The expanding therapeutic landscape of β-thalassaemia.

β-thalassaemia is characterised by ineffective erythropoiesis, chronic anaemia, haemolysis, and progressive iron-related and non-iron-related morbidity. Although transfusion, iron chelation, multidisciplinary care, and allogeneic haematopoietic stem-cell transplantation have substantially improved outcomes, major gaps persist in treatment burden, access, safety, adherence, and curative feasibility. Over the past decade, the therapeutic landscape has expanded rapidly, with disease-modifying agents targeting erythroid maturation and red cell metabolism, including luspatercept and mitapivat, now

Cost-effectiveness and budget impact of Luspatercept in Thai adults with transfusion-dependent beta-thalassaemia: a multi-state Markov model based on national claims data.

BACKGROUND: Luspatercept is a first-in-class erythroid maturation agent that reduces transfusion burden in adults with transfusion-dependent beta-thalassaemia (TDT). However, its cost-effectiveness has not been evaluated in middle-income settings. This study aimed to evaluate the cost-effectiveness and budget impact of Luspatercept for TDT in Thai adults. METHODS: A hybrid decision tree and lifetime Markov cohort model was developed to compare Luspatercept plus best supportive care (BSC) with BSC alone from a societal perspective. Age-specific 12-week transition probabilities across three tra

Bandgap engineering of tantalum nitride thin films by ion‑assisted magnetron sputtering for advanced electronic and radiation detection applications.

INTRODUCTION: Across the globe, children born with perinatal HIV infection (PHIV) are reaching adolescence and young adulthood in large numbers. The majority of research has focused on biomedical outcomes yet there is increasing awareness that long-term survivors with PHIV are at high risk for mental health problems, given genetic, biomedical, familial and environmental risk. This article presents a review of the literature on the mental health functioning of perinatally HIV-infected (PHIV+) adolescents, corresponding risk and protective factors, treatment modalities and critical needs for fut

Management of transfusion-dependent β-thalassaemia in the era of novel therapies: a prioritisation-based matrix for settings with limited resources.

β-thalassaemia is an inherited haemoglobinopathy characterised by ineffective erythropoiesis and chronic anaemia of varying severity, which is predominant in the region extending from the Mediterranean basin and Middle East towards southeast Asia. Patients with severe phenotypes require lifelong transfusions, iron overload monitoring, and chelation. Suboptimal management due to access challenges continues to be directly linked to increased morbidity and mortality in many regions. In the past few decades, an improved understanding of the underlying pathogenesis of β-thalassaemia has led to the

[Beta thalassaemia: a surprising radiological image].

The aetiology of thalassaemia lies in a defect in the synthesis of 1 of the 2 chains, alpha or beta, of the haemoglobin molecule. This failure primarily results in haemolytic anaemia, which is the main clinical feature of the disorder. As a consequence of this haemolysis, erythropoiesis is stimulated, which in turn leads to morphological changes in the bone and hypertrophy of other organs. Radiographic findings reveal a decrease in cortical thickness accompanied by trabecular reduction in the bone, including the facial bones. Commonly, no local treatment is required in the field of maxillofaci

Impact of gender and chelation on the ferritin-liver function test relationship in transfusion-dependent β-thalassaemia.

OBJECTIVES: Transfusion-dependent β-thalassaemia (TDT) requires lifelong blood transfusions, resulting in progressive iron overload and liver injury. Serum ferritin is a practical surrogate for iron burden, while liver function tests (LFTs) may reflect hepatic damage. However, the influence of gender and the chelation regimen on the ferritin-LFT relationship remains underexplored. This study evaluated these associations in a large Iraqi TDT cohort. METHODS: A retrospective cross-sectional study was conducted on 323 patients with TDT at the Center of Hereditary Blood Diseases, Karbala, Iraq. D

Altered cerebral morphometry and individual-based morphological brain network in children with beta-thalassaemia major.

Beta-thalassemia major (TM) is a severe genetic blood disorder that frequently leads to cognitive impairments in pediatric patients, yet its neurological impact remains insufficiently explored. This study investigates alterations in cerebral gray matter morphology and brain network topology in children with TM and their associations with cognitive performance. High-resolution brain MRI data were processed using FreeSurfer to extract cortical morphological features, from which individual-based Morphological Brain Networks (MBNs) were constructed based on vertex-wise similarity across gray matte

Adult survivors of sickle cell disease, transfusion-dependent beta-thalassaemia and childhood acute leukaemia in England: protocol for a mixed methods data linkage and health-related quality of life survey study.

INTRODUCTION: Recent advances in treatment and care have improved survival rates for children and young adults with severe blood disorders such as sickle cell disease (SCD), transfusion-dependent beta-thalassaemia (TDT) and acute leukaemia. However, their quality of life and reproductive and psychosocial outcomes are not yet well studied. For SCD and TDT, robust survival data are mainly limited to North America. Thus, there is a need to fill these knowledge gaps to guide improvements in care, address unmet clinical needs and rigorously assess the efficacy of emerging novel therapies. METHODS

Extracellular Vesicle-Mediated U1 snRNA Delivery Restores Aberrant Pre-mRNA Splicing in Human Cells.

Splicing defects represent a significant class of human genetic disorders, yet strategies to directly correct aberrant splice-site recognition remain limited. The small nuclear RNA (snRNA) U1 plays a critical role in pre-messenger RNA splicing by base-pairing with the conserved 5' splice-site 'GU' dinucleotide. Disruption of this interaction can lead to abnormal splicing or frameshift mutations, contributing to disease pathology. Extracellular vesicles (EVs) can transport small molecules to cells for therapeutic applications. Here, U1 snRNA-overexpressing HEK293T cells were used to generate ap

A novel HBD gene mutation associated with normal-range hb A2 in β-thalassemia carriers.

β-thalassemia is one of the most common single-gene inherited conditions in the world, prevalence of β-thalassaemia in south China is 3–4%,increased Hb A2 level is one of the most important markers of β-thalassemia heterozygous carriers.Interaction of HBD gene defect with β-thalassemia can result in the normal Hb A2 β-thalassemia, potentially leading to a misdiagnosis of β- thalassemia carrier state. This study aimed to identify a novel mutation in HBD gene resulted in normal Hb A2 levels in β-thalassemia carriers, and explore the underlying mechanism of the novel HBD gene mutation using a min

Evaluation of Liver Iron Accumulation With Liver Elastography and Apparent Diffusion Coefficient in Children With Thalassaemia Major.

BACKGROUND: Beta Thalassaemia Major is a hereditary disorder characterised by defective protein synthesis in the beta-globin chain, leading to chronic anaemia. As a consequence of repeated blood transfusions used to manage anaemia, excessive iron accumulation occurs, primarily in the liver and other vital organs, leading to organ damage. PURPOSE: To determine the correlation between liver iron concentration and iron accumulation in the liver due to blood transfusions in paediatric patients diagnosed with Beta Thalassaemia Major, using shear wave elastography and liver magnetic resonance imagi

CCND3 Suppression Ameliorates β-Thalassaemia in a Murine Disease Model: A Potential Therapeutic Strategy.

β-thalassaemia (β-thal) is part of a group of diseases, the β-hemoglobinopathies, affecting the levels or functionality of the β-globin subunit of hemoglobin, which are the most widespread monogenic diseases throughout the world. The severity of β-thal is determined by different genetic factors, but in the gravest form, affected patients are constrained to a program of blood transfusion and iron chelation regimens for their entire life. Although definitive cures, such as bone marrow transplantation or gene therapy, are now available, they are still far from being applied worldwide. Therefore,
