Mean nocturnal baseline impedance predicts increased pharyngeal reflux on hypopharyngeal-esophageal pH-impedance monitoring in patients with laryngopharyngeal symptoms.

Laryngopharyngeal symptoms (LPS) present a diagnostic challenge due to the lack of clear diagnostic gold standard, and suboptimal correlation between traditional esophageal reflux criteria and pharyngeal reflux events. Mean nocturnal baseline impedance (MNBI) is a measure of mucosal integrity that correlates with reflux burden, but evidence for its role in diagnosing laryngopharyngeal reflux disease remains limited. We aimed to characterize the correlation between MNBI and pharyngeal reflux on objective reflux monitoring. This was a retrospective study of adults with LPS undergoing reflux moni

Removal of Fish Bones Embedded in the Hypopharynx Using a Novel Snare Device.

BACKGROUND: Fish bones can become embedded in the throat tissues while eating and are a common emergency department (ED) symptom. They can be difficult to visualize and remove, depending on location and proximity to the glottis. We present 3 cases of fish bone removal from the pharyngeal mucosa of adult patients using a novel, simple snare device that can be assembled using widely available ED supplies. DISCUSSION: All cases failed conventional removal techniques by emergency physicians and otolaryngologists and were destined for the operating room for removal. In all cases, removal was succe

Large mammalian herbivores influence the deployment of microbial extracellular enzymes to control soil carbon cycling.

UNLABELLED: Large mammalian grazers have regulated biogeochemical cycles across 40% of the Earth's terrestrial surface for millennia. While grazer effects on organic matter entering the soil are becoming clear, their effect on the downstream microbial foraging through extracellular-enzymes (EE), which link processes at the cellular level to ecosystem-level flow of matter remain less clear. We assessed how large mammalian grazers influence microbial foraging in soil using two lines of evidence. First, utilising a 14-year-old long-term grazer-exclusion experiment in the Trans-Himalayan ecosystem

Multi-institutional Normal Tissue Complication Probability (NTCP) Prediction Model for Mandibular Osteoradionecrosis: Results from the PREDMORN Study.

PURPOSE: Mandibular osteoradionecrosis (ORN) is a severe late complication affecting patients with head and neck cancer (HNC) treated with radiation therapy (RT) that significantly impacts patients' quality of life and can require costly interventions. Although radiation dose is a key factor, other clinical and demographic risk factors also influence ORN development. Previous predictive models have primarily been single-institutional, limiting their generalizability. In this first analysis from the PREDMORN Consortium, we have aimed to reproduce existing statistical association and modeling an

Disease Site-Specific Outcomes in p16-Positive Non-Oropharyngeal Mucosal Head and Neck Cancer.

OBJECTIVES: To report disease-free survival (DFS) in p16-positive non-oropharyngeal head-and-neck squamous-cell carcinoma (HNSCC). METHODS: Curatively-treated non-oropharyngeal HNSCC (2009-2021) were reviewed. DFS was compared among p16-positive, p16-negative, and p16-untested cases for the overall cohort and by disease site. Multivariable analysis estimated adjusted hazard ratio (aHR) for p16-positivity. RESULTS: Among 3061 non-oropharyngeal HNSCC, 91 p16+, 375 p16-, 2595 untested; median follow-up 4.7 years. P16-positive patients had superior 5-year DFS versus p16-/untested (74% vs. 48%/59

SEOM-TTCC clinical guidelines for the treatment of head and neck cancer (2025).

Squamous cell carcinoma of the lip, oral cavity, oropharynx, hypopharynx, and larynx comprises the ninth most common cancer in Spain. While alcohol and tobacco use have long been the primary etiological factors, the role of human papillomavirus (HPV) in oropharyngeal cancer is increasing. Diagnoses and relapses should always be discussed by an expert multidisciplinary team. For localized stages, surgery or radiotherapy should be used as single-modality treatments, while locoregional stages require combined modality approaches. In the case of recurrent or metastatic disease, curative intent sho

Subsite- and histology-specific epidemiology of head and neck cancers in Japan from 2016 to 2019: an application of a new classification of rare cancers.

