In the United States, sickle cell disease (SCD) is a rare inherited hemoglobinopathy affecting about 100,000 individuals, mostly with African ancestry. SCD causes damage to multiple organ systems and SCD nephropathy (SCDN) is a common complication associated with early mortality. We previously performed a genome-wide association study (GWAS) for SCDN and identified a modest number of genome-wide significant loci. Here, we leveraged the ancestral composition of participants from two well-characterized adult SCD cohorts to boost statistical power and perform a local ancestry-aware GWAS for estim

PURPOSE: Small cell lung cancer (SCLC) remains one of the most aggressive malignancies, with limited treatment options beyond frontline chemo-immunotherapy. Izalontamab brengitecan (iza-bren, BL-B01D1) is a first-in-class bispecific antibody-drug conjugate cotargeting epidermal growth factor receptor (EGFR) and human epidermal growth factor receptor 3 (HER3). We expanded the SCLC cohort to further evaluate the efficacy and safety of iza-bren in patients with extensive-stage SCLC (ES-SCLC).
METHODS: This open-label, multicenter, dose-expansion phase Ib study (ClinicalTrials.gov identifier: NCT

Hemolytic anemia is a rare but potentially life-threatening condition in the emergency department (ED) that requires timely recognition and early intervention. Hemolytic anemia results from either the intravascular or extravascular breakdown of red blood cells. The most common causes of secondary hemolytic anemia include lymphoproliferative disorders, autoimmune disorders, and infections. This case will demonstrate how an urgent diagnosis of hemolytic anemia can lead to the underlying cause: diffuse large B-cell lymphoma. This case illustrates how laboratory evaluation, early subspecialty invo

Glucose-6-phosphate dehydrogenase (G6PD) deficiency, caused by pathogenic variants in the G6PD gene, is the most common X-linked enzymopathy in the world. The clinical manifestations range from drug-induced hemolytic anemia to neonatal hyperbilirubinemia and chronic hemolysis. The G6PD status needs to be accurately assessed in several clinical settings, including drug prescription, assisted reproductive technologies, and the diagnostic workflow of hemolytic anemia. In this study, the contribution of molecular analysis in the diagnostic workflow of patients with suspected G6PD deficiency was re

Human genetic studies have identified key regulators of fetal haemoglobin (HbF) expression, including BCL11A, resulting in therapeutic advances1-8. Yet the mechanisms by which HbF expression is activated remain incompletely understood9. Here we conduct a large multi-ancestry genome-wide association study of HbF levels in 28,279 individuals that identifies 91 conditionally independent associations across 12 genomic regions. In one previously uncharacterized associated region, the high-HbF-linked causal variant rs1010474-C reduces BACH2 expression and elevates HbF levels. Direct perturbation or

Sickle cell disease (SCD) is one of the most common inherited haemoglobin disorders worldwide and is associated with substantial morbidity, premature mortality and lifelong healthcare utilisation. Red blood cell (RBC) transfusion remains a cornerstone of SCD management and is indispensable for the treatment and prevention of life-threatening complications, including acute chest syndrome, ischaemic stroke, severe anaemia and perioperative complications. Despite its undeniable therapeutic benefits, repeated transfusions are accompanied by important adverse consequences, including alloimmunisatio

BACKGROUND: Luspatercept is a first-in-class erythroid maturation agent that reduces transfusion burden in adults with transfusion-dependent beta-thalassaemia (TDT). However, its cost-effectiveness has not been evaluated in middle-income settings. This study aimed to evaluate the cost-effectiveness and budget impact of Luspatercept for TDT in Thai adults.
METHODS: A hybrid decision tree and lifetime Markov cohort model was developed to compare Luspatercept plus best supportive care (BSC) with BSC alone from a societal perspective. Age-specific 12-week transition probabilities across three tra

Drug-induced immune hemolytic anemia (DIIHA) is a rare but potentially life-threatening adverse drug reaction. Although more than 150 drugs have been reported to induce DIIHA, vancomycin, a glycopeptide antibiotic widely used in clinical practice, is rarely associated with hemolysis, making it prone to underdiagnosis or misdiagnosis. This article reports a case of vancomycin-induced DIIHA in a 64-year-old female patient after right revision knee arthroplasty (RKA). This study investigated the serological tests associated with vancomycin and tracks drug antibody titers through longitudinal moni

OBJECTIVE: Glycosylphosphatidylinositol (GPI)-anchored proteins play critical roles in nervous system function. Pathogenic variants in genes involved in GPI-anchor biosynthesis cause early-onset multisystem disorders known as inherited GPI deficiencies. We describe a novel neuromuscular phenotype associated with PIGB deficiency.
METHODS: Patients with neuromuscular disease carrying PIGB variants were identified, and clinical data collected. GPI-anchored protein and free-GPI expression were assessed by flow cytometry and variants validated in a PIGB knockout cellular model.
RESULTS: Biallelic

