Assessment of normal fetal cranial nerve development from 14 to 28 weeks of gestation on post-mortem 3 Tesla MRI.

INTRODUCTION: High-resolution post-mortem Magnetic Resonance Imaging (MRI) is increasingly used to assess anomalies and can serve as a complementary tool to conventional autopsy. The main aim of this study was to detect and measure the diameter of cranial nerves across gestation. METHODS: This observational study was conducted at a tertiary referral hospital and included 150 fetuses from 14 to 28 weeks of gestation with normal brain findings. RESULTS: The optic nerve was identified earliest at 15 weeks, with a mean diameter of 0.62 mm at 15 weeks and 1.49 mm at 27 weeks. The oculomotor nerve

Oculocardiac Reflex during Electrode Placement in Trigeminal Schwannoma Surgery via Cavernous Sinus Approach.

The oculocardiac reflex (OCR) is well known in ophthalmic surgery, but its occurrence during neurosurgical procedures is rare and often unexpected. Mechanical stimulation of trigeminal pathways at the skull base can cause significant hemodynamic changes; however reflex activation during preparatory steps rather than tumor manipulation is rarely described. We report an intraoperative event in a 22-year-old male undergoing resection of a cystic trigeminal schwannoma via a subtemporal transcavernous approach under general anesthesia, during which periorbital needle electrodes were placed for cran

Facial nerve palsy and hypertension; unusual presentation of apparent mineralocorticoid excess (AME) syndrome in a paediatric patient.

We report a rare case of a monogenic form of hypertension where a boy in early childhood presented with a 2-day history of right-sided facial palsy. Severe hypertension was evident with signs of end-organ damage. This report aims to highlight the diagnostic process when a common presentation, such as facial palsy, uncovers a serious and rare underlying condition that demands urgent multidisciplinary care. Further workup revealed that the patient had hypokalaemic metabolic alkalosis with low serum renin and aldosterone levels. His urinary steroid profile was suggestive of Apparent Mineralocorti

Mid- to Long-Term Visual Outcomes After Surgical Treatment of Cavernous Sinus Meningioma.

BACKGROUND AND OBJECTIVES: Cavernous sinus meningiomas (CSMs) are surgically challenging due to their proximity to the internal carotid artery and multiple cranial nerves, and long-term visual trajectories after treatment remain incompletely characterized. This study aimed to define mid- to long-term outcomes, with an emphasis on temporal patterns of visual change, in patients undergoing surgical treatment for CSMs. METHODS: We retrospectively analyzed 30 patients with CSM who underwent initial surgical treatment between April 2004 and October 2024. Clinical, radiological, and pathological da

Multimodality management of large carotid-space paragangliomas: A single-center case series.

BACKGROUND: Large carotid-space paragangliomas (LCSPs) are uncommon, highly vascular tumors that present significant operative challenges because of their intimate relationship to the carotid circulation, lower cranial nerves, and skull base. The central hazard in surgical management is not tumor size alone but uncertainty regarding vascular anatomy, collateral circulation, and operative conditions before incision. We describe a structured multimodality strategy to reduce this uncertainty before critical dissection. CASE DESCRIPTION: A retrospective review was performed of eight consecutive p

Jugulotympanic paraganglioma, preoperative embolization, and transmastoid retrolabyrinthine approach: Two-dimensional operative video.

BACKGROUND: Jugulotympanic paraganglioma (JTP), also known as glomus jugulare tumor, is a neuroendocrine neoplasm. Although it represents <0.5% of head-and-neck tumors, it demonstrates significant local aggressiveness due to its tendency to infiltrate bone, vascular structures, and cranial nerves. Due to the hypervascular nature of these tumors, preoperative embolization has become an important surgical adjunct. The retrolabyrinthine approach provides adequate exposure of the jugular foramen while preserving the facial nerve without mandatory transposition. CASE DESCRIPTION: We present a 52-y

Pediatric Guillain-Barré Syndrome Post Organ Transplant: Acute and Chronic Complication Requiring High Index of Suspicion.

BACKGROUND: Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy most often triggered by infections such as Campylobacter jejuni, Epstein-Barr virus (EBV), or cytomegalovirus (CMV). Although transplant-associated GBS is recognized in adults, it remains rare in pediatric recipients, and distinguishing immune-mediated disease from medication toxicity or critical illness can be challenging. METHODS: We describe two pediatric cases of GBS following organ transplantation, one after liver transplant and one after small bowel transplant. Clinical features, diagnostic eva

Neurophysiological monitoring of cranial nerves III and VI in endoscopic skull base surgery: scoping review and comparative study.

INTRODUCTION: Intraoperative neurophysiological monitoring (IONM) of extraocular cranial nerves (EOCN) during skull base surgery has expanded to reduce post-operative neurological deficits. However, multiple techniques exist, and comparative evidence supporting their routine use in endoscopic endonasal approaches (EEA) remains limited. The aim of the study is to evaluate the reliability, signal quality, and safety of different IONM techniques for CN III and VI during EEA, integrating a scoping literature review with a single surgeon's clinical experience. METHODS: A scoping review identified

Microsurgical resection of skull base meningioma Operative Nuances, Challenges, and Outcome.

