Outcomes following Pancreatoduodenectomy in Patients ≥80 Years: An Analysis of the Prospective Pancreas Registry of the German Society for General and Visceral Surgery (DGAV|Registry Pancreas).

INTRODUCTION: With rising life expectancy, the number of elderly patients undergoing pancreatoduodenectomy is steadily increasing. This study aimed to assess peri- and postoperative outcomes in patients aged ≥ 80 years undergoing pancreatoduodenectomy, using data from the DGAV|registry pancreas. METHODS: All pancreatoduodenectomies performed between September 2013 and April 2024 and recorded in the DGAV|registry pancreas were retrospectively analyzed. Patients were divided into two groups: <80 and ≥80 years. Demographic, perioperative, and postoperative data were compared. A multivariate anal

Probiotics in type 1 diabetes: immunomodulatory mechanisms and the unresolved question of clinical efficacy.

Type 1 Diabetes (T1D) is a common autoimmune disease worldwide, which is associated with imbalanced gut microbiota. Intestinal dysbiosis plays a key role in the pathogenesis of T1D, and it may affect immune tolerance and autoimmune responses of pancreatic islet β-cells. As a potential immunomodulator, probiotics can help restore this balance, regulate immune responses, and reduce the risk of T1D. Although probiotics offer a logical immunomodulatory strategy to restore microbial and immune homeostasis, the translational gap between robust preclinical mechanisms and inconsistent clinical outcome

Polylactic co-glycolic acid (PLGA)-based nanoparticles encapsulating Persea americana seed extract: A potential therapeutic strategy against oxidative stress and diabetes in rat model.

Diabetes mellitus is a chronic metabolic disorder characterized by persistent hyperglycemia arising from insulin resistance or impaired insulin secretion. Conventional and commercially available effective antidiabetic drugs are often associated with adverse effects and reduced efficacy over prolonged use, highlighting the need for safer and plant-based alternatives. Persea americana seed extract (PSE) has antidiabetic potential but suffers from non-specific distribution and poor bioavailability, thus limiting its clinical application. This study developed and evaluated the poly(lactic-co-glyco

Multisystemic corynebacteriosis caused by Corynebacterium pseudotuberculosis in a captive Brazilian porcupine (Coendouprehensilis).

A captive, 3-year-old, male Brazilian porcupine (Coendou prehensilis) was found dead in its enclosure at a wildlife facility in the USA, with no prior clinical signs reported. At necropsy, the animal was in poor body condition with malocclusion of the maxillary incisors. Numerous tan-to-white, firm nodules, ranging from 0.1 to 1.5 cm in diameter, were present in the lungs, heart, pancreas and right kidney. Histologically, multifocal to coalescing pyogranulomas composed of foamy macrophages, multinucleated giant cells and peripheral lymphoplasmacytic infiltrates surrounded central areas of case

Amphicrine Carcinomas of the Pancreas Showing Neuroendocrine and Acinar Cell Differentiation: A Case Report and Literature Review.

This study clarifies the distinct clinicopathological and molecular features of amphicrine pancreatic carcinoma (AmpCA) through a case analysis and literature review. We present a case of AmpCA in a 73-year-old female, characterized by co-expression of acinar cell markers and neuroendocrine markers within individual tumor cells. Molecular profiling revealed high-risk mutations in KRAS G12V, TP53, and RB1, along with a high Ki-67 index (85%). Literature comparison confirms that AmpCA exhibits a unique bidirectional, single-cell differentiation pattern, fundamentally distinguishing it from the b

Assist team self-efficacy, training needs, and operative outcomes in single-port robotic hepatopancreatobiliary surgery: a mixed-methods study to inform protocol development.

Single-port (SP) robotic surgery imposes unique spatial and technical constraints on bedside assistance personnel; however, structured training protocols for hepatopancreatobiliary (HPB) procedures are lacking. A cross-sectional survey using a 7-item Likert self-efficacy instrument was administered to 11 operating room nurses and 19 physician assistants (PAs). Free-text responses were subjected to inductive thematic analysis. Retrospective data from 92 SP robotic HPB cases performed between august 2024 and July 2025 were analyzed. PAs were grouped according to seniority into Group 1 (≥ 5 years

Hourglass optimization in pancreas transplantation: A systematic review (HOP study).

