Duodenum inversum presenting with intestinal obstruction in a 40-day-old infant: a case report.

BACKGROUND: Duodenum inversum is a rare congenital anomaly characterized by an abnormal course of the duodenum with preservation of a normally positioned duodenojejunal junction. Clinical presentation is highly variable, ranging from incidental findings to symptoms of upper gastrointestinal obstruction. Due to its rarity, diagnosis and management remain challenging, particularly in pediatric patients. CASE PRESENTATION: We report the case of a 40-day-old female infant presenting with recurrent postprandial vomiting and poor weight gain since the first days of life. Initial clinical, metabolic

Laparoscopic Right Hemicolectomy Using A Cranial-First Approach: Technical Description and Early Outcomes.

There are various surgical approaches for laparoscopic radical right hemicolectomy. However, the Henle's trunk and middle colic vessels often reveal anatomical variations, and improper handling may lead to vascular injury. This protocol presents a "cranial-first" approach combined with complete mesocolic excision (CME) to attempt to overcome the aforementioned technical difficulties. 86 cases were collected from September 2023 to April 2026. First, the gastrocolic ligament was opened to expose the right gastroepiploic vessels and the pancreatic head. The colic branches of Henle's trunk and the

Unusual Tumor in an Unusual Localization: Sclerosing Extramedullary Hematopoietic Tumor in Breast With Literature Review.

BackgroundSclerosing extramedullary hematopoietic tumor (SEMHT) is a rare entity that can occur in patients with myeloproliferative neoplasms. It has been reported in various localizations including retroperitoneum, mesentery, liver, colon, lymph nodes, thyroid, and orbit. It is considered as late manifestation of the underlying myeloid neoplasm. It is clinically important to distinguish from extramedullary hematopoiesis, as well as mesenchymal tumors.PresentationA 81-year-old female patient with a history of left breast ductal carcinoma in situ who underwent breast lumpectomy followed by radi

Dual single-port laparoscopic surgery for metastatic low-grade serous ovarian carcinoma: a case report of an innovative minimally invasive approach adhering to no-touch isolation technique.

The management of peritoneal metastases in young patients with ovarian cancer demands a delicate balance between oncological radicality and quality of life. We present the case of a 34-year-old woman with a final pathological diagnosis of primary advanced-stage (FIGO IIIC) low-grade serous ovarian carcinoma (LGSC) of the left ovary, with metastases to the sigmoid colon, pelvic peritoneum, vesical peritoneum, diaphragm, and omental surface. The diagnosis was confirmed by comprehensive immunohistochemistry (PAX8+, WT1+, ER+, PR+, CA125+, p53 wild-type) and complete histopathological examination.

Case Report: Erdheim-Chester disease presenting as abdominal cocoon syndrome.

Erdheim-Chester disease (ECD) is a rare histiocytic neoplasm driven by MAPK pathway mutations. Although it frequently involves the bones, heart, and kidneys, initial manifestations predominantly affecting the gastrointestinal tract and mesentery are exceedingly rare, often leading to significant delays in clinical diagnosis. This report describes a 61-year-old female patient who presented with recurrent abdominal pain over several months and was initially diagnosed with a conventional "adhesive bowel obstruction" at an external hospital. However, surgical exploration revealed a classic "abdomi

FRMD6 expression can predict relapse of colorectal carcinoma after cytoreductive surgery with HIPEC.

BACKGROUND: 4.1-ezrin-radixin-moesin (FERM) domain containing protein 6 (FRMD6), a component of Hippo/YAP pathway, behaves as a tumor suppressor gene and is one of the proteins used for colorectal carcinomas (CRC) molecular classification. As its potential prognostic role has not been widely analyzed, this study aims to determine it in the management of CRC with peritoneal involvement. METHODS: Clinicopathological data from 37 patients undergoing cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) for CRC with peritoneal involvement after preoperative chemotherap

From Leiomyoma to Smooth Muscle Tumor of Uncertain Malignant Potential: A Rare Recurrent Right Retroperitoneal Mass in a Young Male.

Smooth muscle tumors of uncertain malignant potential (STUMP) are rare mesenchymal neoplasms that exhibit histopathological features intermediate between benign leiomyoma and leiomyosarcoma. STUMP is overwhelmingly a disease of the uterus in perimenopausal and postmenopausal women; extra-uterine STUMPs are exceedingly uncommon, and involvement of the retroperitoneum in a male patient is exceptionally rare, with only a handful of cases described in the literature to date. We report a unique case of a young male who initially underwent excision of a retroperitoneal leiomyoma and subsequently dev

Pyelolymphatic backflow demonstrated on computed tomography urography in acute ureteric obstruction.

A man in his early 40s presented with acute right flank pain, vomiting and dysuria. Ultrasonography demonstrated an obstructing right upper ureteric calculus with a contralateral renal calculus. CT urography revealed fine, serpiginous contrast-opacified tubular channels extending from the right renal pelvis into the retroperitoneum without evidence of urinoma. Delayed excretory phase imaging demonstrated convergence towards the para-aortic region, consistent with pyelolymphatic backflow rather than collecting system rupture. The imaging pattern reflected markedly elevated intrarenal pressure a

Primary malignant mesothelioma of the tunica vaginalis presenting as hydrocele: imaging pitfalls.

