A Screening Model for Latent Tuberculosis Infection Among Household Contacts of Patients With Tuberculosis Based on the CyTB Test and an Android App: Protocol for a Development and Implementation Study.

BACKGROUND: Latent tuberculosis infection (LTBI) remains a major barrier to tuberculosis elimination, particularly in community settings where household contacts (HHCs) of patients with active tuberculosis are at increased risk of infection and progression to disease. Current tests for LTBI have practical limitations that restrict their use in field and primary care settings. Antigen-based skin testing, combined with digital tools for induration measurement, may improve the accuracy and delivery of HHC screening under routine program conditions. OBJECTIVE: This study aims to develop and evalu

Programmable Multisite Recognition Enables Amplification-Free Nanopore Detection of Mycobacterium tuberculosis in Sputum and Plasma.

Tuberculosis diagnostics remain challenged by low target abundance, strain diversity, and the need for reliable testing in difficult clinical specimens. Here, we develop an amplification-free nanopore sensing platform based on programmable linear DNA nanohook arrays for direct detection of Mycobacterium tuberculosis (Mtb) DNA. This work presents a dual-target, multisite recognition strategy that simultaneously captures two conserved insertion sequences, IS6110 and IS1081, and converts their binding events into resolvable nanopore electrical signatures. This design improves target recovery and

Advancing Tuberculosis Antigen Detection by Selecting Sandwich-Compatible Aptamers Against the Mycobacterium tuberculosis ESAT-6/CFP-10 Heterodimer Using Library-Immobilized Strategies.

Tuberculosis (TB) represents the world's leading infectious disease killer, surpassing COVID-19. Despite being preventable and curable, a substantial proportion of cases remain undiagnosed or unreported, sustaining transmission. The detection of antigens such as the Mycobacterium tuberculosis ESAT-6/CFP-10 heterodimer, which are secreted at different stages of infection, is a promising screening strategy. Traditional antigen tests relying on antibodies suffer from significant problems of specificity and sensitivity. Aptamers, selected in an in vitro technique termed SELEX (Systematic Evolution

Development of GuaB Inhibitors to Treat Mycobacterium tuberculosis Infection.

Mycobacterium tuberculosis (Mtb) is responsible for more deaths per year than any other infectious disease. Resistance of circulating Mtb to the standard of care treatment regimen is an urgent global health crisis for which novel antibiotic treatments are needed. We have developed extremely potent, orally bioavailable, small-molecule inhibitors of inosine 5'-monophosphate dehydrogenase (GuaB) that catalyze the rate-limiting step in de novo guanine biosynthesis. The inhibitors exhibited bactericidal activity against Mtb in extracellular, intracellular, and hypoxic environments that capture feat

Mycobacterium tuberculosis Rv3248c Inhibits Type I Interferon Response and Promotes Intracellular Survival by Mediating Ubiquitin-Proteasome-Dependent Degradation of TBK1.

Mycobacterium tuberculosis (M. tb) is a leading cause of infectious disease mortality worldwide, and M. tb subverts host immunity to persist intracellularly. Type I interferon (IFN-I) modulates host defense during M. tb infection, but the mechanisms controlling their induction remain incompletely defined. In this research, a previously uncharacterized mycobacterial factor, M. tb Rv3248c, which regulates IFN-I response, was identified using pull-down and mass spectrometry. Overexpression of Rv3248c suppresses IFN-β expression in macrophages at both the transcriptional and protein levels. In add

AX-35 Targets the Cytochrome bc1 Complex in Mycobacterium abscessus and Reveals a Novel Resistance Mechanism.

In patients with cystic fibrosis, nontuberculous mycobacteria are opportunistic, intracellular and extracellular pulmonary pathogens that contribute to deterioration of lung function and, ultimately, morbidity from respiratory failure. Among nontuberculous mycobacterial respiratory pathogens, members of the Mycobacterium abscessus complex pose additional treatment complications as they are notoriously drug-resistant. As part of our routine screening against mycobacterial species, one of the lead antituberculosis compounds, the cytochrome bc1 inhibitor AX-35, was found to be active against isol

The need for fully-effective federated analytics of data sources for clinical trials.