BACKGROUND: Head and neck cancers comprise multiple anatomical sites. Due to their rarity, previous studies have often aggregated anatomically or histologically distinct cancers or excluded rare entities altogether. Moreover, the list of head and neck cancer entities has evolved along with the adoption of new pathological insights. METHODS: In our previous study, we proposed a new classification of rare cancers, based on the International Classification of Diseases for Oncology 3.2 coding system, using data from cancer patients at all sites recorded in the National Cancer Registry of Japan. Evolution of endoluminal treatment of Zenker's diverticulum and complete pharyngo-oesophageal obstruction.

PURPOSE OF REVIEW: Endoscopic approaches to surgery of the foregut, in particular, the upper oesophageal sphincter (UES), continue to develop, providing options for management that were previously unavailable. This review examines endoluminal surgical tunnelling approaches (third space endoscopy) to treat Zenker diverticulum and the more rare condition of complete oesophageal stenosis. RECENT FINDINGS: Endoluminal surgical approaches to the UES such as Z-POEM (per-oral endoscopic myotomy) that utilize gastroscopes and upper gastrointestinal endoscopic instrumentation, are increasingly being u

[Transoral robotic surgery in head and neck cancers].

Transoral robotic surgery (TORS) for head and neck cancers has developed since 2005, following the pioneering work of the University of Pennsylvania team. Since then, numerous publications have demonstrated its clinical feasibility and safety, while several clinical trials have confirmed favorable oncologic and functional outcomes. Today, robotic surgery is routinely used as an alternative to conventional transoral approaches for tumors that are difficult to expose, as an alternative to external open approaches for tumors that are not amenable to transoral exposure, and as an alternative to no

Helicobacter pylori infection is causally associated with extra-gastric tumors: a Mendelian randomization study.

BACKGROUND: Helicobacter pylori (H. pylori) infection is a major pathogen associated with gastric cancer. However, the causal relationship between H. pylori infection and extra-gastric tumors has not been fully elucidated. METHODS: A two-sample Mendelian randomization (MR) study was conducted using genome-wide association study (GWAS) data on serum H. pylori IgG levels as the exposure variable and GWAS data on 268 benign and malignant tumors as the outcome variable. Single nucleotide polymorphisms (SNPs) associated with the exposure were identified from the OpenGWAS and GWAS Catalog databases

Plummer-Vinson syndrome: is the immune system the missing piece?

Plummer-Vinson syndrome (PVS) is a rare disorder characterized by a triad of iron-deficiency anemia, cervical dysphagia, and post-cricoid esophageal webs. The exact relationship between anemia and web formation remains unclear, with current hypotheses lacking strong evidence. Although treating anemia generally resolves dysphagia, some cases require endoscopic intervention. PVS is considered a precancerous condition due to its association with squamous cell carcinoma of the hypopharynx or upper esophagus. We present the case of a 53-year-old woman with psoriatic arthritis who exhibited iron-def

Laryngotracheal Anastomosis for Cervical Upper Digestive Tract Reconstruction Following Hypopharyngeal Cancer Resection: A Case Report.

We report a case of cervical upper digestive tract reconstruction using a laryngotracheal flap following resection of hypopharyngeal carcinoma. A 53-year-old man presented with a 1-year history of dysphagia accompanied by a persistent sensation of obstruction. Gastroscopy revealed a cauliflower-like mass involving the right pyriform sinus and postcricoid region, extending to the esophageal inlet. Histopathological examination of biopsy specimens confirmed squamous cell carcinoma. Reconstruction of the surgical defect was achieved by anastomosing the hypopharynx to the cervical esophagus using

An endotracheal tube inserted to the level of the hypopharynx is an easy, safe, and effective technique for ventilation of pediatric patients: A prospective observational study.

BACKGROUND: Difficult intubation and facemask ventilation in infants and small children can lead to rapid oxygen desaturation. An endotracheal tube (ETT) with its tip positioned in the hypopharynx is readily available and bypasses the soft palate, tongue, and epiglottis of the child, thus saving time by providing safe and efficient hypopharyngeal ventilation to ensure proper oxygenation. METHODS: This prospective, observational study included 85 infants and children aged >1 month and ≤ 6 years old, undergoing elective surgery under general anesthesia with orotracheal intubation. The effective
