Movement behavior in adults with sickle cell disease compared to healthy adults: a cross-sectional study.

BACKGROUND: Sickle cell disease (SCD) is one of the most common monogenic disorders in the world. Patients with SCD have chronic hemolytic anemia and experience episodic pain due to vaso-occlusion resulting in, amongst other, musculoskeletal and cardiopulmonary complications. We hypothesize that adults with SCD are less physically active, but limited information is available on the movement behavior of adults with SCD. In the current study, we aimed to evaluate movement behavior in patients with SCD as compared to healthy controls. METHODS: Movement behavior of patients with SCD (≥16 years) w

Illness Identity in Symptomatic Adults with Sickle Cell Disease: Preliminary Findings and Considerations.

Illness identity, or how an individual integrates their disease into their sense of self, is an important psychological concept for medical populations. According to one model, there are four proposed dimensions of illness identity: Acceptance, Rejection, Enrichment, and Engulfment. Acceptance and Enrichment describe an individual's ability to embrace their illness and regard it as a source of positive meaning-making, while Rejection and Engulfment describe an individual's inclination to disregard or be preoccupied by their illness in unhelpful ways. This study aimed to describe illness identi

Prevalence estimates of sickle cell disease among children and adolescents in sub-Saharan Africa: a systematic review and modelling analysis.

BACKGROUND: There is a scarcity of data reporting on the burden of sickle cell disease across many African settings, particularly among children, who have the highest risk of preventable morbidity and mortality in the absence of early diagnosis and care. We aimed to estimate the prevalence of sickle cell disease and the absolute number of paediatric cases in sub-Saharan Africa to inform policy and service responses. METHODS: For this systematic review and modelling analysis, we searched MEDLINE, Embase, Global Health (CABI), and African Journals Online (AJOL) for studies published from Jan 1,

Risk factors for venous thromboembolism in sickle cell disease.

BACKGROUND: Individuals with sickle cell disease (SCD) have a high risk for venous thromboembolism (VTE). OBJECTIVES: To determine the associated risk factors and recurrence rate of VTE in SCD. METHODS: We performed a retrospective review of 719 individuals with SCD followed at our institution between 2013 and 2023 and compared those who developed VTE with those who did not. RESULTS: Of the 719 patients reviewed, 64 (8.9%) patients had 71 VTEs during the study period; 28% of the VTE events were pulmonary embolism and 61% were deep vein thrombosis. The odds ratio (OR) for developing a VTE wa

Associations Between Sickle Cell Disease, Pica, and Enuresis in Pediatric Neurodevelopmental Disorders.

Background: Sickle cell disease (SCD) is a hereditary disorder affecting red blood cells' shape and functional capacity. Individuals with SCD report relatively high co-occurrence of neurodevelopmental disorders (NDDs). In addition, these children also have higher rates of enuresis (incontinence) and pica, disorders prevalent in children with developmental delays. Both enuresis and pica can have negative effects on mental health, but their pathophysiology, especially in SCD, remains unclear. Objectives: The objective of this study was to determine the rates of pica and enuresis in a pediatric S

Acute kidney injury: incidence, risk factors, and early outcomes in hospitalized children with sickle cell disease attending a tertiary hospital in Eastern Uganda.

BACKGROUND: Many sickle cell disease (SCD) patients experience renal damage that impairs function, with acute kidney injury (AKI) significantly affecting both immediate and long-term outcomes. The only Ugandan study on this subject reported a high incidence and mortality of AKI among children with SCD, but did not assess other outcomes. METHODS: This hospital-based prospective study was conducted between January and June 2024 at Jinja Regional Referral. AKI was diagnosed using KDIGO criteria, and participants were followed until discharge or death. Outcomes included length of hospital stay, mo

Dissecting polycomb complexes for enhanced fetal hemoglobin production.

Polycomb repressive complexes PRC1 and PRC2 regulate diverse developmental processes, including the fetal-to-adult switch in hemoglobin production, a process whose reversal is a goal for the treatment of sickle cell disease and β-thalassemia. PRC inhibitors show promise for various disorders, but use is limited because of pleiotropic PRC activities. We explored whether fetal hemoglobin (HbF) can be reactivated in adult erythroid cells by selective perturbations of PRC1 or PRC2 components without complete loss of PRC function. A high-density CRISPR-Cas9 mutagenesis screen identified a region in

Human placental decellularized extracellular matrix improves pancreatic progenitor differentiation from human embryonic stem cells compared with defined extracellular matrix protein substrates.

Latanoprost (Xalatan) is an ester analogue of prostaglandin F2alpha that reduces intraocular pressure (IOP) by increasing uveoscleral outflow. The IOP-lowering efficacy of latanoprost 0.005% lasts for up to 24 hours after a single topical dose, which allows for a once-daily dosage regimen. In patients with ocular hypertension or open-angle glaucoma, a single drop of latanoprost 0.005% solution (about 1.5 microg) administered topically once daily reduced diurnal IOP by 22 to 39% over 1 to 12 months' treatment in well-controlled trials; efficacy was maintained during treatment periods of up to 2

Optical coherence tomography angiography in retinal vascular disease: transformative tool or expensive gimmick?

