Comment on: Clinical impact of anti-SSA antigenic specificity on disease activity and patient-reported outcomes in primary Sjögren's disease: a real-life cohort study.

The postoperative recurrence risk of gastrointestinal stromal tumour (GIST) should be estimated when considering adjuvant systemic therapy. Previous studies in the literature have suggested that small intestinal GISTs are more aggressive than gastric GISTs. We assessed the prognostic role of the primary tumour site in patients with operable GIST to compare the outcomes of gastric and small intestinal GISTs over a decade of treatment. The Surveillance, Epidemiology, and End Results (SEER) database was queried for cases of gastric and small intestinal GISTs between 2004 and 2014 using the GIST-s

Therapeutic divergence between polymyalgia rheumatica and giant cell arteritis: lessons from IL-17A inhibition.

Graft-versus-host disease (GvHD) is a life-threatening complication of hematopoietic stem cell transplantation (HSCT), which is caused by allogeneic T cells recognizing molecules of the recipient as foreign. Endogenous glucocorticoids (GC) released from the adrenal gland are crucial in regulating such inflammatory diseases. Here we demonstrate that genetically engineered mice, that are largely unresponsive to GC, suffer from aggravated clinical symptoms and increased mortality after HSCT, effects that could be tempered by neutralization of IL-6. Interestingly, selective ablation of the GC rece

Bioactive Compounds as Putative Molecules of Neuroplasticity: An Epigenetic Perspective.

Aging populations face rising Alzheimer's disease (AD) incidence, with emerging links between epigenetic dysregulation and cognitive decline. Given their dynamic nature, epigenetic modifications offer potential targets for nutritional intervention, although current evidence remains largely correlational. This review presents a food science-oriented framework examining how dietary bioactive compounds may associate with neuroplasticity modulation through putative epigenetic pathways, including DNA methylation, histone modifications, chromatin remodeling, and noncoding RNAs. While certain food-de

Acute variceal bleeding in a patient with phaeochromocytoma: the critical choice of vasoactive therapy.

A woman in her late 70s with known left-sided phaeochromocytoma presented with acute variceal bleeding secondary to suspected cirrhosis. Terlipressin, a standard first-line vasoactive agent, was contraindicated due to the risk of precipitating hypertensive crisis. Octreotide was successfully used as the preferred alternative, combined with careful alpha-blockade optimisation and endoscopic variceal ligation, achieving complete haemorrhage control without cardiovascular complications. This case highlights the need to recognise contraindications in patients with rare comorbidities and offers gui

Adrenal metastasis from papillary thyroid cancer: staged surgical resection.

Adrenal gland metastasis from papillary thyroid carcinoma (PTC) is a rare and diagnostically challenging event. Approximately 15 cases of adrenal involvement have been published to date. We report a man in his 70s with previously treated PTC (pT2N1bMx) who re-presented with right cervical lymph node recurrence and a right adrenal mass measuring 5.7×6.5 cm detected on staging cross-sectional imaging. A CT-guided biopsy confirmed metastatic PTC. We performed staged surgery - right cervical lymph node dissection followed by open right adrenalectomy 4 weeks later. Serum thyroglobulin fell from >50

Visual cue properties determine innate orientation strategy in Monarch butterflies.

OBJECTIVE: Gestational diabetes mellitus (GDM) has an impact on fetal adrenal gland size and volume, which are greater in the GDM population. This study used 2D and 3D ultrasound scanning to determine the correlation of fetal adrenal gland size and volume with fructosamine levels, HbA1c levels, estimated fetal weight (EFW), and neonatal birth weight in GDM patients. METHODS: This study included eighty singleton pregnant women diagnosed with GDM between 24-28 weeks of gestation. During weeks 32-34 of gestation, the length, width, and depth of the fetal zone and total adrenal gland were measure

'Paraganglioma and pheochromocytoma: a review of current guidelines, new diagnostic and therapeutic approaches'.

