Removal of a lodged endotracheal tube in an anaesthetised French Bulldog using manual laryngeal stabilisation and neuromuscular blockade, and subsequent complications.

A 5-year-old, female neutered French Bulldog presented under anaesthesia with an endotracheal tube (ETT) lodged in the larynx. Cuff inflation and deflation, application of lubricant, ETT rotation, and repeated traction did not allow ETT removal. The ETT was successfully removed under neuromuscular blockade with manual laryngeal stabilisation: the left thumb pressed against the epiglottis intraorally and the left index and middle fingers gripped the larynx externally to immobilise the larynx. The right-hand applied traction to the ETT, which could then be extracted. The dog's larynx obstructed

Latent class analysis-based prognostic classification in laryngeal carcinoma: beyond TNM staging.

INTRODUCTION: The aim of this study is to identify latent clinical-biological profiles using latent class analysis (LCA) and to compare their prognostic capacity with that of the TNM system. MATERIALS AND METHODS: A retrospective study of 489 patients with squamous cell carcinoma of the larynx (2000-2020). LCA was performed using eight indicator variables: age, sex, toxic consumption, sub-location, T category, N category, Karnofsky index, and host index. TNM staging was grouped into three categories (stages I-II, III, and IV) as no survival differences were observed between stages I and II (p

Association between cognitive social capital and saliva-related quality of life among head and neck cancer survivors: a cross-sectional study.

PURPOSE: To evaluate the association between cognitive social capital and patient-reported saliva-related quality of life among head and neck cancer survivors. METHODS: This cross-sectional study included adults treated with curative intent for squamous cell carcinoma of the lip/oral cavity, oropharynx, hypopharynx, or larynx at a referral hospital in southern Brazil. The outcome was patient-reported saliva-related quality of life, assessed using the saliva domain of the University of Washington Quality of Life questionnaire (UW-QOL); higher scores indicate better outcomes. Cognitive social c

Development of intraparotid lymph nodes with special reference to entrapment of the parotid acini in human fetuses.

OBJECTIVE: Lymph nodes within the parotid gland (PG) are known as 1) an origin of Warthin's tumors and 2) a metastatic focus of nearby cancers. This study aimed to investigate factors that provide the specific features. METHODS: Using histological sections from 28 human fetuses at approximately 15-31 weeks, we examined developing acini, ducts and lymph vessels (LVs) in the PG and compared the results with the other exocrine glands. RESULTS: In the PG at 15-16 weeks, developing acini were embedded in a meshwork of LVs that corresponds to a major lymphatic route from the posterolateral head to

Brainstem and vagal dysfunction in pharyngoesophageal and esophageal dysphagia.

Dysphagia affects approximately 16% of the US population and is classified as pharyngoesophageal and esophageal, the former being more clinically urgent because loss of airway protection may cause pulmonary aspiration and death. Safe swallowing depends on a brainstem central pattern generator, modulated by cortical and subcortical input and executed through the cranial nerves, above all the vagus. The latter supplies the pharynx, larynx, upper esophageal sphincter, esophagus and lower esophageal sphincter. Because afferent and efferent pathways converge in the medulla, small lesions in medulla

Time trends of human papillomavirus-attributable head and neck cancers: the BROADEN study.

BACKGROUND: The incidence of human papillomavirus (HPV)-driven oropharyngeal cancer (OPC) is increasing in several Western countries, but this trend is less well understood in other geographical regions and for other head and neck cancers (HNC). The BROADEN (absolute BuRden Of HPV relAteD hEad and Neck) study assessed HPV-attributable fractions (HPV-AFs) of HNC including OPC in France, Germany, Italy, Portugal, Spain, China, and Japan over a 10-year period. METHODS: This retrospective study included patients diagnosed with HNC (oropharynx, oral cavity, nasopharynx, hypopharynx, and larynx can

Treatment of Radiation-Associated Dysphagia with Statins (TRADstat): protocol for a prospective, single-site, single-arm pilot trial.