Severe inflammatory lung disease, including acute respiratory distress syndrome (ARDS), is life-threatening with diverse causes and few targeted therapies. Inborn errors of immunity (IEIs) underlie a subset of cases. We describe a novel IEI caused by biallelic loss-of-function variants in PTPN6, encoding the immunoregulatory phosphatase SHP1. Seven children from five families developed early-onset anemia and severe inflammatory lung disease, at times presenting as ARDS. The causative variants were shown to destabilize SHP1 and abolish its phosphatase activity. These findings expand the genetic

Hereditary spherocytosis (HS) is caused by inherited defects in red blood cell (RBC) membrane and membrane-skeletal proteins, but the metabolic features associated with genotype, splenic selection, RBC injury, and clinical burden remain incompletely resolved. We performed RBC metabolomics in the VASCUSPHERO cohort, including 16 controls and 22 HS samples, and integrated omics data with splenectomy status, RBC deformability, microparticles, phosphatidylserine exposure, reactive oxygen species, calcium, CD47, complete blood count traits, and leukocyte subsets. The HS cohort included 12 spectrin

BACKGROUND: Pregnancy in women living with sickle cell disease (SCD) is associated with increased risk of morbidity and mortality for mother and baby. Outside of pregnancy, serial prophylactic exchange blood transfusion (SPEBT) has proven efficacy as a treatment for acute SCD complications, however, there is inadequate evidence for the safety and benefit for SCD during pregnancy.
AIM: To explore health professionals and women's view on the acceptability and feasibility of a randomised control trial of SPEBT versus usual care in pregnant women with SCD (Transfusion Antenatally in Pregnant wome

Pain is a debilitating complication of sickle cell disease (SCD), driven by recurrent vaso-occlusive crises and chronic pain syndromes involving central sensitization and neuropathic mechanisms. Although opioid medications remain first-line therapy for acute pain, there are limitations that highlight the need for effective adjunctive approaches. Available data support non-opioid pharmacologic therapies including acetaminophen and nonsteroidal anti-inflammatory drugs as components of multimodal analgesia, with emerging evidence for therapies targeting neuropathic and centralized pain. BACKGROUND: Hereditary elliptocytosis (HE) is typically a mild or asymptomatic red blood cell membrane disorder characterized predominantly by elliptocytes, whereas hereditary pyropoikilocytosis (HPP) represents the severe end of the HE spectrum, with early-onset hemolysis, marked poikilocytosis, red cell fragmentation, and pronounced membrane instability. We report a Cambodian girl with a severe HPP/HE-like phenotype associated with a heterozygous SPTA1 variant of uncertain significance, emphasizing the importance of integrating clinical, hematologic, morphologic, and molecular findings to su

INTRODUCTION: While genetic abnormalities underlying lymphoid malignancies (LM) are well-characterized, the environmental and metabolic determinants driving LM remain largely unexplored. Autoimmune diseases are known risks, yet the specific circulating mediators and potential protective dietary factors linking these conditions to LM require systematic elucidation.
OBJECTIVES: This study aimed to comprehensively screen for causal risk factors of LM, identify the metabolic mediators bridging autoimmune diseases and LM, and validate the therapeutic potential of leucine as a protective factor.

Sickle cell disease (SCD) is a severe heritable hemoglobinopathy characterized by chronic hemolysis, episodic vaso-occlusion, and progressive multi-organ damage. Although hydroxyurea remains the most established disease-modifying therapy and that newer approaches such as L-glutamine, voxelotor, crizanlizumab, and gene therapy have expanded the therapeutic landscape, important limitations remain. These include incomplete efficacy, recent clinical or regulatory setbacks, as well as high cost and access barriers. Thus, safe, accessible, and mechanistically targeted treatments remain urgently need

Diabetes is a chronic condition characterised by hyperglycaemia due to impaired insulin secretion, insulin action, or both. Oxidative stress reduces insulin sensitivity and contributes to complications. Glucose-6-phosphate dehydrogenase (G6PD) deficiency, the most common enzymatic disorder of erythrocytes, impairs NADPH production and reduces the ability to counter oxidative stress. Given the involvement of redox imbalance in diabetes pathogenesis, the role of G6PD deficiency in diabetes development and progression remains unclear. Two reviewers independently searched PubMed, Cochrane Library,

Metastasis-directed external beam radiation therapy (EBRT) delivered for limited-volume disease, integrated with systemic radioligand therapy, may improve long-term disease control across the spectrum of metastatic prostate cancer. We examined 88 patients with metastatic prostate cancer who received both lutetium-177 PSMA-617 (LuPSMA) radiopharmaceutical therapy and metastasis-directed external beam radiation therapy (EBRT) between June 2022 and August 2025. EBRT was delivered to a median of three metastatic sites, most commonly bone (69.3 %) and lymph nodes (64.8 %). The PSA50 response rate t