Paragangliomas and pheochromocytomas, recently reclassified to unify the disease as extra-adrenal and intra-adrenal paragangliomas, are rare neuroendocrine tumours. These tumours have an exceptionally strong hereditary component, with up to 40% of cases being driven by germline mutations in susceptibility genes and therefore a cornerstone of modern PPGL care is the genotype-first approach, which heavily dictates initial assessment, surgical strategy, and follow-up. In recent years, the clinical landscape has evolved rapidly, leading to the publication of more than ten new consensus statements

Fatal TAFRO syndrome in which adrenal hemorrhage preceded thrombocytopenia.

TAFRO syndrome is a rare systemic inflammatory disease characterized by thrombocytopenia, anasarca, fever, renal dysfunction, and myelofibrosis, and its follows a rapidly fatal clinical course. A few cases of TAFRO syndrome complicated by adrenal hemorrhage were recently reported; however, its pathogenetic significance remains largely unclear. The present case was a 59-year-old man presenting with left lower back pain. Imaging studies revealed left adrenal hemorrhage. Although an elevated C-reactive protein level was observed in the initial visit, there were no obvious abnormalities in the plat

The Rare Case of a Sertoli Cell Tumor of the Adrenal Gland in a Man With Pathological Dexamethasone Suppression Test: A Case Report.

Sertoli cell tumors are rare gonadal stromal tumors. Extragenital manifestations, particularly in the adrenal gland, are exceptionally uncommon. We present a case of an adrenal Sertoli cell tumor in a male patient, presenting with a pathological (1 mg overnight) dexamethasone suppression test. An 82-year-old otherwise healthy man was referred for evaluation of an incidentally detected left adrenal mass. The patient was asymptomatic with no clinical signs of catecholamine excess, hypercortisolism, hyperaldosteronism, or B-symptoms. Initial CT scan revealed a 44 × 42 mm lesion, which increased t

Insights into diagnosis and management of giant adrenal pseudocysts.

Adrenal cysts are rare lesions that may coexist with, or mimic, malignant adrenal tumours. Their shared imaging characteristics may cause misdiagnosis with potentially harmful outcomes. We report the case of a young man who presented with several months of headaches and mild anaemia. Non-contrast CT imaging revealed a giant left adrenal cystic lesion measuring 19×15×15 cm with indeterminate CT attenuation and internal debris. 18F-fluorodeoxyglucose-positron emission tomography demonstrated low peripheral uptake favouring a benign aetiology. The multidisciplinary recommendation was for excision

Primary adrenal lymphoma presenting as primary adrenal insufficiency.

Although lymphoma may involve the adrenal glands as part of a generalised disease process, primary adrenal lymphoma is a rare entity. We present the case of a man in his late 60s who was admitted via the emergency department with clinical features consistent with adrenal failure. Initial investigations demonstrated anaemia, hypercalcaemia and adrenal insufficiency. Further biochemical work-up showed no evidence of an autoimmune or congenital aetiology of adrenal failure, and diagnostic imaging revealed bilateral adrenal enlargement. Adrenal biopsy revealed a diagnosis of diffuse large B-cell l

Mild autonomous cortisol secretion (MACS) is associated with increased mortality in patients with adrenal adenomas only in the presence of type 2 diabetes: a retrospective cohort study.

BACKGROUND: Hypercortisolism and type 2 diabetes mellitus (T2D) independently contribute to an increased burden of cardiometabolic morbidity and mortality. However, data on the interaction of hypercortisolism and T2D are scarce-thus, we aimed to assess the cortisol-associated morbidity and mortality in the context of T2D and adrenal mild autonomous cortisol secretion (MACS). METHODS: This is a retrospective cohort study including 668 patients with adrenal adenomas, who were visiting the endocrine outpatient clinic at the Vienna General Hospital between 01.01.2000 to 31.12.2022. Of those, 151

Metastasis of lung carcinoma in a pheochromocytoma.

Tumor-to-tumor metastasis (TTM) is an established but rare entity. Lung carcinoma metastasizes commonly to adrenal glands; however, metastases into an already existing adrenal gland tumor are a rare finding. A 78-year-old man was diagnosed with pheochromocytoma, and during the disease staging, lung involvement was revealed. Functional imaging with both 18F-fluorodeoxyglucose (18F-FDG) and 68Ga-DOTA-Tyr3-Octreotate (68Ga-DOTA-TATE) positron emission tomography (PET) was performed: 18FDG-PET showed mild uptake in both the adrenal and pulmonary lesions, whereas 68Ga-DOTA-TATE-PET revealed intense

Current evidence across the treatment spectrum of adrenocortical carcinoma: a comprehensive scoping review.

