Diphtheria outbreak, Northern Territory of Australia, 2025 to 2026.

Because of high vaccination rates, the Northern Territory of Australia had not seen locally transmitted toxigenic Corynebacterium diphtheriae since the 1990s, until cutaneous diphtheria cases emerged in May 2025 and respiratory cases in March 2026. By 30 April 2026, there had been 131 cases (34 respiratory, 97 cutaneous) in an ongoing outbreak characterised by a high proportion of cutaneous cases with transmission driven by crowding and consequent poor skin health. Most patients have had mild disease due to prior vaccination.

Cellulitis Caused by Mixed Infection with Group A Streptococci and Non-Toxigenic tox-Gene-Bearing Corynebacterium diphtheriae: A Case Report.

INTRODUCTION: Cutaneous diphtheria has re-emerged in Europe, often presenting as skin-limited disease. Uncertainty around diagnosis, infection control, and toxigenicity testing can delay appropriate management. CASE PRESENTATION: A 62-year-old woman experiencing homelessness presented with a 2-week history of a painful lower-leg eruption. This evolved from vesicles to bullae and shallow ulcers with a characteristic green-grey pseudomembrane, unresponsive to oral flucloxacillin. Swabs of the ulcer grew Corynebacterium diphtheriae and concurrent group A β-haemolytic streptococci, while throat s

Lack of Detectable Anti-Diphtheria Toxin Antibodies in European Hedgehogs (Erinaceus europaeus) from a Corynebacterium ulcerans Hotspot.

Corynebacterium ulcerans is a zoonotic pathogen causing cutaneous diphtheria. In Belgium, the bacterium is the cause of hedgehog diphtheric disease (HDD) that appears to have emerged from an endemic state within the European hedgehog (Erinaceus europaeus) population. In spring 2025, 11 hedgehogs admitted to a wildlife rehabilitation center were euthanized due to their severe condition. Blood was collected for anti-diphtheria toxin antibody detection, and lesion swabs (if present) and oral cavity swabs were taken, followed by necropsy. Of nine hedgehogs exhibiting HDD-like lesions, five were co

The Return of Diphtheria: Reflections From the Emergency Department.

Diphtheria has re-emerged in Australia in 2026, prompting significant public health concern and placing additional demands on healthcare services. This article examines the early phase of the outbreak, with particular focus on its impact on EDs. The outbreak has disproportionately affected Indigenous communities, highlighting the influence of social determinants of health, geographical isolation, skin infections and waning vaccine-induced immunity on disease transmission and outcomes. Emergency clinicians faced challenges in identifying suspected cases due to the overlap of diphtheria presenta

[Cutaneous diphtheria among the homeless].

We report on a 34-year-old homeless woman with polysubstance addiction, admitted with multiple skin ulcers initially misdiagnosed as insect bites during a dermatological teleconsultation. A wound swab revealed Corynebacterium diphtheriae. This case illustrates the danger of cutaneous diphtheria in vulnerable groups. It emphasizes the need for microbiological diagnostics in chronic wounds, as well as the importance of correct antibiotic therapy and strict isolation measures to prevent outbreaks.

Post-Diphtheritic Myocarditis with Right Ventricular Thrombus: A Rare Cardiac Complication.

BACKGROUND: Myocarditis is a serious consequence of diphtheria; however, the occurrence of a subsequent right ventricular (RV) thrombus is exceedingly rare. We provide an uncommon case of post-diphtheritic myocarditis complicated by a right ventricular thrombus and secondary pneumonia, emphasizing its effective multi-modal therapy and subsequent recovery. CASE PRESENTATION: A 19-year-old female arrived at our emergency department with acute dyspnea and chest pain two weeks after a 3-day hospitalization for pharyngeal diphtheria at Galkacyo Regional Hospital. Molecular epidemiology and antimicrobial resistance determinants of Corynebacterium diphtheriae causing infections in Karachi, Pakistan, 2023-2024.

OBJECTIVES: Diphtheria remains endemic in Pakistan, with cases increasing following the COVID-19 pandemic despite ongoing vaccination programs. This study analyzes the genomic diversity, virulence, and antimicrobial resistance patterns of pharyngeal diphtheria strains collected during the Karachi outbreak. METHODS: Corynebacterium diphtheriae isolates from a tertiary care hospital laboratory in Karachi (August 2023-October 2024) were included. Antimicrobial susceptibility testing and whole-genome sequencing of phenotypically confirmed isolates were performed. Phylogenetic and bioinformatics a

Native Aortic and Mitral Valve Endocarditis Caused by Nontoxigenic Corynebacterium diphtheriae in a Young Male Patient: A Case Report.

