT) (all P < 0.05) based on the last available measurements during hospitalization. Multivariate lo

Burkholderia pseudomallei, the etiologic agent of melioidosis, is a Gram-negative bacterial pathogen that causes severe disease in humans and animals. In this study, we evaluated the safety and immunogenicity of our lead melioidosis subunit vaccine candidate in cynomolgus macaques. To accomplish this, the 6-deoxyheptan capsular polysaccharide (CPS) from Burkholderia thailandensis E555 was purified using a phenol-free extraction process and then conjugated to CRM197 to generate CPS-CRM197. Highly purified, His-tagless B. pseudomallei Hcp1 (Hcp1-TL) was also produced. Animals immunized with CPS-

Burkholderia pseudomallei causes the tropical disease melioidosis. Host mortality primarily results from sepsis-related complications and associated cytokine release. Lipopolysaccharide (LPS), a major virulence factor and mediator of sepsis, is modified by B. pseudomallei in response to external stimuli. LpxO and PagL are lipid A-modifying proteins encoded by the B. pseudomallei genome to decrease recognition by host defenses. The contribution of lpxO and pagL-dependent lipid A modifications to outer membrane permeability and host response has not been clearly demonstrated. Mono-phosphorylated

Background: Burkholderia pseudomallei, the causative agent of melioidosis, infects diverse animal species and reflects environmental contamination. However, the global seroprevalence of B. pseudomallei in animals remains incompletely characterized. Methods: A systematic review and meta-analysis were conducted according to PRISMA guidelines and registered in PROSPERO (CRD420261306404). PubMed, Embase, and Scopus were searched for observational studies reporting seroprevalence of B. pseudomallei in animals. Random-effects meta-analysis was performed to estimate the pooled prevalence with 95% con
