Incidence of Gonadal and Extragonadal Germ Cell Tumours in Patients With Klinefelter Syndrome.

BACKGROUND: Klinefelter's syndrome (KS; 47, XXY) is associated with an altered risk profile for malignancies compared with non-KS males. In particular, several reports have noted a striking association between KS and extragonadal germ cell tumours (EGCTs), especially in the mediastinum, whereas the risk of testicular germ cell tumours (TGCTs) in KS remains unclear. KS patients also have a higher prevalence of cryptorchidism (undescended testes)-a known risk factor for TGCT in the general population-yet it is uncertain if cryptorchidism confers the same cancer risk in the KS population. This st

Expanding the Anatomical Distribution of PRRX1::KMT2D Fusion Mesenchymal Neoplasms: A Rare Mediastinal Case Report.

BACKGROUND: PRRX1-rearranged mesenchymal neoplasms are rare soft tissue tumors with a predilection for the superficial subcutaneous tissue. The PRRX1::KMT2D fusion variant is exceptionally rare, with only three previously reported cases, all of which were located in the intermuscular regions. However, its occurrence in deep visceral sites has not been documented. CASE: A 62-year-old woman was admitted after a routine physical examination revealed a space-occupying lesion in the left thoracic cavity. Contrast-enhanced CT showed a mixed-density mass (10.4 × 8.1 × 3.8 cm) at the left cardiophren

Usefulness of Transparent Illumination Device in Esophageal Atresia for the Detection of the Distal Esophagus.

INTRODUCTION: Pure esophageal atresia without a tracheoesophageal fistula (Gross type A) is technically demanding, particularly with respect to intraoperative identification of the distal esophageal pouch. Biologically transparent illumination (BTI) devices have been used to visualize nasogastric tubes in pediatric patients; however, their application during esophageal reconstruction has not been reported. CASE PRESENTATION: A male neonate with prenatally suspected esophageal atresia was diagnosed postnatally with Gross type A esophageal atresia. After gastrostomy and serial bougienage of bot

Prenatal Diagnosis of Isolated Congenitally Corrected Transposition of the Great Arteries.

Congenitally corrected transposition of the great arteries (ccTGA) is an uncommon congenital heart defect characterized by atrioventricular and ventriculo-arterial discordance resulting from abnormal leftward looping of the primitive cardiac tube. Although this double discordance produces a physiologically corrected circulation, the morphologic right ventricle functions as the systemic ventricle, and the tricuspid valve serves as the systemic atrioventricular valve, predisposing affected individuals to late complications. Prenatal diagnosis of ccTGA is often challenging, particularly in isolat

Neck migration of a K-wire following fixation of clavicle fracture: a case report.

BACKGROUND: Kirschner wires (K-wires) remain a simple and cost-effective option for clavicle fixation, especially in resource-limited settings. However, migration of K-wires is a rare but potentially fatal complication that can involve critical structures in the thorax, mediastinum, or neck. Preventive measures such as bending, burying, and early removal are essential but often overlooked. CASE PRESENTATION: A 35-year-old male underwent K-wire fixation for a displaced left clavicle fracture. Three months postoperatively, he presented with progressive shoulder pain and restricted motion. Imagin

Risk factors and prognostic implications of upper or middle mediastinal lymph nodes metastasis and recurrence in esophagogastric junction carcinoma.

BACKGROUND: The risk factors for upper and middle mediastinal lymph node (UMMLN) metastasis and the prognostic value of mediastinal lymphadenectomy (MLA) in patients with esophagogastric junction cancer (EGJC) remain unclear. This study aims to identify predictors for UMMLN metastasis or recurrence in EGJC with an esophageal invasion length (EIL) of >2 cm to ≤4 cm, and to clarify the histology specific prognostic impact of mediastinal dissection. METHODS: We retrospectively reviewed patients who underwent radical resection for stage II-IV EGJC with an EIL of >2 cm to ≤4 cm between 2000 and 20

Point-of-care ultrasonography for verification of central venous catheter placement in cats and dogs.