Adrenocortical carcinoma is a rare and aggressive malignancy for which high-quality evidence to guide clinical decision-making remains limited. This scoping review aimed to map and synthesize the extent, nature, and direction of evidence across five therapeutic domains in the management of adrenocortical carcinoma. Following the Joanna Briggs Institute methodology and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews guidance, MEDLINE, Embase, and the Cochrane Library were searched without date restrictions. Eligible sources included clinical

First-in-human, open-label, single-arm phase 1 study of [211At] meta-astatobenzylguanidine in patients with pheochromocytoma or paraganglioma.

PURPOSE: The efficacy of current therapeutic options for pheochromocytomas and paragangliomas, such as 131I-meta-iodobenzylguanidine (131I-MIBG), is limited owing to low complete response rates and a requirement for prolonged radiation isolation. In this regard, alpha-emitting meta-astatobenzylguanidine labeled with astatine-211 (²¹¹At-MABG) has demonstrated encouraging non-clinical results in mice, yet its clinical potential remains unclear. This first-in-human, open-label, single-arm, phase 1 clinical study aimed to evaluate the safety, pharmacokinetics, and recommended dose of 211At-MABG.

The cardiometabolic significance of post-dexamethasone cortisol levels below the MACS threshold in adrenal incidentaloma.

PURPOSE: Although post-dexamethasone suppression test (DST) cortisol levels ≤ 1.8 µg/dL are generally considered compatible with nonfunctioning adrenal tumors, accumulating evidence suggests that cardiometabolic risk may vary even below the threshold for mild autonomous cortisol secretion (MACS). This study aimed to evaluate cardiometabolic risk across different post-DST cortisol levels in patients with adrenal incidentaloma using conventional metabolic parameters and atherogenic indices. METHODS: In this retrospective cross-sectional study, 156 patients with adrenal incidentaloma were classi

Genotype-dependent [18F]FDOPA uptake: quantitative evidence from a genetically stratified PPGL cohort.

INTRODUCTION: [18F]FDOPA PET/CT is widely used in the diagnostic work-up of pheochromocytoma and paraganglioma (PPGL), with the latter entity classified into genetic clusters 1a, 1b, and 2 and sporadic tumours, reflecting varying aggressiveness. We aimed to assess diagnostic performance and quantitative [18F]FDOPA uptake across genetic clusters. METHODS: Consecutive PPGL patients with [18F]FDOPA PET/CT at initial diagnosis and available information on germline and/or somatic testing were included. Patients were grouped into genetic clusters. PET quantification included metabolic tumour volume

Effects of a GLP-1 agonist and anti-diabetic therapies on pheochromocytoma/paraganglioma growth.

BACKGROUND: A high proportion of patients with pheochromocytomas and paragangliomas (PPGLs) show glycemic dysregulation. However, studies on the effects of anti-diabetic drugs on PPGL tumor growth are still lacking. METHODS: We evaluated the effects of glucagon-like peptide 1 receptor agonist (GLP-1RA) semaglutide, metformin and insulin on fresh patient-derived PPGL primary cultures (n=54), including 8 cluster 1 tumors and 18 cluster 2/3 tumors (n=4 tumors with RET pathogenic variants [PVs]). We correlated the primary culture data with clinical data from 67 patients enrolled in the prospectiv

The dromedary camel HPA axis: Baseline ultrastructure and functional correlates of cortisol following transport.

BACKGROUND: The hypothalamic-pituitary-adrenal (HPA) axis is central to vertebrate adaptation, yet its structural organization remains poorly characterized in large desert-adapted mammals at the subcellular level. The dromedary camel (Camelus dromedarius) is a species of considerable comparative interest due to its tolerance of extreme environmental conditions, but foundational integrated neuroendocrine anatomy is lacking. Furthermore, whether subcellular structural variation within this axis correlates with functional HPA output has not been investigated enough. METHODS: This study provides