INTRODUCTION: Radiation-associated dysphagia (RAD) is one of the most common and profound side effects of radiotherapy for head and neck cancer (HNC), but there is currently no effective treatment for persistent or late-onset RAD. Pravastatin has shown potential in reversing established fibrosis through modulation of the connective tissue growth factor signalling pathway and decreasing pro-inflammatory and profibrotic cytokines. Treatment of RAD with statins (TRADstat) is a pilot trial designed to evaluate the feasibility (primary outcome), efficacy potential, acceptability and safety of a 12-

Hypopharyngeal granular cell tumor treated with endoscopic submucosal dissection.

A 55-year-old woman was referred to our hospital after screening esophagogastroduodenoscopy revealed an erythematous area in the larynx. Detailed esophagogastroduodenoscopy at our institution initially suggested inflammatory changes. However, pharyngeal observation using the Valsalva maneuver revealed a 4-mm flat, elevated, whitish lesion in the postcricoid region of the hypopharynx. Although endoscopic differential diagnosis was challenging, lymphoid follicles were initially suspected; however, biopsy revealed inflammatory changes. Follow-up esophagogastroduodenoscopy four months later demons

Intubation videoendoscopy in a preterm infant with congenital tracheal stenosis and laryngotracheoesophageal cleft: a case report.

BACKGROUND: The co-occurrence of congenital laryngotracheoesophageal cleft and tracheal stenosis is extremely rare. Diagnosis is difficult and many affected neonates die within hours to days. CASE PRESENTATION: A male neonate was delivered at 32 weeks and 5 days of gestation. Birth weight was 1364 g. Severe asphyxia and respiratory failure were apparent soon after birth. Endotracheal intubation was not successful and esophageal intubation did not provide effective ventilation. Flexible intubation videoendoscopy was performed immediately while continuing mask bagging, which identified congenit

When Postextubation Stridor Is Not Edema: Bilateral Lateral Subglottic Mucosal Tears.

Iatrogenic tracheal injury following endotracheal intubation is a rare but potentially life-threatening complication. Most reported injuries involve the posterior membranous tracheal wall. We describe a case in which a patient required rapid sequence emergency intubation in the ED and, three days later, following extubation, developed acute stridor and increasing oxygen requirements, found to be secondary to bilateral, inferiorly based lateral mucosal tears of the subglottic larynx and superior trachea with an associated grade 1 subglottic stenosis. This case highlights the importance of maint

Common Cold-An Update.

The common cold, a mild yet highly prevalent upper respiratory tract infection (URTI), imposes a substantial societal and economic burden through healthcare costs, lost productivity, and widespread morbidity. Primarily caused by diverse respiratory viruses (e.g., rhinoviruses, coronaviruses), it encompasses a spectrum of acute infections affecting the nasal passages, throat, larynx, and bronchi, manifesting as rhinitis, pharyngitis, or bronchitis. Framed by the "iceberg concept" of disease, most cases are mild or asymptomatic, yet severe outcomes (e.g., exacerbations of chronic conditions) con

Delayed manifestation of infantile nystagmus following a vertigo attack.

Infantile nystagmus syndrome (INS), formerly referred to as congenital nystagmus, typically presents in early infancy. We report a rare case in which nystagmus consistent with INS became clinically apparent after a vertigo attack during adolescence. A 17-year-old female developed acute rotational vertigo. Although the vertigo gradually subsided, she continued to perceive rightward visual motion. Five months later, she was referred to our institution because of persistent spontaneous and gaze-evoked nystagmus. There was no prior history or documentation of nystagmus. Examination revealed jerk-t

When the Tumor Disappears, Must the Larynx Follow?

REMATCH, a single-institution phase II study, evaluated reduced-volume surgery in patients with locally advanced head and neck cancer responding to neoadjuvant immunochemotherapy. Two-year disease-free survival was 90%, with universal laryngeal preservation. The trial advances organ preservation by reducing the extent and intensity of local therapy rather than substituting modalities.

