Fever and Painful Skin Lesions: When to Consider Sweet's Syndrome.

Sweet's syndrome is an acute febrile neutrophilic dermatosis characterised by fever, neutrophilia, and painful cutaneous lesions. We describe a case of a 31-year-old male admitted with a two-week history of fever up to 39°C and painful erythematous papules, scattered and coalescing into plaques over the trunk and limbs, together with pustules on an erythematous base over the palms and dorsum of the hands. No relevant epidemiological exposure or recent drug exposure was identified. Investigation showed an erythrocyte sedimentation rate of 40 mm/h, leucocytosis of 14,380/µL with 77% neutrophils

Myeloid neoplasm with PDGFRA rearrangement presenting in blast-phase with basophilia.

An adult man, from Southern Africa, presented to a public health centre with symptoms of progressive fatigue. Peripheral blood morphology showed marked basophilia and 21% myeloblasts, confirmed by flow cytometry, suggesting acute myeloid leukaemia with basophilia. Bone marrow biopsy revealed increased mast cells, raising systemic mastocytosis as a differential. However, next-generation sequencing identified a PDGFRA fusion, establishing the diagnosis of blast phase of a myeloid/lymphoid neoplasm with PDGFRA rearrangement. The presence of cells with abnormal basophilic granulation and absence o

The clinical course of nasal polyps in relation with histological and radiological findings.

BACKGROUND: Nasal polyps (NPs) show distinct characteristics in histologic features with different inflammatory cells and stromal changes, but their clinical implications remain poorly understood. OBJECTIVE: To determine the associations between NP histology, clinical characteristics, radiologic disease burden on osteomeatal unit computed tomography (OMU-CT) findings, and postoperative recurrence. METHODS: We retrospectively reviewed 219 patients who underwent nasal polypectomy. NPs were histologically classified by predominant inflammatory cells (eosinophilic vs. lymphoplasmacytic) and stro

When the Cure Turns Into a Culprit: Mesalamine and the Eosinophilic Lung.

We report a case of a 74-year-old man with a one-year history of ulcerative colitis in remission who developed low-grade fever, dry cough, progressive dyspnea, loss of appetite, and an eight-kg weight loss over 12 weeks. Oral mesalamine (2.4 g/day) had been initiated 12 weeks before presentation, with respiratory symptoms developing approximately three weeks after initiation. Despite initial antibiotic therapy, his symptoms progressively worsened, and he developed hypoxemia requiring hospitalization. Investigations revealed leukocytosis with marked peripheral eosinophilia, elevated C-reactive

Unilateral eosinophilic fasciitis: a case report and review of the literature.

A 59-year-old Japanese man developed pruritus and swelling of the right dorsal foot after weeding his garden in a crouching position for approximately one hour. Over a period of five months, the swelling, as well as skin induration, progressed unilaterally from the right thigh to the lower leg and dorsal foot. Laboratory tests, including eosinophil fraction and inflammatory markers, were within normal limits, and computed tomography showed no evidence of deep vein thrombosis or malignancy. A full-thickness biopsy including the fascia revealed marked fascial thickening with fibrosis of the subc

An elusive diagnosis: pericardial effusion and hypereosinophilia revealing seronegative eosinophilic granulomatosis with polyangitis: a case report.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a small- and medium-vessel necrotizing vasculitis characterized by asthma, eosinophilia, and multiorgan involvement. Cardiac manifestations, though uncommon, are more prevalent in antineutrophil cytoplasmic antibody (ANCA)-negative patients and carry significant morbidity. Diagnosis can be challenging due to variable presentations and the lack of a definitive test. We present a 60-year-old woman with asthma and chronic rhinosinusitis who developed progressive dyspnea and was found to have a large pericardial effusion with tamponade physio

Diffuse esophageal eosinophilia is associated with increased endoscopic severity and topical steroid non-response in patients with eosinophilic esophagitis.

BACKGROUND AND AIMS: Variability of esophageal eosinophil distributions and its impact on disease presentation and treatment outcomes in eosinophilic esophagitis (EoE) is unclear. We aimed to determine whether isolated proximal or distal involvement differs from diffuse involvement in EoE presentation and treatment outcomes. METHODS: This retrospective cohort study analyzed patients of all ages with newly diagnosed EoE from the UNC EoE Clinicopathologic database. Patients were classified as distal esophagus-only (≥15 eos/hpf), proximal-only (≥15 eos/hpf), and diffuse involvement (≥15 eos/hpf

[ASSOCIATION BETWEEN PEDIATRIC EOSINOPHILIC GASTROINTESTINAL DISORDERS AND NEURODEVELOPMENTAL DISORDERS: A CLINICAL INVESTIGATION].

