Hyperfibrinogenaemia and hyperviscosity in sickle-cell crisis.

Plasma fibrinogen concentration and whole-blood viscosity, the latter measured at two shear rates (23 and 230 sec-1), were estimated during eight episodes of sickle-cell crisis and compared with values in 26 sickle-cell anaemia patients who were not in crisis. Painful crisis was associated with a significant increase in both plasma fibrinogen and whole-blood viscosity. Increased fibrinogen-erythrocyte interaction in vivo may be a significant contributory factor to raising blood viscosity and precipitating vaso-occlusive crisis in sickle-cell disease.

Sickle cell disease in pregnancy.

S-Homozygotes, SC heterozygotes, and S-beta-thalassaemia heterozygotes are the haemoglobinopathies which make up sickle cell disease. Although their clinical features are similar, as regards complications during pregnancy, Hb S-beta-thalassaemia most dangerous, the main causes of mortality being severe anemia, acute sequestration crisis, bacterial infections, painful episodes, and pulmonary bone marrow fat embolism. Folic acid and antimalarials (where these are indicated) are often successful in preventing severe anaemia. It is best to reserve blood transfusion to replace moderate loss or to c

Central retinal artery occlusion (reversible in sickle trait with glaucoma.

We report a case of central retinal artery occlusion in an 18-year-old black woman with sickle-trait haemoglobinopathy and acute glaucoma after hyphaema. The central retinal artery occlusion occurred immediately after treatment of the glaucoma with osmotic agents, raising the possibility that they played a precipitating role. We suggest that osmotic agents be used with extreme caution in sickle patients with glaucoma. The occlusion was treated by anterior chamber paracentesis with eventual return of good vision. The reversibility of retinal and optic nerve function after total ischaemia is dis

Comparison of five methods for preparing blood smears.

An extensive study was undertaken to compare five methods of blood smear preparation. Three mechanical devices for preparing blood smears were compared to the traditional coverslip and manual wedge methods. A series of five slides was prepared from each sample of blood. One hundred-cell leukocyte differential counts, red cell morphology, and platelet distribution were compared. Differential counts along the axis and margins of each slide failed to reveal any significant distribution difference by method. On the other hand, there were a significantly fewer neutrophils as well as higher lymphocy

Patients at high risk for low-birth-weight delivery.

A retrospective analysis of 8,839 singleton deliveries was undertaken to determine the relationships of maternal age, maternal hemoglobin concentrations, and past obstetric performance to the incidence of low-birth-weight (LBC) delivery. Patients under 19 years of age and patients with hemoglobin values less than 9.0 Gm. per 100 ml. were found to have significantly higher incidences of LBW deliveries. In addition, the incidence of LBW delivery was found to vary in proportion to the number of previous LBW deliveries and inversely with the number of previous term deliveries.

The control and propagation of a genetic disease.

Preventive health is an appealing method to better our quality of life. However, in the practice of preventive health, its probabilistic nature and the time lag for results lessen its appeal. The specter of genetic diseases also has a probabilistic nature but has a time lag that spans generations. Thus any preventive health effort for genetic diseases should be studied so the results can be predicted in advance. A number of mathematical genetic problems have been solved that include mutation and phenotype selection factors. This paper takes a large human population that has birth and death fre

The significance of glucose-6-phosphate dehydrogenase deficiency in pregnancy.

A study was undertaken to attempt to understand the significance of glucose-6-phosphate dehydrogenase deficiency, one of the most common genetic enzymatic defects known, in the context of pregnancy. A group of 841 consecutive patients were screened by a sensitive test for this deficiency state, and all deficient subjects available were matched with control subjects and followed prospectively. Little significance to mother and baby was noted, although the study of the babies was incomplete. Previous reports of an increased rate of urinary tract infections and icterus neonatorum were not substan

Thermodynamic studies of polymerization of deoxygenated sickle cell hemoglobin.

