When Weakness Persists Beyond Its Apparent Cause: A Case Report of Myasthenia Gravis.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction characterised by fluctuating, fatigable skeletal muscle weakness. Its overlap with reversible causes of weakness such as hypokalemia can easily delay recognition. We report the case of a 40-year-old man who presented with one week of profuse diarrhea and vomiting, along with two days of generalized limb weakness. Investigations revealed hypokalemia and hypomagnesemia, and his limb weakness partially improved after fluid and electrolyte correction, which raised suspicion of an additional underlying cause contributing

Ganglioneuroma of the sacral canal: an unusual location with MR imaging findings.

Ganglioneuroma is a rare, benign, slow-growing tumor arising from embryonic neural crest cells that differentiate into the sympathetic ganglia. Accordingly, it can occur anywhere along the sympathetic chain, from the clivus to the sacrum, although it is seen most commonly in the mediastinum, retroperitoneum, adrenal glands, and neck. Although involvement at the sacral level is uncommon, ganglioneuroma typically arises from the anterior surface of the sacrum, with presacral lesions occasionally extending through the sacral foramina into the sacral canal. Only two cases of ganglioneuroma confine

Anterior mediastinal epithelioid hemangioendothelioma associated with myelolipoma and extramedullary hematopoiesis mimicking a mature teratoma: A radiologic-pathologic correlation.

Epithelioid hemangioendothelioma (EHE) is an ultra-rare vascular neoplasm with variable biologic behavior. Mediastinal EHE is particularly uncommon and may mimic more prevalent prevascular mediastinal tumors. We report a 52-year-old man with a 10.5 × 8.9 × 9.6 cm mixed-density left upper anterior mediastinal mass containing macroscopic fat, soft-tissue components, and coarse calcification. FDG PET/CT demonstrated mild-to-moderate uptake (SUVmax 4.8) without nodal or distant metastatic disease, and the imaging appearance favored mature teratoma. Resection demonstrated a 9.5 cm conventional EHE

[Hamman Syndrome: A series of 5 cases].

Hamman's syndrome, or spontaneous pneumomediastinum, is an uncommon condition characterized by the presence of free air within the mediastinum in the absence of trauma or invasive procedures. We present a retrospective series of five patients treated at the British Hospital of Buenos Aires between 2018 and 2025. The mean age was 20 years, with a predominance of females (3/5). Four patients had a history of bronchial asthma and one reported concomitant marijuana use. The most common clinical manifestations were chest pain, dyspnea, and subcutaneous emphysema. The diagnosis was confirmed by ches

SMARCA4-deficient undifferentiated thoracic tumors with rare nasal cavity and gluteal metastases: a case report and literature review.

BACKGROUND: SMARCA4-deficient undifferentiated thoracic tumor (SMARCA4-UT) is an extremely aggressive malignancy that primarily affects the lung and mediastinum. It predominantly occurs in males with a heavy smoking history, and the associated survival typically does not exceed 1 year. Currently, no standard treatment regimen has been established for this disease, and a limited response to conventional chemotherapy has been noted in SMARCA4-UT. Some studies have suggested that multimodal management combining immunotherapy with chemotherapy and local consolidation therapy may offer a certain su

Thoracoscopic Management of Recurrent Pneumothorax in an Adult Patient with Cri du Chat Syndrome and Severe Scoliosis: A Case Report.

INTRODUCTION: Cri du chat syndrome (CdCS) is a rare genetic disorder that can be associated with scoliosis and marked thoracic deformity. Although scoliosis has been reported to be associated with an increased incidence of pneumothorax, reports of pneumothorax surgery in patients with severe scoliosis associated with CdCS are extremely rare. Herein, we report a case of recurrent pneumothorax in an adult patient with CdCS associated with severe scoliosis who was successfully treated by thoracoscopic surgery. CASE PRESENTATION: A 41-year-old man with CdCS presented with severe scoliosis, intell

Ultrasound-guided percutaneous catheter drainage of deep neck space abscesses: outcomes and predictors of treatment failure.

