Ciliated foregut cyst of the pancreas mimicking a cystic neoplasm.

Pancreatic cystic lesions are being identified more frequently with widespread use of cross-sectional imaging. While most pancreatic cysts are mucinous neoplasms or pseudocysts, congenital cysts of foregut origin are exceptionally rare in this location, with these being commonly found in the mediastinum. Their occurrence in the hepatobiliary system is rare, and pancreatic involvement is even rarer. Fewer than 20 cases have been reported in literature since it was first described in the 1990s. Ciliated foregut cysts arise from abnormal development of the primitive foregut and, histologically, a

Two-stage surgical resection of a massive primary posterior mediastinal dedifferentiated liposarcoma with bilateral thoracic extension: a case report.

INTRODUCTION AND IMPORTANCE: Primary mediastinal dedifferentiated liposarcoma (DDLPS) is a rare and aggressive tumor, often diagnosed late due to nonspecific symptoms. The optimal surgical strategy for tumors with bilateral thoracic extension remains unclear. CASE PRESENTATION: A 59-year-old woman presented with a 1-year history of exertional dyspnea. Chest computed tomography revealed a large posterior mediastinal mass extending into both thoracic cavities with compression of adjacent structures. A two-stage surgical approach was performed, with initial right thoracotomy followed by left tho

Stable disease after multimodal treatment of SMARCA4-deficient undifferentiated tumor concurrently involving the stomach and mediastinum: a case report.

We report a clinically rare SMARCA4-deficient undifferentiated tumor concurrently involving the stomach and mediastinum. A 54-year-old man presented with a 1-month history of left upper abdominal pain radiating to the anterior chest and back. Contrast-enhanced computed tomography of the chest and abdomen showed thickening and abnormal enhancement of the gastric wall along the greater curvature, multiple enlarged lymph nodes along the lesser curvature, and a posterior inferior mediastinal mass. Biopsies of both the gastric body lesion and the mediastinal mass revealed a poorly differentiated/un

Case Report: A case of mediastinal parathyroid adenoma leading to hypercalcemic crisis complicated by acute kidney injury and postoperative hungry bone syndrome.

A 61-year-old man was admitted to the hospital with fatigue and hypercalcemia, with serum calcium levels as high as 6.16 mmol/L and parathyroid hormone (PTH) levels >5000 pg/mL, accompanied by progressively rising serum creatinine levels (150→266 μmol/L). A neck ultrasound revealed a small nodule in the left parathyroid region, but 99mTc-MIBI imaging identified a focal area of increased radiotracer uptake within the mediastinum. Following multidisciplinary consultation, a thoracoscopic mediastinal mass resection was performed, and pathology confirmed an ectopic parathyroid adenoma (3.2 × 2.8 ×

Posterior Mediastinal Biopsy Gone Awry: Managing Biopsy Needle-Induced Intubation Challenges in Emergency Surgery.

Foreign bodies entering the thorax via a posterior trajectory present distinct challenges in positioning and intubation, particularly during emergency exploration. We present the case of a 37-year-old female who sustained a subclavian artery perforation during a computed tomography-guided biopsy for a posterior mediastinal mass. The biopsy needle advanced to the left common carotid artery. She was brought in the prone position for emergency surgical exploration, which posed significant challenges for airway management. This case highlights the importance of flexible airway strategies, multidis

Unicentric hyaline-vascular Castleman disease of the jejunal mesentery mimicking a gastrointestinal stromal tumor or neuroendocrine tumor: a case report.

BACKGROUND: Castleman disease (CD) is a rare lymphoproliferative disorder classified as unicentric (UCD) or multicentric (MCD). The hyaline vascular variant of UCD most commonly arises in the mediastinum, whereas mesenteric involvement is exceedingly rare. As it can mimic hypervascular mesenteric tumors like neuroendocrine tumors (NETs) and gastrointestinal stromal tumors (GISTs), preoperative diagnosis remains challenging. CASE PRESENTATION: A 36-year-old woman presented with a 3-month history of vague abdominal discomfort, early satiety, and an unintentional 9-kg weight loss. Contrast-enhan

The role of thoracic interventional radiologists in the management of chest infections: a pictorial review.