INTRODUCTION: This prospective study aimed to investigate the use of ultrasonography for verifying central venous catheter placement in hospitalized cats and dogs, in comparison with radiographic assessment. METHODS: The investigation was conducted on client-owned animals. The position of the central venous catheter was checked using both thoracic radiography and ultrasonography. Ultrasonographic examination was performed in three steps: two intercostal scans of the cranial thorax (transverse and longitudinal scans), to visualize the course of the cranial vena cava within the mediastinum, and

Mediastinal Ectopic Pancreas Mimicking Lymphoma with Discordant Histology and Flow Cytometry: A Diagnostic Challenge.

Background: Mediastinal ectopic pancreas (EP) is an exceptionally rare entity that can mimic malignancy. Diagnosis is typically established post-operatively; pre-operative confirmation is challenging. Case Presentation: We describe a 28-year-old man presenting with life-threatening airway obstruction due to a progressive mediastinal mass, requiring emergency tracheal stenting. Diagnostic workup revealed a critical discordance: while CT-guided core biopsy confirmed benign ectopic pancreatic tissue, concurrent flow cytometry identified a monoclonal B-cell population with a high Ki-67 index (~86%

Safety and feasibility of bilateral lung transplantation with video-assisted thoracic surgery.

BACKGROUND: The clamshell incision remains the most common approach for bilateral lung transplantation because it provides excellent exposure of bilateral pleural cavities and the mediastinum. However, it is associated with significant morbidity, including sternal dehiscence and instability. Video-assisted thoracic surgery (VATS) offers a less invasive alternative. This study aimed to evaluate the feasibility, safety, and early postoperative outcomes of the VATS approach. METHODS: We retrospectively analyzed 136 patients who underwent bilateral lung transplantation with extracorporeal membran

Influence of Tube Voltage, Tin Filtration, and Sex on Radiation Dose and Image Quality in Adult Chest Photon-Counting CT: A Phantom-Based Signal-to-Noise-Based Analysis.

PURPOSE: Photon-counting computed tomography (PCCT) offers new possibilities for optimizing image quality while reducing radiation dose. However, the influence of acquisition parameters such as tube voltage and spectral shaping with tin filtration on chest imaging remains insufficiently understood. MATERIALS AND METHODS: Adult male and female anthropomorphic phantoms were scanned using a dual-source PCCT system across a broad range of image quality levels (IQL 1-100). Radiation dose and signal-to-noise ratio (SNR) were evaluated for protocols using 120 kVp and 140 kVp without tin filtration,

Sonographic Characteristics of the Thymus in Healthy Canine Neonates.

The thymus is important for neonatal immune development; however, information on its ultrasonographic appearance in newborn puppies is limited. Because the thymus occupies a substantial portion of the cranial mediastinum in neonates, it may be confused with pathological mediastinal masses during thoracic imaging. Defining its normal ultrasonographic appearance is therefore crucial for accurate clinical interpretation. This study aimed to describe the topographic anatomy and ultrasonographic features of the thymus in neonatal dogs. Ten puppy cadavers were examined. Under ultrasound guidance, th

Minimally Invasive Extirpation of an Eden Type II Dumbbell-Shaped Mediastinal Tumor Using a Posterior and Uniportal Thoracoscopic Approach without Changing the Patient's Position: A Case Report.

INTRODUCTION: Dumbbell-shaped posterior mediastinal tumors, most commonly schwannomas, sometimes extend through the intervertebral foramen into the spinal canal and often require a combined posterior and thoracic approach. Although complete resection is essential, reducing surgical invasiveness remains an important clinical priority. We report a case of an Eden type II dumbbell-shaped schwannoma that was successfully resected using a minimally invasive single-stage approach combining posterior surgery and uniportal thoracoscopic surgery, performed entirely in the prone position without intraop
