Monoclonal Gammopathies in Africa.

People of African descent have a reported higher incidence of multiple myeloma (MM) and increased prevalence of its precursor conditions, monoclonal gammopathy of undetermined significance (MGUS) and smoldering MM (SMM). Despite this, research focusing on people of African descent remains sparse. Even in the absence of robust studies across African populations, major disparities are consistently reported. West Africans and South African Black men have a higher prevalence of MGUS than individuals of European descent. MM has been shown to occur in African individuals at a younger age of diagnosi

GLP-1 RA Use and Major Adverse Cardiovascular Events in Patients With Monoclonal Gammopathy of Undetermined Significance.

IMPORTANCE: Monoclonal gammopathy of undetermined significance (MGUS) is associated with an increased risk of cardiovascular disease. Glucagon-like peptide-1 (GLP-1) receptor agonists (RAs) have demonstrated cardiorenal benefits in patients with type 2 diabetes, but their effectiveness in patients with MGUS remains unexplored. OBJECTIVE: To assess the effectiveness of GLP-1 RAs for primary prevention of major adverse cardiovascular and cerebrovascular events (MACCE) in patients with MGUS and diabetes. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used a propensity score-

Trichoscopy of Terra Firma-Forme Dermatosis of the Scalp: A Case Report.

The pathogenesis of multiple myeloma (MM) and its precursor monoclonal gammopathy of undetermined significance (MGUS) is linked to an aging immune system. Chronic activation of B/plasma cells may contribute to the origin of MGUS, which is frequent in the older individuals. However, only 1% of individuals with MGUS annually experience progression to MM. The immune system can specifically recognize MGUS lesions, and preclinical MM models provide evidence for both innate and adaptive immune surveillance. Multiomic studies have identified several systemic alterations at the MGUS stage, suggesting

Genetic, epigenetic, and molecular determinants of multiple myeloma and precursor plasma cell disorders: a pathophysiological overview.

Multiple myeloma (MM) is a malignancy of clonal plasma cells that arises from precursor conditions, including monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM). Disease progression is driven by a complex interplay of genetic alterations, epigenetic dysregulation, and support from the bone marrow microenvironment. Early events such as chromosomal translocations (e.g. t(4;14)), copy number abnormalities (e.g. del(17p), gain(1q)), and driver mutations in KRAS, NRAS, TP53, and DIS3 promote clonal evolution. These are complemented by non-coding regulato

Synovitis, acne, pustular lesions, hyperostosis, and osteitis syndrome associated with monoclonal gammopathy of undetermined significance: a case report.

Herein, we present a case of synovitis, acne, pustular lesions, hyperostosis, and osteitis (SAPHO) syndrome in which the patient had monoclonal gammopathy. To identify whether the patient had blood proliferative disorders, we performed bone marrow aspiration and positron emission tomography-computed tomography (PET-CT) examination. Finally, we diagnosed the patient with SAPHO syndrome and monoclonal gammopathy of undetermined significance (MGUS). The patient was followed up for two years with repeat bone marrow and PET-CT examinations, and no apparent disease progression or progression to anot

[Clonal diseases in the border zone rheumatology/hematology].

Chronic inflammation and disorders of immunoregulation are basic traits of inflammatory rheumatic diseases and also cofactors in the development and sequelae of plasma cell dyscrasias with associated gammopathy. Therefore, in cases of unclear rheumatological symptoms the possibility of an underlying monoclonal gammopathy of undetermined significance (MGUS) should always be considered. The thorough examination of patients with a monoclonal gammopathy is important to be able to differentiate between MGUS, monoclonal gammopathy of clinical significance (MGCS) and multiple myeloma.

Quantitative risk of underlying disease conditions for progression from monoclonal gammopathy to multiple myeloma: a nationwide cohort study.

BACKGROUND: This nationwide real-world data cohort study developed a risk-scoring model to predict the progression of monoclonal gammopathy of undetermined significance (MGUS) to symptomatic multiple myeloma (MM) using prior disease conditions. MATERIALS AND METHODS: The Health Insurance Review and Assessment Service database, which covers nearly the entire Korean population, was utilized in this study. Out of 5361 MGUS patients, 345 (6.4%) were diagnosed with MM, with 253 (73.3%) of them progressing to symptomatic MM. RESULTS: We identified female sex, chronic pulmonary disease, peptic ulce

Genomic and immune profiling of prognostic risk groups in IgM gammopathy reveals novel biomarkers beyond MYD88 L265P.