Thoracic infections remain a major cause of morbidity and mortality worldwide, requiring accurate and timely diagnosis to guide appropriate treatment. This pictorial review illustrates the role of the interventional thoracic radiologist in the management of infectious processes involving the lung, pleura, mediastinum, and chest wall. We describe a range of non-vascular image-guided percutaneous diagnostic and therapeutic techniques. These include targeted sampling for microbiological identification in complicated pneumonias and cavitary lesions, as well as image-guided drainage of pulmonary, p

Delayed diagnosis of selected thoracic and mediastinal conditions in young adults: a focused narrative review of clinical pitfalls, cognitive biases, and system barriers.

BACKGROUND: Thoracic and mediastinal conditions affecting the esophagus, diaphragm, mediastinum, and airway carry considerable morbidity when diagnosis is delayed. Young adults are often perceived as low risk for serious thoracic disease, and low clinical suspicion, atypical symptoms, and cognitive and healthcare-system barriers can delay recognition. METHODS: This focused narrative review searched PubMed and Scopus, supplemented by Google Scholar, for studies published between January 2000 and May 2026, with selected earlier landmark studies retained where essential. Search terms included "t

Direction of discordance between PET/CT, EBUS-TBNA, and multidisciplinary tumor board nodal assignment in non-small cell lung cancer: A retrospective observational study.

Mediastinal nodal classification in non-small cell lung cancer (NSCLC) at the N0/N1-N2/N3 treatment-defining boundary guides therapeutic strategy and is commonly finalized in multidisciplinary tumor board (MDT) meetings. We compared positron emission tomography/computed tomography (PET/CT) and endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) against MDT assignment and quantified discordance at this threshold. We retrospectively analyzed 114 consecutive NSCLC patients who underwent both PET/CT and EBUS-TBNA before MDT review. Nodal stage was dichotomized as N0/N1 vs

Tortuous Descending Thoracic Aorta in a Patient with a History of Ascending Aorta Replacement as a Confounder in Transesophageal Echocardiographic Evaluation of the Left Atrium: A Procedural Challenge.

HIGHLIGHTS: Descending thoracic aorta tortuosity, sometimes present as a vascular mass in the mediastinum, distorting nearby cardiac chambers, which may confuse physicians during transesophageal echocardiography.Recognizing the potential echocardiographic presentation of aortic tortuosity can aid in the timely identification of this condition during transesophageal echocardiography. UNLABELLED: Descending thoracic aorta tortuosity results from elongation of the descending aorta and is associated with aging. It acts as a risk factor for challenging endovascular procedures and has prognostic si

Fetal hemothorax due to intrapleural rupture of a small anterior thoracic lymphatic malformation: a case report.

BACKGROUND: Fetal pleural effusion is usually chylous, whereas hemothorax is considerably rarer. Trauma, iatrogenic complications, and rupture of intrathoracic tumors have been reported as causes of fetal hemothorax. Lymphatic malformations often enlarge during the fetal period; however, these benign lesions may regress spontaneously after birth or be managed with sclerotherapy. Accurate prenatal diagnosis is therefore crucial for determining whether fetal intervention or postnatal management is the most appropriate strategy. Here, we present a rare case of a fetus with hemothorax presumably c

A Case of Anterior Mediastinal Hodgkin Lymphoma Exhibiting Abundant Intratumoral Vascular Structures on Computed Tomography.

Hodgkin lymphoma shows a bimodal age distribution, predominantly affecting young adults and individuals in their 50s and 60s, and is the most common type of lymphoma occurring in the mediastinum. Typical computed tomography (CT) imaging findings include a bulky mass arising in the anterior mediastinum (prevascular compartment) with homogeneous soft-tissue attenuation. Herein, we report a case with unusual CT findings characterized by abundant internal vascular structures in an 18-year-old man with nodular sclerosis classic Hodgkin lymphoma who presented with hemoptysis. Histopathologic examina

Volume-based quantitative assessment of respiratory-induced organ motion in lung cancers.

