Partial upper sternotomy for resection of a large substernal goiter in a high-risk patient.

Substernal goiters (SGs) with significant mediastinal extension can present complex surgical dilemmas. While most SGs can be resected through a low cervical collar incision, a full median sternotomy may be required in select cases. Morbidly obese patients are susceptible to adverse outcomes following full sternotomy, such as wound infection and sternal dehiscence. Partial upper sternotomy is a safe, effective alternative that offers sufficient access to the mediastinum while minimizing surgical morbidity, making it a valuable approach in this high-risk population. We present the case of a 33-y

[Laparoscopic procedures for recurrent hiatal hernias].

OBJECTIVE: To evaluate the immediate and long-term results of laparoscopic surgical procedures in patients with recurrent hiatal hernias (rHH). MATERIAL AND METHODS: Thirty-six patients with rHH underwent laparoscopic mesh repair and Nissen fundoplication between 2015 and 2024. Nine patients underwent Collis-Nissen esophagogastroplasty due to intraoperative confirmation of short esophagus. RESULTS: Mean surgery time was 3 (2-4.5) hours, length of hospital-stay - 7 (5-15) days. Intraoperative revision established cuff and part of the stomach displacement into mediastinum as a cause of rHH in

Stratifying risk of heart failure death and arrhythmic events: a ¹²³I-meta-iodobenzylguanidine-based multinomial logistic model.

PURPOSE: Differentiating heart failure-related death (HFD) from arrhythmic events (ArEs) is clinically important for patients with chronic heart failure (CHF), as they have distinct mechanisms and therapeutic strategies. We developed and validated a multivariable model to predict HFD, ArEs, and survival using clinical parameters and cardiac 123I-meta-iodobenzylguanidine (123I-mIBG) images. METHODS: We retrospectively analyzed data derived from 997 patients with CHF (mean age 70 ± 13 years, left ventricular ejection fraction (LVEF) 32% ± 13%) over a mean follow-up of 41 ± 27 months. Outcomes w

Familial Ascending Aortic Aneurysm in a Child With a Pathologic Variant of the Lysyl Oxidase Gene.

The lysyl oxidase (LOX) gene encodes the extracellular enzyme lysyl oxidase, which is essential for cross-linking collagen and elastin, key structural proteins that maintain connective tissue stability. Pathogenic variants in the LOX gene are associated with familial thoracic aortic aneurysms. A five-year-old male presented to the emergency room with upper respiratory tract symptoms. A chest radiograph showed a markedly enlarged mediastinum. Further studies revealed a 9-cm thoracic aortic aneurysm confined to the ascending aorta. The child underwent successful surgical intervention.

Long-Term Survival With Minimum Side-Effects in a Post-Op Case of Carcinoma Esophagus With Multiple Recurrences in Mediastinum Treated With Radical Re-Radiotherapy in Mediastinum-A Case Report.

Loco-regional recurrence after radical surgery in esophageal cancer is common. However, there is no universally accepted guideline for managing such loco-regional recurrences. Even after multiple local recurrences in the mediastinum, a patient can consolidate safely by re-radiotherapy in the mediastinal region with minimal late morbidity by judicious use of radiotherapy, if there is an acceptable time gap between two radiotherapy periods.

Foreign Bodies Simulating Mesenchymal Tumors: Laparo-Endoscopic Resolution in Abdomen and Mediastinum.

INTRODUCTION: Retained surgical foreign bodies are underreported due to legal concerns. They may remain asymptomatic or cause acute complications with nonspecific imaging findings, leading to delayed diagnosis. Awareness in previously operated patients is essential, and their impact has driven preventive measures such as standardized counts, detection technologies, and surgical checklists. CASE PRESENTATION: A 38-year-old woman with prior urologic and gynecological surgeries presented with 3 days of left flank pain, fever, nausea, and vomiting. Imaging revealed a large heterogeneous abdominal

Case report: ectopic papillary thyroid carcinoma found in the midline of the hyoid bone and cervical lymph nodes, no thyroid lesions.

BACKGROUND: Ectopic thyroid tissue (ETT) is primarily caused by embryonic dysplasia. It is commonly located laterally in the neck or in the thyroglossal duct, as well as in more remote locations like the mediastinum and the subdiaphragmatic organs. The incidence of ETT is relatively rare, and the risk of malignant transformation is extremely low. The clinical symptoms of ETT are typically linked to the site of ETT involvement, and histopathological or biopsy examination is typically used to confirm the diagnosis of ectopic thyroid cancer. CASE DESCRIPTION: We present a rare case of a 30-year-

Diagnostic accuracy and safety of endoscopic ultrasound-guided fine needle biopsy for evaluating mediastinal pathologies.

BACKGROUND: The mediastinum comprises vital anatomical structures and can be affected by diverse benign and malignant pathologies. Imaging modalities such as computed tomography, magnetic resonance imaging, and positron emission tomography assist in initial evaluation, but histopathological confirmation remains the diagnostic gold standard. Endoscopic ultrasound (EUS) enables real-time, minimally invasive sampling of mediastinal lesions. EUS-guided fine needle biopsy (FNB) demonstrated high diagnostic accuracy with a favourable safety profile. Despite its established utility in high-income set

Double Trouble: Bilateral Fishbone Perforation and Its Successful Retrieval.

