A Novel Connector for Intraoperative Nerve Monitoring Probes in Endoscopic Thyroidectomy: Comparative Efficacy and Preclinical Evaluation in a Porcine Model.

BACKGROUND: This study aimed to develop a connector that adapts standard endoscopic instruments into functional nerve stimulator probes. METHODS: The connector underwent engineering validation in a laboratory and preclinical testing using a porcine model. A handheld standard stimulator probe served as the control. The experimental group consisted of a long monopolar probe and two connector-adapted probes. Recurrent laryngeal nerve (RLN) and vagus nerves were stimulated to compare the efficacy and safety of the adapted probes with the standard probe. RESULTS: The connector, when coupled to en

Automated 3D segmentation of human vagus nerve fascicles and epineurium from micro-computed tomography images using anatomy-aware neural networks.

Objective.Precise segmentation and quantification of nerve morphology from imaging data are critical for designing effective and selective peripheral nerve stimulation (PNS) therapies. However, prior studies on nerve morphology segmentation suffer from important limitations in both accuracy and efficiency. This study introduces a deep learning approach for robust and automated three-dimensional (3D) segmentation of human vagus nerve fascicles and epineurium from high-resolution micro-computed tomography (microCT) images.Methods.We developed a multi-class 3D U-Net to segment fascicles and epine

Closed-loop wearable neurostimulation system with triboelectric sensing to alleviate hemifacial spasms.

Hemifacial spasm (HFS) is a neurological disorder characterized by involuntary contractions of the facial muscles, which can significantly influence patients' quality of life. Traditional diagnostic methods are often subjective, and electromyography (EMG) monitoring is constrained by equipment limitations and interference. Current treatments include short-term Botulinum toxin (BTX) injections and high-risk microvascular decompression (MVD), both of which suffer from low patient compliance. This study introduces a closed-loop facial nerve stimulation system integrated into eyewear, featuring tr

Nerves Stimulate Cross-talk Between Gastric Cancer and Group 3 Innate Lymphoid Cells to Enhance Immunosuppression.

UNLABELLED: The immunosuppressive tumor microenvironment (TME) enables cancer cells to evade clinical immunotherapies. Neural networks are vital components of the TME, and interactions among cancer cells, neuronal cells, and immune cells mediate immunosuppression. Hence, understanding the mechanisms of intercellular cross-talk could inform immunomodulatory approaches to enhance immunotherapy efficacy. In this study, we found that the vagus nerve regulated the cross-talk between gastric cancer cells and group 3 innate lymphoid cells (ILC3), boosting immune resistance in gastric cancer by enhanc

Diagnostic performance of ultrasonography in predicting extrathyroidal extension in papillary thyroid carcinoma.

INTRODUCTION: Extrathyroidal extension (ETE) is a critical prognostic factor influencing treatment decisions in papillary thyroid carcinoma (PTC). While ultrasonography (US) is widely used for preoperative ETE assessment, its diagnostic accuracy varies across anatomical sites and tumor characteristics. This study evaluates the performance of US in predicting pathologic ETE in PTC, focusing on strap muscle, tracheal, and recurrent laryngeal nerve (RLN) invasion, stratified by tumor size, histopathology, and molecular profile. METHODS: A retrospective analysis of surgically treated PTC patients

Unintended Vagus Nerve Stimulation From Cuff Electrode During MRI: Combined Effects of Gradient and Radiofrequency Fields.

PURPOSE: Emissions generated during magnetic resonance imaging (MRI)-including gradient coil induced electric fields and radiofrequency coil induced heating near nerve fiber-may alter neural activation inside patients. This study investigates the combined effects of these emissions on vagus nerve activation in the presence of cuff electrodes. METHODS: Electromagnetic, thermal, and neurophysiological simulations were performed to quantify activation thresholds under MRI-induced fields. The study examined the impact of gradient field exposure and RF-induced heating, particularly for the trapezo

Gut-brain cholinergic signaling mediates the antiseizure effects of Bacteroides fragilis.

Gut dysbiosis has been implicated in epilepsy, yet probiotic efficacy and mechanisms remain unclear. Here, we identify that Bacteroides fragilis (B. fragilis) is markedly reduced in children with epilepsy and show that oral B. fragilis administration suppresses seizures in both pentylenetetrazole- and kainic-acid-induced mouse models. Mechanistically, B. fragilis activates colonic choline acetyltransferase-positive (ChAT+) cells and enhances gut-vagus-brain cholinergic signaling, as demonstrated by vagal recordings, pharmacological blockade, and chemogenetic manipulation, identifying a colonic

Peripheral nerve transection predominantly drives sympathetic nerve sprouting in mouse dorsal root ganglia.