OBJECTIVES: Clinical evidence supporting ultrasound-guided percutaneous catheter drainage (US-PCD) as a first-line treatment for deep neck space abscesses (DNSAs) remains limited. This study aimed to evaluate the outcomes of US-PCD as initial management for DNSAs and to identify factors associated with treatment efficacy. MATERIALS AND METHODS: This multicenter retrospective cohort study included patients with DNSAs who underwent US-PCD as a first-line treatment at four tertiary hospitals in China between 2015 and 2022. All patients were hemodynamically stable and had a stable airway, with pr

Direct SVC Access for Valve Replacement With Percutaneous Venotomy Suture System Hemostasis: A Way In.

BACKGROUND: Adult patients with congenital heart disease often have limited vascular access due to previous procedures and anatomic variations. CASE SUMMARY: This case describes how a percutaneously placed venotomy suture system allowed for direct percutaneous superior vena cava access for delivery sheath insertion for transcatheter pulmonary valve replacement. This system allowed for a novel access approach and the achievement of hemostasis in an otherwise noncompressible site. DISCUSSION: The availability of percutaneously placed venotomy suture placement systems allows for a more daring a

Giant mixed vascular lymphatic malformation in a pediatric patient: a case report.

BACKGROUND: Giant Mixed vascular lymphatic malformation-is a relatively rare congenital anomaly, particularly in pediatric populations. Mediastinal vascular malformations are uncommon in children, and their exact incidence remains poorly defined in the literature. It is not a true neoplasm but rather a low-flow vascular malformation arising from aberrant development of the vascular system during embryogenesis. Due to nonspecific clinical manifestations (e.g., cough, dyspnea) and the deep location of the mediastinum-adjacent to the heart, great vessels, and airways-diagnosis and management pose

Ciliated foregut cyst of the pancreas mimicking a cystic neoplasm.

Pancreatic cystic lesions are being identified more frequently with widespread use of cross-sectional imaging. While most pancreatic cysts are mucinous neoplasms or pseudocysts, congenital cysts of foregut origin are exceptionally rare in this location, with these being commonly found in the mediastinum. Their occurrence in the hepatobiliary system is rare, and pancreatic involvement is even rarer. Fewer than 20 cases have been reported in literature since it was first described in the 1990s. Ciliated foregut cysts arise from abnormal development of the primitive foregut and, histologically, a

Two-stage surgical resection of a massive primary posterior mediastinal dedifferentiated liposarcoma with bilateral thoracic extension: a case report.

INTRODUCTION AND IMPORTANCE: Primary mediastinal dedifferentiated liposarcoma (DDLPS) is a rare and aggressive tumor, often diagnosed late due to nonspecific symptoms. The optimal surgical strategy for tumors with bilateral thoracic extension remains unclear. CASE PRESENTATION: A 59-year-old woman presented with a 1-year history of exertional dyspnea. Chest computed tomography revealed a large posterior mediastinal mass extending into both thoracic cavities with compression of adjacent structures. A two-stage surgical approach was performed, with initial right thoracotomy followed by left tho

Stable disease after multimodal treatment of SMARCA4-deficient undifferentiated tumor concurrently involving the stomach and mediastinum: a case report.

We report a clinically rare SMARCA4-deficient undifferentiated tumor concurrently involving the stomach and mediastinum. A 54-year-old man presented with a 1-month history of left upper abdominal pain radiating to the anterior chest and back. Contrast-enhanced computed tomography of the chest and abdomen showed thickening and abnormal enhancement of the gastric wall along the greater curvature, multiple enlarged lymph nodes along the lesser curvature, and a posterior inferior mediastinal mass. Biopsies of both the gastric body lesion and the mediastinal mass revealed a poorly differentiated/un

Case Report: A case of mediastinal parathyroid adenoma leading to hypercalcemic crisis complicated by acute kidney injury and postoperative hungry bone syndrome.

