Extracellular vesicles modulate endothelial nitric oxide production in patients with β‑thalassaemia/HbE.

Thrombosis is a significant complication in patients with β-thalassaemia/haemoglobin E (HbE), particularly in splenectomised patients. The endothelium is a key regulator of vascular haemostasis and homeostasis, through the secretion of various regulatory molecules. Nitric oxide (NO), produced by endothelial cells (ECs), regulates vascular functions by acting as a potent vasodilator and an inhibitor of platelet activation. Decreased NO bioavailability, a marker of vascular dysfunction, could be a contributing factor leading to thrombosis. Microparticles or medium extracellular vesicles (mEVs) a

Advancing CRISPR genome editing into gene therapy clinical trials: progress and future prospects.

Genome editing has recently evolved from a theoretical concept to a powerful and versatile set of tools. The discovery and implementation of CRISPR-Cas9 technology have propelled the field further into a new era. This RNA-guided system allows for specific modification of target genes, offering high accuracy and efficiency. Encouraging results are being announced in clinical trials employed in conditions like sickle cell disease (SCD) and transfusion-dependent beta-thalassaemia (TDT). The path finally led the way to the recent FDA approval of the first gene therapy drug utilising the CRISPR/Cas

Acute splenomegaly, haemolysis, and thrombocytopenia following luspatercept initiation in a patient with transfusion-dependent thalassaemia: A case report.

Patients with transfusion-dependent thalassaemia (TDT) require chronic, life-sustaining red blood cell transfusions, which contribute to iron overload and associated morbidity and mortality. Luspatercept, an inhibitor of the transforming growth factor-beta pathway, decreases transfusion requirements in a subset of transfusion-dependent β-thalassaemia patients and is increasingly prescribed in clinical practice. Subsequent to regulatory approvals, several reports have identified the risk of extra-medullary haematopoiesis, which resulted in updated safety labelling. In this report, we present a

The double edge of erythropoietic modulation in thalassaemia.

Luspatercept has emerged as a valuable therapeutic option for patients with transfusion-dependent β-thalassaemia, enabling significant reduction in transfusion requirements. In the report by Zaidel et al., a paradoxical clinical scenario is described in which treatment with luspatercept was associated with progressive splenomegaly, thrombocytopenia, increased haemolysis and a marked rise in reticulocytes and nucleated red blood cells, promptly reversible upon drug discontinuation. Luspatercept usually enhances late-stage erythroid maturation, but, in rare cases like this, seems to trigger or e

Liver Abscesses in Patients With Beta Thalassaemia Major: A Case Series and Mini-Review of the Literature.

Treatment of liver abscesses in patients with transfusion-dependent thalassaemia remains a challenging task due to the interplay of multiple factors, including difficult venous access and the co-existence of other organ dysfunction, such as diabetes mellitus. We report case histories of three transfusion-dependent thalassaemia patients with liver abscesses, two of whom had repeated episodes of the disease. We recommend a prolonged regimen of intravenous and oral antibiotics to eliminate the infection, along with more vigilant and regular follow-up using imaging for early detection of recurrenc

Long-term efficacy and safety of mitapivat in non-transfusion-dependent α- or β-thalassaemia: An open-label phase 2 study.

Non-transfusion-dependent thalassaemia (NTDT) can result in serious complications and comorbidities that can impact patients' quality of life. Mitapivat, a first-in-class, oral, small-molecule allosteric activator of red blood cell pyruvate kinase, is under investigation in adults with thalassaemia. Through its mechanism of action, mitapivat increases adenosine triphosphate, leading to improvements in red blood cell health, ineffective erythropoiesis and haemolysis. An open-label, multicentre, phase 2 study (NCT03692052) is evaluating mitapivat 100 mg twice daily in adults with NTDT. We previo

[CRISPR as a functional cure for hemoglobinopathies].

Severe haemoglobinopathies, including sickle cell disease and β-thalassaemia, represent significant global health burdens. CRISPR technology enables precise genetic editing of haematopoietic stem cells, with current therapies focused on boosting fetal haemoglobin production for a functional cure. This review finds that, while promising, ex vivo approaches require advanced facilities and substantial resources, limiting accessibility where the need is highest. Future development of in vivo methods may expand global access, addressing the urgent need for scalable and affordable treatments for the

CRISPR-based therapeutic genome editing for inherited blood disorders.