BACKGROUND: MYD88 L265P is an early mutation in IgM monoclonal gammopathy of undetermined significance (MGUS) and asymptomatic Waldenström macroglobulinemia (WM). Given the high prevalence of the MYD88 mutation observed in epidemiological studies, its presence is not sufficient to drive disease progression. In fact, a recent risk model of progression reported that the impact of other laboratory biomarkers was superior to the MYD88 mutation's presence. Due to the low incidence of these clinicopathological entities, there is a need for a better characterization of tumor and immune cells that can

Immunoglobulin G4-related autoimmune hepatitis with overlapping multiple autoimmune diseases: A case report.

RATIONALE: The prevalence of autoimmune liver disease in Sjogren syndrome (SS) ranges from 1.7% to 47%. Hematologic involvement is commonly observed in SS patients, with 1.5% of patients presenting with monoclonal gammopathy of undetermined significance (MGUS). Chronic antigenic stimulation of B lymphocytes in autoimmune diseases may eventually lead to clonal proliferation or MGUS. Nevertheless, studies reporting the concurrent manifestation of IgG4-related autoimmune hepatitis (IgG4-AIH), Primary biliary cholangitis (PBC), SS, and MGUS are scarce. PATIENT CONCERNS: This article reports a rar

Risk factors for multiple myeloma and its precursor diseases.

Multiple myeloma (MM) is a common hematologic malignancy that originates from precursor conditions such as monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM). Identifying its risk factors is crucial for early intervention. The etiology of MM is multifactorial, involving race, familial clustering, gender, age, obesity, cytogenetic abnormalities, and environmental exposures. Among these, cytogenetic abnormalities and modifiable factors play pivotal roles in MM pathogenesis and progression. 1) cytogenetic abnormalities. Primary abnormalities [e.g., hyp

Anti-plasma cell therapy in patients with monoclonal gammopathy-associated refractory pure red cell aplasia: A report of three cases and literature review.

Acquired pure red cell aplasia (aPRCA) is a clinical syndrome that may be secondary to a diverse array of diseases. It is rarely secondary to monoclonal gammopathy of undetermined significance (MGUS). Currently, there is no consensus on when to initiate anti-plasma cell therapy. This study reports three cases of patients with MGUS-associated aPRCA who responded to anti-plasma cell therapy after failing conventional immunosuppressive treatment. Patient 1 achieved complete remission after treatment with the BD (bortezomib and dexamethasone) and BRD (bortezomib, lenalidomide, and dexamethasone) r

Assessing the performance of the Iceland screens, treats, or prevents multiple myeloma (iStopMM) model in a multicultural Bronx cohort: implications for monoclonal gammopathy of undetermined significance risk stratification.

The Iceland Screens, Treats, or Prevents Multiple Myeloma (iStopMM) risk stratification model, developed to predict ≥10% abnormal plasma cells in the bone marrow in monoclonal gammopathy of undetermined significance (MGUS) patients, was developed in a predominantly White and genetically homogeneous Icelandic population, lacking external validation. Our study aimed to externally validate this model in a racially and ethnically diverse Bronx population. The medical records of patients at Montefiore Medical Center (2002-2023) were searched to identify patients with MGUS who had undergone a bone m

Precursor plasma cell disorders: Classification, risk stratification, and emerging role of early interception.

Precursor plasma cell disorders include monoclonal gammopathy of undermined significance (MGUS) and smoldering multiple myeloma (SMM). These conditions carry a variable risk of progression to symptomatic myeloma and there are ongoing efforts to improve risk stratification to identify patients that are at highest risk of progression. Advanced imaging plays a crucial role in diagnosis and monitoring, and more sensitive tools to measure serum monoclonal proteins and circulating tumor cells are being developed. The data for early intervention in SMM continues to evolve, with several phase III stud

Monoclonal gammopathy of clinical significance - a group name for diseases caused by monoclonal immunoglobulin and/ or free light chains. A change in the approach to non-malignant gammopathies.

BACKGROUND: The group of monoclonal gammopathies includes both malignant diseases, such as multiple myeloma and Waldenström's macroglobulinemia, and benign proliferations of plasma cells or lymphoplasmacytic cells with the formation of usually low concentrations of monoclonal immunoglobulin and/or free light chains. The term monoclonal gammopathy of undetermined significance (MGUS) is used for conditions with non-malignant proliferation. AIM: In the last 25 years, understanding of the various forms of diseases caused by products of benign lymphoplasmacytic or plasma cell proliferation has sig
