Accounting for urgency: Implications of risk adjustment for surgical site infection after emergency colon surgery at safety net hospitals.

BACKGROUND: Colon operations performed for urgent indications or trauma are associated with a higher risk of surgical site infection. Prior models for surgical site infection surveillance did not account for this urgency, potentially yielding inadequate risk adjustment for surgical site infection at safety net hospitals that perform a disproportionate number of these emergency operations. METHODS: We used data from the 2021 Healthcare Cost and Utilization Project State Inpatient and Emergency Department Databases from California and Florida. We evaluated index encounters during which colon su

Case Report: Delayed complete transection of the sigmoid colon after severe pelvic fracture leading to septic shock.

Severe pelvic fractures caused by high-energy trauma may be complicated by bowel injuries. These injuries may be evident at initial presentation or remain occult, resulting in delayed diagnosis. Delayed complete transection of the sigmoid colon associated with pelvic fractures is extremely rare. We describe a 57-year-old woman who was admitted to the Intensive Care Unit (ICU) following a traffic accident that resulted in a Young-Burgess lateral compression type III pelvic ring injury, hemorrhagic shock, impaired consciousness, and multiple associated injuries. Abdominal CT on post-injury days

Observation versus maintenance immunotherapy after PD-1-induced clinical complete response in dMMR/MSI-H colorectal cancer managed non-operatively: a multicenter cohort study.

BACKGROUND: Whether maintenance immunotherapy is necessary after clinical complete response (cCR) to programmed cell death protein 1 (PD-1) inhibition in mismatch repair-deficient/microsatellite instability-high colorectal cancer remains unclear. We evaluated its associations with survival and treatment-related toxicity. METHODS: This multicenter cohort study included patients treated at five hospitals in China between January 2018 and March 2025 who achieved cCR after PD-1 inhibitor therapy and entered non-operative management. All patients underwent endoscopy and site-appropriate cross-sect

MZF1 Inhibits TGFβ-1/SMAD Signaling by Transcriptionally Activating SnoN to Augment Tumorigenic Potential in Colon Cancer.

Colon cancer progression is associated with transcriptional and signaling abnormalities. MZF1, linked to leukemogenesis, has an unclear role in colon cancer. This study examined its function in colon tumorigenesis and signaling-targeted therapies. Multi-omics (RNA-seq, CUT&Tag) identified MZF1 targets, validated in vitro/in vivo using xenografts and epithelial-specific MZF1 knockout mice. Virtual screening found MZF1 inhibitors. MZF1 activated SnoN transcription, suppressing TGF-β/SMAD signaling. Overexpression enhanced proliferation and colony formation; knockdown reversed these. In vivo, MZF

Rapidly Progressive Buschke-Löwenstein Tumor Managed With Wide Local Excision and Healing by Secondary Intention: A Case Report and Literature Review.

Buschke-Löwenstein tumor (BLT) is a rare, locally aggressive anogenital tumor associated predominantly with low-risk human papillomavirus (HPV) types 6 and 11. BLT is classically described as a slow-growing lesion predominantly affecting middle-aged men who are immunocompromised; cases of rapid progression are rarely reported. We present the case of a 23-year-old male with a progressively enlarging perianal mass over 1 year that was accompanied by rectal pain and intermittent bleeding. He reported prior unprotected heterosexual intercourse, though serologic testing for HIV, syphilis, and hepat

Response surface-optimized Cycas revoluta leaf extract attenuates experimental ulcerative colitis through anti-inflammatory and oxidative stress-related mechanisms.

Cycas revoluta Thunb. has traditionally been used for the management of gastrointestinal and inflammatory disorders, but its bioactive constituents and anti-inflammatory potential remain insufficiently characterized. This study aimed to optimize the extraction of phenolic-rich compounds from C. revoluta leaves and evaluate the anti-inflammatory and coloprotective effects of the optimized extract against experimental ulcerative colitis (UC). Four extraction techniques, namely Soxhlet extraction, maceration, ultrasound-assisted extraction (UAE), and microwave-assisted extraction (MAE), were comp

Ebastine attenuates DSS-induced ulcerative colitis via modulation of NF-κB, oxidative stress and pro-inflammatory cytokines: evidence for antihistamine repurposing.