Solubilities of deoxygenated sickle cell hemoglobin (deoxy-Hb S), at varying pH and temperature over a range of concentrations encompassing those found in erythrocytes, were measured. The technique involved ultracentrifugation, which gave values of the supernatant concentration and the mass of the sedimented material. The data establish that the solubility of doexy-Hb S is the saturation concentration and is independent of initial concentration. The mass of the pellet phase increases linearly with initial concentration. Moreover, the saturation concentration represents the critical concentrati

Severe proliferative retinopathy as the only sign of sickle cell hemoglobin C disease.

A 48-year-old black woman developed severe bilateral hypoxic proliferative retinopathy without other clinical manifestations. The hemoglobin level was 10.6 to 11.5 g/100 ml, reticulocyte level was 2.2%, targeted and sickled red blood cells were seen on blood smears, and hemoglobins S and C were demonstrated by electrophoresis. Glucose tolerance test was normal. The development of neovascular proliferation, vitreous hemorrhage, and retinal detachment unassociated with other clinical symptoms is unusual in sickle cell hemoglobin C disease.

betaS Chain turnover in reticulocytes of sickle trait individuals with high or low concentrations of haemoglobin S.

Reticulocytes, isolated from the blood of sickle cell trait donors with either low (25-30%) or high (40-42%) haemoglobin S(Hb S) concentrations, were incubated with [3H]leucine for various times from 1.25 to 60 min. Samples of the total soluble fractions of the cells were denatured with urea and mercaptoethanol. The mixtures were analysed by electrophoresis on cellulose acetate strips. The specific radioactivities (dpm/mg) of the separated betaS and betaA globin chains were determined. The betaS/betaA ratios of globin chain specific radio activities in the reticulocytes of the 'low Hb S' donor

Neonatal screening for haemoglobinopathy. Results in 7691 Manchester newborns.

Over a period of one year the blood samples collected for phenylketonuria testing from 7691 Manchester newborns were screened by haemoglobin electrophoresis. An abnormality was detected in 47 (0-61%) of the babies. No cases of homozygous haemoglobinopathy were found. The overall incidence of sickle-cell trait was 0-38%, but for the Black population it was 10%. Four Black babies and one White baby had alpha-thalassaemia. No other haemoglobinopathies were found in the White babies and no Asian baby had alpha-thalassaemia. Haemoglobin A2 was precociously developed in three babies, two of whom wer

Biological variation in the heterogeneous distribution of haemoglobin F among erythrocytes.

The development of highly specific fluorescent labelled antibodies against haemoglobin F presented an opportunity to investigate variables which might influence distribution of this haemoglobin among individual erythrocytes. Earlier investigations revealed heterogeneous distribution within healthy individuals and in individuals with sickle cell disease and other haemoglobinopathies. The current study demonstrates quantitatively that there are normal biological determinants of variability in the frequency of F-containing erythrocytes in individuals who have a haemoglobin A electrophoretic pheno

beta+-Thalassaemia, Haemoglobin S and Hereditary elliptocytosis in a Zairian Family. Ischaemic costal necroses in a child with sickle cell beta+-Thalassaemia.

In a family of Low-Zaïre, a child with sickle cell beta+-thalassaemia disease, aged 7, presented 33.6% haemoglobin A. The clinical evolution was benign, except for the occurrence of an extensive bilateral ischemic necrosis of the ribs. This is the third case of sickle cell thalassaemia with the haemoglobin phenotype SFA reported in Zaïrians, whereas the SF phenotype has repeatedly been found: beta+-thalassaemia seems therefore much rarer in Zaïrians than betadegrees-thalassaemia. Besides the thalassaemia trait and the sickle cell trait, hereditary elliptocytosis was also segregating in this fa

Increased susceptibility to infection in sickle cell disease: defects of opsonization and of splenic function.

Patients with sickle cell disease (SCD) have an increased susceptibility to bacterial infections and die more frequently from infection than any other cause. Their sera do not promote the phagocytosis of pneumococci normally, apparently because of an inability to utilize the alternate pathway for C3 fixation as a means of opsonization. The splenic dysfunction which exists in SCD may result in depressed synthesis of alternate pathway factor(s), as well as in decreased phagocytic clearance of bacteria.
