Biliary stent migration complicated by sigmoid colon perforation with iliopsoas and acetabular involvement.

Biliary stenting is a common treatment for obstructive jaundice in patients with benign or malignant biliary pathologies, particularly when surgical intervention is high-risk due to comorbidities. Although generally safe, stent-related complications can occur. They are typically classified as early or late. Stent migration is one such complication and is often asymptomatic. However, in rare instances it can lead to gastrointestinal perforation. We present a highly unusual case of a migrated biliary stent that perforated the sigmoid colon, traversed the lateral pelvic wall, and focally eroded t

Dosimetric Comparison of Fixed-Configuration Versus Image Guided Needle Insertion in Cervical Brachytherapy.

PURPOSE: Brachytherapy is an integral part of cervical cancer management. In interstitial brachytherapy, image guided needle insertion can be logistically challenging in resource-limited settings. This study aimed to evaluate whether a fixed-configuration (FC) needle insertion method yields dosimetric results and toxicity profile comparable to the standard, image guided technique. METHODS AND MATERIALS: The study included cervical cancer patients, FIGO IB2-IVa, who underwent external beam radiation (EBRT) followed by interstitial brachytherapy, 28Gy/4 fractions, via Utrecht applicator. Patien

DZ-1-artesunate conjugate induces mitochondria-mediated, reactive oxygen species-dependent apoptosis in colorectal cancer tumoroids.

We previously reported that the heptamethine cyanine dye-conjugated artesunate (DZ-1-ART) induces apoptosis in monolayer (2D) cell culture models. However, in 2D cultures, cells grow on flat, rigid plastic surfaces that fail to recapitulate the three-dimensional architecture of tumor tissues. This artificial environment alters cell polarity, morphology, and mechanical signaling, leading to non-physiological behavior. To overcome these limitations, we developed patient-derived tumoroids to assess the tumoricidal efficacy of the DZ-1-ART conjugate. In this study, tumoroids were established from

The Role of Intestinal Ultrasonography in Children with Inflammatory Bowel Disease.

BACKGROUND/AIMS: Endoscopic and histopathological evaluation are important in the diagnosis and follow-up of inflammatory bowel disease (IBD), but they are invasive methods. This study aimed to evaluate the use of ultrasonography (USG), a noninvasive examination, in diagnosing and monitoring children with IBD. MATERIALS AND METHODS: The study included IBD and non-IBD patients who underwent a prospective diagnostic colonoscopy. After colonoscopy, bowel wall thickness (BWT) of the patients was measured using USG. Ultrasonographic, endoscopic, histological, and laboratory findings were then comp

Colonic Histoplasma capsulatum Infection Simulating a Polypoid Lesion: Reactivation of a Long-Dormant Disease in a Patient With Chronic Renal Failure.

A 73-year-old Japanese man undergoing hemodialysis for chronic renal failure, with a history of sigmoid colon low-grade adenoma, underwent follow-up colonoscopy. A 5 mm rectal polypoid lesion was identified and resected by endoscopic mucosal resection. Histopathological examination revealed granulomas and clusters of histiocytes containing intracytoplasmic yeast-like organisms. Polymerase chain reaction analysis of paraffin-embedded tissue identified the genome of Histoplasma capsulatum. The patient had visited Brazil 30 years earlier, where he was presumed to have acquired the infection witho

Simultaneous Robot-Assisted Radical Nephrectomy and Sigmoid Colectomy Using the da Vinci 5 System.

Synchronous renal cell carcinoma (RCC) and colorectal cancer are uncommon, and the optimal surgical strategy remains controversial. We report a patient with synchronous RCC and sigmoid colon cancer successfully treated with a one-stage, robot-assisted combined approach using the da Vinci 5 system. A 60-year-old woman presented with severe anemia. Imaging revealed a left-sided renal tumor, sigmoid colon cancer, and a solitary pulmonary metastasis. Both tumors were considered surgical indications, and early intervention was required due to the risk of bowel obstruction. Robot-assisted radical ne

Mastery Learning in an Intestinal Ultrasound Workshop for Inflammatory Bowel Disease: Evaluating Its Effectiveness in Enhancing Skill Acquisition.

BACKGROUND: Intestinal ultrasound (IUS) is a non-invasive tool for monitoring inflammatory bowel disease (IBD), offering high diagnostic accuracy and greater patient convenience than gastrointestinal endoscopy. The present study evaluated the feasibility of a mastery learning approach in Indonesia's inaugural IUS workshop to enhance skill acquisition among physicians. METHODS: A retrospective study was conducted on 37 physicians who participated in a two-day IUS workshop employing a mastery learning approach that included flipped learning, lectures, a pre-test, hands-on sessions with real-tim

Mastery Learning in Intestinal Ultrasound Training: A Meaningful Step Forward, With Miles Still to Go.