Inflammatory bowel disease (IBD) is a chronic inflammatory disorder of the gastrointestinal tract, driven by immune dysregulation, oxidative stress, and the release of pro-inflammatory cytokines. Histamine, an important mediator in gut inflammation, has immunomodulatory properties. Ebastine, a second-generation histamine H1-receptor antagonist, is primarily used for allergic conditions, but its potential role in managing IBD has not been fully explored. This study aimed to evaluate whether ebastine could prevent colitis induced by dextran sodium sulfate (DSS) in mice. The severity of the condi

A macrophage-targeted TIMD4 Peptide-Nanoparticle attenuates colitis symptoms and monocyte recruitment in a mouse model.

Macrophages play a pivotal role in the pathogenesis of inflammatory bowel disease (IBD). T-cell immunoglobulin and mucin domain-containing molecule 4 (TIMD4) is involved in the regulation of tissue-resident macrophages. TIMD4 expression is elevated in colonic tissues from patients with ulcerative colitis (UC) and mice with dextran sulfate sodium (DSS)-induced colitis, but its role in IBD pathogenesis remains unclear. In this study, we found that Timd4 deficiency aggravated DSS-induced colitis, accompanied by increased infiltration of monocytes into the colonic tissues. Treatment with a CCR2 an

Disease-free survival and overall survival after neoadjuvant immunotherapy in mismatch repair-deficient colon cancer in the Netherlands: results of the single-arm, multicentre, phase 2, NICHE-2 trial.

BACKGROUND: Neoadjuvant immune checkpoint blockade induces high pathological response rates in patients with locally advanced mismatch repair-deficient (dMMR) colon cancer, yet long-term survival outcomes have not been established. Previously, we reported safety (a primary endpoint) and pathological response (a secondary endpoint) data from the NICHE-2 trial. Here, we report the primary efficacy endpoint of 3-year disease-free survival and the exploratory endpoint of 5-year overall survival. METHODS: In this single-arm, multicentre, phase 2, NICHE-2 trial, patients with previously untreated,

Internal herniation through Petersen's defect following Roux-en-Y gastric bypass presenting as subacute bilateral lower extremity edema due to IVC compression - a case report.

A 38-year-old woman with a history of Roux-en-Y gastric bypass (RYGB) 2 years prior presented to the bariatric clinic on multiple occasions with postprandial abdominal pain and bloating and bilateral lower extremity edema. Abdominal pain did not improve despite antiacid medication and dietary recommendations. Computed tomography scan of the abdomen revealed an internal hernia (IH) through Petersen's defect (a potential space between the Roux limb and transverse colon), with a mass effect on the inferior vena cava by herniated bowel. Diagnostic laparoscopy confirmed the diagnosis. Reduction of

Endoscopic and histologic healing distribution and persistent rectosigmoiditis in moderate-to-severe ulcerative colitis.

BACKGROUND: Mucosal healing in ulcerative colitis (UC) is believed to follow a proximal-to-distal pattern. We evaluated colonic healing distribution in patients with moderate-to-severe UC receiving adalimumab. METHODS: Patients from the SERENE UC (Study of a Novel Approach to Induction and Maintenance Dosing with Adalimumab in Patients with Moderate to Severe Ulcerative Colitis) trial with full colonoscopies and segmental biopsies at screening, weeks 8, and 52 were included for endoscopic and histologic analyses if baseline Mayo endoscopic subscore (MES) or Robarts Histopathology Index (RHI)

Heterotopic Cutaneous Adnexa Arising from the Ascending Colon Presenting as a Chronic Abdominal Wall Sinus: A Rare Congenital Anomaly and Diagnostic Challenge.