BACKGROUND: Pancreas transplantation provides durable metabolic control for selected patients with insulin-dependent diabetes mellitus but remains uniquely vulnerable to early technical failure. Ischemia-reperfusion injury is central to graft loss, yet the relative impact of discrete, time-dependent intervals during donor management, procurement, and implantation is incompletely defined. This systematic review evaluates how procurement-related temporal variables influence early complications and pancreas graft survival. METHODS: A systematic review was conducted in accordance with Preferred R

Pancreatic hamartoma presenting with sleep-related behavioral abnormalities due to neuroglycopenia: a case report.

Pancreatic hamartoma (PH) is an exceedingly rare benign non-neoplastic lesion of the pancreas. Owing to its nonspecific clinical and radiological features, it is prone to misdiagnosis. Among its various subtypes, functionally active pancreatic hamartomas with distinct endocrine activity have only been sporadically reported in the literature as individual cases. We report the case of a 58-year-old female patient whose initial presentation was characterized by paroxysmal behavioral abnormalities during nocturnal sleep, manifesting as sudden agitation, arm-flinging, and purposeless stereotyped mo

Antihyperglycemic and pancreatic tissue-preserving effects of L. sativum oil in streptozotocin-induced diabetic rats: exploratory analysis of PDX-1 and GCK.

BACKGROUND: Diabetes mellitus constitutes a major global health burden. L. sativum has long been employed in traditional medicine for glycemic management, an effect attributed to its rich phytochemical constituents, including flavonoids, phenolic acids, and glucosinolates. This study investigated the ameliorative effects of L. sativum oil against streptozotocin-induced type 1 diabetes in rats, with particular emphasis on pancreatic and duodenal homeobox 1 (PDX-1) and glucokinase (GCK) gene expression. METHODS: A total of 48 young male Wistar rats were randomly divided into four experimental g

Molecular signaling networks that connect tissue dysfunction to systemic insulin resistance.

Insulin resistance is conventionally described as a single condition, but it is better understood as the point at which several tissue-specific signaling failures, each following its own molecular logic and its own timeline, converge into one measurable phenotype. This review treats insulin resistance as a network rather than a collection of independent organ pathologies. Because the components of this network rest on very different kinds of evidence, each node and edge is graded explicitly by study design and causal status, and the time-staged sequence is presented as a falsifiable proposal r

Equitable outcomes for critically urgent living donor liver transplantation in acute alcoholic hepatitis.

BACKGROUNDS/AIMS: The justification for living donor liver transplantation (LDLT) for acute alcoholic hepatitis (AAH) remains debated, particularly in urgent settings. We compared outcomes of recipients undergoing urgent LDLT for AAH versus other causes and identified predictors of mortality. METHODS: We analyzed 317 adults in the Korean national registry who underwent urgent LDLT from 2017 to 2021 within seven days of emergency approval. AAH was defined using National Institute on Alcohol Abuse and Alcoholism criteria, excluding coexisting liver disease. Recipient, donor, and perioperative v

Engineering stem cell-derived islets for type 1 diabetes: genetic strategies for immune evasion and stress resilience.

Type 1 diabetes (T1D) is characterized by autoimmune-mediated destruction of pancreatic β-cells, resulting in lifelong insulin dependence and disease burden. Stem cell-derived islet (SCI) replacement therapies have emerged as a promising strategy to restore endogenous insulin production; however, immune rejection, inflammatory stress, and intrinsic β-cell vulnerability remain major barriers to durable graft survival. Recent advances in gene editing and stem cell engineering have accelerated the development of next-generation SCIs designed to evade immune recognition while enhancing resistance

Dual CHK1 and WNT Inhibition as a Synergistic Therapy for Gastrointestinal Neuroendocrine Carcinoma.

