Axillary lymph node status following non-identification of the sentinel lymph node in breast cancer.

INTRODUCTION: International guidelines provide no consensus on the surgical management of patients with sentinel lymph node (SLN) non-identification. This study aimed to identify factors associated with SLN non-identification and to determine axillary nodal status following axillary lymph node dissection (ALND) in contemporary clinical practice. MATERIALS AND METHODS: A retrospective cohort study included all patients with unilateral breast cancer who underwent sentinel lymph node biopsy (SLNB) at a tertiary university hospital between 2010 and 2022. All patients underwent preoperative ultras

Preoperative Radiologic Assessment of the Axilla in Locally Advanced Breast Cancer After Neoadjuvant Systemic Therapy: A Contemporary Comparison.

BACKGROUND: Prior studies comparing different imaging modalities in assessing axillary response after neoadjuvant systemic therapy (NST) in locally advanced breast cancer (LABC) are limited. This study evaluated the efficacy of different imaging modalities in identifying residual lymphadenopathy after NST in modern cohort of LABC patients. METHODS: A retrospective review was performed examining LABC patients undergoing NST followed by surgical axillary staging from 2018 to 2023. Patients with LABC (T3-4,N1-3 or any T stage with N2-3 disease) were included. Inflammatory breast cancer was exclu

Concerns and Consultation Needs for Hospital and Community Pharmacists at Metastatic or Recurrent Cancer Diagnosis: A Web-Based Survey in Japan.

Patients diagnosed with metastatic or recurrent cancer experience uncertainty and distress; however, their consultation needs remain insufficiently quantified. We conducted a web survey in Japan (January 23-29, 2025) among adults (≥18 years) with a history of metastatic or recurrent cancer (n=522). Participants selected concerns from 21 items across four domains, and for each endorsed concern, they indicated whether they wished to consult hospital and/or community pharmacists; consultation intention was calculated among those endorsing each item. Hospital-community differences were evaluated u

Tattoo Pigment Migration Within the Breast and Axilla: Radiologic-Pathologic Correlation.

Tattoo pigment migration can appear on breast imaging, including mammography and ultrasound, where it may mimic malignant findings. In the literature, 19 cases have been reported. Here, we describe an additional three cases. Most published cases involve women undergoing screening mammography with incidental identification of pigment migration. On imaging, pigment can present as high-density deposits on mammography and echogenic foci on ultrasound, mimicking calcifications. Reported sites of deposition most commonly include axillary lymph nodes and intramammary nodes. If no other morphologic ab

Manganese-doped zinc oxide Nano-agonists activating the cGAS-STING pathway to potentiate anti-tumor immunotherapy and reprogram the immunosuppressive tumor microenvironment.

The cyclic GMP-AMP synthase - stimulator of interferon genes (cGAS-STING) innate immune pathway is an intriguing, but still challenging, target for cancer immunotherapy. The current small molecule STING agonists have several limitations, such as low membrane permeability, rapid enzymatic degradation, lack of tumor accumulation and dose-limiting systemic toxicity. In this work, we report the design and synthesis of the manganese-doped zinc oxide nanoparticles (Mn-ZnO NPs), as a dual-functional tumor microenvironment (TME) responsive nano-agonist platform and comprehensive biological validation

Breast Cancer Location Predicts Sentinel Lymph Node Metastasis in Clinically Node Negative Patients With Invasive Breast Cancer Treated With Upfront Surgery.

BACKGROUND: Assessment of clinicopathological factors is standard for predicting the sentinel lymph node (SLN) status in patients with clinically node negative breast cancer. Limited studies have specifically addressed the value of cancer location as an independent factor predicting SLN status. This study aimed to evaluate whether clinicopathological factors, and specifically cancer location, predict SLN status. METHODS: This retrospective study was performed in a cohort of consecutive patients diagnosed with T1 to T3 clinically node negative invasive breast cancer managed at a single institu

Clinicopathological characteristics, diagnostic challenges, and systemic implications of metastatic tumors to the eyelid: a systematic review.

