Maternal diet and placental outcomes: Evidence of sexual dimorphism among mothers with overweight and obesity.

INTRODUCTION: Maternal diet may influence placental development and efficiency, reflected by placental weight (PW) and the birthweight-to-placental weight ratio (BW:PW). Mothers with overweight or obesity (OWOB) are at increased risk of adverse placental outcomes, yet evidence on maternal dietary influences on placental outcomes in OWOB remains limited. METHODS: We conducted secondary analysis of data on 423 mother-child pairs with maternal BMI ≥25 kg/m2 from two Irish cohorts (Lifeways Cross-Generation Cohort (n = 66) and the Pregnancy Exercise and Nutrition Research Study (PEARS) randomized

Placental adaptation to maternal overnutrition as an evolutionary trade-off.

The mammalian placenta is genetically identical to the fetus, exposing it to unique selective pressures. In humans, placental evolution has been explored through the lens of nutritional scarcity, with selective pressures assumed to diminish as resources increase. Here, we challenge this assumption, arguing that maternal overnutrition constitutes a distinct and underappreciated selective pressure. Maternal inability to maintain fuel homeostasis exposes the fetus to nutrient overload, elevating fetal insulin and oxygen demand. Selection has favored placental adaptive responses to the resulting o

Placental Iron Homeostasis: Navigating the Delicate Balance Between Transport and Ferroptosis.

As an essential micronutrient, iron plays a vital role in oxygen transport, deoxyribonucleic acid synthesis, and energy production and is a redox-active cofactor for many biological processes. However, ferrous iron (Fe2+) is toxic to cells because it reacts with hydrogen peroxide in the Fenton reaction to generate the hydroxyl (free) radical, a key player in oxidative stress, leading to lipid peroxidation and iron-dependent cell death (ferroptosis). Therefore, iron homeostasis is precisely regulated. Maintaining appropriate iron metabolism during pregnancy is a major challenge, as it requires

Decreased function of fatty acid desaturase 1 is associated with placental foetal vascular malperfusion and highlights the key role of the maternal-foetal interface.

BACKGROUND: Placental foetal vascular malperfusion (PFVM) is a chronic disorder characterised by impaired blood flow and abnormal vascular development, inducing adverse neonatal outcomes. Long chain polyunsaturated fatty acids (LC-PUFAs) play a crucial role in placenta health. We aimed to explore the role of genetic variation in LC-PUFA metabolism in PFVM risk as well as underlying mechanisms. METHODS: Associations were examined between two functional polymorphisms (rs174538 and rs174556) of the fatty acid desaturase 1 (FADS1) gene encoding the rate-limiting enzyme for LC-PUFA metabolism and

Mindfulness-based stress reduction during pregnancy is associated with changes in placental fatty acid metabolism and endocrine function.

INTRODUCTION: Mindfulness-based stress reduction (MBSR) is a promising non-pharmacological approach to improve mental health during pregnancy. This study examined whether maternal MBSR is associated with changes in placental metabolic and endocrine function. METHODS: Placental samples were collected from participants in the IMPACT BCN randomized clinical trial, assigned to either MBSR or control care. Fatty acid composition was analysed by gas chromatography, gene expression by qPCR, and protein levels by Western blotting in placentas from male and female neonates. RESULTS: MBSR altered plac

Placental immunopathology and congenital transmission of Zika virus and Trypanosoma cruzi: comparative mechanisms, immune disturbances, and potential co-infection scenarios.

Congenital transmission of Zika virus (ZIKV) and Trypanosoma cruzi (T. cruzi) remains a major public health concern in the Americas. Both pathogens cross the placental barrier and cause severe neonatal outcomes, yet no confirmed congenital co-infections have been reported, and their combined effects on maternal-fetal health are unknown. Critical gaps persist regarding placental immunopathology, fetal immune programming, and long-term consequences of prenatal exposure. This review synthesizes current evidence on the routes and mechanisms of placental transmission of ZIKV and T. cruzi, their tro

Immunologic and placental responses to the VSV-EBOV vaccine in a pregnant mouse model.

Ebola virus disease is a severe viral hemorrhagic illness with high maternal and perinatal mortality. Although the replication-competent vesicular stomatitis virus-based Ebola vaccine (VSV-EBOV) has been deployed successfully in outbreak settings, pregnant women were excluded from clinical trials, leaving critical safety gaps. To address this, we developed a murine model to assess pregnancy outcomes following VSV-EBOV exposure at different gestational stages. Pregnant C57BL/6 mice vaccinated at embryonic day 7 (E7), E10, or E15 mounted robust anti-Ebola virus (anti-EBOV) antibody responses com

Interferon-λ signaling to maternal dendritic cells protects against congenital Zika virus infection.

Interferon-λ (IFN-λ, type III IFN) mediates antiviral immunity at anatomic barriers, including the maternal-fetal interface. To investigate the effects of IFN-λ during congenital Zika virus (ZIKV) infection, we infected mice lacking the IFN-αβ receptor (Ifnar1-/-) or both the IFN-αβ and IFN-λ receptors (Ifnar1-/-Ifnlr1-/-) at embryonic day 9 and found that loss of maternal IFN-λ signaling resulted in greater transplacental transmission, highlighting a protective effect of IFN-λ at the maternal-fetal interface. To investigate the spatial relationship between ZIKV infection and antiviral respons

Neighborhood stress and social vulnerability in relation to placental metals concentrations.