Fishbones are a common aerodigestive foreign body because of their irregular shape and the complex anatomy of the throat and esophagus. They are usually lodged in the upper esophagus and may sometimes cause severe complications. We present a 61-year-old man with dysphagia and chest discomfort 8 days after eating fish. CT imaging revealed a fishbone horizontally lodged across the esophagus with bilateral perforation into the mediastinum. Endoscopic removal was successfully performed under surgical standby. This case highlights a rare bilateral perforation, emphasizing the importance of CT imagi

Modified Pectus Up and the Portuguese Case Series.

INTRODUCTION: Pectus excavatum (PE) is the most common congenital chest wall deformity, with an incidence of 1 in 400 births. Surgical approaches vary depending on the severity of the condition. Recently, the taulinoplasty approach - "Pectus Up®" - was described as an alternative to lift the sternum through external traction, avoiding invading the mediastinum or pleural cavity. We describe our progressively modified taulinoplasty technique and case series. MATERIALS AND METHODS: From 2022 to 2024, 13 consecutive patients with pectus excavatum underwent surgical correction with a modified taul

The complementary roles of endobronchial ultrasound-guided needle aspiration, mini-forceps, and cryobiopsy in the investigation of mediastinal lesions.

BACKGROUND: Convex probe endobronchial ultrasound (EBUS) bronchoscopy-guided transbronchial needle aspiration (TBNA) is the mainstay of mediastinal and hilar lesion sampling, and EBUS-TBNA allows for the acquisition of cytologic material. However, certain clinicopathologic conditions require the acquisition of histopathologic specimens. This can be accomplished via EBUS-guided transbronchial mini-forceps biopsy (TBFB) and cryobiopsy (TBCB), but the incremental value of these -modalities in various clinicopathologic conditions has yet to be explored. OBJECTIVE: Define the incremental value of

Intramedullary Gas Detected on Computed Tomography in Cases of Decompression Sickness: A Case Series.

Decompression sickness (DCS) is caused by nitrogen gas bubbles in the blood that lead to bubble embolization and tissue compression. Intramedullary gas bubbles may also be observed; however, their clinical significance remains unclear. Herein, we describe our experiences with three cases that experienced DCS in which intramedullary gas bubbles were incidentally discovered on computed tomography (CT) performed before treatment. Case 1 involved a 28-year-old male who had repeatedly dived to 30 m and surfaced. After diving, the patient experienced pain in the left lower extremity. CT revealed bub

Cardiac sympathetic nerve activity following mitral transcatheter edge-to-edge repair verified by 123I-metaiodobezylguanidine (MIBG) scintigraphy among disproportionate secondary mitral regurgitation.

This study evaluated the cardiac sympathetic nerve (CSN) activity following mitral transcatheter edge-to-edge repair (M-TEER) using 123I-metaiodobenzylguanidine (MIBG) scintigraphy among disproportionate secondary mitral regurgitation (SMR). Among the 31 patients who underwent M-TEER between November 2018 and December 2020, 20 patients who underwent pre-procedural and 1-year post-procedural MIBG scintigraphy were included. The ratio of the pre-procedural effective regurgitant orifice area (EROA) to the left ventricular end-diastolic volume (LVEDV) was retrospectively assessed. Patients with hi

Salvage Antiperistaltic Left Colon Conduit for Caustic Esophageal Stricture: A Viable Alternative Route in a Hostile Mediastinum.

Corrosive ingestion is a serious condition that can result in multiple esophageal strictures, often necessitating complex surgical reconstruction when endoscopic management fails. Colonic interposition is an established option for esophageal replacement, but conduit necrosis remains a potentially fatal complication. We present details of a rare salvage antiperistaltic left colon conduit for a patient with corrosive esophageal stricture, who failed endoscopic dilatation and a right ileocolonic conduit.

A Rare Case of Poorly Differentiated Lower Esophageal Cancer with Neuroendocrine Differentiation and α-Fetoprotein Production.

INTRODUCTION: Neuroendocrine differentiation and α-fetoprotein production (AFP) are 2 rarely reported features in poorly differentiated gastric carcinomas. We report an extremely rare case of esophageal carcinoma (EC) featuring both histologic types. CASE PRESENTATION: A 67-year-old man was referred to our hospital with dysphagia and weight loss. Upper gastrointestinal endoscopy revealed a type 3 tumor in the lower esophagus, 32-40 cm from the incisors, with luminal stenosis. The endoscopic biopsy specimen showed pathologic features of poorly differentiated carcinoma, and immunostaining revea

Surgical repair of peripherally inserted central catheter extravasation via a transmanubrial approach.

The transmanubrial approach (TMA) offers great exposure for managing complex superior mediastinal pathology while preserving functional anatomy. Here, we report a case of extravascular perforation caused by peripherally inserted central catheter with mediastinal abscess formation, which was successfully treated using a TMA. A 76-year-old woman developed a fever 3 months after the insertion of a peripherally inserted central catheter for parenteral nutrition. Imaging showed catheter tip migration with venous perforation, accompanied by abscess formation in the superior mediastinum. Owing to the