BACKGROUND: Eosinophilic gastrointestinal disorders (EGIDs) are classified into eosinophilic esophagitis (EoE) and non-EoE EGIDs, with the latter being more common in Japan. Although recent studies have suggested an association between EGIDs and neurodevelopmental disorders (ND), domestic data remain limited. This study examined the relationship between ND and non-EoE EGIDs. METHODS: We evaluated 33 children diagnosed with non-EoE EGIDs and determined the prevalence of comorbid ND. Clinical characteristics-including sex, age at onset, treatment patterns, and relapse-were examined according to

Clinical and Laboratory Dynamics of Bear Meat-Associated Trichinellosis in Hokkaido, Japan.

Trichinellosis is a rare infection associated with bear meat consumption found in Japan, where limited access to parasite-specific antibody testing makes definitive diagnosis challenging. In the present study, 19 patients, comprising 16 confirmed and three probable cases, treated at a tertiary care hospital in Hokkaido, Japan, during three independent outbreaks in 2018, 2019, and 2025, were retrospectively analyzed. Patients prepared brown bear meat as a roast (n = 18) or a cutlet (n = 1) and consumed it as a main dish (n = 7) or in smaller portions of a few slices (n = 12). Ten patients consu

Exploration of miRNAs promoting eosinophilia and type 2 immunity.

BACKGROUND: MicroRNAs (miRNAs) are evolutionary conserved, small non-coding RNAs that regulate gene expression and modulate cellular responses. In this study, we profiled miRNAs regulated by interleukin-33 (IL-33), a key inflammatory mediator of allergic responses and type 2 immunity that promotes bone marrow eosinophil development and subsequent airway recruitment. METHOD: C57BL/6 mice were exposed to recombinant IL-33 intranasally to induce airway eosinophilia. Cellular composition was determined by differential cell counts in bronchoalveolar lavage fluid and bone marrow. RNA profiling was

The Use of Montelukast in Pediatric Asthma and Anti-Inflammatory Effects - Easy Choice or a Balancing Act?

Montelukast is a selective cysteinyl leukotriene receptor 1 antagonist with an established, though nuanced, role in the management of asthma and allergic rhinitis. By inhibiting leukotriene-mediated signaling, montelukast targets a pathway involved in bronchoconstriction, mucus secretion, vascular permeability, eosinophil recruitment, cytokine release, and airway remodeling. Available evidence indicates that montelukast may reduce several markers of allergic inflammation, including fractional exhaled nitric oxide, peripheral and tissue eosinophilia, eosinophil-derived mediators, adhesion molec

Severe strongyloidiasis and Gram-negative bacterial superinfections: epidemiology, clinical implications, and therapeutic strategies.

PURPOSE OF REVIEW: Strongyloidiasis is a neglected tropical disease affecting over 600 million people worldwide. Its severe forms carry a fatality rate exceeding 60%, yet death is rarely attributable to the parasite alone: the terminal event is often bacterial sepsis or meningitis of enteric origin. We review recent evidence on Gram-negative and other bacterial superinfections in severe strongyloidiasis, and their diagnostic and therapeutic implications. RECENT FINDINGS: Cohort studies show that bacteraemia is the commonest complication of severe disease, affecting 38.3% of cases against 4.1%

Organizing pneumonia in Crohn's disease: a case report and literature review.

Organizing pneumonia (OP) is a rare pulmonary manifestation in patients with inflammatory bowel disease (IBD). Distinguishing between a primary extra-intestinal manifestation of Crohn's disease (CD) and mesalazine-induced lung injury remains a major clinical challenge, as both conditions are steroid-responsive. We report the case of a 31-year-old woman with CD who developed progressive dyspnea and migratory pulmonary infiltrates one month after initiating mesalazine. Laboratory findings revealed marked peripheral eosinophilia (1276/mm3) and a mixed eosinophilic bronchoalveolar lavage profile (

[Principles of treating patients with chronic obstructive pulmonary disease].

Chronic obstructive pulmonary disease (COPD) is a heterogeneous condition characterized by inflammation of the airways and lung parenchyma, manifested by persistent respiratory symptoms such as dyspnoe and cough. As a global pandemic, it affects approximately 10.3% of the population over the age of 40 and remains among the leading causes of death with an increasing mortality trend. In Hungary, the estimated actual prevalence is approximately three times higher than the number of officially registered cases. COPD imposes a significant economic burden, primarily driven by the hospital care requi

Human angiostrongyliasis: the past, present, and future.

Human angiostrongyliasis is an emerging foodborne zoonosis caused predominantly by Angiostrongylus cantonensis and Angiostrongylus costaricensis, which produce distinct neurological and abdominal forms of disease, respectively. Human infection occurs primarily through ingestion of raw or undercooked gastropod intermediate hosts, paratenic hosts such as freshwater prawns and crabs, or fresh produce contaminated with infective third-stage larvae (L3). A. cantonensis is the principal cause of neuroangiostrongyliasis and eosinophilic meningitis, whereas A. costaricensis causes abdominal angiostron