Non-toxigenic Corynebacterium diphtheriae strains are emerging as a cause of invasive disease, including highly virulent endocarditis, and have been associated with aggressive valvular destruction and embolic complications in reported cases without classical risk factors. A 26-year-old man with a history of former intravenous drug use and recent inhalational drug use presented with symptoms of systemic infection. He did not have any known pre-existing cardiac disease. Blood cultures grew C. diphtheriae. A transthoracic echocardiogram revealed large (>1 cm) aortic and mitral vegetations associa

Evaluation of Indigenous status completeness in vaccine preventable disease notification data in the NNDSS.

BACKGROUND: High quality Indigenous status data for vaccine preventable diseases (VPDs) in the National Notifiable Diseases Surveillance System (NNDSS) is important for evaluation of immunisation programs and ultimately for improving health outcomes for Aboriginal and Torres Strait Islander peoples. We evaluated Indigenous status completeness, and factors influencing it, for VPDs in the NNDSS. METHODS: Literature review (published and grey); descriptive analysis of NNDSS data for selected VPDs over the 2010-2019 period; standardised online survey (containing closed- and open-ended questions)

Agreement between BTS G-walk and GaitLab in spatiotemporal and pelvic angle measurements in active older adults.

BACKGROUND: Aging impacts gait, a vital health indicator in older adults, reducing speed and increasing double support time, linked to falls and disability. The World Health Organization advises 150 min/week of moderate or 75 min/week of vigorous exercise, plus strength training, to sustain neuromuscular integrity and locomotor capacity associated with healthy gait patterns in older adults. While 3D motion capture is the gold standard for gait analysis, its cost and complexity limit use, boosting interest in portable inertial sensors like the BTS G-Walk. Yet, their accuracy in active older adu

Toxigenic Corynebacterium ulcerans in raw milk of a cow with acute mastitis: case report and historical review on milk-transmitted diphtheria.

PURPOSE: Historically, a considerable number of diphtheria outbreaks, mainly in the UK and the USA, have been epidemiologically or bacteriologically linked to the consumption of milk and dairy products. However, with the introduction of milk pasteurization and hygienic improvements in livestock farming and animal welfare, no cases of milk-linked human diphtheria outbreaks or bovine mastitis due to toxigenic corynebacterial have been reported in the recent decades. Here we report the first isolation of a toxigenic Corynebacterium ulcerans strain from the milk of a cow with acute mastitis within

A case of fatal respiratory diphtheria imported from Poland to Germany: possible link to an undetected imported diphtheria cluster in Poland?

PURPOSE: Diphtheria is a re-emerging vaccine-preventable disease, mainly caused by toxigenic Corynebacterium diphtheriae. METHODS: Here, we report a fatal respiratory diphtheria infection in a Polish woman travelling to Germany possibly linked to a cutaneous diphtheria infection in a homeless man living in the same geographic area. Laboratory diagnostics involving MALDI-TOF MS, tox-gene PCR, Lateral Flow Immuno Assay, a modified Elek test and Next Generation Sequencing (NGS) identified the causative strain as cotrimoxazole-resistant toxigenic Corynebacterium diphtheriae biotype mitis, sequenc

Adult cutaneous diphtheria by non-toxicogenic strain: Case report and review of literature.

We report an unusual case of recurrently infected post-folliculitis keloid. The diagnosis of cutaneous diphtheria was made by isolation of Corynebacterium diphtheriae cultured from pus collected from the skin lesion. The strain was non-toxigenic on multiplex real-time PCR assay. A prior pus sample from the lesion had grown Methicillin resistant Staphylococcus aureus (MRSA). The patient was treated with oral erythromycin for 14-days. Chemoprophylaxis was advised to the close contacts. The present case emphasizes the importance of investigation for unusual etiologies in patients with non-respond

Autochthonous outbreak of respiratory diphtheria caused by Corynebacterium diphtheriae, Germany, September 2024.

In September 2024, a school-aged child (P1), unvaccinated against diphtheria, was hospitalised with severe respiratory diphtheria caused by toxigenic Corynebacterium diphtheriae. P1 subsequently died from the disease. The child's mother (P2) had pharyngitis 9 days before the onset of symptoms of P1 and subsequently tested positive for C. diphtheriae. In multilocus sequence typing (MLST), the C. diphtheriae isolates from P1 and P2 were of sequence type (ST) 574. In core genome (cg)MLST, they were clonal, suggesting recent human-to-human transmission. This indicates the first autochthonous respi