A man in his late 70s presented with progressive, painless scrotal swelling. The initial ultrasound was reported as a simple hydrocele, and he was scheduled for surgery. At operation, friable nodules on the tunica vaginalis were biopsied, confirming malignant epithelioid mesothelioma. Staging CT showed no pleural or peritoneal disease, supporting a primary tunica vaginalis origin. As the tunica vaginalis is an embryological continuation of the peritoneum, it is exceptionally rare for mesothelioma to arise here as a true primary rather than as a secondary spread from the peritoneum. This case h

Concomitant Hydatid Disease of the Lower Extremity and Retroperitoneum: A Case Report of Two Rare Locations.

Cystic echinococcosis is an endemic parasitic disease in Morocco, characterized by cystic lesions, most commonly in the liver or lungs, with lesions in other locations being less common. We report the case of a 38-year-old woman presenting with chronic pain and swelling in her left thigh. An ultrasound of the thigh revealed a multilocular mass located on the posteromedial aspect of the thigh. An MRI of the left thigh showed a multilocular subfascial cystic mass containing multiple fluid-filled vesicles and a sinuous hypointense membrane (serpent sign). An MRI of the thoracolumbar spine and an

Gastrin and cholecystokinin 2 receptor, and gastrin-releasing peptide receptor in cancer: From pathogenesis to precision medicine.

Gastrin regulates gastric acid secretion and mucosal homeostasis via the cholecystokinin-2 receptor (CCK2R). Aberrant signaling via CCK2R (cholecystokinin family) and the distinct class A GPCR, gastrin-releasing peptide receptor (GRPR; bombesin family), drives oncogenesis across diverse GI and non-GI malignancies through calcium mobilization, MAPK/PI3K activation, EGFR transactivation, and constitutive splice variants like CCK2Ri4sv. Furthermore, precursor forms such as progastrin remodel the tumor microenvironment by interacting with non-CCK binding sites (p78, Annexin A2), converting fibrobl

Intraperitoneal Parasitic Leiomyoma With Torsion.

Extrauterine leiomyomas are rare histologically benign tumors. These usually arise in the pelvis and may develop an auxiliary blood supply and become parasitic. Parasitic leiomyoma occurring in patients without prior abdominal or pelvic surgery is extremely rare, with fewer than 30 reported cases in the literature. We report a rare case of parasitic leiomyoma in a 41-year-old woman with no uterine leiomyoma or history of previous surgery. She presented with recurrent lower abdominal pain of one-month duration, and ultrasound revealed a solid, heterogeneously hypoechoic lesion in the hypogastri

Incidental discovery of a massive primary retroperitoneal serous cystadenoma following trauma.

A woman in her 70s presented with severe abdominal pain following a mechanical fall. A CT of the abdomen revealed a massive cystic lesion with evidence of rupture, while MRI localised the lesion to the left retroperitoneum; however, its aetiology remained unclear. She underwent laparotomy with extensive adhesiolysis and excision of a large multiloculated retroperitoneal cyst containing approximately 6500 mL of serous and haemorrhagic fluid. Histopathological examination demonstrated a benign serous cystadenoma lined by ciliated serous epithelium with tubal metaplasia and no evidence of atypia.

Extraperitoneal versus transperitoneal robot-assisted laparoscopic total prostatectomy: Comparison after propensity score matching.

BACKGROUND: Robot-assisted laparoscopic radical prostatectomy (RALRP) has become the standard surgical approach for the treatment for localized prostate cancer. Two main surgical site access are commonly used: transperitoneal (TP) and extraperitoneal (EP). While high-volume specialized centers have demonstrated comparable outcomes between these two techniques, data from real-world clinical practices remain limited. The aim of our study was to compare the morbidity and oncological outcomes of RALRP via TP and EP in a standard clinical setting, outside of high-specialty centers. METHOD: We cond

From operating room to pathology report: Delphi consensus on pathological assessment of peritoneal cytology and biopsies in staging laparoscopy for gastric cancer.

BACKGROUND: Staging laparoscopy with peritoneal cytology and biopsies is recommended in selected patients with gastric cancer (GC), but practical guidance for assessment of peritoneal lavage/ascites and peritoneal biopsies is lacking. We aimed to develop consensus-based recommendations for handling, assessing and reporting of these specimens. METHODS: A modified Delphi study was conducted among Dutch experts in GC (cyto)pathology. In round 1, sixty-two statements on clinical information, specimen requirements, initial assessment, ancillary testing, biomarker testing and reporting were rated o

Well-differentiated papillary mesothelial tumor: an incidental finding in esophagogastrectomy mimicking metastasis.

Well-differentiated papillary mesothelial tumor is a benign mesothelial tumor that can be found in the peritoneum, pleura, or tunica vaginalis. It is usually an incidental finding during surgery or radiological examinations. Because it is usually a multifocal process, it can be mistaken for metastatic disease, particularly in patients with a history of cancer. We present the case of a 61-year-old female patient with adenocarcinoma of the lower esophagus who had omental nodules identified incidentally during a partial esophagogastrectomy; these nodules were histologically diagnosed as a well-di

Primary Amenorrhea due to Mayer-Rokitansky-Küster-Hauser Syndrome Managed with Laparoscopic Davydov Procedure: A Case Report.

Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is a congenital Müllerian anomaly characterized by uterovag-inal agenesis in phenotypic females with normal second-ary sexual characteristics and preserved ovarian function. Vaginal reconstruction may be required to allow satisfac-tory sexual function, particularly when nonsurgical dilation is declined, not tolerated, or not preferred. Laparoscopic Davydov vaginoplasty uses autologous peritoneum to line the neovaginal canal and may provide favorable anatomical outcomes when postoperative dilation is performed consis-tently.
