Does exposing all parathyroids during total thyroidectomy with central lymph node dissection protect long-term function? A prospective cohort study.

BACKGROUND: Hypoparathyroidism (HypoPT) is the most common complication after total thyroidectomy (TT) and central lymph node dissection (CLND) for thyroid carcinoma. However, there's still no effective surgical strategy to prevent its incidence. This study evaluated the impact of sufficient intraoperative exposure of the parathyroid glands (PG) on postoperative parathyroid function, the rate of parathyroid autotransplantation, and incidental parathyroidectomy. METHODS: Prospective data from 133 consecutive patients who underwent TT and CLND by a single surgeon at the First Affiliated Hospita

Parathyroid adenoma causing hypercalcemic crisis masquerading as acute abdomen in a 7-year-old: a case report.

BACKGROUND: Hypercalcemic crisis from a functioning parathyroid adenoma (PA) is exceptionally rare in prepubertal children and typically presents with a non-specific complaint such as polyuria or lethargy. We report a 7-year-old girl who was urgently referred for acute, colicky abdominal pain and repeated vomiting, which clinically mimicked acute appendicitis. During the work-up, extensive metabolic tests incidentally revealed severe hypercalcemia and elevated intact parathyroid hormone (PTH), which led to the diagnosis of PA-induced hypercalcemic crisis. Her initial manifestation was an acute

Enhanced parathyroid identification and preservation in total thyroidectomy using combined near-infrared autofluorescence and carbon nanoparticles.

OBJECTIVE: Postoperative hypoparathyroidism remains a common complication after total thyroidectomy. This study aimed to assess whether combining near-infrared autofluorescence (NIRAF) imaging with carbon nanoparticle suspension (CN) improves parathyroid gland (PG) identification and functional preservation in patients undergoing total thyroidectomy for papillary thyroid carcinoma (PTC). METHODS: A total of 80 patients with PTC undergoing total thyroidectomy with central neck dissection (CND) were randomly divided into two groups. The CN group (n = 40) received CN alone to distinguish PGs, wh

Experience-informed interpretation of contrast-enhanced CT for parathyroid localization in Sestamibi-Negative Primary Hyperparathyroidism.

PURPOSE: Accurate preoperative localization of the pathological parathyroid gland is critical for the success of minimally invasive parathyroidectomy (MIP) in primary hyperparathyroidism (PHPT). Localization, however, remains challenging in patients with negative or non-localizing 99mTc-sestamibi scintigraphy (MIBI). This study evaluated whether experience-informed interpretation of contrast-enhanced neck computed tomography (CT) improves localization accuracy in this subgroup. METHODS: This retrospective study included PHPT patients who underwent parathyroidectomy between April 2021 and Marc

Parathyroid autotransplantation: a phase-based framework for surgical decision-making.

Parathyroid autotransplantation (PAT), a technique in which parathyroid tissue is excised from its original site and transplanted to an alternative site within the same individual, serves as a pivotal preventive and therapeutic strategy for hypoparathyroidism. Its success is not guaranteed by a single intervention but is determined by a series of crucial decisions that directly influence autograft survival and function, such as the physiological or pathological state of the parathyroid gland, revascularization, and the quality of the surgical process. This review comprehensively delineates the

Diagnostic accuracy of a thyrothymic ligament-based nomenclature for ultrasound classification of inferior parathyroid gland location.

BACKGROUND: Preoperative knowledge of inferior parathyroid gland (IPTG) location and vascular supply is critical for in situ preservation, yet evidence on imaging accuracy for IPTG localization remains limited. OBJECTIVE: To evaluate the diagnostic accuracy of an ultrasound classification system for IPTG location guided by the thyrothymic ligament (TTL). METHODS: This prospective, single-center study enrolled consecutive patients scheduled for thyroidectomy from January 2025 to September 2025. The reference standard was surgical exploration confirmed by negative mitoxantrone hydrochloride st

Artificial Intelligence (AI) and Parathyroid Gland Preservation During Thyroidectomy.

Preserving the parathyroid glands (PGs) is crucial during thyroidectomy to avoid hypoparathyroidism and its related complications. Recently, artificial intelligence (AI) based techniques have been introduced intraoperatively, allowing surgeons to reduce the level of subjectivity and assist with the identification of PGs accurately and quickly. The aim of this narrative review is to assess the current evidence on the application of AI in parathyroid gland (PG) preservation. PubMed, Google Scholar, and Embase were used for the search for various types of studies and reviews dating from 2015 up t

Primary hyperparathyroidism in pregnancy: a case report and review of the literature.

Primary hyperparathyroidism during pregnancy represents an exceedingly rare clinical entity. Its early manifestations are frequently subtle and easily masked by physiological adaptations of gestation, frequently leading to diagnostic delay. Persistent hypercalcemia carries substantial hazards to both maternal and fetal wellbeing, while relevant clinical literature remains limited. Herein, we present an advanced maternal age patient with recurrent pregnancy loss complicated by second-trimester persistent nausea and vomiting. Serum biochemistry demonstrated marked hypercalcemia and elevated para

Recurrent Hyperparathyroidism Due to Hyperactive Ectopic Intrathymic Parathyroid Gland 20 Years After Parathyroidectomy.

