Beyond Iron Deficiency: Unveiling the Prevalence of Folate and Vitamin B12 Deficiencies in Major Surgical Patients.

INTRODUCTION: Preoperative anaemia is associated with increased morbidity and mortality in surgical patients. While iron deficiency is a well-recognized cause, the contribution of other nutritional deficiencies, such as folate and vitamin B12, remains underexplored. Therefore, this study aimed to assess the prevalence and role of folate and vitamin B12 deficiencies in preoperative anaemia among patients undergoing major surgery. METHODS: This retrospective observational study included 410 patients aged ≥18 years who underwent major surgery and were evaluated at a preoperative anaemia clinic i

AIMS: To investigate whether localization of beta-catenin is helpful in differentiating primary ovarian mucinous carcinoma and colorectal adenocarcinoma metastatic to the ovary. Extra-ovarian cancers which metastasize to the ovaries, especially from colorectal adenocarcinoma, frequently mimic primary ovarian carcinomas, particularly endometrioid and mucinous types. Distinguishing primary ovarian carcinoma from metastatic colorectal carcinoma is important for both therapeutic and prognostic reasons. Even after thorough histological examination, metastatic colorectal adenocarcinomas are still of

Evaluation of Anemia in the Older Population in a Tertiary Care Hospital.

Background The prevalence and severity of anemia among older people are significant health concerns. Hence, the initial medical service provider for older people with any symptoms or signs admitted under any department should be aware of the blood hemoglobin status and if anemia is present and its severity at the very outset, rather than later. This can help treat the presenting problem, along with the workup and management of anemia at the earliest. Hence, we thought of assessing the severity of anemia in the older age group in the eastern coastal belt of India. Methods A prospective observat

Assessment of Micronutrient Deficiencies in Exclusively Breastfed Infants: A Cross-Sectional Study.

Background/Objectives: Micronutrient deficiencies during infancy remain a major public health concern, particularly in developing countries. Although exclusive breastfeeding is the optimal source of nutrition for infants up to six months of age, limited studies have simultaneously evaluated multiple micronutrient statuses in this population. This study aimed to assess the levels of vitamin D, iron, and other micronutrients-including vitamins A, E, B12, folic acid, zinc, and copper-in exclusively breastfed infants. Methods: This cross-sectional study was conducted between 2022 and 2024 at a uni

Severe Normocytic Anemia and Spontaneous Hematomas in Profound Vitamin C Deficiency.

Scurvy is considered rare in developed countries but can occur in patients with a poor diet that lacks vitamin C. Risk factors include, but are not limited to, eating disorders such as anorexia nervosa, social isolation and dietary habits, and limited access to vegetables and produce due to low socioeconomic status. We describe a case of a middle-aged man who presented with bilateral thigh pain and inability to stand up or walk, skin rash, and swelling in his ankles. Initially, he thought he "pinched a nerve" during his daily exercise and that this caused his discomfort in his thighs. He was f

Intramuscular Vitamin B12 Treatment in Transcobalamin II Deficiency: Case Series Clinical Outcomes.

BACKGROUND: Transcobalamin II (TC II) deficiency is a rare autosomal recessive disorder that typically manifests in early infancy. Symptoms include failure to thrive, vomiting, weakness, and pancytopenia. If left undiagnosed and untreated, it can be life-threatening. TC II is crucial for transporting cobalamin (vitamin B12), which plays a vital role in homocysteine and methylmalonic acid metabolism. It serves as a cofactor in neurotransmitter synthesis and protein methylation processes. METHODS: In this study, we reviewed the clinical presentation, treatment approaches, and long-term outcomes

PRAYAS: individual patient data meta-analysis database for Pooled Research and Analysis for Yielding Anemia-free Solutions in India.

PURPOSE: The PRAYAS Individual Patient Data Meta-analysis (IPD-MA) database aims to estimate the prevalence of anemia among children under 18 years, non-pregnant and non-lactating (NPNL) women, and pregnant women (by trimester), with further stratification by age group, year, and region of India. Beyond prevalence, it seeks to address the etiological contribution of iron and other erythropoietic micronutrient deficiencies and to evaluate the effectiveness of anemia prevention and treatment interventions, including factors associated with non-response. This will directly support India's "test-t

Hematological Correlations as Predictors of Disease Manifestations in Psychiatric Inpatients.

Background/Objective. Psychiatric disorders exhibit significant symptomatic and etiopathological heterogeneity, complicating diagnosis and treatment. Hematological parameters may serve as indicators of overall health and predictors of psychiatric symptom manifestation and remission, particularly in long-term hospitalized patients. This study evaluated hematological and biochemical markers, including vitamin B12, vitamin D, and glucose levels, to explore their potential role in psychiatric disorders and disease progression. Methods. This prospective observational study was conducted from 1 Janu

Imerslund-Gräsbeck syndrome presenting with a 12-year history of intermittent proteinuria and anemia: a case from the Middle East.

BACKGROUND: Imerslund-Gräsbeck Syndrome (IGS) is a rare autosomal recessive disorder characterized by selective cobalamin (vitamin B12) malabsorption and often accompanied by proteinuria. Mutations in CUBN or AMN genes underlie the condition, which usually manifests in childhood with megaloblastic anemia, failure to thrive, or recurrent infections. CASE PRESENTATION: We report a 15-year-old Iranian boy with a 12-year history of recurrent anemia and intermittent proteinuria. He presented with fatigue, nausea, and anorexia. Laboratory evaluation showed pancytopenia, macrocytic anemia (Hb 7.5 g/

Iron Deficiency Anemia and Oxidative Stress in Type 2 Diabetic Patients on Metformin: A Meta-Analysis.