Therapeutic genome editing promises to transform medicine. Pivotal discoveries have provided a diverse and versatile set of tools to correct pathogenic mutations or produce protective alleles using CRISPR-based technologies. These innovative therapies are especially adaptable for blood and immune disorders, where clinical methods allow haematopoietic stem cells (HSCs) to be mobilized, harvested, engineered ex vivo and transplanted back into a patient to permanently replace their blood system. This paradigm has been exemplified with the first US Food and Drug Administration (FDA)-approved CRISP

CRISPR/Cas9 for achieving postintervention HIV control.

PURPOSE OF REVIEW: Recent advances in gene therapy have led to the first clinically approved CRISPR/Cas9 therapy for β-thalassaemia and sickle cell disease. Gene therapy could play an important role in targeting HIV persistence and achieving postintervention HIV control. Here, we review recent updates in CRISPR/Cas9-based HIV gene therapy approaches, including CCR5-editing (protect), proviral targeting (excise or modify), and immune cell engineering (attack). RECENT FINDINGS: Recent studies provide additional safety data for use of CRISPR/Cas9-based gene therapies, however low in vivo editing

Impact of lifetime anaemia and iron control on outcomes in β-thalassaemia: Data from the longitudinal de-LIGHT study.

Evidence on the impact of anaemia and iron control on morbidity and mortality in β-thalassaemia is limited to short-term studies assessing spot measures. We conducted a retrospective longitudinal cohort study of 557 patients with β-thalassaemia followed from diagnosis for a median of 38 years. There was a significant association between the lifetime proportion spent on transfusion and thalassaemia-related mortality (adjusted hazard ratios (HR) 0.987 for each 1-percent increase, p = 0.014). In both patients with non-transfusion-dependent β-thalassaemia (NTDT) and transfusion-dependent β-thalass

In vivo genome editing of human haematopoietic stem cells for treatment of blood disorders using mRNA delivery.

Ex vivo autologous haematopoietic stem cell (HSC) gene therapy provides a promising treatment option for haematological disorders. However, current methods involve complex processes and chemotherapeutic conditioning, leading to limited accessibility for treatment and major side effects. Here we develop antibody-free targeted lipid nanoparticles (LNPs) for mRNA delivery to HSCs in vivo, enabling efficient base editing of the γ-globin gene (HBG1/2) promoter target in human HSCs to reactivate fetal haemoglobin in derived erythroid cells. Delivery of ABE8e/sgRNA mRNA with optimized LNPs achieves e

Clinical patterns of thalidomide in the treatment of transfusion-dependent β-thalassaemia in children: a prospective single-arm study in China.

BACKGROUND: Previous studies in β-thalassaemia have shown that thalidomide is effective in reducing transfusion volume and is safe for short-term therapy of 6 months or less. The aim of this prospective study was to investigate long-term benefits and side effects of thalidomide in the treatment of transfusion-dependent β-thalassaemia (TDT) in children. METHODS: We analyzed the data from children and adolescents with TDT enrolled in a single-arm prospective study. The overall response rate, the incidence of discontinuation due to adverse events, the effect on height growth, and the effect on n

Evaluation of Five Plasma miRNAs as Biomarkers for Minimally Invasive Staging of Liver Fibrosis in β-Thalassaemia Patients.

Iron overload-driven liver fibrosis is a major concern in β-thalassaemia patients, but non-invasive or minimally invasive biomarkers for fibrosis staging remain limited. This study evaluated five plasma microRNAs (let-7a, miR-21, miR-29a, miR-34a, and miR-122) as potential markers for distinguishing liver fibrosis stages in β-thalassaemia. Plasma samples from 40 patients with fibrosis stages F0-F1 to F4 were analysed using RT-qPCR, normalised against the arithmetic mean of reference miRNAs miR-16 and miR-221. Expression levels of candidate miRNAs showed no statistically significant variation a