Primary hyperparathyroidism is most commonly caused by a single adenoma, with multiglandular disease accounting for a smaller proportion of cases. A recurrent hyperparathyroidism decades after prior parathyroidectomy is uncommon and often presents with nonspecific multisystem symptoms that delay diagnosis. Ectopic parathyroid tissue, particularly in intrathymic or retrosternal locations, represents a challenging but important cause of persistent or recurrent disease. A 43‑year‑old woman with a history of three‑gland parathyroidectomy 20 years earlier presented with a year of progressive fatigu

Preoperative, Intraoperative, and Postoperative Parathyroid Pathology: Clinical Pathologic Collaboration for Optimal Patient Management.

Parathyroid disease typically presents with parathyroid hyperfunction as result of neoplasia or a consequence of non-neoplastic systemic disease. Given the parathyroid gland is a hormonally active organ with broad physiologic implications and serologically accessible markers for monitoring, the diagnosis of parathyroid disease is predominantly a clinical pathologic correlation. We provide the current pathological correlates of parathyroid disease and discuss preoperative, intraoperative, and postoperative pathology consultative practice for optimal patient care.

Predictive value of a normal ipsilateral parathyroid gland for single-gland disease in primary hyperparathyroidism.

BACKGROUND: When treating primary hyperparathyroidism (PHPT), surgeons use visual cues to guide decision-making and judge disease extent. We assessed assess whether visual identification of a normal-appearing ipsilateral parathyroid gland (IPG) during parathyroidectomy predicts single-gland disease in patients with PHPT. METHODS: We retrospectively analyzed patients undergoing parathyroidectomy for PHPT. Diagnostic performance of a normal IPG appearance was assessed, and logistic regression identified independent predictors of single-gland disease. RESULTS: 572 patients were included. Mean a

Tertiary hyperparathyroidism with ectopic mediastinal parathyroid adenoma in a patient with chronic kidney disease and cardiovascular complications: a case report.

BACKGROUND: Hyperparathyroidism includes conditions marked by excessive secretion of parathyroid hormone (PTH), resulting in disruptions in calcium-phosphate metabolism. In individuals with chronic kidney disease (CKD), differentiating secondary hyperparathyroidism from tertiary hyperparathyroidism is frequently challenging due to the overlapping biochemical and clinical characteristics. Ectopic mediastinal parathyroid adenomas are uncommon and complicate localisation and therapy further. CASE PRESENTATION: We present a 66-year-old female with chronic kidney disease, hypertension, and ischaem

Reduced dosage of Kmt2d modifies Tbx1 haploinsufficiency toward phenotypes of 22q11.2DS.

Haploinsufficiency of TBX1, which occurs in 22q11.2 deletion syndrome (22q11.2DS), leads to a heterogeneous spectrum of clinical manifestations, including craniofacial anomalies, immunodeficiency, and congenital heart defects. The variability in syndromic presentation between patients may be partially explained by variants in chromatin regulatory genes that act to further modify TBX1 function. To investigate this relationship, we selected KMT2D as a candidate gene because of its role in the etiology of Kabuki syndrome, which shares overlapping features with 22q11.2DS. We demonstrate that condi

Near-infrared autofluorescence parathyroid imaging during total thyroidectomy to reduce the rate of postoperative hypoparathyroidism: A prospective two-center randomized clinical trial with a comprehensive long-term follow-up.

BACKGROUND: Postoperative hypoparathyroidism remains the most common complication after total thyroidectomy. Near-infrared autofluorescence leverages the intrinsic fluorescence of parathyroid glands, enabling intraoperative visualization and potentially improving gland preservation, thereby reducing postoperative hypoparathyroidism. METHODS: A randomized clinical trial conducted at two Danish centers assigned total thyroidectomy patients to either the near-infrared autofluorescence or control group. Blood samples and data on hypoparathyroidism treatment were collected preoperatively, postoper

Subacute thyroid injury and relative parathyroid preservation following ionizing radiation: altered HSP-90 immunoexpression in a rat model.

Ionizing radiation is widely used in the treatment of head and neck malignancies. However, its subacute effects on adjacent endocrine tissues have not yet been fully elucidated. This study aimed to investigate the structural changes that develop in the thyroid and parathyroid glands following irradiation and to evaluate HSP-90 immunoexpression in these tissues. The researchers initially randomized eighteen male Wistar-Hannover rats into control and irradiation groups (n = 9 per group). Rats in the irradiation group underwent 6-MV photon irradiation of the head and neck target region using a no

Intraoperative near-infrared autofluorescence in parathyroidectomy: associations with gland morphology, body mass index, and histopathology.

BACKGROUND: Identifying parathyroid glands during surgery is challenging. Recent studies suggest that near-infrared autofluorescence (NIRAF) detection can improve their recognition. However, the correlation between positive NIRAF detection and clinical features in parathyroidectomy remains unclear. This study aimed to explore correlation between positive NIRAF detection and clinical features in parathyroid surgery, aiming to improve the application of this technology. METHODS: Prospective data were collected from patients undergoing parathyroidectomy using handheld NIRAF devices. Data include

Autofluorescence can aid in differentiating normal from pathologic parathyroid glands during parathyroidectomy.

BACKGROUND: Several studies suggest that during parathyroidectomy near-infrared autofluorescence may be able to help differentiate hyperfunctional from normal parathyroid glands. However, to date, no clear metric for employing autofluorescence in this manner in real-time during surgery has been described. This study investigated whether a specific measure of autofluorescence heterogeneity can aid discrimination of pathologic from normal parathyroid glands. METHODS: In patients undergoing curative parathyroidectomy for classic or normohormonal primary hyperparathyroidism, a near-infrared autof