INTRODUCTION: Clinical trials are usually analysed in a single environment allowing for flexible analysis including adjustment or stratification by subgroup: 'one-stage' analysis of individual-level data. Health systems datasets, often distributed across geography and providers, can streamline clinical trials. Data are increasingly accessible in secure data environments (SDEs). Future trial analyses may involve working across multiple SDEs. Row-level data and identifiable data often cannot leave, requiring a 'two-stage approach', where summary data from each SDE are meta-analysed. OBJECTIVE:

Finding a needle in a haystack: Evaluation of Cartridge-Based Nucleic Acid Amplification Test in comparison with microscopy and culture for diagnosis of Extrapulmonary Tuberculosis - A study from a tertiary care centre, Kerala.

Introduction. Mycobacterium tuberculosis has held an exceptional presence in medical research, especially in recent years (1). While pulmonary infections largely remain the focus of literature and public health initiatives (2), extrapulmonary tuberculosis (EPTB) infections have been gaining prominence due to their increasing prevalence, particularly in populations characterized by a low incidence of tuberculosis (2-4). Although EPTB infections, such as tuberculous meningitis, carry a high mortality risk (5), delays in diagnosis and treatment persist. These delays are driven by their diverse, n

Genomic insights into non-tuberculous mycobacteria isolated from patients in a Chinese hospital.

Non-tuberculous mycobacteria (NTM) are increasingly recognized as important opportunistic pathogens, yet the genomic basis of their antimicrobial resistance and virulence in clinical settings remains incompletely understood. We performed whole-genome sequencing on 28 clinical isolates of NTM collected from Chinese patients (primarily bronchoalveolar lavage fluid) between 2022 and 2025, combined with broth microdilution susceptibility testing and comprehensive bioinformatics analysis. The collection comprised diverse species, including Mycobacterium intracellulare, Mycobacterium chimaera, Mycob

Age-dependent immune heterogeneity and the limited diagnostic utility of peripheral blood biomarkers in pediatric tuberculosis: a real-world cohort study with multidimensional model evaluation.

BACKGROUND: Pediatric tuberculosis (TB) remains a diagnostic challenge due to atypical presentations and low pathogen detection rates. Overlapping clinical features with Mycoplasma pneumoniae (MP) infection further complicate differential diagnosis. Immune-inflammatory parameters are age-dependent, yet how age modifies these profiles in children with TB and MP co-infection remains poorly understood. OBJECTIVE: To investigate age-dependent heterogeneity in immune-inflammatory profiles among children with tuberculosis (TB) and to evaluate the additive diagnostic value of peripheral blood biomar

CT-Based Clinical-Radiomics for Differentiating Talaromyces marneffei Spondylitis From Tuberculous Spondylitis Before Pathogen Confirmation: A Multicentre Diagnostic Study in an Endemic Region.

BACKGROUND: Talaromyces marneffei spondylitis (TMS) is an uncommon but clinically important form of talaromycosis in endemic regions. Where talaromycosis and tuberculosis overlap, TMS may be worked up as tuberculous spondylitis (TBS), delaying fungal investigations and antifungal treatment. OBJECTIVES: To assess whether pretreatment CT, routine clinical variables and radiomics features can help differentiate TMS from TBS at the initial assessment stage. PATIENTS/METHODS: This retrospective multicentre study included 263 patients meeting the prespecified composite reference standard (83 TMS a

Alignment-Free Tool for Assemblies and Reads in Genomic Surveillance of Tuberculosis.

Whole-genome sequencing strengthens tuberculosis surveillance, but incomplete lineage schemes can misclassify isolates. We developed Pathotypr, an alignment-free tool for assemblies and reads, using 2,145 markers derived from 26,813 genomes. Across 88,071 samples, Pathotypr improved concordance, expanded uncommon lineage coverage, and detected resistance markers, supporting molecular epidemiology and public health surveillance.
