Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND: Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital time (symptom onset to first surgical assessment) has not been explored in contemporary global data. METHODS: We conducted an international, multicentre, prospective, observational cohort study (AlliGatOr) of patients undergoing emergency appendicectomy for suspected acute append

Myxobolus binotati n. sp. (Myxozoa: Myxobolidae) from the ovaries of spotted barb, Barbodes binotatus (Valenciennes, 1842) in Terengganu, Malaysia.

To date, only 14 species have been reported worldwide from the ovaries of fish hosts. In the present study, a novel species, Myxobolus binotati n. sp., was described from the ovary of Barbodes binotatus. Spores were oval in frontal view and lemon-shaped in sutural view, measuring 10.7 × 8.3 × 6.2 μm. They possessed two unequal pyriform polar capsules, with larger measuring 4.8 ± 0.4 (4.0-5.8) μm in length, 2.8 ± 0.2 (2.4-3.4) μm in width, while smaller measuring 2.7 ± 0.3 (2.1-3.1) μm in length, 1.6 ± 0.2 (1.3-1.9) μm in width. Polar tubules coiled 7-8 and 4 turns in the larger and smaller cap

Influence of various reproduction substrates on the survival, health, and reproductive output of broodstock Mediterranean medicinal leech (Hirudo verbana).

The captive breeding of non-traditional aquatic species is becoming increasingly important for both niche animal production and the conservation of overexploited taxa; however, breeding protocols for these species remain largely undocumented. The Mediterranean medicinal leech (Hirudo verbana), which is clinically used in reconstructive surgery and listed on CITES Appendix II, deposits its cocoons on a moist terrestrial substrate; this makes the reproductive substrate a critical but untested management variable. We compared four substrates available to leech breeders, namely unamended fresh lak

A validated forensic STR profiling method for the Peregrine Falcon (Falco peregrinus) to identify individuals and investigate familial relationships.

Falconry, and the captive breeding of Peregrine Falcons (Falco peregrinus) and their hybrids is a thriving industry in both the UK and the Middle East, with thousands of birds traded internationally on an annual basis. Wild Peregrine Falcons, having suffered a population collapse in the 1960s, are protected by UK legislation and the Convention on International Trade in Endangered Species of Flora and Fauna (CITES) lists Peregrines on Appendix I, which restricts international trade in these birds. As such, in the UK, captive-bred birds must be registered within a few weeks of hatching and birds

Hydrocele stone presentation and crystallographic analysis: a unique case report.

INTRODUCTION AND IMPORTANCE: Hydroceles are typically benign fluid collections around the testes. Rarely, scrotal calculi, or "hydrocele stones," form within the tunica vaginalis, possibly due to chronic inflammation, torsion of the appendix testis, minor trauma, or other mechanisms. These stones are infrequently reported, and cases with detailed crystallographic analysis are exceptionally rare. CASE PRESENTATION: We present a 43-year-old male with bilateral hydroceles, a history of psoriasis and dyslipidemia, and prior right inguinal hernia repair. Ultrasound revealed a moderate right hydroc

MRI of acute appendicitis in pregnancy: navigating diagnostic pitfalls and challenges.

Non-contrast magnetic resonance imaging (MRI) is the preferred cross-sectional imaging modality for suspected appendicitis in pregnancy, yet its interpretation is uniquely challenging. This pictorial review moves beyond simple appendiceal diameter thresholds to the practical pitfalls encountered in daily practice. We show how the enlarging gravid uterus displaces the appendix and how physiologic changes mask inflammatory signs. We also address the diagnostic uncertainty of the equivocal appendix and the need to recognize subtle secondary features on MRI. We then detail several important entiti

Heterotopic pancreas as a rare lead point of ileal intussusception mimicking acute appendicitis in a child: a case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Heterotopic pancreas is an uncommon developmental anomaly characterized by pancreatic tissue located outside the normal pancreas without anatomical or vascular continuity. Although it is most frequently identified in the stomach and duodenum, involvement of the ileum is rare, particularly in children. When present in the small intestine, it may act as a pathological lead point for intussusception, posing a considerable diagnostic challenge because the underlying lesion is often not identified by preoperative imaging. PRESENTATION OF CASE: We report the case of a 6

Two-stage laparoscopic management of De Garengeot hernia with gangrenous appendicitis: a case report and literature review.