Sympathetic sprouting in dorsal root ganglia (DRG) is a feature of sympathetically maintained pain (SMP) after peripheral nerve injury, yet the factors determining its occurrence remain unclear. Here, we compare transection and crush injury models to determine whether injury type or site influence sympathetic remodeling and pain. Using tyrosine hydroxylase-immunoreactivity staining and Phox2b reporter mice to selectively label sympathetic fibers, we found that an L5 spinal nerve transection triggered robust sympathetic fiber sprouting and elevated norepinephrine (NE) levels in the DRG, correla

Vagus nerve-induced cardiac arrest during percutaneous nephrolithotomy: a clinical challenge.

BACKGROUND: Vagal overexcitation is a rare trigger of cardiac arrest during percutaneous nephrolithotomy, and is closely related to the abundant vagal branches in the renal hilum region. When surgical instruments or high-pressure perfusion directly stimulate the vagus nerve, sudden bradycardia or even asystole may be triggered. CASE PRESENTATION: A 56-year-old man undergoing percutaneous nephrolithotomy under general anesthesia experienced cardiac arrest due to vagus nerve stimulation following the injection of a small volume of iohexol into the renal calyx. Cardiopulmonary resuscitation was i

The ansa cervicalis and superior cervical ganglion contribute to the fiber composition of the human hypoglossal nerve and its branches.

The hypoglossal nerve is considered a pure motor nerve. Yet, there are indications that it contains sensory and sympathetic fibers in the periphery. We analyzed changes in the fiber composition along the human hypoglossal nerve, delineating the pathways through which non-motor fibers enter this nerve and describing the direction in which these fibers extend. Five key positions along the nerve were selected for fiber analyses. Position 1 was intracranial. Positions 2-5 were extracranial and in relation to the C1 component of the ansa cervicalis, which is attached to the hypoglossal nerve. Nerve

Vagal control of pulmonary peripheral circulation in the rattlesnake modulates relevant hemodynamic adjustments to temperature change.

The vagal influence over large pulmonary arteries has been clearly demonstrated in Squamates. Additionally, diverse in situ experiments suggest that pulmonary circulation was either not responsive or much less responsive than the systemic circuit to known local, humoral, and nervous modulators. We suggest that the systemic alterations should be counterbalanced by pulmonary circuit modulation in awaken animals with a cardiac shunt, and the lack of pulmonary circuit responses is a collateral effect of the anesthesia used in in situ experiments. The use of a decerebrate experimental model allowed

Effects of Aging on Superior Laryngeal Sensory and Motor Function in a Rat Model.

OBJECTIVE: Aging is a risk factor for diminished laryngeal sensation, dysphagia, and aspiration events; however, the mechanisms underlying age-related swallow dysfunction are not well understood. Some of these changes are thought to be mediated through superior laryngeal nerve (SLN) dysfunction. The purpose of this study was to measure the effects of aging on SLN motor and sensory function in a rat model. STUDY DESIGN: Animal study. SETTING: Tertiary-care center. METHODS: Evoked response studies were performed by SLN stimulation. Outcome measures included compound motor (CMAP) and sensory n

Cortical excitation does not drive changes in skin sympathetic nerve activity during single-pulse transcranial magnetic stimulation in humans.

We have shown that single-pulse transcranial magnetic stimulation (TMS) of the dorsolateral prefrontal cortex (dlPFC) inhibits muscle sympathetic nerve activity. However, this was likely due to arousal caused by the TMS pulses themselves, rather than altering the underlying neuronal circuitry. In extension, we have aimed to explore the effects of single-pulse TMS on skin sympathetic nerve activity (SSNA), which is more sensitive to arousal. It was hypothesised that TMS-evoked arousal would increase SSNA but would not generate de novo bursts from the dlPFC. Microneurographic recordings were tak

Vagus nerve and cancer symptom science.

