Circulating erythropoietin concentration associates with thromboembolism in sickle cell disease.

Elevated erythropoietin (EPO) concentration associates with thrombotic risk in hypoxic conditions, hereditary erythrocytosis and treatment of anaemia with recombinant EPO. We evaluated sickle cell disease (SCD) patients from the University of Illinois at Chicago (UIC) and the Treatment of Pulmonary Hypertension and SCD with Sildenafil Therapy (Walk-PHaSST) study and found that higher serum EPO concentration associated with increased thromboembolic risk (combined odds ratio [OR] = 1.9, p = 0.0029, N = 557). Percent haemoglobin F and haemoglobin concentration strongly correlated with EPO concent

Causal association between gastroesophageal reflux disease and anemia: Mendelian randomization analysis.

BACKGROUND: Previous observational studies have indicated a possible link between gastroesophageal reflux disease (GERD) and anemia, but the causal relationships and specific anemia subtypes involved remain unclear. METHODS: To explore the causal relationship between GERD and anemia risk, we conducted a two-sample Mendelian randomization study using the inverse-variance weighting (IVW) analytical method. After stringent selection, 75 and 76 independent single nucleotide polymorphisms were used as instrumental variables for GERD in analyses of anemia and iron-deficiency anemia (IDA), respectiv

Uridine-responsive epileptic encephalopathy: Precision treatment across the age spectrum - a case series.

INTRODUCTION: CAD deficiency (Type 50 early infantile epileptic encephalopathy) is a rare, yet treatable neurometabolic disorder characterised by refractory seizures, developmental delay, and dyserythropoietic anaemia. Timely recognition and treatment with uridine can dramatically alter the clinical course but remains challenging due to phenotypic variability and diagnostic limitations. CASE PRESENTATIONS: We describe five patients with biallelic pathogenic CAD variants, presenting across the age spectrum from neonatal period to adulthood. Common features included pharmacoresistant epilepsy,

Identification of breakpoint regions and single nucleotide variations of RHD hybrid alleles by long-read sequencing.

BACKGROUND AND OBJECTIVES: RHD*03N.01 (RHD*DIIIa-CEVS(4-7)-D) and RHD*01N.06(RHD*D-CEVS(4-7)-D) are hybrid alleles that encode a partial C antigen, resulting in increased risk of C alloimmunization in individuals without a conventional C antigen. As detection of the hybrid alleles remains challenging, new markers can aid accurate identification, ensuring provision of C-negative blood to individuals at risk. MATERIALS AND METHODS: To identify breakpoint regions and distinct intronic single nucleotide variations (SNVs) of the hybrid alleles, Pacific Bioscience (PacBio) long-read sequencing was

Impact of SLCO1B1 Gene Polymorphisms on Vadadustat Pharmacokinetics.

BACKGROUND AND OBJECTIVE: Vadadustat, an oral hypoxia-inducible factor prolyl hydroxylase inhibitor, is transported into hepatocytes via OATP1B1, which is encoded by the SLCO1B1 gene. Although interindividual variability in vadadustat pharmacokinetics (PK) has been reported, the influence of SLCO1B1 polymorphisms and the clinical relevance of coproporphyrin-I (CP-I) as a biomarker of OATP1B1 activity on vadadustat exposure remain unclear in Japanese patients with chronic kidney disease-related anemia. This study evaluated the effects of SLCO1B1 polymorphisms on vadadustat plasma concentration

Integrative analysis of single nucleotide polymorphism of PDE7B (rs2327669) with biopsychosocial and biochemical parameters in β-thalassemia major patients in Pakistani Cohort.