BACKGROUND: Metastatic tumors to the eyelid are rare but clinically significant, often indicating advanced systemic malignancy or presenting as the first sign of an occult primary. Histopathology is the diagnostic cornerstone, yet the spectrum of clinicopathological patterns, immunohistochemistry, and common diagnostic pitfalls has not been systematically synthesized at the patient level. METHODS: We conducted a PRISMA 2020-compliant systematic review (protocol registered on the Open Science Framework). PubMed/MEDLINE, Embase, and Scopus were searched on 7 March 2026 without date restrictions

Survival outcomes of axillary de-escalation following neoadjuvant chemo-immunotherapy in clinically node-positive triple-negative breast cancer: a national cancer database study.

BACKGROUND: Adding immune checkpoint inhibitors (ICIs) to neoadjuvant chemotherapy (NAC) enhances systemic efficacy in triple-negative breast cancer (TNBC). However, its impact on the survival outcomes of axillary surgical de-escalation remains undefined. This study evaluates whether chemo-immunotherapy mitigates survival risks historically associated with omitting axillary lymph node dissection (ALND) for sentinel lymph node biopsy (SLNB) in clinically node-positive (cN+) disease. METHODS: In this retrospective cohort study using the National Cancer Database (2018-2022), we identified women

[18F]FDG PET/MRI for axillary staging in node positive breast cancer patients receiving primary systemic therapy. A prospective single arm trial.

BACKGROUND: Management of axillary nodes in breast cancer (BC) remains controversial. Increasingly, node-positive patients receive primary systemic therapy (PST), achieving axillary downstaging in up to 70% of cases. However, current imaging methods, despite being pivotal in the choice for PST, show limited accuracy in assessing treatment response. This study evaluated the diagnostic performance of hybrid [18F]FDG PET/MRI as non-invasive, single-step tool for post-PST nodal assessment. METHODS: This prospective, single-arm, monocentric interventional trial (SNBvsPET/MRI1, ClinicalTrials.gov N

Comparison of ultrasound visibility and retrieval success of two ultrasound-visible clips in targeted axillary dissection after neoadjuvant chemotherapy.

BACKGROUND: Targeted axillary dissection (TAD) is a less invasive alternative to axillary lymph node dissection (ALND) for patients with clinically node-positive breast cancer who convert to node-negative status after neoadjuvant chemotherapy (NAC). However, the effect of preoperative ultrasound visibility of different clip types on TAD outcomes remains unclear. METHODS: We conducted a single-center retrospective cohort study of patients with biopsy-proven axillary node-positive breast cancer who underwent NAC followed by TAD between August 2017 and December 2024. Two ultrasound-visible clips

Development and validation of a nomogram for predicting axillary reverse mapping lymph node metastasis in breast cancer patients.

BACKGROUND: Axillary reverse mapping (ARM) aims to preserve upper limb lymphatic drainage during breast cancer surgery to reduce lymphedema risk, but the metastatic potential of ARM lymph nodes and its preoperative predictors remain unclear. METHODS: A retrospective analysis was conducted on a development cohort of 142 breast cancer patients, with an independent temporal validation cohort of 54 consecutive patients. Indocyanine green and methylene blue were used to map upper limb and breast lymphatic drainage, respectively. Identified ARM lymph nodes were preserved and pathologically examined

De-escalating the axilla: oncologic and morbidity outcomes of targeted axillary dissection versus axillary lymph node dissection following neoadjuvant systemic therapy in clinically node-positive breast cancer within a single institution.

BACKGROUND: Axillary lymph node dissection (ALND) has been standard for regional control in clinically node-positive breast cancer, but causes significant morbidity, including lymphedema and arm dysfunction. Targeted axillary dissection (TAD) is a less invasive alternative after neoadjuvant systemic therapy (NST), which can downstage nodal disease. Emerging evidence suggests that TAD offers oncologic outcomes similar to ALND with less morbidity. METHODS: We conducted a retrospective cohort study of patients with clinically node-positive (cN1) breast cancer treated between 2017 and 2025 at the

[Long-term treatment with sacituzumab govitecan in a patient with brain metastases from triple-negative breast cancer, PD-L1-negative, BRCA1/2 wild-type.].