Intestinal ultrasound (IUS) is increasingly recognized as a vital, non-invasive tool for managing inflammatory bowel disease (IBD) due to its real-time assessment capabilities and patient comfort. However, the lack of standardized training pathways remains a significant barrier to its widespread adoption, particularly in low- and middle-income settings. This editorial evaluates the implementation of a mastery learning-based workshop in Indonesia designed to enhance IUS skills among physicians. The mastery learning framework-incorporating flipped learning, deliberate practice, and real-time fee

Hyperprogression after Anti-Programmed Death-1 Therapy in a Case of Sigmoid Colon Cancer with Lynch Syndrome.

INTRODUCTION: Lynch syndrome is a hereditary condition caused by germline defects in mismatch repair (MMR) genes, resulting in microsatellite instability-high (MSI-H) tumors. Immune checkpoint inhibitors (ICIs) have demonstrated efficacy in the treatment of MSI-H advanced cancers. However, hyperprogressive disease (HPD) following ICI therapy is a rare and paradoxical phenomenon. We report a case of advanced sigmoid colon cancer associated with Lynch syndrome that developed HPD after pembrolizumab treatment. CASE PRESENTATION: A 51-year-old woman with advanced sigmoid colon cancer and a histor

Characteristics and follow-up results of bladder augmentation surgery at a pediatric center in South America.

BACKGROUND: Augmentation cystoplasty (AC) remains the standard reconstructive option for children with refractory lower urinary tract dysfunction (LUTD) and high-risk urodynamic patterns threatening renal function. However, postoperative morbidity and long-term renal outcomes remain clinically relevant concerns. OBJECTIVE: To characterize postoperative complications following AC, evaluate functional and renal outcomes, and identify risk factors associated with intermediate and late adverse events. STUDY DESIGN: Retrospective cohort study including patients ≤18 years with neurogenic or congen

Synchronous Gastric Metastasis From Sigmoid Colon Cancer: A Case Report.

Colorectal cancer is the third most common cancer worldwide, and as such is a significant global health concern. Distant metastases of colorectal cancer to the lung, liver, and bone are well documented, while gastrointestinal tract (GIT) metastases are very rare. Herein, we report a case of gastric metastasis from sigmoid colon cancer. A 78-year-old male was diagnosed with both gastric and colorectal cancer, which occurred synchronously. The presence of two primary cancers was diagnosed, and radical subtotal gastrectomy and anterior resection were performed simultaneously. The postoperative pa

Ustekinumab resistance in individuals with ulcerative colitis is associated with an alteration of a subset of proinflammatory mucosal regulatory T cells.

BACKGROUND: Ustekinumab is a monoclonal antibody therapy targeting interleukin-12 and interleukin-23 for the treatment of inflammatory bowel diseases, including ulcerative colitis (UC). While these pathways remain quite attractive for UC therapy, response to ustekinumab can be variable. There is an urgent need to better understand the underlying mucosal immune alterations associated with treatment to guide therapy decisions. This study aims to examine the mucosal immune signatures in individuals with UC with variable treatment response to ustekinumab. METHODS: Sigmoid colon tissue from indivi

[Wolffian adnexal tumors (FATWO)].

Tumors originating from the remnants of the mesonephral (Wolf's) duct, known as FATWO (Female Adnexal Tumor of Probable Wolffian Origin), belong to the category of rare neoplasms of the female reproductive system. The tumor was first described in 1973 by doctors Kariminijad and Scully from the University of Texas M.D. Anderson Cancer Center. Since then, about 100 cases of FATWO have been recorded worldwide. The localization of the tumor is associated with the threefold laying of the Wolf duct in embryogenesis: from the ovarian gate, along the mesosalpinx, in the wide ligament of the uterus alo

Intestinal Lipoma as a Cause of Sigmoid Colo-Colic Intussusception in an Adult: A Case Report.

Adult intussusception is an uncommon and often unexpected diagnosis, typically driven by an underlying structural lesion. We report the case of a 43-year-old woman who presented with a month-long constipation, five days of worsening intermittent abdominal pain associated with nausea, bilious vomiting, and anorexia. Contrast-enhanced computed tomography (CECT) revealed a sigmoid colo-colic intussusception caused by a 4 cm intraluminal fat-density mass consistent with a lipoma. The patient underwent successful surgical reduction with segmental resection and primary anastomosis. Histopathology co