BACKGROUND: β-Thalassemia major typically manifests between six months and two years of age. It is characterized by chronic anemia, and a range of multisystemic complications lead to increased morbidity and disease burden. It has a significant influence on patients' mental and social health, making a biopsychosocial approach necessary for all encompassing therapy. This study examines how biopsychosocial, biochemical variables and genetic variants, namely the PDE7B gene SNP rs2327669, is associated with β-thalassemia major. METHODOLOGY: Analysis of 400 patients and 400 controls, ages 2 to 18 y

Cryo-EM structures of human FANCJ reveal the mechanism of G-quadruplex unwinding and disease-associated mutations.

Guanine-rich nucleic acid sequences can fold into G-quadruplex (G4) structures that regulate DNA replication, transcription, and translation. Fanconi anemia group J helicase (FANCJ) resolves G4 structures at stalled replication forks. Despite its central role in genome maintenance, the molecular basis of G4 recognition and unwinding by FANCJ has remained unclear. Here, we report cryo-EM structures of human FANCJ bound to a G4-containing DNA substrate and ATPγS. The structures reveal direct engagement of the G4 by the Fe-S domain. Structure-guided mutagenesis demonstrates that this interface is

Evaluating mitapivat for the treatment of alpha or beta thalassemia.

INTRODUCTION: Thalassemia is a group of diverse genetic disorders with worldwide distribution that affects hemoglobin synthesis. Until recently, the therapeutic approach to thalassemia was symptomatic, relying on red blood cell transfusions, treatment of comorbidities and of disease-related complications. However, novel therapeutic agents have recently been developed and are gradually being integrated into routine clinical practice. AREAS COVERED: One of the most promising agents is mitapivat (AG348), an oral pyruvate kinase activator that enhances the erythrocytic adenosine triphosphate (ATP

Inflammatory Cytokine Dysregulation and Interleukin Gene Polymorphisms in Iraqi Pediatric Sickle Cell Anemia: Implications for Genetic Risk Stratification and Targeted Therapy.

BACKGROUND: Sickle cell anemia (SCA) involves inflammation that worsens disease symptoms and complications. The inflammatory profiles and genetic determinants among the pediatric Iraqi population are not well described. The current study aimed to analyze pediatric SCA in Iraq for cytokine dysregulation and interleukin gene polymorphism. PROCEDURE: A case-control study was performed involving 216 pediatric SCA patients and 330 healthy controls. Serum levels of six interleukins (IL-1β, IL-2, IL-4, IL-8, IL-10, and IL-17) were quantified using the ELISA technique; the gene polymorphisms for the

Oligonucleotide Synthesis Errors Are a Source of Untoward Variation in HDR-Mediated Gene Editing.

Background/Objectives: Single-stranded oligonucleotides (ssODNs) are used as donor templates for therapeutic gene editing by CRISPR-Cas9 cleavage and homology-directed repair (HDR). Although ssODN sequence fidelity is critical to the safety and efficacy of editing, standard quality control methods cannot resolve individual nucleotide errors. Methods: We performed deep sequencing of ssODNs from three manufacturers and amplicons from edited hematopoietic stem/progenitor cells. Results: We find that synthesis errors are present in all ssODNs tested at rates that vary more than two-fold among manu

NAD+ precursor treatment prevents cardiomyopathy but disrupts erythroid maturation in mitochondrial progeria.

Nicotinamide adenine dinucleotide (NAD+) plays a central role in energy metabolism, and its decline is linked to various degenerative diseases. While NAD+ restoration holds therapeutic promise, its long term, tissue-specific consequences remain poorly understood. We investigated effects of nicotinamide riboside (NR) supplementation for "mutator" mice manifesting mitochondrial progeria. Our results reveal strikingly divergent outcomes: in proliferative bone marrow, NR-treated mutators show reductive stress with accumulation of NADH/NADPH, altered amino acid, nucleotide, folate levels, and impai

Clinical insights into Candidatus Mycoplasma haemominutum-associated anaemia in naturally infected cats: flea-associated risk and paradoxical clinicopathological findings.