Patients with cancer are often polysymptomatic due to significant disease and treatment-related morbidity. Vagus nerve stimulation (VNS), a type of neuromodulation, affects various physiological processes that are highly relevant to many cancer-related symptoms. It is an emerging therapy that uses the omnipotent nature of the nerve and is known to ameliorate conditions such as depression, epilepsy, gastrointestinal disorders and migraine. Given the evidence base for VNS and its proven utilisation outside of cancer, we suggest research and clinical application of this treatment modality in supp

MRI findings in pediatric idiopathic intracranial hypertension: evaluating the oculomotor nerve sheath as a potential new diagnostic marker.

PURPOSE: Idiopathic intracranial hypertension (IIH) in the pediatric population remains diagnostically challenging, particularly in the absence of papilledema, and increasingly depends on ancillary neuroimaging findings to support clinical assessment. The aim of this study was to systematically evaluate established magnetic resonance imaging (MRI) and magnetic resonance venography (MRV) markers of elevated intracranial pressure in children and to investigate oculomotor nerve cistern (OMC) distention as a potential novel adjunctive imaging biomarker of pediatric IIH. METHODS: This retrospectiv

Inhibition of the P2Y2 Receptor Promotes Facial Nerve Function by Enhancing Neuron Autophagy.

OBJECTIVE: Facial nerve injury induces autophagy and apoptosis in facial nerve nucleus motoneurons of the CNS, impairing nerve regeneration and functional recovery. The function of P2Y2R after facial nerve injury remains to be determined. This study hypothesizes that inhibiting P2Y2R may play a protective role in facial nerve injury by modulating the autophagy signaling pathway. METHODS: An in vivo mouse model of facial nerve crush injury was utilized in this study. Mice received either a P2Y2R agonist or antagonist through intrathecal injections of 10 μL/daily for 4 weeks. This study measure

A triple-node heart-brain neuroimmune loop underlying myocardial infarction.

Myocardial infarction (MI) triggers adverse cardiac events, immune responses, and nervous system activation, but the neural and neuroimmune mechanisms remain understudied. Using single-cell RNA sequencing (scRNA-seq) and tissue clearing, we identified transient receptor potential vanilloid-1 (TRPV1)-expressing vagal sensory neurons (VSNs) that increase ventricular innervation post MI. Ablating these VSNs mitigated MI pathology, reducing infarct size, abnormal electrocardiograms, cardiac dysfunction, sympathetic innervation, and pro-inflammatory cytokine interleukin 1β (IL-1β). Single-nuclei RN

Roseburia intestinalis Offers Vagus-Dependent Neuroprotection Against Parkinson's Disease.

Parkinson's disease (PD) is characterized by dopaminergic neurodegeneration and increasingly associated with gut microbiota alterations. Roseburia intestinalis (R. intestinalis) is consistently reduced in PD; however, its functional contribution remains unknown. We performed two complementary mouse experiments using a 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP)-induced PD model. In the primary intervention experiment, mice received live or heat-killed R. intestinalis, followed by behavioral assessments and multi-layer analyses, including immunofluorescence, western blotting, enzyme-lin

Efficacy of glossopharyngeal nerve block in managing pain in head and neck cancer: a retrospective cohort study.

BACKGROUND: Head and Neck Cancer (HNC) causes severe pain from tumor invasion, nerve involvement, and treatment effects. Conventional analgesia, including opioids, may be inadequate or poorly tolerated. Glossopharyngeal nerve block (GNB) offers targeted relief, though evidence in cancer pain is limited. METHODS: This retrospective cohort study evaluated efficacy and safety of GNB in HNC patients. Eighty-two patients with refractory pain received GNB using 2.2 mL of 40 mg triamcinolone 5 mg bupivacaine via peristyloid or pharyngeal wall approach under ultrasound guidance. Pain was assessed usi

Chronic Stress Induces Hepatic Steatosis via Brain-Hepatic Sympathetic Axis Mediated Catecholamine Resistance.

Chronic stress is epidemiologically linked to metabolic dysfunction-associated steatotic liver disease (MASLD), yet the underlying mechanisms remain unclear. In mice exposed to chronic restraint stress (CRS), we observed weight-independent hepatic steatosis with marked degeneration of sympathetic fibers. Stress elevated circulating norepinephrine levels but blunted hepatic β-adrenergic/cyclic adenosine monophosphate (cAMP) signaling accompanied by downregulation of β3-adrenergic receptor (β3-AR), indicating hepatic catecholamine resistance. Blocking hepatic sympathetic input prevented stress-a