Iron deficiency anemia (IDA), one of the most prevalent nutritional disorders globally, is increasingly recognized among patients with type 2 diabetes mellitus (T2DM). Metformin, the first-line antidiabetic drug, is widely used in T2DM management and known for its favorable metabolic effects. However, growing evidence suggests that long-term metformin use may contribute to micronutrient deficiencies, including vitamin B12 and potentially iron, which could impair hematological status and exacerbate oxidative stress in diabetic individuals. Oxidative stress, characterized by an imbalance between

Unmasking Pernicious Anemia: A Reversible Cause of Pancytopenia Due to Severe Vitamin B12 Deficiency.

Vitamin B12 (cobalamin) is a vital cofactor in DNA synthesis and hematopoiesis, and its deficiency can lead to a wide spectrum of clinical manifestations, ranging from asymptomatic macrocytosis to severe hematologic and neurologic complications. While megaloblastic anemia is the most common presentation, prolonged or profound deficiency can result in pancytopenia due to ineffective hematopoiesis and intramedullary cell death. This can mimic more serious bone marrow disorders, including leukemia or aplastic anemia, potentially leading to invasive diagnostic procedures if the deficiency is not p

The Autoimmune Gastritis Puzzle: Emerging Cellular Crosstalk and Molecular Pathways Driving Parietal Cell Loss and ECL Cell Hyperplasia.

Autoimmune gastritis (AIG) is a chronic, organ-specific autoimmune disease characterized by progressive destruction of gastric parietal cells driven by autoreactive CD4+ T-cells, epithelial stress pathways, and microbial factors. Parietal cell loss results in achlorhydria, intrinsic factor deficiency, and vitamin B12 malabsorption, ultimately leading to pernicious anemia. Compensatory hypergastrinemia promotes enterochromaffin-like (ECL) cell hyperplasia and contributes to the development of type 1 gastric neuroendocrine neoplasms (gNENs). These clinical consequences are well recognized, yet t

Dibothriocephalus nihonkaiensis infection accompanied with vitamin B12 deficiency: A case report.

Diphyllobothriosis is a tapeworm infection and is associated with the consumption of raw or undercooked fish. Among causative pathogens of human tapeworm infection, Dibothriocephalus nihonkaiensis is the second most prevalent species, contributing to over 2000 cases of human infection annually. Although D. nihonkaiensis is primarily distributed in the northern Pacific region, several cases have recently been reported from other parts of the world, including the United States. As there is a concern regarding the further spread of D. nihonkaiensis infection, accurate diagnosis and understanding

Plummer-Vinson syndrome in a postpolio patient: a rare intersection of neuromuscular and nutritional sequelae.

A woman in her 40s, with a history of childhood paralytic poliomyelitis, presented with progressive difficulty swallowing, fatigue and unintended weight loss. Due to her limited mobility and chronic tiredness, the diet gradually became restricted, lacking iron, vitamin B12, calcium and protein. She frequently skipped meals and avoided nutrient-dense foods. Clinical signs pointed to multinutrient deficiency, and laboratory results confirmed iron deficiency anaemia along with other deficiencies. Upper gastrointestinal endoscopy revealed a postcricoid oesophageal web, consistent with Plummer-Vins

Comparison of diagnostic algorithms used in guidelines on nutritional anaemias in adults.

BACKGROUND: Anaemia is highly prevalent and commonly caused by nutritional deficiencies. Guidelines often include an algorithm to find the underlying cause. Here, we compared diagnostic algorithms and suggested laboratory tests for anaemia in clinical practice guidelines in countries with similar healthcare systems, focusing on iron, vitamin B12 and folate deficiency in the general adult population. METHODS: We searched for diagnostic guidelines on anaemia in the Trip, Guidelines International Network and country specific databases. To be selected, the guidelines had to include diagnostic cri

Vitamin B12 Deficiency Disguised As Hemolytic Anemia: A Case Presentation.

This case report describes an 18-month-old male presenting with hemolytic anemia and lethargy, who was ultimately diagnosed with severe vitamin B12 deficiency. The child exhibited global developmental delays, including a lack of speech and walking skills. Initially suspected as intravascular hemolytic anemia, the normal reticulocyte count led to further investigation, which revealed low cobalamin (vitamin B12) levels. The patient received vitamin B12 injections, resulting in normalized cobalamin levels. Additional evaluations ruled out metabolic disorders and other etiologies for his anemia. F

Methylcobalamin Deficiency Presenting as Thalamic Syndrome in the Elderly: Association or Chance?

Vitamin deficiency is common in the geriatric population and is responsible for majorly imbalanced hematological, neurological, and neuropsychiatric functioning. Methylcobalamin deficiency or vitamin B12 deficiency can be underestimated in some cases and be misdiagnosed as other illnesses, such as thalamic syndrome. Timely diagnosis of this deficiency is essential, especially in the geriatric population, as it might cause irreversible structural brain damage. This is also presented as elevated levels of homocysteine and methylmalonic acid. Clinically, it presents with the following symptoms: l
