Portal cavernoma cholangiopathy in pediatric extrahepatic portal vein obstruction with or without shunt surgery.

BACKGROUND: Portal cavernoma cholangiopathy (PCC) has recently been recognized as an abnormality of the intrahepatic and extrahepatic bile ducts caused by external compression by the portal cavernoma or ischemia in the biliary region. Although extrahepatic portal vein obstruction (EHPVO) is common in children with portal hypertension, the association between surgical treatment for EHPVO and the incidence of PCC in children is not well known. METHODS: We retrospectively reviewed the medical records of 14 cases of childhood-onset EHPVO in our department. PCC was defined by intra- and extrahepat

A Novel Bioabsorbable Covered Stent for Advancing Bile Duct Injury Management: A Preclinical Study in a Porcine Model (With Video).

Self-expandable metallic and plastic stents have been used for biliary tract injuries, but they are not entirely adequate as treatments. This study investigated the potential of our novel self-expandable bioabsorbable covered stent (SEBCS) to treat bile duct injuries. We developed a novel SEBCS by covering a self-expandable bioabsorbable stent with a bioabsorbable tube. Five pigs underwent laparotomy after being placed under general anesthesia. A 5-mm incision was made in the extrahepatic bile duct, followed by the insertion of an SEBCS. Postoperatively, hepatobiliary enzyme levels were measur

Management of obstructive jaundice induced by a retained bullet in the common hepatic duct: A case report.

BACKGROUND: Traumatic injuries to the extrahepatic bile ducts are rare, with an incidence of 0.4%-0.6% in cholecystectomy procedures among adults, particularly following the introduction of laparoscopic cholecystectomy. Among these, the presence of foreign bodies within the biliary tree is exceptionally rare, with obstructive jaundice caused by a bullet lodged in the common hepatic duct being particularly uncommon. This case report aims to share the diagnostic process and the challenges in managing such a rare condition. CASE PRESENTATION: A 41-year-old female with a 13-year history of an acc

Bioengineered Bile Duct for Liver Regenerative Medicine and Bile Duct Reconstruction.

Bioengineered tubular bile ducts are an emerging strategy in liver regenerative medicine and extrahepatic bile duct reconstruction, offering potential solutions to address biliary diseases and improve outcomes in liver transplantation. Disorders of the biliary system, such as biliary atresia, primary sclerosing cholangitis, and bile duct injury, often result in irreversible damage and cannot be adequately treated with conventional therapies. This review explores the latest advancements in the bioengineering of both intrahepatic and extrahepatic bile ducts, focusing on approaches that leverage

Pitfalls of extrahepatic bile duct cytology.

Cytological examination of extrahepatic bile ducts represents a challenging diagnostic field, often limited by sample quality and cellularity. Despite the availability of detailed classifications and cellular morphology descriptions, distinguishing benign from malignant lesions remains difficult in clinical practice. This paper reviews the current WHO classification system for pancreatobiliary cytopathology, with a focus on diagnostic categories specific to extrahepatic bile ducts. It discusses typical cytomorphological features, differential diagnoses, and the use of ancillary techniques such

Transient response to cisplatin-etoposide chemotherapy for early postoperative recurrence of large cell neuroendocrine carcinoma of the distal bile duct: a case report.

BACKGROUND: Large cell neuroendocrine carcinoma (LCNEC) arising from the extrahepatic bile duct is a highly uncommon and aggressive malignancy. Due to the absence of an established standard and a lack of detailed reports on effective chemotherapy for recurrent biliary LCNEC, we present a case that demonstrated a temporary response to cisplatin and etoposide. CASE PRESENTATION: A Japanese man in his 70 s presented with jaundice and weight loss. Imaging studies identified a tumor in the distal bile duct, and a biopsy of the bile duct confirmed a neuroendocrine tumor. The patient underwent a rad

Primary Sclerosing Cholangitis: Diagnostic Criteria.

Primary sclerosing cholangitis is a chronic cholestatic liver disease characterized by inflammation and fibrosis of intra- and/or extrahepatic bile ducts leading to the formation of multifocal strictures alternated to bile duct dilatations. The diagnosis of the most common subtype of the disease, the large duct PSC, is based on the presence of elevation of cholestatic indices, the association of typical cholangiographic findings assessed by magnetic resonance cholangiography and the exclusion of causes of secondary sclerosing cholangitis. Liver biopsy is not routinely applied for the diagnosis

Modulation of canonical Wnt signaling regulates peribiliary mesenchymal identity during homeostasis and injury.

BACKGROUND: The matrix and associated mesenchyme of the extrahepatic bile ducts are distinct, which could drive diseases with a predilection for these ducts, such as primary sclerosing cholangitis. We aimed to understand the molecular drivers of peribiliary mesenchymal cell (PMC) identity in the extrahepatic bile ducts and dissect how this changed in the context of injury using an entirely in vivo approach with transcriptomic analysis. METHODS AND RESULTS: Single-cell sequencing with a receptor-ligand analysis showed that PMCs had the most interactions with surrounding cells. Wnt4, Wnt5a, and

A case of spindle and giant cell-type undifferentiated carcinoma of the extrahepatic bile duct.

Spindle and giant cell type undifferentiated carcinoma of the extrahepatic bile duct is an uncommon malignancy. We report a case involving the common bile duct in a 72-year-old male with jaundice who was admitted to our hospital. Diagnostic imaging, including abdominal computed tomography and magnetic resonance imaging, revealed a mass in the distal common bile duct, accompanied by dilatation of both intra- and extrahepatic bile ducts and regional lymph node enlargement. Endoscopic retrograde cholangiography demonstrated stenosis in the distal common bile duct, with a biopsy confirming adenoca

Biliary atresia: the development, pathological features, and classification of the bile duct.

Biliary atresia is an occlusive biliary disease involving intrahepatic and extrahepatic bile ducts. Its etiology and pathogenesis are unclear. There are many manifestations of bile duct involvement in biliary atresia, but little is known about its occurrence and development. In addition, different classification methods have been proposed in different periods of biliary atresia, each with its advantages and disadvantages. The combined application of biliary atresia classification will help to improve the survival rate of patients with native liver. Therefore, this article reviews the developme

The clinical impact of macrophage polarity after Kasai portoenterostomy in biliary atresia.

INTRODUCTION: Biliary atresia (BA) is a cholestatic hepatopathy caused by fibrosing destruction of intrahepatic and extrahepatic bile ducts, and its etiology has not been clearly revealed. In BA, liver fibrosis progression is often observed even after Kasai portoenterostomy (KPE), and more than half of cases require liver transplantation in their lifetime in Japan. Macrophages play an important role in liver fibrosis progression and are classically divided into proinflammatory (M1) and fibrotic macrophages (M2), whose phenotypic transformation is called "macrophage polarity." The polarity has

Biliary stem cells in health and cholangiopathies and cholangiocarcinoma.

PURPOSE OF REVIEW: This review discusses evidence regarding progenitor populations of the biliary tree in the tissue regeneration and homeostasis, and the pathobiology of cholangiopathies and malignancies. RECENT FINDINGS: In embryogenesis biliary multipotent progenitor subpopulation contributes cells not only to the pancreas and gall bladder but also to the liver. Cells equipped with a constellation of markers suggestive of the primitive endodermal phenotype exist in the peribiliary glands, the bile duct glands, of the intra- and extrahepatic bile ducts. These cells are able to be isolated a