Recognizing that social and environmental exposures may co-occur and impact perinatal outcomes, we examined associations between neighborhood stress and social vulnerability, and placental metals concentrations. Pregnant participants (n = 242) in the UPSIDE study (Rochester, NY) completed the City Stress Inventory which has two subscales: neighborhood disorder (e.g., vacant buildings) and violence (personal experiences of violence). Neighborhood disorder was considered as quartiles (Q) and violence was considered binary (any/none). Participant addresses were linked to the CDC's Social Vulnerab

Gastrointestinal and Urological Tract Injuries During Cesarean Delivery: A Surgical Review With National Epidemiologic Data.

BackgroundCesarean delivery is the most common major surgical procedure performed worldwide, accounting for approximately one-third of US deliveries. Gastrointestinal (GI) and urological (URO) tract injuries are uncommon but disproportionately morbid complications. Increasing complexity, particularly repeat cesarean and placenta accreta spectrum (PAS), and uneven on-site subspecialty coverage have expanded the role of general surgeons as intraoperative consultants for these injuries, especially outside large tertiary centers.MethodsWe analyzed the National Inpatient Sample (NIS, 2005-2019) to

Gestation and Lactation-Dependent Effects of Experimental Hyperthyroidism.

Thyroid hormones (T3 and T4) play an essential role in reproductive health and can influence the functions of several endocrine and reproductive tissues, including ovaries, placenta, and mammary glands. Their interactions with other steroid hormones, such as glucocorticoids, estrogens, and progesterone, can alter the balance necessary for reproductive success. Hyperthyroidism can lead to significant endocrine dysregulation that affects multiple reproductive stages. This endocrine disorder causes several alterations in the estrous cycle, restricting conception when the condition is chronic. Dur

Maternal immunoglobulin G anti-A and anti-B antibody titres for predicting neonatal immune sensitisation in ABO-incompatible pregnancies: The Maternal Anti-A and Anti-B Antibody Titres in ABO-Incompatible Pregnancies study.

BACKGROUND AND OBJECTIVES: Maternal immunoglobulin G (IgG) anti-A and anti-B antibodies can cross the placenta and cause ABO haemolytic disease of the foetus and newborn (ABO-HDFN). We aimed to evaluate whether maternal IgG anti-A and anti-B titres predict neonatal direct antiglobulin test (DAT) positivity in ABO-incompatible pregnancies. MATERIALS AND METHODS: In this prospective observational cohort study, 889 consecutive ABO-incompatible mother-newborn pairs were enrolled between November 2025 and June 2026. Maternal IgG anti-A and anti-B titres were measured after 0.01 M dithiothreitol tr

Nanotechnology-Driven Precision Diagnostics and Therapeutics for Preeclampsia.

Preeclampsia originates in placental dysfunction yet it is detected only after maternal hypertension and proteinuria or other organ damage emerge, by which point disease-modifying interventions are not available, and delivery of the baby and placenta remains the only cure. Nanotechnology offers a route to reduce this diagnostic-therapeutic latency. Nanostructured biosensors and liquid biopsy platforms detect placental proteins, cell-free RNA/DNA, and extracellular vesicles at femtomolar concentrations and in a multiplex manner, enabling earlier risk stratification when integrated with machine-

Obstetric pharmacotranscriptomics.

INTRODUCTION: Pregnancy produces gestational-age and tissue-specific transcriptional remodeling in maternal blood, placenta, decidua, fetal membranes, and fetal compartments that can modify pathways governing drug disposition and response. Obstetric pharmacotranscriptomics seeks to relate these dynamic expression states to medication exposure, target engagement, efficacy, and toxicity. AREAS COVERED: This narrative review defines obstetric pharmacotranscriptomics, distinguishes pregnancy- and disease-associated expression from medication-induced transcriptional effects, and evaluates current

Oropouche virus disrupts neurodevelopment and exhibits congenital infection potential.

Oropouche virus (OROV) historically caused a self-limiting disease, yet recent strains have been clinically linked to congenital infection and neurodevelopmental disease. These observations highlight the need to study OROV as a congenital pathogen. Here, using both a historical and currently circulating strain, we show OROV infects neurons across differentiation states in human forebrain organoids. Compared with the neurotropic congenital pathogen Zika virus (ZIKV), OROV exhibits a heightened capacity for neuroinfection and pathology in both forebrain organoids and neonatal mice. The increased

Development and validation of a risk prediction model for preterm birth in women concurrent with Hepatitis B virus carrier status and gestational diabetes mellitus.

BACKGROUND: Preterm birth is a major adverse neonatal outcome that has been associated with maternal hepatitis B virus carrier status. Co-occurrence of hepatitis B virus carrier status carrier status and gestational diabetes mellitus is associated with an increased risk of preterm birth compared with hepatitis B virus carrier status alone. This study aimed to develop and externally validate a clinically applicable multivariable prediction model incorporating both clinical risk factors and maternal blood biomarkers to estimate preterm birth risk among women with concurrent hepatitis B virus car

Impaired erythroid maturation in murine embryos upon loss of the preeclampsia-associated serine protease prostasin Prostasin loss impairs erythropoiesis.

In humans, the membrane-bound serine protease prostasin encoded by Prss8 is associated with preeclampsia, a gestational hypertension disorder affecting blood supply of the placenta. Deficiency of Prss8 in mice resulted in embryonic lethality by embryonic day (E) 14.5 and impaired maturation of placental labyrinth. We found in Prss8-/- embryos at E11.5 and E12.5 a reduced frequency of both circulating primitive and erythro-myeloid progenitor (EMP)-derived definitive erythroblasts in the yolk sac, placenta and fetal liver. Single-cell RNA sequencing analyses revealed a downregulation of fetal he