UNLABELLED: Gastrointestinal neuroendocrine carcinoma (GIS-NEC) is a highly aggressive malignancy with limited therapeutic options. To identify a therapeutic target, we performed transcriptomic profiling of 34 GIS-NEC and 30 well-differentiated gastrointestinal neuroendocrine tumors (GIS-NET) and found that checkpoint kinase 1 (CHEK1) is significantly overexpressed in GIS-NEC. Although CHEK1 expression was not prognostic across overall GIS-NEC, it was associated with poor outcome specifically in pancreatic NEC, in which it correlated with replication stress-associated genes, including cell div

Clinicopathologic Characteristics and Prognostic Value of Rhabdoid Features in Pancreatic Undifferentiated Carcinoma.

Undifferentiated carcinoma (UC) is a rare, highly aggressive subtype of pancreatic cancer; UC with rhabdoid features (UCRF) is particularly associated with poor prognosis. This study aimed to compare clinicopathologic characteristics and outcomes of UC between groups of patients defined by presence versus absence of RF. We retrospectively reviewed patients diagnosed with UC by EUS-guided tissue acquisition from 2017 to 2024. Histologically, 11 patients were classified as anaplastic UC and two had UC with osteoclast-like giant cell. Six patients had UCRF. Only 2 patients underwent surgery, whil

Excess Liver Fat in Type 2 Diabetes Stratified by Intrapancreatic Fat Deposition: A Meta-Analysis of Dual-Organ MRI Studies.

Both liver fat content (LFC) and intrapancreatic fat deposition (IPFD) are frequently increased in Type 2 diabetes mellitus (T2DM). We meta-analysed dual-organ MRI studies comparing LFC between individuals with T2DM and non-diabetic controls and explored whether study-level differences in LFC varied according to IPFD burden. Seventeen studies (2142 participants) were included. Individuals with T2DM had significantly higher LFC than non-diabetic controls (standardised mean difference [SMD] 0.89, 95% CI 0.60-1.18). In exploratory subgroup analyses, liver fat excess was larger in studies with mea

Pilot study of a low-attenuation anechoic gastrointestinal contrast filling agent for enhancing transabdominal ultrasound examination of the pancreas.

BACKGROUND: Transabdominal ultrasound is constrained by gastric gas and collapsed folds when evaluating the pancreas. This study developed a low-attenuation anechoic gastrointestinal filling contrast agent (LAA-GFCA) composed of food-grade ingredients and performed a safety evaluation. METHODS: Acoustic characterization of LAA-GFCA was performed using the water-immersion insertion substitute method, with phantom experiments designed to evaluate imaging performance. Pre-clinical safety studies were conducted in rabbits, followed by a clinical pilot study in healthy volunteers to assess safety

Body Roundness Index and Cancer Risk in Postmenopausal Women: Findings from the Women's Health Initiative.

Background Body Roundness Index (BRI) is an anthropometric measure that combines waist circumference and height. It reflects central adiposity more accurately than body mass index (BMI). Evidence linking BRI to long-term cancer risk, particularly in postmenopausal women, remains limited. Methods We examined associations between BRI and incident site-specific cancers (n=33,892) in 144,865 postmenopausal women aged 50-79 years at enrollment into the Women's Health Initiative and followed for a median of 17.9 years. BRI was assessed at baseline in all participants and at year 3 in 60% of particip

Clinical Application of Three-dimensional Laparoscopic Artery-first Pancreaticoduodenectomy in Treating Pancreatic Head Malignant Tumors.

Pancreaticoduodenectomy is a technically demanding procedure associated with substantial surgical risk. Conventional laparoscopic approaches have several limitations, including restricted visualization, which can make exposure of the superior mesenteric artery and identification of vessels and lymph nodes challenging. In patients with vascular adhesion or tumor invasion, surgical dissection is particularly difficult and may increase the risk of intraoperative vascular injury and hemorrhage. In addition, poor delineation of anatomical planes may result in incomplete tumor dissection, inadequate