A chronic abdominal wall sinus tract caused by heterotopic cutaneous adnexa arising from the colonic serosa is extremely rare and readily misdiagnosed. We report a 39-year-old woman with congenital scoliosis and a 30-year history of intermittent, odorless, non-feculent discharge from a cutaneous opening in the right lower abdominal wall; she had no prior abdominal surgery. Contrast-enhanced computed tomography (CT) revealed a non-enhancing sinus tract extending from the ascending colon toward the abdominal wall, whereas colonoscopy showed normal colonic mucosa with no fistulous communication.

Mulberry Polyphenols Ameliorate Metabolic Abnormalities Accompanied by Spatial Microbial and Short-Chain Fatty Acid Alterations in Type 2 Diabetic Mice.

The gut microbiota may contribute to the anti-diabetic effects of mulberry polyphenols (MPs), although the associated spatial microbial patterns and metabolic alterations remain unclear. We investigated microbiota composition in fecal, cecal, and colonic samples and temporal changes in fecal short-chain fatty acids (SCFAs) during an 8-week MP intervention in type 2 diabetic (T2DM) mice using 16S rRNA sequencing and gas chromatography. MPs effectively reduced fasting blood glucose and improved diabetes-associated pathological alterations. Spatially, MP administration was associated with distinc

Urgent Colectomy for Colonic Necrosis Following Shiga Toxin-Producing Escherichia coli O157 Infection in an Adult.

INTRODUCTION: Shiga toxin-producing Escherichia coli (STEC) O157 infection is usually treated conservatively; surgery is rarely required and mostly reserved for children with hemolytic uremic syndrome (HUS). We report a case of STEC O157 requiring urgent surgery in an adult without HUS. CASE PRESENTATION: A 73-year-old woman presented with abdominal pain and diarrhea. Abdominal CT showed diffuse wall thickening in the ascending and transverse colon. Infectious enteritis and acute exacerbation of chronic renal failure were diagnosed, and antibiotics and continuous hemodiafiltration were initia

Medullary adenocarcinoma of the ascending colon mimicking appendicitis after prior appendectomy.

Medullary adenocarcinoma is a rare colorectal carcinoma that may be difficult to recognize when inflammation dominates the presentation. A 65-year-old woman with a remote appendectomy presented with 1 month of postprandial abdominal pain, weight loss, and constipation. Repeated computed tomography demonstrated marked pericecal inflammation and a blind-ending structure interpreted as possible appendicitis despite her surgical history. Diagnostic laparoscopy identified an enlarged mesenteric lymph node, and excision unexpectedly showed metastatic medullary carcinoma of colonic origin with loss o

Parabacteroides distasonis ameliorates methylphenidate hydrochloride-induced colonic inflammatory responses and associated gastrointestinal distress.

BACKGROUND: Attention-deficit/hyperactivity disorder (ADHD) is a common neurodevelopmental disorder in childhood, with methylphenidate hydrochloride (MPH) as its first-line pharmacotherapy. Though effective, MPH is associated with underrecognized gastrointestinal adverse events warranting further investigation. Recently, the gut microbiota has attracted attention for treating gastrointestinal distress, but research on its role in alleviating MPH-induced gastrointestinal reactions remains scarce. METHODS: Herein, Wistar-Kyoto rats (WKY) served as controls alongside spontaneously hypertensive r

GART expression is associated with a proliferative and purine-metabolic ISC-like epithelial state in ulcerative colitis.

OBJECTIVE: To characterize the epithelial state associated with GART expression in ulcerative colitis (UC) and determine whether this association reflects a GART-specific program or a broader proliferative and purine-metabolic phenotype. METHODS: Four independent bulk transcriptomic cohorts were analyzed and synthesized by random-effects meta-analysis. Human colonic scRNA-seq datasets (GSE214695 and SCP259) were integrated, with principal comparisons repeated using donor-aware pseudobulk or mixed-effects approaches and adjustment for sequencing depth, gene detection, and cell-cycle covariates