BACKGROUND: The specific pathophysiological drivers and host-pathogen-environment factors that dictate the pathogenicity of Candidatus Mycoplasma haemominutum (CMhm) remain poorly defined, hindering accurate clinical interpretation and effective disease control. This study provides detailed mechanistic insights into CMhm-associated anaemia in naturally infected cats by integrating analyses of concurrent diseases and comprehensive clinicopathological panels, alongside molecular characterisation in an under-represented region. Two groups of CMhm-infected cats (anaemic, n = 13; non-anaemic, n = 2

Severe rhabdomyolysis as a likely isolated presentation of glucose-6-phosphate dehydrogenase (G6PD) deficiency.

An otherwise healthy young adult male with known glucose-6-phosphate dehydrogenase (G6PD) deficiency presented with severe rhabdomyolysis following routine exercise in the absence of haemolytic anaemia. While G6PD deficiency classically presents with oxidative stress-induced haemolysis, usually reported in the context of infection, medications or toxins, rhabdomyolysis is a rare and atypical manifestation. In this case, no clear precipitating factor for rhabdomyolysis was identified, and the patient did not develop haemolysis. Work up ruled out typical causes of acquired rhabdomyolysis. This p

Glucose-6-phosphate dehydrogenase deficiency is associated with improved survival in patients with acute myeloid leukemia treated with venetoclax and azacitidine.

BACKGROUND: Glucose-6-phosphate dehydrogenase (G6PD) deficiency impairs cellular redox balance through reduced NADPH production and is the most common enzymatic disorder-causing anemia. Venetoclax combined with azacitidine (Ven-Aza) targets leukemic stem cells by disrupting oxidative phosphorylation and inducing mitochondrial stress. This study hypothesized that G6PD deficiency may enhance the efficacy of Ven-Aza in acute myeloid leukemia (AML) by reducing leukemic cell metabolic resilience. METHODS: The authors studied 73 consecutive patients with newly diagnosed (ND) AML treated with Ven-Az

HbA1c underperforms in identifying abnormal glucose tolerance in the presence of G6PD deficiency: Insight from the Africans in America study.

G6PD deficiency (G6PD-D) variants are associated with lower hemoglobin A1c (HbA1c) concentrations, raising concerns about the diagnostic efficacy of HbA1c for abnormal glucose tolerance (Abnl-GT) in Africans, in whom risk of G6PD-D and Abnl-GT is high. G6PD-D is assessed using genotyping or an enzymatic assay, but because G6PD-D is X-linked, the enzymatic assay is necessary for determining status for women heterozygous for deficiency variants. We assessed: 1) ability of HbA1c to detect Abnl-GT by G6PD-D; 2) concordance of genotyping and enzymatic assay for G6PD-D in sub-Saharan Africans living

Ethnic Variation in G6PD Deficiency: Epidemiology and Mutation Spectrum in Southern China's Multiethnic Hub, Nanning.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency exhibits high prevalence in malaria-endemic regions and areas along the historic Maritime Silk Road. This study investigated the burden and genetic architecture of G6PD deficiency in Nanning, the ethnically diverse capital of Guangxi in southern China, where Southeast Asian gene flow has shaped population genetics. We screened 14 403 individuals for G6PD activity at The First Affiliated Hospital of Guangxi Medical University, defining deficiency as activity < 60% of adjusted male median (AMM = 2045 U/L). Among 2513 deficient cases (prevalence

Newborn screening in an Eastern Indian province: prevalence and trends from a burgeoning newborn screening unit.

Neonatal screening has not been demanded by pediatricians nor parents in India because of limited resources, inadequate health information, early hospital discharges, a large rate of deliveries at home, and challenges with follow-up of results. This study was conducted based on the laboratory e-records from the Newborn Screening Unit, where Dried Blood Spot-based analysis was performed for G6PD deficiency, congenital adrenal hyperplasia, neonatal hypothyroidism, biotinidase deficiency, and galactosemia. The total study population comprised 3210 neonates. Overall, 329 children (10.2%) had some
