Biomarkers for the diagnosis and phenotyping of acute chest syndrome in patients with sickle cell disease.

INTRODUCTION: Acute Chest Syndrome (ACS) is a serious and often fatal complication of Sickle Cell Disease (SCD). In the absence of specific biomarkers that indicate the development of ACS, diagnosis can be nebulous and difficult. In this narrative literature review, we examine the current evidence for novel biomarkers associated with acute and long-term propensity toward ACS. AREAS COVERED: A search focused on terms related to 'acute chest syndrome,' 'sickle cell anemia,' and 'biomarkers' was created and run by a medical librarian in MEDLINE (Ovid), Google Scholar, and Embase (Ovid). Four-hun

Development of a Novel Patient-Reported Outcome Diary to Capture and Monitor Home-Managed Pain Crises in Patients with Sickle Cell Disease.

INTRODUCTION: Sickle cell pain crises (SCPCs) are debilitating pain events that significantly impact quality of life in patients with sickle cell disease (SCD), but no fit-for-purpose tool exists to capture patients' experiences of them. We developed a patient-reported outcome (PRO) tool to capture patients' SCPC experiences and ensured its content validity through cognitive debriefing (CD) interviews with adolescents and adults with SCD. METHODS: In this non-interventional, qualitative research study, a targeted literature review (TLR) was conducted to create preliminary items and a prelimin

Weather-Related Exposures and Emergency Department Visits for Pediatric Sickle Cell Complications.

OBJECTIVE: Sickle cell disease (SCD) is an inherited disorder that leads to significant morbidity from health complications beginning in childhood. Weather-related factors may influence the frequency of SCD complications, but evidence regarding the impact of weather changes in the pediatric population remains limited. Our objective was to determine the association between weather-related exposures and odds of emergency department (ED) visits for acute complications in youth with SCD. METHODS: We conducted a retrospective case-crossover study of ED encounters among patients aged 0 to 21 years

The GLOBE Trial: Efficacy and Safety of L-Glutamine Plus Hydroxyurea Versus Hydroxyurea Alone in Sickle Cell Anemia - A Double-Blind, Randomized Study.

OBJECTIVE: Hydroxyurea (HU) reduces complications of sickle cell anemia (SCA), but the response is variable. L-glutamine, an antioxidant that improves redox balance, is implicated in a distinct pathophysiological pathway and may provide additional clinical benefit when added to HU. We evaluated HU plus L-glutamine versus HU alone in pediatric/adolescent SCA. MATERIALS AND METHODS: In a 6-month, double-blind, placebo-controlled trial, 53 patients with HbSS or HbS/β0-thalassemia were randomized to the HU + L-glutamine (n=27) or the HU + placebo (n=26) group while continuing HU at ~20 mg/kg/day.

Multimodal imaging features to diagnose abdominal complications of sickle cell disease.

Sickle cell disease (SCD) is a hereditary hemoglobinopathy, marked by chronic hemolysis and recurrent vaso-occlusive crises, which frequently leads to a wide range of abdominal complications. Sickled erythrocytes have reduced deformability and increased adhesion to vessel walls, leading to chronic vascular occlusion, inflammation, and hemolysis. Comprehensive imaging plays a pivotal role in the early detection and assessment of these complications, thereby mitigating morbidity and severe outcomes. Ultrasound (US), computed tomography (CT), and magnetic resonance imaging (MRI) are the three mod

Safety and effectiveness of voxelotor in individuals with sickle cell disease in the RETRO and PROSPECT US registries.

Two registry studies, RETRO and PROSPECT, were conducted to evaluate real-world safety and effectiveness outcomes in US participants with sickle cell disease (SCD) who received voxelotor as standard of care. RETRO collected retrospective data of participants aged ≥12 years at 9 US sites for ∼1 year after voxelotor initiation. PROSPECT collected retrospective and prospective data of participants aged ≥4 years at 24 US sites, with a 5-year follow-up planned. Both studies collected data for 1 year before voxelotor initiation. RETRO enrolled 216 participants. PROSPECT enrolled 265 participants bef

Inflammatory markers in sickle cell disease during vaso-occlusive crisis.

Sickle cell disease (SCD) is a monogenic blood disorder in which vaso-occlusive crises (VOC) are the main cause of hospitalization. Along with vascular adhesion, inflammation plays a central role in VOC. This study compared inflammatory and hematological markers in SCD patients during VOC and steady state, examined their association with outcomes such as hospital stay, acute chest syndrome (ACS), and mortality, and evaluated the impact of hydroxyurea. A total of 109 VOC patients admitted to the Department of General Medicine, VIMSAR, and 40 steady-state patients attending the outpatient clinic

Loss of energy homeostasis contributes to hepatic damage development in sickle cell disease.

Sickle cell disease (SCD) is characterized by the expression of an abnormal hemoglobin variant (HbS) that promotes distortion and early destruction of red blood cells, resulting in hemolytic anemia, vaso-occlusive crisis, ischemia and, ultimately, tissue damage. Hepatic function is specially compromised in SCD patients; however, the underlying pathological mechanisms remain largely unknown. In the current study, we confirmed the presence of hepatic damage in a murine model of SCD and, through a label free quantitative proteomic approach, we identified significant alterations in protein express

Prevention and Management of Acute Chest Syndrome Without Automated Red Cell Exchange Among Patients With Sickle Cell Disease Not Requiring ICU Care: A Systematic Review and Network Meta-Analysis.

Acute chest syndrome (ACS) is a severe complication of sickle cell disease (SCD) associated with significant morbidity and mortality, necessitating optimized prevention and management strategies for improved patient outcomes. This review does not evaluate red blood cell exchange, as no randomized controlled trials meeting our inclusion criteria reported outcomes for this intervention. A thorough literature review identified interventions for ACS in SCD patients. Seventeen randomized controlled trials (RCTs) underwent assessment using the Cochrane Risk of Bias 2 tool, and a frequentist network

Adiponectin levels during steady state and vaso-occlusive crisis in Nigerian children with sickle cell anaemia.

Sickle cell anaemia (SCA) is a chronic inflammatory hemoglobinopathy marked by recurrent vaso-occlusive crises (VOC) and end-organ damage. Adiponectin, an adipokine with anti-inflammatory and insulin-sensitizing properties, is well-studied in metabolic disorders but remains underexplored in SCA. This study assessed serum adiponectin levels in Nigerian children with SCA during steady state and VOC. We conducted a cross-sectional study involving 50 children with homozygous sickle cell genotype (HbSS) and 46 age- and sex-matched healthy controls with normal adult haemoglobin genotype (HbAA). Adip

Fulminant Acute Chest Syndrome and Multiorgan Failure in Sickle Cell Disease With Chronic Hepatic Vasculopathy: A Fatal Synergy.

Sickle cell disease (SCD) is a multisystem disorder characterized by recurrent vaso-occlusive events and progressive organ damage. Among its chronic complications, sickle hepatopathy, driven by sinusoidal obstruction, ischemia, and cholestasis, remains underrecognized despite its major prognostic significance. During acute crises, hepatic dysfunction may amplify systemic inflammation, impair metabolic reserve, and precipitate rapid multiorgan failure. We report the case of a 28-year-old man with homozygous SCD and biopsy-confirmed chronic hepatic vaso-occlusive disease who presented after disc
