A Case of Simultaneous Glomus Tympanicum, Glomus Jugulare, and Bilateral Carotid Paraganglioma.

Paragangliomas are rare neuroendocrine tumors that can arise anywhere from the skull base to the pelvis, but most frequently occur in the head and neck, with the carotid body being the most common site. Subtypes are named according to site of origin, such as glomus tympanicum (behind tympanic membrane) and glomus jugulare (at or near jugular foramen). These tumors may cause cranial nerve deficits depending on location. The following case describes a 51-year-old male who first presented with long-standing right-sided facial paralysis who later developed progressive neurologic symptoms. Imaging

Management of Shamblin Type III Carotid Body Tumor With Preoperative Embolization, Surgical Resection, and Rescue Endovascular Stroke Treatment: A Case Report.

BACKGROUND This report describes the case of a 51-year-old woman with a history of transient ischemic attack and a diagnosis of a large Shamblin type III carotid body tumor (CBT) encasing the carotid artery. The CBT was managed by preoperative embolization and surgical resection, which was complicated by postoperative stroke and arterial damage requiring surgical repair. CBTs are usually slow-growing, pulsatile cervical tumors that can cause clinical symptoms secondary to catecholamine secretion in functional lesions or symptoms related to compression of adjacent structures in nonfunctional le

Refining Prognostic Stratification in Head and Neck Paragangliomas: Insights from a SEER-Based Population Study.

OBJECTIVES: Head and neck paragangliomas (HNPGLs) are rare neuroendocrine tumors with heterogeneous behavior, making prognostic assessment challenging. Although generally indolent, a subset exhibits malignant potential, and robust prognostic determinants remain insufficiently defined. This study aimed to identify clinical and pathological factors independently associated with overall survival (OS) in patients with HNPGLs. METHODS: We performed a retrospective Surveillance, Epidemiology, and End Results (SEER)-based cohort study (1975-2021). Demographic, clinicopathologic, and treatment variab

[Carotid body tumors].

Glomus caroticum tumors (GCT) are rare, mostly benign paragangliomas and the clinical relevance stems from their complex anatomical relationship to vessels and cranial nerves. Their heterogeneous clinical presentation, potential hormonal activity and genetic associations require a differentiated approach to diagnostics and treatment, which is often not widely familiar because of their rarity. This article outlines the key diagnostic and therapeutic characteristics as well as current evidence-based management strategies to provide a practical, clinically relevant guide.

Surgical Management of Carotid Body Tumors at a High-Altitude Center: A 22-Year Single-Institution Experience.

BACKGROUND: To evaluate surgical management and outcomes of carotid body tumors (CBTs) by Shamblin classification over 22 years at a center 1,853 m above sea level. METHODS: Among 107 patients who underwent surgery for 109 cervical masses suspected to be CBTs between 2000 and 2021, six patients with non-CBT histopathology were excluded. The final cohort comprised 101 patients with 103 confirmed CBTs, including two patients with bilateral disease. Tumor-level variables were reported for 103 tumors, whereas demographics and postoperative outcomes were reported once per patient. Continuous varia

Case report and literature review: a case of rare papillary thyroid carcinoma combined with paraganglioma.

BACKGROUND: Papillary thyroid carcinoma (PTC) primarily metastasizes to ipsilateral cervical lymph nodes. Paragangliomas (PG) in the head and neck region may be located at the carotid bifurcation, causing anatomical separation of the internal and external carotid arteries. METHODS: A rare case of PTC with PG at our center is reviewed to explore the relationship and differential diagnosis between PG and cervical lymph node metastasis (CLNM) in thyroid carcinoma. Based on the characteristics of this case, we retrieved relevant domestic and international case reports and relevant studies. This s

Syncope and obstructive sleep apnea: Mechanisms and clinical scenarios.

Obstructive sleep apnea (OSA) is a highly prevalent sleep disorder affecting a considerable proportion of the adult population. It is characterized by chronic intermittent hypoxia (CIH), resulting from recurrent partial (hypopnea) or complete obstruction (apnea) of the upper airway during sleep. Clinically, OSA produces fragmented sleep, loud snoring, excessive daytime somnolence, cognitive dysfunction, and nocturnal diaphoresis. Importantly, OSA is recognized as an independent risk factor for systemic hypertension and is associated with cardiac arrhythmias and stroke. The CIH contributes to t

Acute Hypoxia Inhibits K2P3/9 (TASK1/3) Independently of Cytosolic ATP in Carotid Body Type 1 Cells.