OBJECTIVES: Skull base meningiomas represent a distinct subgroup of intracranial tumours because of their proximity to cranial nerves, major vascular structures, and the brainstem. Although advances in imaging, neuroanaesthesia, and skull base approaches, marked improvement in illumination devices, provision of improved haemostatic agents, greater availability of more precise surgical instruments, and better modalities for skull base reconstruction have led to an inevitable evolution of safe skull base neurosurgery, complete resection remains technically demanding. This study evaluates operati

Injuries of the upper cervical spine represent 1/3 of all cervical spine injuries and approximately 40 % result by the death. Every level of the cervical spine can be injured - fractures of condyles of the occipital bone (CO), atlantooccipital dislocation (AOD), fractures of the Atlas (C1), atlantoaxial dislocation (AAD) and fractures of the axis (C2). Most of cases in younger patients are caused by high-energy trauma, while by elderly people, because of the osteoporosis, is needed much less energy and even simple falls can cause the injury of the cervical spine. That´s why the etiology of inj

The Modulatory Effects of Anesthetics and Analgesics on Neurophysiological Monitoring and Underlying Mechanisms.

Intraoperative Neurophysiological Monitoring (IONM) is an indispensable surgical tool that offers invaluable insights into neurological function across a spectrum of anatomical areas. By comprehensively assessing the integrity of the brain, brainstem, spinal cord, cranial nerves, and peripheral nerves, IONM plays a pivotal role in guiding surgical decision-making and optimizing patient outcomes, particularly in the context of high-risk procedures. Intraoperative drugs, especially anesthetics and/or analgesics, differentially modulate neurophysiological monitoring, which remarkably affects the

Automated Mapping the Pathways of Cranial Nerve II, III, V, and VII/VIII: A Multi-Parametric Multi-Stage Diffusion Tractography Atlas.

OBJECTIVE: Cranial nerves (CNs) play a crucial role in various essential functions of the human brain, and mapping their pathways from diffusion MRI (dMRI) provides valuable preoperative insights into the spatial relationships between individual CNs and key tissues. However, mapping a comprehensive and detailed CN atlas is challenging because of the unique anatomical structures of each CN pair and the complexity of the skull base environment. METHOD: In this work, we present what we believe to be the first study to develop a comprehensive diffusion tractography atlas for automated mapping of

Klumpke's Palsy Following Minor Trauma: A Rare Presentation in a Young Adult.

The brachial plexus is a complex network that transmits motor and sensory signals between the spinal cord and the upper limbs. Klumpke's palsy, a rare injury affecting the lower brachial plexus (C8-T1 roots), commonly occurs in newborns or adults after severe trauma and is characterized by a claw-hand deformity, sensory loss along the medial forearm and hand and weakness in the small muscles of the hand. Adult-onset Klumpke's palsy following minor trauma is exceptionally rare. We report a unique case of isolated right-sided Klumpke's palsy in a 31-year-old male after a trivial fall from bed. N

Tasting pain-relief through tongue stimulation: results from a feasibility study looking into the use of cranial nerve non-invasive neuromodulation in healthy individuals and chronic poststroke patients.

PURPOSE: Chronic pain is a common consequence of stroke, often impairing survivors' quality of life. By targeting cranial nerves and brainstem regions involved in pain modulation, cranial nerve non-invasive neuromodulation (CN-NINM) offers a promising approach to pain relief. The aims of this study were to investigate the feasibility of using CN-NINM in people with chronic stroke and in healthy individuals, and to test the immediate effect of this modality on experimental pain. PARTICIPANTS AND METHODS: Thirteen adults with chronic stroke and 13 age-matched healthy adults were recruited for a

Identification of low threshold off-target activation pathways during stimulation of carotid baroreceptor afferents in swine.

OBJECTIVE: Electrical stimulation of the baroreceptors pathways at the carotid sinus bulb - known as baroreflex activation therapy (BAT) - is intended to change autonomic tone and ultimately reduce blood pressure and heart rate. BAT is pre-market approved by the United States Food and Drug Administration (FDA) for the treatment of heart failure and received an FDA humanitarian device exemption for drug resistant hypertension. However, responder rates are limited by side-effects including numbness in the head and neck, altered speech, respiratory constriction, dry cough, vomiting, and altered s

Usefulness of a 3 Dimensional-printed Head Model with Two-layered Dura and Cerebellar Tentorium for Transpetrosal Approach Training.

The combined transpetrosal approach provides effective exposure to the petroclival region and ventral brainstem, although its technical demands necessitate advanced neurosurgical expertise. Proficiency in skull base drilling and meticulous separation of the dual dural layers is crucial. Recently, three-dimensional printing has gained attention as a tool for surgical training. To evaluate its potential in transpetrosal approach education, we developed a three-dimensional head model that reproduces the soft brain tissue, cranial nerves, vasculature, and tumors.