Indications, procedural standards, and training requirements for flexible nasolaryngoscopy in pulmonology - a Delphi study protocol.

Flexible nasolaryngoscopy (FNL) is a well-established, minimally invasive procedure for assessing the nasal passages, nasopharynx, and larynx. Its role in pulmonology is expanding, particularly for evaluating vocal cord dysfunction/inducible laryngeal obstruction, chronic cough, and other upper airway conditions relevant to the "united airways" model of care. However, no consensus-based guidance currently exists on the indications, procedural standards, or training requirements for FNL performed by pulmonologists. This protocol describes a modified two-round Delphi study that will survey an in

Thyroid and Parathyroid Dysfunction Following Larynx-Trachea-Thyroid-Parathyroid Allotransplantation.

Larynx-trachea-thyroid-parathyroid allotransplantation can restore essential laryngeal functions, but postoperative endocrine changes remain poorly understood. This study assessed thyroid and parathyroid function in four recipients. All developed early thyroid hormone alterations consistent with euthyroid sick syndrome, followed by variable endocrine outcomes. Two experienced thyrotoxicosis associated temporally with graft rejection, while another developed hypothyroidism and elevated parathyroid hormone during late rejection. Transient parathyroid hormone suppression was also observed. In con

Timing of recurrence in papillary thyroid carcinoma: Analysis of recurrent cases after initial surgery.

OBJECTIVE: Papillary thyroid carcinoma (PTC) generally has an excellent prognosis; however, recurrence may occur over a wide range of postoperative periods, and factors associated with the timing of recurrence remain unclear. Previous studies have evaluated early or late recurrence using predefined time points or specific recurrence patterns. We aimed to evaluate the association between clinicopathological and treatment-related factors and the timing of recurrence across the entire postoperative period. METHODS: We retrospectively analyzed 69 patients with recurrent PTC among 677 patients who

Neuroendocrine Tumors (NETs) of the Larynx.

To review the current state of knowledge regarding neuroendocrine tumors (NETs) of the larynx, with a particular focus on their epidemiology, etiopathogenesis, clinico-pathological features, immunohistochemical profile, and differential diagnostics. Narrative review. Diagnostic criteria for laryngeal NETs were recently defined in the 5th edition of the WHO Classification of Head and Neck Tumors, which newly introduces the entity of NET, grade 3 (G3 NET). However, some of these criteria are still considered provisional. The most common NET of the larynx is the G2 NET. These tumors typically aff

PET Tumour Segmentation in Head and Neck Cancer: Reproducibility, Reader Dependence and Concordance with CT-Defined Tumour Volume in a 154-Patient Cohort.

Purpose: Volumetric PET biomarkers carry substantial prognostic value in head and neck squamous cell carcinoma (HNSCC), yet their clinical implementation is hampered by segmentation heterogeneity. A fundamental and unresolved question is which segmentation method is most suitable for routine clinical use, both for a reader experienced in PET segmentation and for a reader without PET segmentation experience, such as an ENT surgeon or a radiation oncologist. This study addressed this question by assessing the intra- and interobserver reproducibility of four PET segmentation methods in MIM Maestr

Laryngeal vs. Esophageal Acid-Mediated Injury in Bulimia Nervosa: A Systematic Review.

Bulimia nervosa (BN) is a psychiatric eating disorder that can potentially be life-threatening and is characterized by recurrent binging and/or purging. During purging episodes that involve self-induced vomiting, laryngopharyngeal reflux (LPR) can occur as gastric acid is forcefully pushed through the esophageal sphincter and anteriorly to the acid-sensitive laryngopharyngeal tissue. While the esophageal mucosal lining can tolerate repeated exposure to acid before sustaining significant erosion, the laryngeal mucosa exhibits greater sensitivity, reacting to even mild exposure with inflammation