PURPOSE OF REVIEW: Optical coherence tomography angiography (OCTA) has rapidly evolved from an experimental imaging technique into a routine component of many retina practices. By providing noninvasive, depth-resolved visualization of retinal and choroidal vasculature without dye injection, OCTA promises safer, faster, and more detailed assessment of retinal vascular disease than fluorescein angiography. However, there is still debate about its clinical applicability and its role in everyday ophthalmology practice, which we review in this article. RECENT FINDINGS: Over the past decade, OCTA h

Transformed but Not Cured: The Ethics of Describing Gene-Editing Therapy for Sickle Cell Disease.

In December 2023, the U.S. Food and Drug Administration approved gene-editing therapies as sickle cell disease treatments. Such approvals for gene-editing not only mark radical scientific innovations for populations living with sickle cell disease (SCD) across the United States but also generate an expectation of a potential cure-the end or eradication of an illness and its effects. This essay, however, cautions against framing gene-editing therapy as a "cure" for SCD. Our argument illustrates that, even if gene editing is proven to permanently normalize the hematologic function of the body, t

Red Blood Cell Exchange in Sickle Cell Disease Care: A Comprehensive Review.

Erythrocytaphoresis or RBC exchange (RBCX) is a key intervention in the management of sickle cell anemia for both chronic prophylaxis and emergent clinical care during sickle cell disease (SCD) emergencies. RBCX removes sickled RBCs while replacing them with donor RBCs and reducing hemoglobin S (HbS) levels. Unlike simple transfusion, it minimizes iron overload and avoids fluid overload, making it a mainstay for the management of acute stroke, acute chest syndrome, and other settings where fluid overload can complicate disease morbidity. Chronic RBCX has shown benefit in not only improving oxy

Acute Soft Head Syndrome, A Rare Complication of Sickle Cell Anemia: A Case Report.

Acute soft head syndrome (ASHS) is a rare clinical complication of sickle cell disease (SCD). It presents as a progressive, non-traumatic, and tender swelling of one or more areas of the scalp. Aetiopathophysiology is still less understood but hypothesized to be a result of loss of cortical skull bone integrity from increased extramedullary hematopoiesis. Diagnosis is clinically made. However, imaging is very necessary to rule out an intracranial involvement, commonly an epidural hematoma. ASHS with no intracranial involvement is managed conservatively. We present the case of a 15-year-old ado

A randomized, double-blind, controlled, parallel group study with amustaline/glutathione pathogen reduced red blood cells in regions at potential risk for Zika virus transfusion-transmitted infections (RedeS Study)-protocol for a phase 3 clinical trial.

BACKGROUND: Red blood cell (RBC) transfusion may be lifesaving, however transfusion-transmitted infections (TTI) and transfusion-associated graft-versus-host disease (TA-GVHD) still pose a threat despite current donor selection and testing requirements. Amustaline (S-303)/glutathione (GSH) pathogen reduction (PR) is designed to inactivate infectious microbes and leukocytes in RBCs. The RedeS study evaluates the safety and efficacy of amustaline/GSH PR-RBCs compared to conventional RBCs in a broad spectrum of patients requiring acute and/or repeated RBC transfusion support. METHODS: RedeS is a

The Journey of Gene Therapy in Sickle Cell Disease: How Molecular Advances Meet Clinical Care.

Sickle cell disease (SCD) is a monogenic disorder responsible for recurrent vaso-occlusive crises, progressive organ damage, and shortened life expectancy. For decades, allogeneic hematopoietic stem cell transplantation from a matched sibling donor has been the only established cure, but its reach remains limited by donor availability and transplant-related toxicity. The approval of two autologous gene therapy products in 2023, exagamglogene autotemcel (exa-cel) and lovotibeglogene autotemcel (lovo-cel), marked a turning point for the SCD population and the gene therapy field in general. Sickle Cell Disease: Can genetic variability influence pregnancy outcomes?

UNLABELLED: Pregnancy in Sickle Cell Disease (SCD), a severe hereditary genetic condition, highly prevalent in Sub-Saharan African countries, is associated with increased risk of complications and severe outcomes in pregnancy, like intrauterine growth restriction, low birth weight, premature birth, miscarriage, stillbirth, pre-eclampsia, and maternal mortality. Several factors have been identified as associated with the heterogeneity of SCD phenotypes, namely the hemoglobin subunit beta (HBB) haplotype and -3.7 kb α-thalassemia deletion. OBJECTIVE: This study aimed to identify pregnancy compl

Advancing child health: forecasting the next great research achievements.

Advances in pediatric research have improved the lives of infants, children, adolescents, and society at large. A decade ago the American Academy of Pediatrics identified seven research achievements in the past 40 years that reduced morbidity and mortality worldwide: preventing disease with life-saving immunizations, reducing sudden infant death with "Back to Sleep," curing Acute Lymphoblastic Leukemia, helping premature babies breathe with surfactant, preventing Human Immunodeficiency Virus transmission from mother to baby, increasing life expectancy for children with Sickle Cell Anemia and C