PURPOSE: To comprehensively evaluate respiratory-induced organ motion and deformation using volume-based analysis, overcoming the limitations of conventional point-based methods in radiation therapy for lung malignancies. MATERIAL AND METHODS: The Internal target volume (ITV) and organs at risk (OARs) were delineated on averaged-phase images from 4D-CT datasets of 11 lung cancer patients. Deformable image registration using an enhanced non-parametric algorithm generated deformation vector fields (DVFs) between each respiratory phase and a reference phase. Voxel-level displacements within each

A Multidimensional CT-Based Tumor Location Metric for Predicting Lymph Node Metastasis and Occult Lymph Node Metastasis in Lung Adenocarcinoma.

IntroductionLymph node (LN) metastasis of lung adenocarcinoma, particularly occult lymph node metastasis (OLNM), remains difficult to predict using conventional imaging and basic positional parameters, adversely affecting prognosis. This study aimed to: (1) evaluate a new CT index (a/A + b/B) for predicting OLNM in cN0 patients, and (2) create and validate a nomogram for predicting LN metastasis probability in CT-suspected cases.MethodsThis single-center retrospective study involved 387 lung adenocarcinoma patients (225 non-LN, 84 OLNM, and 78 with CT-suspected/pathologically confirmed LN meta

Effect of Personalized Approach to CT Scan Length on Radiation Dose in Parathyroid SPECT/CT.

Parathyroid SPECT/CT is the standard examination for localization of hyperfunctioning parathyroid tissue before surgery for hyperparathyroidism. The effective dose (ED) delivered by CT comprises approximately one third of the radiation dose in this examination. Parathyroid SPECT/CT typically includes 1 CT covering the mediastinum because of the risk of ectopic adenomas. This study investigates whether the use of personalized CT (pCT) on the basis of SPECT images can reduce scan length and CT-derived ED, while maintaining the ability to detect ectopic adenomas, compared with full field-of-view

Endoscopic Ultrasound With Bronchoscope-Guided Fine-Needle Aspiration (EUS-B-FNA) in a six-Month-Old Infant: A Case Based Review of Literature.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is an established, minimally invasive modality for evaluating mediastinal pathologies in adults. However, its application in infants remains extremely limited because of airway size constraints and procedural safety concerns. We report the case of a 6-month-old infant presenting with recurrent lower respiratory tract infections and a posterior mediastinal lesion causing bronchial compression. Contrast-enhanced computed tomography demonstrated a well-defined cystic lesion in the right posterior mediastinum, with differ

Robot-assisted thoracoscopic resection of a costal cartilage chondrosarcoma mimicking anterior mediastinal teratoma: a case report.

BACKGROUND: Chondrosarcoma arising from the costal cartilage is extremely rare. When such a tumor extends toward the mediastinum, it may present as a well-defined mass that is difficult to distinguish from a primary anterior mediastinal tumor. We report a case diagnosed preoperatively as a mature teratoma but later proven to be a grade 1 chondrosarcoma of the costal cartilage. CASE PRESENTATION: A 66-year-old asymptomatic woman was incidentally found to have an abnormal shadow on chest imaging. Contrast-enhanced computed tomography revealed a well-defined, calcified mass measuring 37 × 31 × 2

Case Report: A case of thoracic schwannoma: diagnostic work-up of an incidentally detected mediastinal lesion.

Schwannomas represent the most common neurogenic tumors of the posterior mediastinum. Although typically slow-growing and asymptomatic, degenerative changes such as cystic transformation may alter their imaging appearance and mimic necrotic lymphadenopathy or malignant mediastinal masses. A 61-year-old man underwent chest CT for respiratory symptoms. Careful image review revealed an incidental subcarinal mediastinal lesion initially suspected to represent necrotic lymphadenopathy. Further evaluation with contrast-enhanced CT, 18F-FDG PET/CT and endobronchial ultrasound-guided transbronchial ne