Hypoxia stimulates ventilation through a chemosensory reflex that is initiated by inhibition of the O2-sensitive K+ channels such as K2P3/9 in carotid body Type 1 glomus (CB) cells. Because K2P3/9 activity is sensitive to cytosolic ATP concentration ([ATP]cyt, K1/2 ≈ 2.5 mM), it has been proposed that hypoxia suppresses mitochondrial ATP production and thereby reduces [ATP]cyt to inhibit K2P3/9 activity. To test this hypothesis, we developed a KATP channel-based bioassay using a patch-cramming approach to measure [ATP]cyt in living cells. Co-expression of Kir6.2(ΔC36-R50G) and SUR1(G1485D) in

The carotid body in sepsis: From homeostatic defense to immunometabolic dysregulation.

Sepsis is a life-threatening syndrome caused by a dysregulated host response to infection and involves failure of immune, metabolic, respiratory, cardiovascular, thermal, endocrine, and autonomic regulation. In this review, we propose that the carotid body may act as a peripheral interoceptive checkpoint in the transition from adaptive host defense to immunometabolic decompensation. The discussion is organized around four interconnected dimensions. First, we examine inflammatory and metabolic signals that may be detected by the carotid body during sepsis, including cytokines, lactate, glucose,

Metastatic Carotid Body Paraganglioma Following Three Decades of Indolent Growth: A Case Report.

Paragangliomas (PGLs) are rare neuroendocrine tumors arising from extra-adrenal chromaffin cells, with head and neck paragangliomas (HNPGLs) accounting for only a small proportion of all cases. Although most HNPGLs are benign, slow-growing, and nonfunctional, a minority develop metastatic disease, for which reliable histopathologic predictors remain lacking. We report the case of a 63-year-old man who presented with a progressively enlarging left-sided neck mass that had been present for more than 30 years before evaluation. Imaging demonstrated a carotid body paraganglioma with cervical lymph

Glucagon-Like Peptide-1 Receptor Agonists for Obstructive Sleep Apnea: A Review.

IMPORTANCE: Obstructive sleep apnea (OSA) affects nearly 1 billion adults worldwide and is independently associated with cardiovascular disease, stroke, and increased mortality. While continuous positive airway pressure (CPAP) remains the criterion standard treatment, adherence rates of only 30% to 60% limit its population-level effectiveness. Obesity is present in 60% to 70% of patients with OSA and represents the primary modifiable risk factor, yet, until recently, no pharmacological therapies specifically targeted this pathophysiological mechanism. Carotid body mitochondria exhibit normal oxygen affinity despite COX4I2 enrichment.

The carotid body (CB) is the key peripheral oxygen sensor. CB mitochondria are hypothesised to be uniquely adapted with unusually low intrinsic oxygen affinity which, in association with nitric oxide (NO) and reactive oxygen species signalling, enables acute responsiveness to hypoxia. However, CB mitochondrial physiology or intrinsic oxygen affinity have never been measured directly. We sought to address this key gap by isolating sheep CB mitochondria and comprehensively characterising their phenotype and contrasting them to a non-oxygen sensing tissue, left ventricular myocardium (LV). Photon-Counting Detector CT Improves Visualization of the Normal Carotid Body Compared with Energy-Integrating Detector CT.

BACKGROUND AND PURPOSE: The carotid bodies are small chemoreceptor organs at the carotid bifurcation. Their depiction on conventional CT angiography is inconsistent, with reported detection rates on energy-integrating detector CT (EID-CT) of 62% to 91%. This study evaluates whether photon-counting detector CT (PCD-CT) improves carotid body visualization and describes their normal imaging appearance. MATERIALS AND METHODS: This retrospective study included 37 patients who underwent both PCD-CT and EID-CT head and neck angiography, yielding 74 carotid bifurcations. Two neuroradiologists, readin

Interventional Treatment of Arterial Hypertension in The Modern Era: An Updated Review from the Young Investigator Group of the European Society of Hypertension.

Achievement of blood pressure targets in hypertensive patients is a challenge for clinicians despite the different classes of drugs currently available. For this reason, taking advantage of the technological progress, new interventional strategies have been proposed. Each of these approaches acts on autonomic and vascular regulation, critical components of hypertension pathophysiology with the aim to recalibrate aberrant sympathetic activation and vascular dysfunction. Renal artery denervation is the only modality currently supported by guideline recommendations in selected patients. Barorefle

Somatic-only SDHD variant with tumor-specific loss of heterozygosity in metastatic carotid body tumor: a case report with review of literature.

Carotid body tumors (CBTs) are rare paragangliomas in which genetic predisposition, particularly pathogenic variants in succinate dehydrogenase (SDHx) genes, plays an important role in tumorigenesis. Previous genetic studies of CBTs have primarily focused on germline SDHx variants, whereas somatic alterations remain poorly characterized. Among SDHx genes, SDHB variants are known to be associated with a higher metastatic risk, while SDHD variants are generally linked to a lower metastatic rate. We report the case of a 41-year-old man who presented with a painless right-sided neck mass. Imaging