We report the clinical case of a 64-year-old female patient who underwent a mastectomy with sentinel lymph node biopsy followed by adjuvant chemotherapy with anthracyclines and taxanes for triple-negative, BRCA wild-type invasive ductal carcinoma. Approximately five years later, she presented with recurrence of the disease in the brain and mediastinal lymph nodes, which was treated with neurosurgical resection of the brain lesion, radiotherapy to the tumour bed and first-line chemotherapy with capecitabine. After 4 months, due to progression of extra-cranial disease and poor gastrointestinal t

Machine Learning Classification of Axillary Lymph Nodes Using Microwave Signals.

Axillary Lymph Nodes (ALNs) can be affected by breast cancer, and the number of affected ALNs is a determinant factor in breast cancer staging. Microwave imaging (MWI) has emerged as a promising technique for ALN assessment, addressing limitations in conventional imaging modalities. This study investigates, for the first time, the classification of ALNs and axillary regions from microwave signals, without image reconstruction. Classification is performed considering realistic morphological characteristics of ALNs reported in the literature and is based solely on geometric differences, which di

Dose equivalence calculation in radiotherapy: A Call for Standardization.

Accurate dose equivalence calculations are essential in radiotherapy to balance tumor control with tissue preservation. Despite decades of research, clinical implementation remains highly variable. A structured workshop analysis involving radiation oncologists, medical physicists, and technicians revealed alarming heterogeneity: when applying equivalent dose models (LQ/LQL) to identical clinical scenarios, mean inter practitioner agreement reached only 25.8% (Jaccard index), with consensus as low as 10% for head-and-neck cases. Proposed dose-per-fraction adjustments for identical situations ra

Timing of Loss of Transplacental Antibodies in Infants Born to Mothers Living With Human T-cell Lymphotropic Virus Type 1.

BACKGROUND: To date, evidence suggesting the optimal timing of antibody testing in human T-cell lymphotropic virus type 1 (HTLV-1)-exposed infants is lacking. Testing HTLV-1-exposed infants must balance the need for repeat testing if done too early while minimizing loss to follow-up with protracted follow-up intervals. METHODS: Single-center, retrospective cohort analysis of HTLV-1 serologic testing and breastfeeding practices in infants born to HTLV-1 seropositive pregnant individuals between 01/01/2016 and 10/03/2026. RESULTS: Nineteen children were identified, of whom 5 had positive serol

Cardio-Oncology Risk Phenotypes in Cancer Patients Treated with Chemotherapy: A Data-Driven Analysis.

Cardiovascular disease (CVD) is a major long-term complication in cancer survivors treated with chemotherapy, yet risk varies markedly across individuals. Using the UK Biobank, we studied chemotherapy-treated cancer patients without baseline CVD and identified data-driven cardio-oncology risk phenotypes with unsupervised clustering (UMAP embedding and HDBSCAN). We evaluated associations between phenotypes and incident CVD using Cox proportional hazards models and assessed 5-year risk discrimination with and without phenotype labels. Among 18,693 patients, 9 stable phenotypes were identified, c

Case Report: Concurrent thyroid tuberculosis and tuberculous lymphadenitis initially misdiagnosed as malignancy by 18F-FDG PET/CT.

BACKGROUND: Tuberculous lymphadenitis (TBL) is a common manifestation of extrapulmonary tuberculosis (EPTB). However, establishing a definitive diagnosis can be challenging, particularly in primary healthcare settings, when the clinical presentation is atypical or when TBL coexists with another rare form of EPTB-thyroid tuberculosis (TB). Whether primary or secondary, TB remains an exceedingly uncommon clinical entity, even in the context of the persistently high overall prevalence of tuberculosis in China. Thyroid tuberculosis accounts for approximately 0.1%-0.4% of all thyroid diseases, with