A 61-year-old man was admitted to the hospital with fatigue and hypercalcemia, with serum calcium levels as high as 6.16 mmol/L and parathyroid hormone (PTH) levels >5000 pg/mL, accompanied by progressively rising serum creatinine levels (150→266 μmol/L). A neck ultrasound revealed a small nodule in the left parathyroid region, but 99mTc-MIBI imaging identified a focal area of increased radiotracer uptake within the mediastinum. Following multidisciplinary consultation, a thoracoscopic mediastinal mass resection was performed, and pathology confirmed an ectopic parathyroid adenoma (3.2 × 2.8 ×

Posterior Mediastinal Biopsy Gone Awry: Managing Biopsy Needle-Induced Intubation Challenges in Emergency Surgery.

Foreign bodies entering the thorax via a posterior trajectory present distinct challenges in positioning and intubation, particularly during emergency exploration. We present the case of a 37-year-old female who sustained a subclavian artery perforation during a computed tomography-guided biopsy for a posterior mediastinal mass. The biopsy needle advanced to the left common carotid artery. She was brought in the prone position for emergency surgical exploration, which posed significant challenges for airway management. This case highlights the importance of flexible airway strategies, multidis

Unicentric hyaline-vascular Castleman disease of the jejunal mesentery mimicking a gastrointestinal stromal tumor or neuroendocrine tumor: a case report.

BACKGROUND: Castleman disease (CD) is a rare lymphoproliferative disorder classified as unicentric (UCD) or multicentric (MCD). The hyaline vascular variant of UCD most commonly arises in the mediastinum, whereas mesenteric involvement is exceedingly rare. As it can mimic hypervascular mesenteric tumors like neuroendocrine tumors (NETs) and gastrointestinal stromal tumors (GISTs), preoperative diagnosis remains challenging. CASE PRESENTATION: A 36-year-old woman presented with a 3-month history of vague abdominal discomfort, early satiety, and an unintentional 9-kg weight loss. Contrast-enhan

The role of thoracic interventional radiologists in the management of chest infections: a pictorial review.

Thoracic infections remain a major cause of morbidity and mortality worldwide, requiring accurate and timely diagnosis to guide appropriate treatment. This pictorial review illustrates the role of the interventional thoracic radiologist in the management of infectious processes involving the lung, pleura, mediastinum, and chest wall. We describe a range of non-vascular image-guided percutaneous diagnostic and therapeutic techniques. These include targeted sampling for microbiological identification in complicated pneumonias and cavitary lesions, as well as image-guided drainage of pulmonary, p

Delayed diagnosis of selected thoracic and mediastinal conditions in young adults: a focused narrative review of clinical pitfalls, cognitive biases, and system barriers.

BACKGROUND: Thoracic and mediastinal conditions affecting the esophagus, diaphragm, mediastinum, and airway carry considerable morbidity when diagnosis is delayed. Young adults are often perceived as low risk for serious thoracic disease, and low clinical suspicion, atypical symptoms, and cognitive and healthcare-system barriers can delay recognition. METHODS: This focused narrative review searched PubMed and Scopus, supplemented by Google Scholar, for studies published between January 2000 and May 2026, with selected earlier landmark studies retained where essential. Direction of discordance between PET/CT, EBUS-TBNA, and multidisciplinary tumor board nodal assignment in non-small cell lung cancer: A retrospective observational study.

Mediastinal nodal classification in non-small cell lung cancer (NSCLC) at the N0/N1-N2/N3 treatment-defining boundary guides therapeutic strategy and is commonly finalized in multidisciplinary tumor board (MDT) meetings. We compared positron emission tomography/computed tomography (PET/CT) and endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) against MDT assignment and quantified discordance at this threshold. We retrospectively analyzed 114 consecutive NSCLC patients who underwent both PET/CT and EBUS-TBNA before MDT review. Nodal stage was dichotomized as N0/N1 vs
