Untreated patients suffering from tuberculoid, lepromatous and indeterminate leprosy, their domiciliary contacts, and healthy controls, all living in Guadeloupe, West Indies, were tested by an ELISA for detecting IgM antibodies to the terminal disaccharide of the phenolic glycolipid-I antigen of Mycobacterium leprae. 27 histologically confirmed indeterminate leprosy cases were evaluated clinically and histopathologically at the SLR & TC, Karigiri, South India in 1985. Identical slides from 200 biopsies obtained from individuals suspected of having leprosy during the course of an epidemiological survey in Northern Malawi were examined sequentially and independently by three histopathologists, using a standard protocol to report their findings. The disability rate in this study was 16.23%. The highest disability rate of 30.76% was found in those aged 45 years and above. Disabilities were found to be more common among male leprosy patients with a disability rate of 21.05% compared to that of 11.45% found among female leprosy patients. In a house to house study of leprosy cases at Jaipur (148) a majority (91) had NL leprosy, 50 had LL leprosy and remaining 7 had indeterminate leprosy. More cases (84.5%) had a poor housing, overcrowding, low socioeconomic status. Out of 59 persons giving history of contact a majority had it at home and others at work places. The period of contact was more than five years in 74.6% cases. Most of the cases had lesions on exposed or easily accessible parts of the body.

Indeterminate histopathology with nerve involvement was seen in clinically uninvolved skin in 100% cases, irrespective of clinical type. This was an interesting observation for TT and BT cases: in such cases, it might be a reflection of the disease process which is generalised right from the beginning.

One hundred and five leprosy patients including 37 cases of LL and LI, 22 cases of BL, 3 cases of BB, 17 cases of BT, 23 cases of TT and 3 cases of indeterminate type during the period of 1980-81 have been examined clinically. In an earlier study (Saoji et al, 1980) we looked for anomalous (additional) isoenzymes in homogenates of 17 active and 25 of regressing lepromatous leprosy, three of erythema nodosum leprosum, eight of BT, 19 of TT and one of indeterminate leprosy. Biopsies from 69 patients with leprosy were stained to demonstrate mycobacterial antigens using immunoperoxidase methods. The same biopsies were cut and stained using Fite-Faraco, TRIFF and hematoxylin-eosin for classifying the patients and to demonstrate mycobacteria.