INTRODUCTION AND IMPORTANCE: De Garengeot hernia (DGH), the presence of the appendix within a femoral hernia sac, is exceptionally rare, and acute appendicitis in this setting is even less common. Management is challenging, particularly regarding mesh placement in contaminated fields. This report describes the first two-stage laparoscopic repair with bilateral mesh placement for gangrenous DGH in a young patient. CASE PRESENTATION: A 44-year-old woman presented with right-sided groin pain and swelling. Examination revealed a tender right inguinal mass without systemic signs. Computed tomograp

Neonatal appendicitis with rectal bleeding: an unusual presentation.

BACKGROUND: Neonatal appendicitis is an exceptionally rare condition with a high risk of delayed diagnosis and perforation. Its nonspecific clinical presentation may mimic other causes of acute abdomen and gastrointestinal bleeding in newborns, particularly necrotizing enterocolitis. CASE PRESENTATION: A 36-week, 2500-g male neonate developed abdominal distension, irritability, decreased oral intake, and bright red rectal bleeding on the second day of life. Abdominal radiography demonstrated multiple air-fluid levels, while ultrasonography revealed dilated bowel loops and free intraperitoneal

Comparing the Illness Perception, Help-Seeking Behaviors, and Healthcare Determinants Between the Rural and Suburban Indigenous People in Malaysia: A Socio-Biological Perspective.

Due to the health disparities among the Indigenous population, this theory-driven study aimed to compare the suburban and rural Indigenous populations in their illness perceptions, help-seeking behaviors, and determinants of healthcare utilization. Between 2022 and 2023, we conducted a community-based structured survey with the Orang Asli (OA, Indigenous people) residing in suburban (N = 70) and rural regions (N = 131) of Peninsular Malaysia. The orally administered survey consists of demographic and socio-economic info, indigenously developed measurements of illness expression, explanations f

Incidental Appendiceal Adenocarcinoma Discovered After Ileocecectomy for Recurrent Small Bowel Obstruction: A Case Report.

We report a case of moderately differentiated appendiceal adenocarcinoma discovered on histopathologic examination following ileocecectomy for recurrent SBO in a woman in her early 50s with a history of stage III rectal adenocarcinoma in remission. Notably, serial surveillance imaging over two years had demonstrated a radiographically normal appendix prior to the obstructive presentation. Immunohistochemical and histopathologic features confirmed a metachronous primary malignancy rather than metastatic recurrence. This case exemplifies the diagnostic challenge posed by occult appendiceal neopl

One year follow up outcomes of endoscopic retrograde appendicitis treatment for symptomatic appendicoliths.

Symptomatic appendicoliths may cause recurrent abdominal pain or distension, but appendix-preserving treatment options are limited. We retrospectively evaluated endoscopic retrograde appendicitis treatment (ERAT) in 78 patients with CT- or ultrasound-confirmed appendicoliths. Symptoms were assessed using a modified abdominal pain or distension questionnaire (APQ), and outcomes were evaluated separately in adults and minors. APQ scores decreased at 1 week, 1 month, 6 months, and 1 year after ERAT. At 1 year, complete remission was achieved in 37 of 47 adults (78.7%) and 27 of 31 minors (87.1%).

A Twist in the Tale: Rare Diagnosis of Lipomatosis of the Appendix.

A 66-year-old white male with a history of diverticulosis and prediabetes presented with right lower quadrant (RLQ) abdominal pain lasting 1 month. On physical examination, the abdomen was soft without tenderness or palpable masses. CT imaging showed an enlarged appendix with a diameter of 1 cm with no surrounding inflammation. A laparoscopic appendectomy was performed, and the patient's symptoms resolved within 3 weeks. Histopathology revealed lipomatosis of the appendix, an extraordinarily rare diagnosis which has only been reported five times prior to our case. This case study aims to incre

Development and Content Validation of the Epidermolysis Bullosa Oral Health Assessment Form (EB-OHAF).

OBJECTIVES: Epidermolysis Bullosa (EB) is a rare skin disorder with significant phenotypic variations according to the primary type and subtype of the condition. As a rare disease with uncommon oral features and high phenotypic variability, identifying all characteristics is important to define a patient-specific treatment strategy and monitor the impact of different strategies on the oral outcomes. This project aims to develop and validate an EB-specific oral health assessment tool that helps clinicians to identify every specific oral feature that can be presented in patients with different t
